1.Association of microRNA gene polymorphisms with risk, clinicopathological characteristics and therapeutical efficacy among Chinese patients with Crohn's disease.

OBJECTIVE:
To assess the association of microribonucleic acid (miRNA) gene polymorphisms with the risk and clinicopathological characteristics of Crohn's disease (CD) and the influence of miRNA gene variants on the response to ustekinumab (UST) treatment among CD patients.
METHODS:
From January 2018 to February 2025, 312 patients diagnosed with CD and 527 gender- and age-matched normal controls were selected as the study subjects at the Department of Gastroenterology of the Second Affiliated Hospital of Wenzhou Medical University. Genotypes of miR-155 (rs767649), miR-21 (rs13137), miR-124 (rs531564) and miR-146a (rs57095329, rs2431697) were determined with multiplex polymerase chain reaction-ligase detection reaction (PCR-LDR) technique. The patients were divided into different subgroups according to the Montreal Classification Criteria for CD. Harvey-Bradshaw index (HBI) and simplified endoscopic score for CD were respectively applied to assess the clinical and endoscopic disease activity of CD. Unconditional logistic regression model was employed to analyze the distribution of miRNA gene polymorphisms between the two groups, as well as their influence on the clinicopathological characteristics of CD patients. Among them, 185 CD patients received first-line UST treatment, with the first sufficient dose of UST (6 mg/kg) administered intravenously. Based on the changes in HBI at week 8, the response of patients to UST treatment was evaluated. Unconditional logistic regression model was employed to analyze the distribution of miRNA gene polymorphisms between clinically responsive group (the decline of HBI ≥ 3 scores compared to week 0) and non-responsive group. All of the P values were adjusted by Bonferroni correction. This study has been approved by the Medical Ethics Committee of the Second Affiliated Hospital of Wenzhou Medical University (Ethics No.: 2025-K-12-01).
RESULTS:
No significant difference was found in the distribution of miRNA gene polymorphisms between the two groups (all P > 0.05). The variant genotype (TC+CC) of rs2431697 was more common among patients with terminal ileal-type and ileocolic-type CD than those with the colonic-type CD (OR = 4.98, 95%CI: 1.49~16.68, P = 0.009, adjusted P = 0.045). However, the opposite conclusion was drawn for the homozygous variant genotype (TT) of rs13137 and variant genotype (GC+CC) of rs531564 (OR = 0.37, 95%CI: 0.18~0.76, P = 0.007, adjusted P = 0.035; OR = 0.36, 95%CI: 0.18~0.73, P = 0.004, adjusted P = 0.020). Compared to patients with non-stricturing and penetrating CD, the variant genotype (AG+GG) and variant allele (G) of rs57095329 were more common in those with stricturing and penetrating CD (OR = 4.06, 95%CI: 2.46~6.71, P < 0.001, adjusted P < 0.005; OR = 3.12, 95%CI: 2.06~4.73, P < 0.001, adjusted P < 0.005). However, the frequencies of variant genotype (AT+TT) and variant allele (T) of rs13137 were lower among patients with stricturing and penetrating CD than in those without (OR = 0.25, 95%CI: 0.15~0.41, P < 0.001, adjusted P < 0.005; OR = 0.45, 95%CI: 0.33~0.63, P < 0.001, adjusted P < 0.005). Additionally, the variant genotype (AG+GG) and variant allele (G) of rs57095329 were more common among those with moderately to severely endoscopic activity than those with mildly endoscopic activity (OR = 2.01, 95%CI: 1.19~3.42, P = 0.009, adjusted P = 0.045; OR = 2.04, 95%CI: 1.28~3.25, P = 0.003, adjusted P = 0.015). In total 117 cases had shown clinical response by week 8, while 68 cases showed no response. Compared with t he clinically non-responsive group, the variant genotype (TC+CC) and variant allele (C) of rs2431697 were more common in the clinically responsive group (OR = 3.86, 95%CI: 1.80~8.32, P = 0.001, adjusted P = 0.005; OR = 2.60, 95%CI: 1.34~5.06, P = 0.005, adjusted P = 0.025). However, the variant genotype (TA+AA) of rs767649 was less frequent in the clinically responsive group than the non-responsive group (OR = 0.40, 95%CI: 0.21~0.74, P = 0.004, adjusted P = 0.020). The same conclusion was drawn for the variant genotype (AT+TT) and variant allele (T) of rs13137 when the clinically responsive group was compared with the non-responsive group (OR = 0.30, 95%CI: 0.14~0.63, P = 0.002, adjusted P = 0.010; OR = 0.54, 95%CI: 0.35~0.82, P = 0.005, adjusted P = 0.025).
CONCLUSION
Genetic polymorphisms of miRNAs are not associated with the risk of developing CD. The miR-146a (rs57095329) variant may increase the endoscopic activity of CD and the risk for stenosis or penetration. However, the miR-146a (rs2431697) variant may increase the risk of ileal involvement. The miR-21 (rs13137) variant may reduce the risk of ileal involvement and the risk of stenosis or penetration. The miR-124 (rs531564) variant may reduce the risk of ileal involvement. Among patients receiving UST treatment, the miR-146a (rs2431697) variant may increase the clinical response by week 8. However, both the miR-155 (rs767649) and miR-21 (rs13137) variants may decrease the clinical response by week 8.

