Objective::This study aimed to evaluate the major adverse cardiovascular and cerebrovascular events (MACCEs) and overall safety profile associated with iodixanol in Chinese patients undergoing percutaneous coronary intervention (PCI).Methods::Patients at 30 centers in China registered in the OpenClinic v3.6 database from October 30, 2013, to October 7, 2015, were included in the study. The primary endpoint was in-hospital MACCEs including target lesion revascularization (TLR), stroke, stent thrombosis, cardiac death, and PCI-related myocardial infarction (MI) within 72 h post-PCI. Secondary endpoints were MACCEs from 72 h to 30 d post-PCI and other safety events within 30 d post-PCI.Results::A total of 3,042 patients were enrolled. The incidence of MACCEs within 72 h post-PCI was 2.33% ( n = 71), including cardiac death (0.03%, n = 1) and PCI-related MI (2.30%, n = 70). The incidence of MACCEs from 72 h to 30 d post-PCI was 0.16% ( n = 5), including cardiac death (0.10%, n = 3), PCI-related MI (0.03%, n = 1), and TLR for stent thrombosis (0.03%, n = 1). The incidence of composite angiographic or procedural complications was 2.86% ( n = 87); 233 (7.86%) patients had results suggesting contrast-induced acute kidney injury. Conclusions::These findings indicate that the use of iodixanol in Chinese patients undergoing PCI is associated with a low incidence of MACCEs, confirming its safety in this population.

Objective:To explore the effects of hyperbaric oxygen(HBO) combined with radiochemotherapy on the immune function and prognosis of patients with breast cancer.Methods:A total of 144 breast cancer patients admitted in the Department of Breast Surgery, Shanxi Provincial Cancer Hospital from June 2017 to December 2018 were selected as the research objects and randomly divided into control group ( n=72) and observation group ( n=72) according to random number table method. The control group was given the conventional radiochemotherapy treatment, and the observation group was treated with the hyperbaric oxygen (HBO) therapy on the basis of the treatment in the control group. The changes of the immunoglobulin (IgG and IgM) and their complements (C3 and C4), CD3 + , CD4 + , and CD8 + T lymphocytes of the two groups were compared before and after the treatment. The adverse reactions, complications, tumor recurrence and metastasis of the patients in the two groups were compared in a 1-year follow-up. Results:After 6 courses of the treatment, the immunoglobulin (IgG and IgM) and their complements (C3 and C4) levels of the patients in the observation group were significantly higher than those in the control group. The difference was statistically significant ( P<0.05). CD4 + T lymphocyte count was also obviously higher than that of the control group ( P<0.05). One-year follow-up showed that the bone marrow toxicity, gastrointestinal reaction, and hair loss of the patients in the observation group were significantly better than those in the control group, and the incidences of breast cancer recurrence and metastasis were significantly lower than those in the control group with statistically significant difference ( P<0.05 or P<0.01). Conclusion:HBO therapy, as an assisting treatment, can effectively enhance the immunity of breast cancer patients after radiochemotherapy, reduce the occurrence of adverse reactions, and delay tumor recurrence and metastasis.

OBJECTIVE： To investigate the effects of Danhong injection （DHI） on gene expression profile of acute myocardial infarction （AMI） model rats. METHODS： Male SD rats were randomly divided into sham operation group， model group and DHI group （0.76 mL/kg）， with 10 rats in each group. AMI model was established by ligation of left anterior descending coronary artery in model group and DHI group. After modeling， sham operation group and model group were given constant volume of normal saline intramuscularly， and DHI group was given relevant medicine intramuscularly， once a day， for consecutive 14 days. After last administration， myocardial tissue in the marginal zone of infarction was separated. The change of gene expression profile was detected by gene chip technique. Using fold-change of relative expression as index， differentially expressed microRNA （miRNA） were screened. On the basis of retrieving their corresponding genes， gene ontology （GO） and KEGG pathway enrichment analysis were carried out by using DAVID bioinformatics resource database and KEGG pathway database， respectively. TargetScan database was used to predict the target gene messenger RNA （mRNA） corresponding to differentially expressed miRNA. Cytoscape 3.6.1 software was used to construct and analyze the miRNA-mRNA network. Agilent GeneSpring GX v11.5 software was used to screen target genes and miRNA related to inflammation in the above networks. RESULTS： Compared with sham operation group， there were 22 differentially expressed miRNAs in model group， 5 up-regulated and 17 down-regulated. Compared with model group， there were 26 differentially expressed miRNAs in DHI group， and all of them were up-regulated. The differentially expressed miRNAs related to DHI therapy for AMI included rno-let-7a-5p， rno-let-7d-5p， rno-let-7f-5p， rno-miR-26b-5p， rno-miR-29b-3p， cel-miR-39-3p， cel-miR-39-5p， rno-miR-142-5p， rno-miR-191a-5p， rno-miR-409a-3p. Results of GO analysis and KEGG pathway enrichment analysis showed that differentially expressed miRNAs were mainly concentrated in membrane-bound organelles， cytoplasm， endometrial system and other cell components. The molecular functions such as protein binding and ion binding were exerted through biological processes such as anatomical structure development， multicellular tissue development and development process，which were mainly enriched in calcium signaling pathway， PPAR signaling pathway， VEGF signaling pathway， cell apoptosis， glycosylphosphatidylinositol anchored biosynthesis， valine， leucine and isoleucine degradation， etc. miRNA-mRNA network analysis showed that there were 25 target gene mRNAs corresponding to differentially expressed miRNA and 24 miRNAs related to it. There were 6 inflammation-related target genes （IL6， IL1b， TNF， TLR4， CRP， CXCL12） in this network， involving 19 differentially expressed miRNAs. CONCLUSIONS： The therapeutic effect of DHI on AMI may be related to regulating the expression of related miRNA， affecting signal transduction of calcium ion， PPAR and VEGF pathways， and regulating the secretion of inflammatory markers such as interleukin， chemokine and C-reactive protein.

