OBJECTIVES:
To evaluate the safety and efficacy of thiotepa (TT)-containing conditioning regimens for allogeneic hematopoietic stem cell transplantation (HSCT) in children with inborn errors of immunity (IEI).
METHODS:
Clinical data of 22 children with IEI who underwent HSCT were retrospectively reviewed. Survival after HSCT was estimated using the Kaplan-Meier method.
RESULTS:
Nine patients received a traditional conditioning regimen (fludarabine + busulfan + cyclophosphamide/etoposide) and underwent peripheral blood stem cell transplantation (PBSCT). Thirteen patients received a TT-containing modified conditioning regimen (TT + fludarabine + busulfan + cyclophosphamide), including seven PBSCT and six umbilical cord blood transplantation (UCBT) cases. Successful engraftment with complete donor chimerism was achieved in all patients. Acute graft-versus-host disease occurred in 12 patients (one with grade III and the remaining with grade I-II). Chronic graft-versus-host disease occurred in one patient. The incidence of EB viremia in UCBT patients was lower than that in PBSCT patients (P<0.05). Over a median follow-up of 36.0 months, one death occurred. The 3-year overall survival (OS) rate was 100% for the modified regimen and 88.9% ± 10.5% for the traditional regimen (P=0.229). When comparing transplantation types, the 3-year OS rates were 100% for UCBT and 93.8% ± 6.1% for PBSCT (P>0.05), and the 3-year event-free survival rates were 100% and 87.1% ± 8.6%, respectively (P>0.05).
CONCLUSIONS
TT-containing conditioning for allogeneic HSCT in children with IEI is safe and effective. Both UCBT and PBSCT may achieve high success rates.

Objective:To evaluate the safety of neoadjuvant therapy with pembrolizumab combined with 5-fluorouracil (5-FU) and cisplatin in patients with advanced temporal bone squamous cell carcinoma (TBSCC), and its impact on tumor response rate and disease-free survival (DFS).Methods:This prospective, single-arm, open-label clinical study enrolled patients with advanced (Stage Ⅲ/Ⅳ) TBSCC from Sun Yat-sen Memorial Hospital. Patients received 2-3 cycles of neoadjuvant therapy with pembrolizumab, 5-FU, and cisplatin, followed by definitive surgery. Postoperatively, patients received 6 cycles of pembrolizumab combined with radiotherapy. The primary endpoint was the 2-year disease-free survival (DFS) rate. Secondary endpoints included objective response rate (ORR) and safety indicators. Survival analysis was performed using the Kaplan-Meier method. Adverse events (AE) were assessed using the National Cancer Institute′s Common Terminology Criteria for Adverse Events (CTCAE), version 5.0. Statistical analyses were conducted using SPSS software, version 22.0.Results:From August 2021 to April 2024, 16 patients with advanced TBSCC were enrolled (13 males and 3 females), with a median age of 54 years and a median follow-up time of 2.32 years. Following neoadjuvant therapy, the objective response rate (ORR) was 64.3% (9/14), and the disease control rate (DCR) was 92.9% (13/14). The 2-year DFS rate was 86.6%. Common treatment-related adverse events (TRAE) included leukopenia (56.3%, 9/16), nausea and vomiting (50.0%, 8/16), diarrhea, oral mucositis, and elevated liver function tests (25.0%, 4/16). One patient (6.25%) experienced a grade 3 adverse event. Conclusion:Neoadjuvant pembrolizumab-chemotherapy significantly enhances objective response rate and disease-free survival in advanced TBSCC.