2.Research advances in Infantile liver failure syndrome.

Pediatric acute liver failure (PALF) is a rare and critical clinical syndrome with a poor prognosis. Its etiology is complex, with a significant proportion of cases having remained classified as indeterminate or cryptogenic PALF. With the application of high-throughput sequencing technologies, a spectrum of disorders caused by specific genetic metabolic defects and characterized by stress-sensitive Recurrent acute liver failure (RALF) has been gradually unveiled, collectively termed Infantile liver failure syndrome (ILFS). Although the molecular mechanisms underlying the subtypes ILFS1, ILFS2, and ILFS3 differ by involving aminoacyl-tRNA synthetase defects, vesicular transport disorders, and autophagy abnormalities, respectively, they share a common clinical phenotype of RALF triggered by fever or infection. This article has systematically reviewed the clinical phenotypic spectrum, molecular genetic characteristics, differential diagnosis strategies, and therapeutic advances of the three ILFS subtypes, with the goal of improving early clinical recognition and precise intervention, and providing an important reference for evaluating the prognosis of different subtypes.

3.Starvation Ketoacidosis Secondary to Retatrutide-Induced Gastroparesis: A Case Report

Introduction
Retatrutide is an investigational triple agonist targeting
glucagon-like peptide-1, glucose-dependent insulinotropic
polypeptide, and glucagon receptors. Early clinical trials have demonstrated promising weight loss effects. Gastrointestinal side effects are common and dose-dependent;
however, severe complications such as gastroparesis leading
to starvation ketoacidosis are rarely reported. Increasing
public interest in emerging anti-obesity pharmacotherapies
has led to unsupervised access to investigational medications through unregulated channels. We report a case of
starvation ketoacidosis associated with retatrutide-induced
gastroparesis following unsupervised use of medication
obtained from unregulated sources.
Case:
A 39-year-old female healthcare worker with prediabetes
(hemoglobin A1c 5.7%), class I obesity (body mass index
31.2 kg/m²), and endometriosis presented with persistent
vomiting and diarrhea. She reported using retatrutide for
weight loss, initially obtained from an informal source,
although the authenticity and origin of the medication
could not be verified. She self-titrated retatrutide over 7
weeks, escalating from 1 mg twice weekly to 6 mg weekly,
with only mild nausea. Subsequently, she purchased
the medication from another unverified online source.
Following this change, she developed worsening nausea at
the 6 mg weekly dose, prompting a dose reduction to 5 mg
weekly. Two days after the most recent dose, she developed
intractable vomiting and diarrhea. On presentation, she
was clinically dehydrated. Venous blood gas revealed
metabolic acidosis (pH 7.33, bicarbonate 18.8 mmol/L) with
elevated serum ketones (4.5 mmol/L) and normal lactate
(1.2 mmol/L). Blood glucose was 4.5 mmol/L, and serum
amylase was normal. She was treated with intravenous
fluids, fixed-rate insulin infusion, and antiemetics.
Ketoacidosis resolved by day 3, with improvement of
gastrointestinal symptoms by day 5.
Conclusion
This case highlights a serious complication associated with
retatrutide and underscores the risks of unsupervised use
of investigational weight-loss therapies obtained from
unregulated sources. It also emphasizes pharmacovigilance
as emerging antiobesity therapies become widely used.
Careful dose titration, close clinical monitoring, and
use within regulated medical channels are essential for
patient safety.

