Digital technologies have become an integral part of complete denture restoration. With advancement in computer-aided design and computer-aided manufacturing (CAD/CAM), tools such as intraoral scanning, facial scanning, 3D printing, and numerical control machining are reshaping the workflow of complete denture restoration. Unlike conventional methods that rely heavily on clinical experience and manual techniques, digital technologies offer greater precision, predictability, and efficacy. They also streamline the process by reducing the number of patient visits and improving overall comfort. Despite these improvements, the clinical application of digital complete denture restoration still faces challenges that require further standardization. The major issues include appropriate case selection, establishing consistent digital workflows, and evaluating long-term outcomes. To address these challenges and provide clinical guidance for practitioners, this expert consensus outlines the principles, advantages, and limitations of digital complete denture technology. The aim of this review was to offer practical recommendations on indications, clinical procedures and precautions, evaluation metrics, and outcome assessment to support digital restoration of complete denture in clinical practice.

2.The treatment of an edentulous patient with conventional complete denture and CAD/CAM complete denture

In patients with fully edentulous jaw, treatment of complete dentures should be carried out in many stages when following the conventional methods. Therefore there were disadvantages such as multiple visits to dental clinic is inevitable. In addition, errors caused by polymerization shrinkage, which happens during the fabrication of denture, and difficulties in reproduction of damaged or lost denture were considered as disadvantages. But nowadays, computer-aided design and computer-aided manufacturing (CAD/CAM) system is widely used in dentistry and it has begun to expand its spectrum in manufacturing complete dentures. Using CAD/CAM system to fabricate complete dentures can reduce the number of patient's visit and clinical chair time, since taking impression, recording jaw relation, and selection of artificial teeth are performed at the same time during the first visit, and delivering of dentures during the second visit is possible. In addition, because 3D-Printing technology is used, errors by polymerization shrinkage can be reduced. Among the companies that fabricate complete dentures using CAD/CAM system, DENTCA CAD/CAM denture (DENTCA Inc., Los Angeles, CA, USA) is the most commercialized company. In this case, we treated patients of complete dentures using conventional complete denture method and DENTCA CAD/CAM denture system in the same patient. We would like to report this case because we have achieved good results not only in functional aspects of pronunciation, chewing, and swallowing but also in aesthetic aspects.

3.Fabrication of additive manufacturing interim denture and comparison with conventional interim denture: A case report

With development of digital dentistry, the 3-dimensional (3D) manufacturing industry using computer-aided design and computer-aided manufacturing (CAD/CAM) has grown dramatically in recent years. Denture fabrication using digital method is also increasing due to the recent development of digital technology in dentistry. The 3D manufacturing process can be categorized into 2 types: subtractive manufacturing (SM) and additive manufacturing (AM). SM, such as milling is based on cutting away from a solid block of materal. AM, such as 3D printing, is based on adding the material layer by layer. AM enables the fabrication of complex structures that are difficult to mill. In this case, additive manufacturing method was applied to the fabrication of the resin-based complete denture to a 80 year-old patient. During the follow-up periods, the denture using digital method has provided satisfactory results esthetically and functionally.

4.Comparison of internal adaptation of removable partial denture metal frameworks made by lost wax technique and printing technique of pattern using CAD

Lost wax technique of casting wax patterns has been used principally to make metal frameworks of removable partial dentures. Since the development of digital technology and CAD, metal frameworks can be built through digital surveying and framework designing. Many cases proved that resin patterns made by CAD printing method were well adapted to internal oral structure and final metal frameworks also showed good internal adaptation as well. The metal frameworks of a removable partial denture were made by both lost wax technique and CAD technique and were applied to a patient with severe alveolar bone loss. Using CAD data and fit checker, internal adaptation of both metal frameworks were evaluated by comparing the gap between surveyed crown and its structure. This study is to prove that metal frameworks by both techniques showed adaptation that can be applied in clinical field.

5.A case of removable dentures using digital method.

Generally dentures are manufactured by conventional method, however the frequency of fabricating denture using digital method is increasing due to the recent development of digital technology in dentistry. The digital method of manufacturing denture is classified into two systems; 3D scan of the impression to arrange the artificial teeth on the CAD (Computer-aided design) and 3D printing to produce the resin-based complete denture, or 3D scan of the model to design of the framework using CAD, resin pattern formation by 3D printing and casting of metal framework of complete denture or removable partial denture. In this case report, electronic surveying and design the metal framework of the dentures were performed using CAD program, and plastic resin patterns fabricated by 3D printing were casted for upper full denture and lower removable partial denture. During follow-up periods, dentures using digital method have provided satisfactory results esthetically and functionally.

6.RPD framework fabrication using computer-aided design (CAD) and rapid prototyping.

