ObjectiveTo elucidate the critical role of rehabilitation medical records (including electronic records) in rehabilitation medicine's clinical practice and management, comprehensively analyzed the structure, core content and data standards of rehabilitation medical records, to develop a standardized medical record data architecture and core dataset suitable for rehabilitation medicine and to explore the application of rehabilitation data in performance evaluation and payment. MethodsBased on the regulatory documents Basic Specifications for Medical Record Writing and Basic Specifications for Electronic Medical Records (Trial) issued by National Health Commission of China, and referencing the World Health Organization (WHO) Family of International Classifications (WHO-FICs) classifications, International Classification of Diseases (ICD-10/ICD-11), International Classification of Functioning, Disability and Health (ICF), and International Classification of Health Interventions (ICHI Beta-3), this study constructed the data architecture, core content and data standards for rehabilitation medical records. Furthermore, it explored the application of rehabilitation record summary sheets (home page) data in rehabilitation medical statistics and payment methods, including Diagnosis-related Groups (DRG), Diagnosis-Intervention Packet (DIP) and Case Mix Index. ResultsThis study proposed a systematic standard framework for rehabilitation medical records, covering key components such as patient demographics, rehabilitation diagnosis, functional assessment, rehabilitation treatment prescriptions, progress evaluations and discharge summaries. The research analyzed the systematic application methods and data standards of ICD-10/ICD-11, ICF and ICHI Beta-3 in the fields of medical record terminology, coding and assessment. Constructing a standardized data structure and data standards for rehabilitation medical records can significantly improve the quality of data reporting based on the medical record summary sheet, thereby enhancing the quality control of rehabilitation services, effectively supporting the optimization of rehabilitation medical insurance payment mechanisms, and contributing to the establishment of rehabilitation medical performance evaluation and payment based on DRG and DIP. ConclusionStructured rehabilitation records and data standardization are crucial tools for quality control in rehabilitation. Systematically applying the three reference classifications of the WHO-FICs, and aligning with national medical record and electronic health record specifications, facilitate the development of a standardized rehabilitation record architecture and core dataset. Standardizing rehabilitation care pathways based on the ICF methodology, and developing ICF- and ICD-11-based rehabilitation assessment tools, auxiliary diagnostic and therapeutic systems, and supporting terminology and coding systems, can effectively enhance the quality of rehabilitation records and enable interoperability and sharing of rehabilitation data with other medical data, ultimately improving the quality and safety of rehabilitation services.

Purpose:
Gold-standard urodynamic examination is widely used in the diagnosis and treatment of lower urinary tract dysfunction. The purpose of urodynamic quality control is to standardize urodynamic examination and ensure its clinical reference value. In our study, we attempted to use a deep learning (DL) algorithm model, mainly for the recognition of typical urodynamic signal, to help physicians complete high-quality urodynamic examinations.
Methods:
Urodynamic image data from 2 cohorts of adult patients with neurogenic bladder were used: (1) 300 patients with neurogenic bladder in our center from 2012 to 2018 (1,960 images used to train and validate the DL model); and (2) 100 patients with neurogenic bladder from 2020 to 2021 (695 images used to test the performance of the DL model). This resulted in a total of 2,655 images to train, validate and test the DL algorithm to predict the urdynamic signals.
Results:
Yolov5l had the best detection performance and the highest comprehensive index score (F1, 0.81; mean average precision, 0.83). Our study is a retrospective single-center study, and the generalization ability of the model has not been verified.
Conclusions
DL algorithms can help operators identify typical urodynamic signals in real time, improve the interpretation and quality of urodynamic examination, and benefit patients.