4.Severe Hypertriglyceridemia-Induced Acute Pancreatitis in Pregnancy: A Case Report and Review of Management

Introduction:
Severe hypertriglyceridemia during pregnancy is a rare
yet potentially life-threatening disorder. It is frequently
linked to acute pancreatitis and poses considerable risks
for both maternal and fetal morbidity.
Case:
We report a 35-year-old G4P3 female at 33 + 6 weeks
gestation with underlying type 2 diabetes mellitus who
presented with acute abdominal pain, nausea, vomiting,
and poor oral intake, initially mistaken for preterm labor
and sepsis. Further evaluation revealed profound hypertriglyceridemia (triglycerides 79.1 mmol/L) with markedly
elevated total cholesterol (>32 mmol/L) and raised serum
amylase, consistent with hypertriglyceridemia-induced
acute pancreatitis. She was managed in a multidisciplinary
setting with intravenous insulin infusion, dextrose supplementation, aggressive fluid resuscitation, and empiric
antibiotics. Due to clinical deterioration and ongoing risk of
maternal complications, she underwent emergency lower
segment caesarean section and exploratory laparotomy
at 34 weeks of gestation. Intraoperative findings included
inflammatory intra-abdominal fluid and a bulky pancreas,
supporting the diagnosis. Rapid biochemical improvement was observed following treatment, with triglyceride levels
decreasing by approximately 70% within 24 hours (79.1–
24.0 mmol/L) and achieving >90% reduction over 10 days.
Postpartum management included initiation of omega-3
fatty acids, fibrate, and statin therapy, resulting in sustained
lipid control. Ophthalmologic examination demonstrated
lipemia retinalis, further confirming severe dyslipidemia.
Conclusion
This case highlights the importance of early recognition of
severe hypertriglyceridemia in pregnancy, particularly in
patients with metabolic risk factors such as diabetes. Prompt
initiation of insulin therapy can achieve rapid triglyceride
reduction and may obviate the need for plasmapheresis in
selected cases. Delivery should be considered in the setting
of maternal instability. Long-term lipid management
postpartum is essential for preventing recurrence and
reducing future metabolic risk. A multidisciplinary
approach remains critical in optimizing both maternal and
fetal outcomes in this rare but serious condition.

