1.Effect of finite element method in treatment of developmental dysplasia of the hip in children

BACKGROUND:Developmental dysplasia of the hip often leads to limb deformities in children,and the research related to its diagnosis and treatment has been gradually clarified.Recently,the finite element method has been paid attention to by scholars in the research related to developmental dysplasia of the hip because of its advantages. OBJECTIVE:Through literature search and review of the relevant research progress of finite element method in children's developmental dysplasia of the hip and treatment,analyze and summarize its advantages and disadvantages,and explore the direction of further research in the future. METHODS:PubMed,SCI,CBM,and CNKI were searched for relevant articles published from January 2014 to November 2023 with the key words of"developmental dysplasia(dislocation)of the hip,dysplasia of the hip,finite element analysis(method),pavlik harness,fixation in herringbone position,biomechanics,pelvic osteotomies,pemberton,salter,dega,periacetabular osteotomy,children"in Chinese and English.A small number of long-term articles were included,and 62 articles were finally included for analysis through screening. RESULTS AND CONCLUSION:(1)The mechanical environment of hip joint in children with developmental dysplasia of the hip was abnormal.The pressure in acetabulum was uneven.The stress increased and concentrated;the joint contact area decreased,and the local stress concentrated in femoral neck.(2)In the Pavlik sling and herringbone fixation,the mechanical environment of the hip was improved;the concentrated high stress area disappeared and the joint contact area increased,but the excessive abduction angle led to the increase of stress in the acetabulum and the lateral femoral head.(3)After pelvic osteotomy,the stress environment of hip joint and sacroiliac joint was improved.There was no single hinge in the three kinds of osteotomy,and the stress load position was different according to the age of the children.(4)After peri-acetabular osteotomy,the joint contact pressure was close to normal,but it was difficult to recover in patients with non-spherical femoral head.(5)The postoperative X-ray film findings could not show that the joint contact mechanics was the best.(6)It is indicated that the information that cannot be measured in the body can be obtained by using the finite element method,which can be operated in a virtual environment without the limitation of time and ethics.It can directly see the stress change area of normal and developmental dysplasia of the hip,explain the effectiveness of treatment from the point of view of mechanics,establish a specific finite element model and tailor-made operation plan for patients who need osteotomy.There is no standard or unified standard for the finite element modeling of developmental dysplasia of the hip and the material characteristic parameters of children's hip joint.Due to the inherent limitations of finite element method,it is impossible to analyze the model that contains bone,cartilage,ligament,muscle and other elements at the same time.The operation of finite element analysis is difficult,although it has advantages,it is not universal,and the current research sample size is small,which needs to be further expanded and verified.
2.Human immunodeficiency virus-associated Hodgkin lymphoma: a clinical analysis of 22 cases

Objective:To investigate the clinical characteristics, therapeutic efficacy and prognosis of patients with human immunodeficiency virus (HIV)-associated Hodgkin lymphoma.Methods:A retrospective case series study was conducted. The clinical data of 22 HIV-associated Hodgkin lymphoma patients in Chongqing University Cancer Hospital from December 2013 to June 2022 were retrospectively analyzed. Their clinical features, laboratory results, treatment, and prognosis were analyzed. Kaplan-Meier method was used to perform survival analysis.Results:The age [ M ( Q1, Q3)] of 22 patients was 44 years old (36 years old, 53 years old); 18 cases were male, 4 cases were female; clinical staging was stage Ⅲ in 5 patients and stage Ⅱ in 17 patients. All 22 patients were infected with HIV through sexual transmission, with 10 cases transmitted through man sex with man and 12 cases transmitted through heterosexual transmission. Nine patients were found to be infected with HIV at the time of diagnosis of lymphoma, and 13 patients presented with lymphoma at 22.2 months (12.3 months, 38.4 months) after diagnosis of HIV infection. Of the 22 patients, 3 abandoned treatment; 19 patients were treated with antiretroviral therapy combined with ABVD regimen chemotherapy, 9 patients had complete remission, and 10 patients had partial remission. After follow-up of 46.8 months (24.8 months, 64.5 months), the 5-year progression-free survival rate was 83.9%, and the 5-year overall survival rate was 89.5%. Conclusions:HIV-associated Hodgkin lymphoma exhibits an invasive process in clinical practice, and standardized antiretroviral therapy combined with ABVD regimen chemotherapy can lead to long-term survival for patients.
3.A method for radiation dose assessment of β-rays and γ-rays in mixed β-γ fields