ObjectiveTo investigate the mechanism of Atractylodis Macrocephalae Rhizoma(AMR) in the treatment of slow-transmission constipation(STC) by observing the effects of AMR on short-chain fatty acids and intestinal barries in STC mice. MethodForty-eight male KM mice were randomly divided into blank group, model group, AMR low-, medium-, high-dose groups(2.5, 5, 10 g·kg-1) and mosapride group(2.5 mg·kg-1). Except for the blank group, all groups were gavaged with loperamide suspension(5 mg·kg-1) twice daily for 14 d to construct the STC mouse model. At the same time, each drug administration group was given the corresponding drug by gavage for consecutive 14 d, the blank and model groups were gavaged with equal volume of distilled water. The effects of the treatment of AMR on body mass, defecation frequency, fecal water content and intestinal propulsion rate of mice were observed, the pathological changes of mouse colon were observed by hematoxylin-eosin(HE) staining and periodic acid-Schiff(PAS) staining, the levels of gastrin(GAS) and motilin(MTL) in serum were detected by enzyme-linked immunosorbent assay(ELISA), gas chromatography-mass spectrometry(GC-MS) was used to detect the contents of short-chain fatty acids(SCFAs) in mouse feces, real-time fluorescence quantitative polymerase chain reaction(Real-time PCR) and Western blot were used to determine the mRNA and protein expression levels of zonula occludens-1(ZO-1), Occludin, and Claudin-1 in the colon of mice. ResultCompared with the blank group, the body mass, defecation frequency, fecal water content and intestinal propulsion rate of mice in the model group were significantly decreased(P<0.05, P<0.01), the arrangement of colonic tissues was disordered, and the number of goblet cells was reduced, the levels of GAS and MTL in serum were significantly decreased(P<0.01), and the levels of SCFAs in the feces were on a decreasing trend, with the contents of acetic acid, propionic acid, butyric acid, isobutyric acid and valeric acid were significantly decreased(P<0.05, P<0.01), the mRNA and protein expression levels of ZO-1, Occludin and Claudin-1 in the colonic tissues were significantly decreased(P<0.01). The above results suggested that STC mouse model was successfully constructed. Compared with the model group, the body mass, defecation frequency, fecal water content and intestinal propulsion rate of mice in AMP administration groups all increased significantly(P<0.05, P<0.01), the mucosal layer of the colonic tissues was structurally intact without obvious damage, and the number of goblet cells increased, serum levels of GAS and MTL were significantly increased(P<0.01), the contents of SCFAs in the feces were all on a rising trend, with the contents of acetic, propionic, butyric and isobutyric acids rising significantly(P<0.05, P<0.01), the mRNA and protein expression levels of ZO-1, Occludin and Claudin-1 in the colonic tissues were significantly increased(P<0.05, P<0.01). ConclusionAMR is able to improve the constipation symptoms in STC mice, and its mechanism may be related to increasing the contents of SCFAs in the intestine as well as promoting the mRNA and protein expression levels of ZO-1, Occludin and Claudin-1 in the colon.

The current situation of childrearing in the elderly and low birth rate showed that the prevention and treatment of infertility risk caused by the decline of male reproductive function are increasingly becoming prominent. The decline of male reproductive function is closely related to the hypothalamic-pituitary-gonadal axis, oxygen free radicals,blood-testis barrier, and the apoptosis of spermatogenic cells.Traditional Chinese medicine believes that the kidney stores essence and is in charge of reproduction, and the decline of reproductive function is closely related to the imbalance of yin and yang of the kidney.Therefore,kidney-supplementing method is helpful for treating the decline of reproductive function caused by kidney deficiency.Kidney-supplementing prescriptions were effective on systematically regulating the function of the hypothalamic-pituitary-gonadal axis, thus to significantly delay the decline of reproductive function, were effective on removing excessive oxygen free radicals in the body and protecting cells from oxidative damage, and were effective on improving spermatogenesis by restoring the blood-testis barrier and regulating the apoptosis of spermatogenic cells. The representative kidney-supplementing prescriptions for improving the decline of reproductive function caused by kidney deficiency include Wuzi Yanzong Pills,Shenqi Pills,Yougui Pills,Liuwei Dihuang Pills,Zuogui Pills and Guilu Erxian Soft Extract. The above formulas presented no single therapeutic mechanism but multiple-way,multiple-level and multiple-target mechanism, and the formulas can comprehensively regulate the internal environment of the body,so as to achieve the purpose of improving male reproductive function. The kidney-supplementing therapy of traditional Chinese medicine plays an irreplaceable role in the treatment of male reproductive function decline. The in-depth study of its therapeutic mechanism will provide guide for clinical practice and will supply theoretical basis for improving male reproductive function decline.