Introduction:
Pancreatic neuroendocrine tumors (PNETs) are rare,
comprising less than 3% of all pancreatic neoplasms.
These tumors are broadly classified as functioning or nonfunctioning. Insulinoma is the most common functioning
PNET. Non-functioning PNETs often present significant diagnostic issues and may be misdiagnosed as pancreatic
adenocarcinoma, especially when immunohistochemical
evaluation is omitted during histological examination. We
describe a case of metastatic insulinoma that was misdiagnosed as a poorly differentiated pancreatic adenocarcinoma.
Case:
A 65-year-old female first presented to a private hospital
in 2023 with obstructive jaundice and was found to
have a pancreatic head lesion that was causing biliary
obstruction. As a result, a biliary stent was placed. She
underwent aortocaval lymph node biopsy, and the result
showed poorly differentiated pancreatic adenocarcinoma.
Nonetheless, she refused surgical and oncological
intervention. Even so, she remained clinically stable and
maintained good functional status for 2 years.
In 2025, she presented to Hospital Canselor Tuanku
Muhriz with recurrent hypoglycemia fulfilling Whipple’s
triad. The unexpectedly indolent clinical course prompted
reassessment of the initial diagnosis. Biochemical evaluation
confirmed endogenous hyperinsulinemic hypoglycemia,
with inappropriately elevated insulin (11.03 µIU/mL) and
C-peptide levels (1,089 pmol/L). Computer tomography
of the abdomen revealed multiple hepatic lesions and
progressive lymphadenopathy, suggestive of metastatic
disease. Re-evaluation of the initial histopathological
specimen showed a well-differentiated neuroendocrine
tumor (Grade 1, Ki-67 ~2%). Hence, the diagnosis was
revised to metastatic insulinoma.
She was referred to the hepatobiliary surgical team for
surgical debulking, but the procedure was deemed highrisk and likely to have high mortality due to the extent of
the disease. She was managed with diazoxide and longacting somatostatin analogues for glycemic control.
Conclusion
This case highlights the critical importance of diagnostic
vigilance when evaluating pancreatic neoplasms. Persistent
or unexplained clinical courses, especially when endocrine
symptoms arise, should prompt thorough reassessment.
Immunohistochemical confirmation is essential to avoid
misdiagnosis and ensure optimal patient management.

6.Metastatic Uterine Leiomyosarcoma Presenting as an Overt Gastric Mass and Gastrointestinal Bleeding Masquerading as GIST: a Rare Diagnostic Pitfall

Introduction:
While uterine leiomyosarcoma, an aggressive smooth muscle malignancy,
has a strong propensity for hematogenous spread—most commonly to the lungs and
liver—gastric metastasis remains exceedingly rare. When it occurs, it presents as a
masquerading lesion that mimics primary gastric tumors, creating a significant diagnostic
pitfall for both clinicians and pathologists.
Case Description:
A 75-year-old female presented with recurrent melena. Esophagogastroduodenoscopy revealed a friable, bell-shaped mass measuring 7 × 6 cm in the
gastric fundus, suspicious for GIST. Histopathology showed spindle to epithelioid tumor
cells arranged in intersecting fascicles and solid sheets with vesicular nuclei, conspicuous
nucleoli, numerous mitosis, and tumor necrosis. IHCs demonstrated strong SMA positivity
but negative CD117, raising consideration of undifferentiated GIST. However, subsequent
DOG1 staining was negative, arguing strongly against GIST. Additional markers including
S100, PanCK, CD3, CD20, CD15, and CD30 were negative. Further IHCs revealed
diffuse, desmin and H-caldesmon positivity, supporting smooth muscle differentiation.
Review of prior records revealed TAHBSO in 2023 for uterine leiomyosarcoma.
Discussion:
Metastatic involvement of the stomach is rare and most commonly arises
from breast or lung primaries. Gastric metastases are rare and most commonly originate
from breast or lung primaries. Metastatic uterine leiomyosarcoma presenting as a solitary
gastric mass is exceedingly uncommon and may closely mimic primary gastric tumors
such as GIST both endoscopically and histologically, representing an important diagnostic
pitfall.
Conclusion
This case highlights a rare metastatic pattern of uterine leiomyosarcoma
presenting as an overt gastric mass with GI bleeding. Awareness of this deceptive
presentation and careful clinicopathologic correlation are essential for accurate diagnosis.
To our knowledge, this represents one of the few reported cases worldwide and possibly
the first documented in the Philippines.

7.Early predictors of rescue therapy and colectomy in acute severe ulcerative colitis.