Delayed gastric emptying(DGE)is a common complication after pancreatoduodenectomy. Due to its complex causes and difficul-ties in treatment,it has been a difficult problem in general surgery. By analyzing the improvement in the surgical procedure of pancreaticodu-odenectomy in recent years and the influence of the establishment of predictive scoring model on DGE,this article puts forward new direc-tions for the prevention and treatment of DGE. It is pointed out that effective preoperative assessment and reasonable selection of surgical pro-cedures may help to reduce the incidence rate of DGE after surgery.

Objective:To invest the risk factors associated with delayed hemorrhage after pancreaticoduodenectomy occurred.Methods:A retrospective analysis was performed from January 2012 to June 2015 Shengjing hospital 511 cases of patients who accepted pancreaticoduodenectomy.Results:The incidence of PD postoperative delayed hemorrhage was 8.81％ (45/511).Univariate analysis shows:Preoperative bilirubin levels,pancreatic fistula,biliary fistula,postoperative abdominal infection are all the meaningful related factors(P＜0.05).With the application of multivariate logistic regression analysis,pancreatic fistula (OR=2.158) and abdominal infection (OR=3.051) were independent risk factors for postoperative delayed hemorrhages (P＜0.05).Conclusion:Dealing with the postoperative complications of PD like pancreatic fistula,abdominal infection and others,early detection and rapid diagnosis and correct treatment can effectively reduce the incidence of delayed hemorrhage.

Objective To explore the protective mechanism of HSP70 protein in traumatic brain injury (TBI)‐related acute gastric mucosal lesions in mice .Methods Forty adult male Balb/c mice were randomly divided into sham (A) ,TBI (B) ,TBI+ geranylgeranylacetone (GGA) (C) ,and TBI+saline (D) groups .TBI was induced via the Feeney impact model .GGA (800 mg/kg) was administered via oral tube beginning before the model was built in group C .The expressions of HSP70 protein in brain and gastric mucosa were determined by immunohistochemistry , and the apoptotic index was detected by TUNEL method .Results The injury area in mouse brain and gastric mucosa was greater in group B than in groups A and C (P<0 .05) .After model induction ,the content of HSP70 protein in group B was markedly higher in the brain and gastric mucosa ,which was notably higher than in group A (P<0 .05) .Obviously apoptotic cells were observed in groups B and D ,which were significantly higher than in groups A and C .GGA pretreatment enhanced the up‐regulated expression of HSP70 and decreased the apoptotic index distinctly ;HSP70 expression was higher in group C than in groups B and D ,but the apoptotic index was lower (P<0 .05) .Conclusion GGA can induce HSP70 protein expression in mouse brain and gastric mucosa .HSP70 is involved in the process of apoptosis inhibition .GGA can be used in the prevention and therapy of TBI‐related acute gastic mucosal lesions .

OBJECTIVE:To analyze the characteristics and rules of new and serious ADR induced by Chinese patent medicine in Wuhan during 2012-2013,and to improve the monitoring levels of Chinese patent medicine-induced ADR and clinical rational drug use. METHODS:New and serious Chinese patent medicine-induced ADR cases reported by 16 districts of Wuhan during 2012-2013 were classified and analyzed statistically. RESULTS:A total of 245 cases of new and serious TCM ADR were reported in 2012-2013,accounting for 13.61% of all reports;the incidence of ADR in patients above 51 years old was the highest,account-ing for 55.51%;42.04%of new and serious ADR induced by Chinese patent medicine occurred within 30 minutes after using medi-cine;among suspected drugs,intravenous dripping was the main way to cause new and serious ADR induced by Chinese patent medicine (50.39%);blood-regulating formula was the main cause of new and serious ADR induced by Chinese patent medicine (40.80%);new and serious ADR induced by Chinese patent medicine mainly manifested as systemic damage(25.97%),followed by lesion of skin and its appendants(18.81%). CONCLUSIONS:It is needed to strengthen the rational use of Chinese patent medi-cine and the supervision of TCM injection for activating blood circulation to dissipate blood stasis,strengthen the supervision and improvement of instruction content of Chinese patent medicine. TCM Pharmacists should carry out the clinical pharmaceutical care.