Objective:To test a new method with thermoluminescent dosimeters (TLDs) to determine the β-ray and γ-ray doses of β-γ mixed radiation fields.Methods:TLDs for personal dose monitoring were irradiated in the reference radiation fields of β-rays ( 90Sr/ 90Y, 85Kr) and γ-rays ( 137Cs). Across the range of 2.0-15.0 mSv, the linearity of TLD response and normalized response with respect to 137Cs were determined at the depths of Hp(10) and Hp(0.07). Using TLD detector readings at the depths of Hp(10) and Hp(0.07), β- and γ-ray doses in the mixed radiation fields were determined, and the result were verified. Results:For Hp(10) and Hp(0.07) under γ-ray exposure and Hp(0.07) under β-ray exposure, the coefficient of determination ( R2) were all >0.998. For the 90Sr/ 90Y source and 85Kr source, the average values of response values normalized with respect to 137Cs at different doses were 1.14 and 0.18, respectively; and the normalized response values derived from the slope values of the dose-response curves for the two sources were 1.17 and 0.18, respectively. The ratios of measurements of Hp(10) to Hp(0.07), kR, for the 85Kr source were close to 0, while the kR values for the 137Cs source were close to 1. Using the average value of kR and the slope value of kR for calculation, the maximum relative deviations between the calculated values and conventional values for Hp(10) γ, Hp(0.07) γ, and Hp(0.07) β were 6.1% and 6.0%, respectively. Conclusions:This method can be applied for the assessment of β-ray and γ-ray doses in β-γ mixed radiation fields of a single β source and single γ source.
4.Research advances in the minimally invasive treatment of benign prostatic obstruction

The number of benign prostatic obstruction (BPO) patients in China is increasing, and patients tend to be younger and younger. The former "gold standard" scheme of transurethral resection of the prostate (TURP) is more suitable for patients with prostate volume ranging from 40 mL to 80 mL, which may lead to excessive resection in patients with small prostate volume and low efficiency in patients with large prostate volume. New minimally invasive techniques have been introduced,including prostate artery embolization, laser surgery (such as holmium, green, diode, and thulium), minimally invasive simple prostatectomy, transperineal laser ablation, prostatic urethral lift,and robot-assisted water jet ablation of the prostate. These methods are alternatives to TURP and increasingly used in the treatment of BPO. This article reviewed the advances in minimally invasive treatment of BPO.
5.Advances in the study of the m6A methyltransferase METTL3 promoting bladder cancer progression via AFF4/NF-κB/MYC signaling network

Bladder cancer (BCa) is one of the most common cancers in urology,whose pathogenesis is still unclear. Methyltransferase-like 3(METTL3) is the most important part of N6-methyladenosine methyltransferase complex (m6A MTC),which mediates the methylation of mRNA to regulate the stability and translation process of mRNA. Researches have shown that METTL3 can promote BCa development via AFF4/NF-κB/MYC signaling network,which involves many kinds of signaling molecules. In addition,METTL3 can affect the expressions of AFF4,NF-κB and MYC,so as to affect their downstream signaling pathways and finally promote the malignant progression of tumor.
6.Research progress of urethral sling in the treatment of male urinary incontinence after prostatectomy

Stress urinary incontinence (SUI) following prostatectomy is an important and common problem in male, which can significantly reduce the quality of life. Urethral sling is a promising treatment for male SUI. It is easy to operate and can avoid the insertion of prosthesis, making it highly acceptable to patients. In recent years, with the advancement of technology, several kinds of slings have been put into clinical application and have achieved great outcomes. This article reviews the application of urethral sling in the treatment of SUI after prostatectomy.
7.Research advances in the clinical diagnosis and treatment of pheochromocytoma and paranganglioma

8.A food poisoning incident suspected to be caused by proteus in a city of Dehong Prefecture,Yunnan Province

Objective To Investigate the cause of a food poisoning incident in a city in Dehong Prefecture, determine the scope of the incident, and to formulate effective prevention and control measures. Methods Field epidemiology and case-control study methods were used to formulate case definition; carry out case search; find suspicious food combined with clinical manifestations, dining history and other information; and collect samples from the suspicious food, cases and environment for laboratory testing. Results A total of 160 cases were found and the incidence rate was 26.02% (160/615). The main clinical manifestations were diarrhea, leukocytosis, vomiting, nausea and abdominal pain. The average incubation period was 10.89±5.09 h, with 2 h as the shortest and 28 h as the longest. The epidemiological curve suggested the point source exposure mode. 9.Research advances of urinary cell-free DNA in the detection and therapy of bladder cancer

Bladder cancer is a common malignancy in the genitourinary system and the current therapeutic approaches are unsatisfactory. Urinary cell-free DNA (ucf DNA) has the ability to give comprehensive and crucial information on cancer as it carries genetic messages from cells shedding directly into urine as well as transporting from circulation. The ucf DNA of patients with bladder cancer carries disease information, suggesting that ucf DNA may have the ability to detect, monitor, and prognosticate patients with bladder cancer. The ucf DNA analysis bridges the gap between current techniques and enhances diagnostic and detection capabilities, and has a very promising future in term of translation into clinical practice. This article reviewed the progress of clinical applications of ucf DNA in bladder cancer.
10.The review of inspection methods of X-ray lead protective clothing

The quality evaluation of X-ray protective clothing at home and abroad is mainly intended for the quality inspection of lead protective clothing. There is no unified standard to be developed for the test of lead protective clothing during use. When lead protective clothing with defective quality is used many times, this phenomenon will pose a greater safety hazard to radiation workers and patients. Therefore, in order to protect the health of radiation workers and patients, the implementation of regular testing and quality evaluation of lead protective clothing and the development of a standardized management system are important. This review introduces the domestic and international inspection method of X-ray lead protective clothing, summarizes the inspection and evaluation method in use, including shield transmission coefficient, defect detection and uniformity test, puts forward relevant suggestions to provide reference for the managers and practitioners of relevant institutions.