INTRODUCTION:
Acute severe ulcerative colitis (ASUC) is a significant cause of disease morbidity. One-third of patients with ASUC are steroid refractory. Rescue therapy may not successfully induce remission, necessitating colectomy. We aimed to identify predictors of rescue therapy and colectomy in ASUC assessed within 24 h of admission for early risk stratification.
METHODS:
We conducted a retrospective cohort study of 58 admissions for ASUC among 47 patients from August 2002 to January 2022. Serum biomarkers assessed were measured on admission. Primary outcomes were the need for rescue therapy during the same admission and colectomy within 1 year of admission.
RESULTS:
Rescue therapy (all with infliximab) was given in 20 (34.5%) of the admissions. Colectomy was done within 1 year for nine (15.5%) of the admissions. An elevated C-reactive protein (CRP) of >30 mg/L (relative risk [RR] 1.63), a CRP-albumin ratio of >0.85 (RR 1.63), and a composite factor of both CRP > 30 mg/L and age ≥60 years (RR 2.37) were significantly associated with the need for rescue therapy. Hypoalbuminaemia ≤ 25 g/L (RR 4.35) and the use of biologics at presentation (RR 1.54) were significantly associated with colectomy within 1 year of admission, while a CRP of ≥ 80 mg/L was a significant protective factor (RR 0.70).
CONCLUSION
Patients with ASUC who have elevated CRP or CRP-albumin ratio on admission should be considered at risk for steroid-refractory disease. Those with hypoalbuminaemia on admission and using biologics at presentation are more likely to require colectomy in the first year after admission for ASUC.

8.Effects of electroacupuncture at changbing fang on autophagy of colonic cells and gut microbiota in ulcerative colitis of rats.

OBJECTIVE:
To observe the effects of electroacupuncture (EA) at changbing fang (prescription for intestinal disease) on autophagy of colonic cells and gut microbiota in rats with ulcerative colitis (UC), and to explore the mechanism of EA in the treatment of UC.
METHODS:
Thirty-two SD male rats were randomly divided into a control group, a model group, an EA group and a sham-EA group, with 8 rats in each group. Except the control group, the UC rat model was established by free drinking of 5% dextran sulfate sodium solution for 7 days in the other groups. In the EA group, changbing fang was adopted, in which, EA was applied at "Tianshu" (ST25) and "Shangjuxu" (ST37), at disperse-dense wave and frequency of 10 Hz/50 Hz, for 20 min in each intervention. In the sham-EA group, shallow transcutaneous puncture was performed at the sites, 5 mm away from the points as the EA group, with the same parameters as the EA group. The intervention was delivered once daily for 3 consecutive days. The body weight was measured daily and the disease activity index (DAI) score was calculated before and after intervention. After intervention completion, the colon length was measured. Using HE staining, the colon morphology was observed and the score of colonic pathology was assessed. With ELISA adopted, the contents of tumor necrosis factor (TNF-α), interleukin (IL)-1β, IL-2 and IL-10 in the serum of the rats were detected. The ultrastructure of the colon tissue was observed under electron microscopy. Using Western blotting, the protein expression was detected for microtubule-associated protein 1 light chain 3 (LC3)Ⅱ, LC3Ⅰ, autophagy-related genes (ATG) 5, ATG12, sequestosome 1 (p62), phosphatidylinositol 3-kinase (PI3K), phosphorylated protein kinase B (p-AKT), protein kinase B (AKT), and phosphorylated mammalian target of rapamycin (p-mTOR), mammalian target of rapamycin (mTOR) in the colon tissue. The mRNA expression of PI3K, AKT and m-TOR in the colon tissue was detected by real-time fluorescence quantitative PCR. The 16S rRNA gene sequencing was used to analyze the structure of gut flora in the feces of rats.