Chinese Journal of Neuromedicine 2025;24(7):656-664
Objective:To investigate the heterogeneity of tumor density-enhancement complex (TDEC) based on MRI parameters in predicting the treatment response and prognosis for recurrent glioblastoma (rGBM) to guide the formulation of personalized clinical treatment strategies. Methods:A prospective cohort study was performed; 66 patients with postoperative rGBM were enrolled from Department of Neurosurgery, Affiliated Hospital of Jiangsu University. Multi-sequence MRI was performed, and diffused and enhanced data of the rGBM were utilized to construct TDEC as intratumoral sub-regions via pixel co-localization technique. Correlations among rGBM with different volume proportions of TDEC types and correlations of rGBM with different volume proportions of TDEC types with rGBM volume were analyzed in rGBM after bevacizumab (BEV) combined with radiotherapy. A pixel co-localization decoupling method was applied to assess the treatment response efficiency in individual TDEC subcomponents. The rGBM imaging phenotypes were identified through unsupervised clustering analysis, and progression-free survival (PFS) and overall survival (OS) between patients with different phenotypes were compared. The predictive value of TDEC heterogeneity in PFS and OS of rGBM patients under BEV plus radiotherapy was assessed. Results:Four distinct TDEC sub-regions (TDEC1-4) were identified; a significant negative correlation was observed between volume proportions of TDEC2 and TDEC3 ( r s=-0.558, P<0.001), as well as between volume proportions of TDEC3 and TDEC4 ( r s=-0.782, P<0.001), while TDEC composition (volume proportions of TDEC2-4) showed no significant correlation with tumor volume ( P>0.05). Following BEV combined with radiotherapy, significant sub-region-specific TDEC volume changes were observed (tumor volume minification rate of TDEC1[ΔV TDEC1]: 16.7% [13.8%, 20.1%]; ΔV TDEC2: 25.4% [21.9%, 29.0%]; ΔV TDEC3: 27.6% [23.5%, 31.2%]; ΔV TDEC4: 8.4% [6.1%, 10.7%], P<0.05); volume proportion of TDEC3 was positively correlated with tumor volume minification ( r s=0.702, P<0.001), whereas volume proportion of TDEC4 was negatively correlated tumor volume minification ( r s=-0.933, P<0.001). The volume reduction of TDEC1-3 was driven by combined effects of tumor cellular and enhancement components, while volume reduction of TDEC4 was primarily attributed to changes in tumor cellularity (ΔV ADC: 9.3%; ΔV T1C: 0.8%). Two distinct TDEC phenotypes with different survival outcomes were identified in rGBM patients (silhouette coefficient=0.584; TDEC type I: n=23; type II: n=43); significant difference in PFS and OS was noted between patients with TDEC type I and type II (PFS: χ2=11.191, P=0.001; OS: χ2=9.733, P=0.002). TDEC phenotype was an independent influencing factor for survival of rGBM patients under BEV combined with radiotherapy (PFS: HR=2.738, 95% CI: 1.815-3.938 , P=0.003; OS: HR=2.507, 95% CI: 1.851-3.660, P=0.007). Conclusion:TDEC sub-region helps efficiently characterize the rGBM heterogeneity; rGBM imaging phenotypes identified based on TDEC sub-region can independently predict the clinical outcomes: the prognosis of TDEC type I patients is better than that of TDEC type II patients.

ObjectiveTo analyse the efficacy and mechanism of Guizhi Fulingwan in regulating sex hormone disorders in rats with benign prostatic hyperplasia (BPH). MethodsThirty male SD rats were randomly divided into a sham group, a model group, a finasteride group (0.45 mg·kg-1·d-1), and low-dose and high-dose groups of Guizhi Fulingwan (0.135, 0.337 5 g∙kg-1∙d-1), with six in each group. The BPH model was prepared by subcutaneous injection of 3.5 mg∙kg-1∙d-1 testosterone propionate after debridement surgery in all groups except the sham group. The rats in the sham group and the model group were administered with an equal volume of saline by gavage, and the rest of the groups were administered with the corresponding medicinal solution by gavage for 35 days. Histopathology in rats was evaluated by prostate wet weight, volume, index, and hematoxylin-eosin (HE) staining. The serum sex hormone levels of testosterone (T), dihydrotestosterone (DHT), and estradiol (E2) were determined by enzyme-linked immunosorbent assay. The protein expression of the androgen receptor (AR) was detected by immunohistochemistry. The serum metabolism profiles of rats in the sham group, the model group, and the high-dose group of Guizhi Fulingwan were compared by ultra-high performance liquid chromatography tandem Fourier transform mass spectrometry (UHPLCQ Exactive) to screen for metabolic markers and to obtain relevant metabolic pathways. ResultsCompared with those in the sham group, the wet weight, volume, index, serum sex hormone level, and AR protein expression of the prostate in the model group were all elevated (P<0.05, P<0.01), and the histomorphology showed pathological changes. Compared with those in the model group, the wet weight, volume, index, serum sex hormone level, and AR protein expression of the prostate in the intervention groups showed a decreasing trend (P<0.05, P<0.01), and histopathology was improved. Serum metabolomics analysis obtained a total of 40 metabolic markers related to the intervention effect of Guizhi Fulingwan, such as dehydrosafynol, hyoscyamine, and lumichrome, which were involved in the pathways of autophagy, riboflavin metabolism, and retrograde endocannabinoid signaling. ConclusionGuizhi Fulingwan can effectively regulate sex hormone disorders in BPH rats, and its mechanism may be related to autophagy, riboflavin metabolism, and retrograde endocannabinoid signaling.