Nowadays, digital dentistry is generally applied to prosthodontics with fabrication of inlays or any other fixed prostheses by utilizing CAD/CAM (computer-aided design/computer-aided manufacturing) technology and intraoral scanner. However, in fabricating removable prosthesis, there are some limitations for digital technology to substitute conventional casting method. Therefore, approaching removable prostheses fabrication with CAD/CAM technology would be a meaningful trial. In this case report, Kennedy class III mandibular edentulous patient who was in need of increasing the vertical dimension of occlusion was treated with removable partial denture using CAD and rapid prototyping technique. Surveying and designing the metal framework of the partial denture was performed with CAD, and sacrificial plastic pattern was fabricated with rapid prototyping technique. During the follow up period of nine months, the removable partial denture has provided satisfactory results in esthetics and function.

7.Three-dimensional displacement of implant-supported cantilever fixed partial denture under oblique loading.

The purpose of this study was to research the three-dimensional displacements of implant-supported cantilever fixed partial denture (CFPD) under oblique loading. One Beagle dog was used in this experiment. Two immediate implants of ITI were inserted in the mandible of the dog, and the implant-supported CFPD which used the implants as abutments was made in vitro fresh mandible. Then the digital laser speckle technique was employed to measure the three-dimensional displacements of CFPD under different oblique loading. We found that when an oblique loading was exerted on the pontic, the displacement increased with increasing of load. Under equal loading, the displacement of the abutment near to the pontic was smaller than that of the pontic but greater than that of the abut-ment far from the pontic. When oblique loading was exerted on the abutment, the displacement of the direct loaded abutment was greater than that of the other abutment and the pontic. Under the.eeual loading, the displacement of implant-supported CFPD of loading on pontic was greater than that of loading on abutments. The experiments demonstrated that implant-supported cantilever fixed partial denture (CFPD) is an effective and advisable therapy for totally? or partially edentulous patients. However, it is also suggested that the clinicians should avoid exerting oblique loading, especially the obliqe loading of the pontic when th e CPDF is used.

8.Study on the digital laser speckle technique of three-dimensional displacement of cantilever fixed partial denture under oblique loading.

The purpose of this study was to investigate the three-dimensional displacement of the cantilever fixed partial denture (CFPD) under an oblique loading. One Beagle dog was used in this experiment. The CFPD, which used the second premolar and the first molar as abutments and the second molar as pontic, was made in vitro fresh mandible of the Beagle dog, and the digital laser speckle (DLS) technique was employed to measure the three-dimensional displacement of the CFPD under the oblique loading for the first time. We found that when an oblique loading was exerted on the pontic, the displacement of CFPD was the greatest, and the displacement of the abutment near the pontic was smaller than that of the pontic but greater than that of the abutment far from the pontic. We also found that when an oblique loading was exerted on the abutment, the displacement of the directly loaded abutment was greater than that of the other abutment and the pontic. Under the oblique loading, the displacement increased with increasing of load. The experiment demonstrated that it would be advisable for the clinicians to avoid oblique loading, especially the oblique loading of the pontic when using the CPDF. The DLS technique may be one kind of methods for measuring the three-dimensional displacements of the small and irregular objects.

9.Accuracy evaluation of a new three-dimensional reproduction method of edentulous dental casts, and wax occlusion rims with jaw relation.

The article introduces a new method for three-dimensional reproduction of edentulous dental casts, and wax occlusion rims with jaw relation by using a commercial high-speed line laser scanner and reverse engineering software and evaluates the method's accuracy in vitro. The method comprises three main steps: (i) acquisition of the three-dimensional stereolithography data of maxillary and mandibular edentulous dental casts and wax occlusion rims; (ii) acquisition of the three-dimensional stereolithography data of jaw relations; and (iii) registration of these data with the reverse engineering software and completing reconstruction. To evaluate the accuracy of this method, dental casts and wax occlusion rims of 10 edentulous patients were used. The lengths of eight lines between common anatomic landmarks were measured directly on the casts and occlusion rims by using a vernier caliper and on the three-dimensional computerized images by using the software measurement tool. The direct data were considered as the true values. The paired-samples t-test was used for statistical analysis. The mean differences between the direct and the computerized measurements were mostly less than 0.04 mm and were not significant (P>0.05). Statistical significance among 10 patients was assessed using one-way analysis of variance (P<0.05). The result showed that the 10 patients were considered statistically no significant. Therefore, accurate three-dimensional reproduction of the edentulous dental casts, wax occlusion rims, and jaw relations was achieved. The proposed method enables the visualization of occlusion from different views and would help to meet the demand for the computer-aided design of removable complete dentures.

10.Inflatable hollow obturator prostheses for patients undergoing an extensive maxillectomy: a case report.

The presence of a large palatal or maxillary defect after partial or total maxillectomy for tumor, trauma or congenital deformation poses a challenge to prosthodontists, particularly when the use of an implant cannot be considered. This case report described the use of an air valve in a hollow silicone obturator to manufacture an inflatable obturator that could be extended further into undercut area to retain itself. The inflatable obturator exhibited adequate retention, stability and border sealing, thereby improving the masticatory,pronunciation and swallowing functions of patients. It may be a suitable alternative treatment option to an implant-retained obturator.