RESULTS:
From day 1 to day 7, compared with the control group, the body weight decreased in the model group, EA group, and SEA group (P<0.05, P<0.01). From day 9 to day 10, the EA group showed an increase in body weight compared with the model group and SEA group (P<0.05, P<0.01). Before intervention, the DAI score in the model group, EA group, and SEA group was higher than the score of the control group, respectively (P<0.01). After intervention, the DAI score in the EA group was reduced compared with the model group and SEA group (P<0.01). Compared with the control group, in the model group, the colon length of rats was shorter (P<0.01); it showed the distorted crypts, thinner mucosal layer, reduced goblet cells, inflammatory cell infiltration and the disarranged histological structure; and the pathological score of the colon tissue increased (P<0.01); the serum contents of TNF-α and IL-1β increased (P<0.01), and those of IL-2 and IL-10 decreased (P<0.01). The structure of colon epithelial cells was disarranged, with cilia pelt off, and a large number of vacuoles in the cytoplasm; the mitochondria were swollen, with unclear structure and cristae partially disappeared; and few autophagosomes were observed. The value of LC3Ⅱ/LC3Ⅰand the protein expression of ATG5 and ATG12 in the colon tissues were reduced (P<0.01), the protein expression of p62 and PI3K, and the values of p-AKT/AKT, and p-mTOR/mTOR increased (P<0.01), and mRNA expression of PI3K, AKT and mTOR was elevated (P<0.01). The indexes of Chao1, Ace and Shannon decreased (P<0.01). At the phylum level, the relative abundance of Firmicutes decreased (P<0.05), that of Bacteroidetes and Proteobacteria increased (P<0.05, P<0.01). At the genus level, the relevant abundance of Lactobacillus decreased (P<0.05), while that of Lachnospiraceae_NK4A136_group and Phascolarctobacterium increased (P<0.01, P<0.05 ). Compared with the model group and SEA group, in the EA group, the colon length increased (P<0.01), the infiltration of inflammatory cells was reduced, the arrangement of intestinal epithelial cells was arranged regularly, with a small amount of shedding, and the pathological score of the colon tissue decreased (P<0.01). The serum contents of TNF-α and IL-1β decreased (P<0.01), and those of IL-2 and IL-10 increased (P<0.01). The colonic epithelial cells were arranged relatively, the morphology of organelles was basically normal, and autophagosomes were visible. The value of LC3Ⅱ/LC3Ⅰand the protein expression of ATG5 and ATG12 in colon tissue increased (P<0.01, P<0.05), the protein expression of p62 and PI3K, and the values of p-AKT/AKT, and p-mTOR/mTOR decreased (P<0.01); and mRNA expression of PI3K, AKT, m-TOR was reduced (P<0.01). The indexes of Chao1, Ace and Shannon increased (P<0.01). At the phylum level, the relative abundance of Firmicutes increased (P<0.01), while that of Bacteroidetes decreased (P<0.01). At the genus level, the relative abundance of Lactobacillus increased (P<0.05), whereas that of Lachnospiraceae_NK4A136_group decreased (P<0.01). When compared with the model group, the relative abundance of Proteobacteria decreased (P<0.05), and that of Phascolarctobacterium was reduced (P<0.05) in the EA group.
CONCLUSION
EA at changbingfang alleviates UC symptoms probably through inhibiting the PI3K/AKT/mTOR signaling pathway to regulate colonic autophagy and improve the intestinal flora.