Anxiety disorder is a highly prevalent psychological illness, and research has shown that obesity is a significant risk factor for its development. This study explored the ameliorative effects and mechanisms of saponins from Panax japonicus(SPJ) on anxiety disorder in mice fed a high-fat diet(HFD). Fifty C57BL/6J mice were randomly divided into normal control diet(NCD) group, HFD group, and low-and high-dose SPJ groups. At week 12, six mice from the HFD group were further divided into a control group(treated with DMSO) and an exogenous fibroblast growth factor 21(FGF21) group(administered rFGF21). The anxiety-like behavior of the mice was assessed using the open field test and elevated plus maze test. Hematoxylin-eosin(HE) staining and oil red O staining were performed to observe pathological changes in the liver and adipose tissue. Glucose metabolism was evaluated through the glucose tolerance test(GTT) and insulin tolerance test(ITT). Western blot analysis was performed to detect the expression of FGF21 and its downstream-related proteins in the liver and cortex, along with the expression of brain-derived neurotrophic factor(BDNF), disks large homolog 4(DLG4), and synaptophysin(SYP) in the cortex. Real-time quantitative fluorescent PCR(qPCR) was used to detect the expression of FGF21 and its receptor genes in the liver and cortex. Immunofluorescence staining was employed to examine the expression of neuronal activator c-Fos, FGF21, and the FGF21 co-receptor β-klotho in the cerebral cortex. The results showed that SPJ significantly improved the frequency of activity in the open arms of the elevated plus maze and the central area of the open field in HFD mice, up-regulated the expression of BDNF, DLG4, and SYP, and effectively alleviated anxiety-like behaviors in HFD mice. Compared with the NCD group, HFD mice exhibited up-regulated expression of FGF21 in the liver and cerebral cortex, while the expression of fibroblast growth factor receptor 1(FGFR1) and β-klotho was significantly down-regulated, suggesting that HFD mice exhibited FGF21 resistance. SPJ markedly up-regulated the β-klotho levels in HFD mice, reversing FGF21 resistance. Further comparison with exogenously administered FGF21 revealed that SPJ activates brain cortical regions in a consistent manner, and additionally, SPJ promotes the number and colocalization of c-Fos and β-klotho positive cells in the brain cortex. In summary, SPJ effectively alleviates anxiety-like behaviors in HFD mice. Its mechanism is associated with up-regulation of β-klotho expression in the brain, reversal of FGF21 resistance, and subsequent activation of neurons in the cerebral cortex and amygdala.

The theory of"spleen and stomach deficiency and excess transformation"originates from Huangdi Neijing, which is based on the five elements theory. It systematically elucidates the physiological interconnections and pathological transmission relationships among spleen, stomach, and the five zang and six fu viscera. This theory was continuously developed and refined by later physicians. It was first systematically summarized and deepened by LI Dongyuan in his work Piwei Lun,which elaborates on the pathological transmission relationships of other zang-fu viscera after spleen and stomach deficiency. From the perspective of LI Dongyuan's theory of"spleen and stomach deficiency and excess transformation",this paper discusses the pathological relationships between spleen-earth,lung-metal,and kidney-water,and proposes that spleen-earth deficiency is the pathological basis for the onset of allergic rhinitis.Based on the pathological evolution following spleen-earth deficiency, the traditional Chinese medicine syndromes of allergic rhinitis were categorized into three types:earth deficiency with metal weakness, earth dryness with metal desiccation, and water cold with metal excess and earth decline. The treatment of allergic rhinitis should prioritize the spleen-earth,employing acrid and dispersing herbs with light properties to elevate the spleen,sweet-warm and sour-astringent herbs to tonify the spleen, and diuretic and dampness-resolving herbs to activate the spleen, thereby restoring spleen-earth function.Simultaneously, treatment should regulate lung-metal and kidney-water according to different pathological evolutions, incorporating cold-cool or acrid-warm herbs as appropriate, combining cold and warm properties, and treating both the manifestation and root cause of the disease.
