1.Evaluation of Leydig cell activity using single-dose hCG stimulation in prepubertal children suspected of hypogonadism: experience from a tertiary institution

Purpose:
Multiple protocols exist for the dosing and duration of human chorionic gonadotropin (hCG) and the sampling schedule of the hCG stimulation test. This study analyzes the testosterone response following a single-dose hCG test.
Methods:
This observational study analyzes 103 prepubertal undervirilized males who underwent a single-dose hCG test. A dosage of 1,500 IU was used for those under 2 years of age and 5,000 IU for those over. Testosterone levels were measured before and 72 hours after the hCG injection, and the fold of increase was analyzed. As part of the unit protocol, those with a suboptimal response to a single-dose test (n=19) underwent a 3-day hCG test.
Results:
A significant 23.65-fold increment of testosterone, with a poststimulated value of 167.26 (interquartile range [IQR], 62.30–279.15) ng/dL, was observed following a single dose of hCG. Of the 103 subjects, 19 (18.4%) had a subnormal response with testosterone levels of 8.20 (IQR, 3.48–29.70) ng/dL. A 3-day test on these 19 subjects showed a testosterone level of 18.4 (IQR, 10.6–64.2) ng/dL, which is statistically significant. However, the 3-day hCG test revealed an adequate response in only 3 patients. The remaining 16 did not achieve the expected outcome, and 15 of these patients had laboratory evidence of hypogonadism either genetically or biochemically.
Conclusion
A single-dose hCG stimulation test could serve as an alternative to a 3-day hCG test in the initial assessment of Leydig cell function, thereby avoiding repeated injections, hospital visits, and school absenteeism.

This narrative review synthesizes the current literature to comprehensively compare these modalities in terms of accessibility, monitoring mechanisms, patient adherence, cost-effectiveness, and social support. Telerehabilitation offers unparalleled convenience, real-time monitoring, and personalized feedback through wearables, thereby fostering greater patient engagement and adherence compared to conventional rehabilitation. However, conventional rehabilitation provides face-to-face interactions, immediate feedback, and a more personalized touch, albeit with logistical challenges and higher costs. This review emphasizes the significance of patient preferences, technological access, and healthcare infrastructure in selecting the appropriate approach. It also calls for further research into long-term outcomes, cost-effectiveness, and the optimal integration of wearable technology in diabetes management programs. Ultimately, both telerehabilitation and conventional rehabilitation demonstrate considerable potential in empowering individuals with T2DM, underlining the imperative for tailored and patient-centric interventions in diabetes care. The review also stresses the significance of integrating patient preferences and their level of comfort with technology when deciding on treatment approaches. It also takes into account the diverse socioeconomic contexts and healthcare infrastructures globally, which can affect the viability and efficacy of both telerehabilitation and conventional rehabilitation. Moreover, the integration of wearable technology in diabetes management programs holds promise for enhancing self-management capabilities and promoting healthier lifestyles. 5.Tubular Hybrid Surgery: A Varying Paradigm

In general, the scientific literature recommends extended fusion of the lumbar decompressed level adjacent to a fused level. Tubular decompression of lumbar canal stenosis defies this concept as it minimizes collateral damage. This study explores the concept of tubular hybrid surgery, where tubular decompression is performed adjacent to a fused level. A descriptive study of the technique is presented with a few case examples. The concept of tubular hybrid surgery wherein tubular minimally invasive spinal fusion is performed at one level and microscopic tubular decompression is executed at the adjacent levels is discussed, along with clinical and biomechanical evidence and case examples. The tubular hybrid method represents a significant advancement in minimally invasive spine surgery. Larger long-term, multicentric, prospective studies are necessary to establish the benefits of the procedure.
6.A New Trajectory of the Percutaneous S1 Pedicle Screw: A Prospective Analysis With 2–4 Years of Follow-up

Purpose:
S1 pedicle screw loosening remains a significant challenge in lumbosacral fixation due to the unique anatomical and biomechanical properties of the sacrum. Traditional trajectories often face limitations, including poor screw hold and an elevated risk of complications. This study introduces the Vishal Peshattiwar (VP) trajectory for S1 pedicle screw placement, aiming to optimize screw positioning, improve construct stability, and reduce associated complications.
Methods:
A prospective analysis was conducted on 117 patients undergoing L5–S1 minimally invasive transforaminal lumbar interbody fusion (MIS-TLIF) and multilevel MIS-TLIF including the L5–S1 level between June 2019 and August 2022. In total, 234 S1 pedicle screws were placed using the VP trajectory under O-arm-guided 3-dimensional computed tomography navigation. Screw parameters, including length, entry point, and angulation, were analyzed. Finite element analysis (FEA) was performed using Altair OptiStruct software to assess displacement forces and elemental stresses.
Results:
The average screw length was 45.90 mm in men and 44.88 mm in women. The caudocranial angle ranged from 50° to 65°, and the lateromedial angle from 18° to 25°. FEA demonstrated reduced displacement forces and elemental stresses, indicating improved construct stability. At a 2- to 4-year follow-up, complications were minimal, with 1 case of bilateral screw breakage and 1 dural tear.
Conclusion
The VP trajectory provides an improved approach for S1 screw placement, offering enhanced biomechanical stability, reduced complications, and better long-term outcomes. Its application is most effective with minimally invasive surgical techniques and navigation assistance.

Hemophilia A (HA) is an X-linked inherited bleeding disorder caused by reduced factor VIII (FVIII) levels.Approximately 10–15% of patients with severe HA (SHA) do not present with the anticipated bleeding pattern. Here, we assessed the phenotypic severity of hemophilia A using rotational thromboelastometry (ROTEM) and activated partial thromboplastin time-clot waveform analysis (APTT-CWA).
Methods:
Patients diagnosed with hemophilia A were enrolled. Clinical phenotype assignment was performed according to the published literature, and patients were classified into four phenotypic subgroups. The whole blood sample was first run on ROTEM in INTEM mode using platelet-poor plasma, APTT was run, and the APTT-CWA graph was simultaneously recorded.
Results:
A total of 66 patients were recruited for this study. Statistically significant differences were observed between the four phenotypically categorized groups using ROTEM and APTT-CWA. On comparing patients with mild/ moderate-to-severe phenotypes (Group II) with SHA without inhibitors (Group IV), no significant difference was found for all parameters of ROTEM or APTT-CWA. The MCF, MA30, MAXV, and Alpha angle values using ROTEM were found to be the lowest in patients with SHA with inhibitors, which helped differentiate them from those with SHA without inhibitors. However, these two groups could not be differentiated using the APTT-CWA parameters.
Conclusion
ROTEM can be used to distinguish patients with SHA with inhibitors from those with SHA without inhibitors using a combination of parameters with high sensitivity and specificity. However, APTT-CWA cannot be used to differentiate these patient groups.