9.Effects of electroacupuncture on early enteral nutrition tolerance and autonomic nerve activity in patients with acute pancreatitis.

OBJECTIVE:
To observe the therapeutic effect of electroacupuncture (EA) in improving early enteral nutrition tolerance in patients with acute pancreatitis (AP) under the concept of accelerated rehabilitation, and to explore the related mechanism based on the changes in autonomic nerve characteristics.
METHODS:
A total of 42 patients with AP were randomized into an observation group (21 cases, 1 case dropped out) and a control group (21 cases, 1 case dropped out). The control group received standard basic treatment for AP. On the basis of the treatment in the control group, EA was applied in the observation group, bilateral Zusanli (ST36), Yixian point (Extra), Tianshu (ST25), Neiguan (PC6) and Zhongwan (CV12) were selected as the main points, and the supplementary points were selected according to syndrome differentiation. Ipsilateral Zusanli (ST36) and Yixian point (Extra) were connected to EA, using discontinuous wave, in frequency of 2 Hz, 30 min a time, once a day for 6 continuous days. The enteral nutrition tolerance score was observed before treatment and after 3 and 5 days of treatment; the visual analogue scale (VAS) score for abdominal pain was observed before treatment and after 3 days of treatment; the time of reaching the feeding goal and hospital stay was recorded; the levels of C-reactive protein (CRP) and amylase were measured before treatment and after 5 days of treatment; the heart rate variability (HRV) indexes (standard deviation of NN intervals [SDNN], average standard deviation of NN intervals [SDANN], root mean square of successive NN interval differences [rMSSD], low frequency [LF] and high frequency [HF], ratio of low frequency to high frequency [LF/HF]) were monitored in the two groups.
RESULTS:
After 3 and 5 days of treatment, the enteral nutrition tolerance scores were decreased compared with those before treatment in both groups (P<0.01), the reductions in the observation group were larger than those in the control group (P<0.01). After 3 days of treatment, the VAS scores for abdominal pain were decreased compared with those before treatment in both groups (P<0.01), the reduction in the observation group was larger than that in the control group (P<0.01). The time of reaching the feeding goal and hospital stay in the observation group was shorter than that in the control group (P<0.05). After 5 days of treatment, the CRP and amylase levels were decreased compared with those before treatment in both groups (P<0.01), the reduction of CRP level in the observation group was larger than that in the control group (P<0.01). In the observation group, SDNN, SDANN and LF/HF were lower than those in the control group (P<0.05, P<0.01), while rMSSD was higher than that in the control group (P<0.01). SDNN, SDANN and LF/HF were positively correlated with the enteral nutrition tolerance scores after 3 and 5 days of treatment (P<0.05), while rMSSD was negatively correlated with the enteral nutrition tolerance scores after 3 and 5 days of treatment (P<0.01).
CONCLUSION
Electroacupuncture can improve enteral nutrition tolerance in patients with AP by regulating autonomic nervous function, alleviating the inflammation, promoting accelerated recovery, and reducing the length of hospital stay.

10.Vagus nerve modulates acute-on-chronic liver failure progression via CXCL9.

BACKGROUND:
Hepatic inflammatory cell accumulation and the subsequent systematic inflammation drive acute-on-chronic liver failure (ACLF) development. Previous studies showed that the vagus nerve exerts anti-inflammatory activity in many inflammatory diseases. Here, we aimed to identify the key molecule mediating the inflammatory process in ACLF and reveal the neuroimmune communication arising from the vagus nerve and immunological disorders of ACLF.
METHODS:
Proteomic analysis was performed and validated in ACLF model mice or patients, and intervention animal experiments were conducted using neutralizing antibodies. PNU-282987 (acetylcholine receptor agonist) and vagotomy were applied for perturbing vagus nerve activity. Single-cell RNA sequencing (scRNA-seq), flow cytometry, immunohistochemical and immunofluorescence staining, and clustered regularly interspaced short palindromic repeats/CRISPR-associated protein 9 (CRISPR/Cas9) technology were used for in vivo or in vitro mechanistic studies.
RESULTS:
The unbiased proteomics identified C-X-C motif chemokine ligand 9 (CXCL9) as the greatest differential protein in the livers of mice with ACLF and its relation to the systematic inflammation and mortality were confirmed in patients with ACLF. Interventions on CXCL9 and its receptor C-X-C chemokine receptor 3 (CXCR3) improved liver injury and decreased mortality of ACLF mice, which were related to the suppressing of hepatic immune cells' accumulation and activation. Vagus nerve stimulation attenuated while vagotomy aggravated the expression of CXCL9 and the severity of ACLF. Blocking CXCL9 and CXCR3 ameliorated liver inflammation and increased ACLF-associated mortality in ACLF mice with vagotomy. scRNA-seq revealed that hepatic macrophages served as the major source of CXCL9 in ACLF and were validated by immunofluorescence staining and flow cytometry analysis. Notably, the expression of CXCL9 in macrophages was modulated by vagus nerve-mediated cholinergic signaling.
CONCLUSIONS
Our novel findings highlighted that the neuroimmune communication of the vagus nerve-macrophage-CXCL9 axis contributed to ACLF development. These results provided evidence for neuromodulation as a promising approach for preventing and treating ACLF.
