Rectal cancer, which accounts for approximately 40% of colorectal cancers, remains a major clinical concern. Recent advances in diagnostic imaging, surgical techniques, radiotherapy, and systemic treatment have steadily improved rectal cancer outcomes. Considering this, the Korean Rectal Cancer Multidisciplinary (KRCM) Committee has aimed to provide clinicians and policymakers with up-to-date, evidence-based clinical practice guidelines to support optimal decision-making, reflecting current evidence, the Korean healthcare context, and patient values and preferences. The Clinical Practice Guidelines for Rectal Cancer version 2.0 were developed through multidisciplinary collaboration with related academic societies, building upon and updating the KRCM Clinical Practice Guidelines version 1.0 (titled “Multidisciplinary guidelines for the management of rectal cancer”). These consensus guidelines of the KRCM were established based on a comprehensive literature review, evidence synthesis, with recommendation development guided by the GRADE (Grading of Recommendations Assessment, Development and Evaluation) methodology, and consideration of applicability in real-world clinical practice under the national health insurance system. Each recommendation has been presented with its strength and level of evidence.

Objective: This study aimed to standardize the Korean version of the dysphagia risk assessment for the community-dwelling elderly (K-DRACE), identify normative values in the Korean elderly, and age-specific swallowing characteristics. Methods: Internal consistency and test-retest reliability were confirmed in 489 community-dwelling elderly aged 65 years or older in Korea. Concurrent validity was verified by correlation with the Korean version of eating assessment tool (K-EAT-10). A receiver operating characteristic (ROC) analysis was performed to assess utility and estimate the cut-off value. Norms were identified in 5-year age increments, from 65 to 85＋. Results: The Cronbach’s α value of K-DRACE was .815, test-retest reliability was .834 (95% CI .735-.909). It was significantly positively correlated with K-EAT-10 (r=.629, P＜.01). K-DRACE was thus deemed suitable as a screening tool to identify the risk of dysphagia (area under the curve [AUC]=.828), with an optimal cut-off of 3.5 (Youden’s index=.494). The total score of K-DRACE increased with age, with individuals aged 85 years and older being at risk of dysphagia (total score ＞4). The highest mean score was found in the oral phase. Conclusion K-DRACE demonstrated sufficient reliability, validity, and feasibility in the Korean elderly population and can be effectively utilized in community-based swallowing rehabilitation. This study confirms the need for specialized assessment and intervention in the oral phase for the oldest age group (85+) and emphasizes the importance of managing dysphagia in the community-dwelling elderly.

Sarcopenia is defined as a decline in muscle mass and strength and a deterioration of the physical functions caused by a decrease in lean body mass and an increase in fat with aging that is becoming a key condition of frailty in an aging society. Sarcopenia was assigned a disease code (M63.84) in the United States in 2016, with South Korea following suit by assigning it a disease code (M62.5) in the eighth revision of the Korean Standard Classification of Diseases in 2021. Discussions on the diagnostic criteria for sarcopenia are actively taking place in Asia, Europe, and other regions. On the other hand, although the presence of muscle loss is assessed, there are no established criteria for determining its severity. Furthermore, no effective treatment has been established because several factors affect muscle regeneration, including aging, lack of physical activity, endocrine dysfunction, metabolic diseases, chronic inflammation, and oxidative stress. Nevertheless, various candidate substances remain in clinical trial stages. This paper evaluated a clinical approach to sarcopenia by briefly reviewing its definition and diagnosis, summarizing the pathophysiology, and providing an overview of recent research and ongoing treatment developments.

Immunosenescence gradually deteriorates the function of the immune system, making elderly patients susceptible to infection, while reducing rejection of organ transplants. Therefore, age-adaptive immunosuppression is necessary in the elderly. We evaluated clinical outcomes such as rejection and infection rate when using basiliximab and rabbit anti-thymocyte globulin (r-ATG) as induction agents in elderly and young organ transplant recipients. Methods: We retrospectively reviewed patients who underwent kidney transplantation (KT) between June 2011 and April 2019. We enrolled 704 adult KT patients and classified the patients into groups according to patient age. We compared the outcomes of infection and biopsy-proven acute rejection (BPAR) according to the type of induction agent (basiliximab and r-ATG [4.5 mg/kg]). Results: The patient group included 520 recipients (74.6%) in the younger recipient group and 179 recipients (25.4%) in the older recipient group. When r-ATG was used as an induction agent, BPAR within 6 months occurred less (p = 0.03); however, infections within 6 months were higher in older recipients. Deaths due to infection were more common in older recipients (p = 0.003). Conclusion: It may be necessary to use less intensive induction therapy for older recipients, of which dose reduction of r-ATG is one option.
