2.Herbal Textual Research on Gentianae Macrophyllae Radix in Famous Classical Formulas

In this paper, through consulting relevant records in materia medica, medical and prescription books, and combining with modern literature, the name, origin, producing area, collection and processing of Gentianae Macrophyllae Radix in famous classical formulas from The Catalogue of Ancient Famous Classical Formulas (The First Batch) was systematically sorted out and textual research was carried out, in order to provide a basis for the development of the famous classical formulas containing Gentianae macrophyllae Radix. After textual research, it was found that Gentianae Macrophyllae Radix was the rectification of name in the past dynasties. In addition, there were other names such as Qinjiao, Qingua and Qinzhua. Gentiana macrophylla, G. straminea, G. dahurica and G. siphonantha were the main origin of this herb in ancient literature. Among them, G. macrophylla is the mainstream. In the Southern and Northern dynasties, G. straminea and G. macrophylla produced in northern Sichuan were recommended as the best. In the early Tang dynasty, G. macrophylla from the Liupan Mountain area at the border of Shanxi and Gansu provinces was the mainstream. During the Northern Song dynasty, G. siphonantha from Linxia and Qilian Mountain of Gansu province and G. macrophylla from eastern Shaanxi province were two new producing areas. In the Ming and Qing dynasties, the abundant base and production areas of Gentianae Macrophyllae Radix were gradually formed. In the past dynasties, harvesting was carried out in spring and autumn, and stored mainly by aeration drying or shade drying treatment. The processing methods are mainly the raw products after the net selection, cutting and drying, in addition to the frying, processing with wine and milk. G. macrophylla is recommended as the first choice for the herbal medicine involved in the famous classical formulas. Among them, wild products produced in Gansu and Shaanxi are the best, and raw products are recommended to be used. At the same time, it is suggested that G. siphonantha should be added to the subsequent edition of Chinese Pharmacopoeia as one of origins of Gentianae Macrophyllae Radix.
3.A simple and easily implemented risk model to predict 1-year ischemic stroke and systemic embolism in Chinese patients with atrial fibrillation

Background::Accurate prediction of ischemic stroke is required for deciding anticoagulation use in patients with atrial fibrillation (AF). Even though only 6% to 8% of AF patients die from stroke, about 90% are indicated for anticoagulants according to the current AF management guidelines. Therefore, we aimed to develop an accurate and easy-to-use new risk model for 1-year thromboembolic events (TEs) in Chinese AF patients.Methods::From the prospective China Atrial Fibrillation Registry cohort study, we identified 6601 AF patients who were not treated with anticoagulation or ablation at baseline. We selected the most important variables by the extreme gradient boosting (XGBoost) algorithm and developed a simplified risk model for predicting 1-year TEs. The novel risk score was internally validated using bootstrapping with 1000 replicates and compared with the CHA 2DS 2-VA score (excluding female sex from the CHA 2DS 2-VASc score). Results::Up to the follow-up of 1 year, 163 TEs (ischemic stroke or systemic embolism) occurred. Using the XGBoost algorithm, we selected the three most important variables (congestive heart failure or left ventricular dysfunction, age, and prior stroke, abbreviated as CAS model) to predict 1-year TE risk. We trained a multivariate Cox regression model and assigned point scores proportional to model coefficients. The CAS scheme classified 30.8% (2033/6601) of the patients as low risk for TE (CAS score = 0), with a corresponding 1-year TE risk of 0.81% (95% confidence interval [CI]: 0.41%-1.19%). In our cohort, the C-statistic of CAS model was 0.69 (95% CI: 0.65-0.73), higher than that of CHA 2DS 2-VA score (0.66, 95% CI: 0.62-0.70, Z = 2.01, P = 0.045). The overall net reclassification improvement from CHA 2DS 2-VA categories (low = 0/high ≥1) to CAS categories (low = 0/high ≥1) was 12.2% (95% CI: 8.7%-15.7%). Conclusion::In Chinese AF patients, a novel and simple CAS risk model better predicted 1-year TEs than the widely-used CHA 2DS 2- VA risk score and identified a large proportion of patients with low risk of TEs, which could potentially improve anticoagulation decision-making. The research studies the effect of different fertilization treatments on yield and accumulation of secondary metabolites of Codonopsis pilosula by using single factor randomized block design, in order to ensure reasonable harvesting time and fertilization ratio, and provide the basis for standardized cultivation of C. pilosula. According to the clustering results, the nitrogen fertilizer benefitted for the improvement of root diameter and biomass of C. pilosula. The phosphate fertilizer could promote the content of C. pilosula polysaccharide. The organic fertilizers could increase the content of lobetyolin. With the time going on, C. pilosula's yield, polysaccharide and ehanol-soluble extracts increased while the content of lobetyolin decreased. According to various factors, October is a more reasonable harvest period. Organic fertilizers are more helpful to the yield and accumulation of secondary metabolites of C. pilosula.

BACKGROUNDThis study aimed to evaluate the effects of standard rescue procedure (SRP) in improving severe trauma treatments in China.
METHODSThis study was conducted in 12 hospitals located in geographically and industrially different cities in China. A standard procedure on severe trauma rescue was established as a general rule for staff training and patient treatment. A regional network (system) efficiently integrating prehospital rescue, emergency room treatments, and hospital specialist treatments was built under the rule for information sharing and improving severe trauma treatments. Treatment outcomes were compared between before and 1 year after the implementation of the SRP.
RESULTSThe outcomes of a total of 74,615 and 12,051 trauma cases were collected from 12 hospitals before and after the implementation of the SRP. Implementation of the SRP led to efficient cooperation and information sharing of different treatment services. The emergency response time, prehospital transit time, emergency rescue time, consultation call time, and mortality rate of patients were 24.24 ± 4.32 min, 45.69 ± 3.89 min, 6.38 ± 1.05 min, 17.53 ± 0.72 min, and 33.82% ± 3.87% (n = 441), respectively, before the implementation of the standardization and significantly reduced to 10.11 ± 3.21 min, 22.39 ± 4.32 min, 3.26 ± 0.89 min, 3.45 ± 0.45 min, and 20.49% ± 3.11%, separately (n = 495, P < 0.05) after that.
CONCLUSIONSStaff training and SRP can significantly improve the efficiency of severe trauma treatments in China.

8.Incidence and predictors of radial artery spasm during transradial coronary angiography and intervention.

BACKGROUNDRadial artery spasm (RAS) is the most common complication in transradial coronary angiography and intervention. In this study, we designed to investigate the incidence of RAS during transradial procedures in Chinese, find out the independent predictors through multiple regression, and analyze the clinical effect of RAS during follow-up.
METHODSPatients arranged to receive transradial coronary angiography and intervention were consecutively enrolled. The incidence of RAS was recorded. Univariate analysis was performed to find out the influence factors of RAS, and logistic regression analysis was performed to find out the independent predictors of RAS. The patients were asked to return 1 month later for the assessment of the radial access.
RESULTSThe incidence of RAS was 7.8% (112/1427) in all the patients received transradial procedure. Univariate analysis indicates that young (P = 0.038), female (P = 0.026), small diameter of radial artery (P < 0.001), diabetes (P = 0.026), smoking (P = 0.019), moderate or severe pain during radial artery cannulation (P < 0.001), unsuccessful access at first attempt (P = 0.002), big sheath (P = 0.004), number of catheters (> 3) (P = 0.048), rapid baseline heart rate (P = 0.032) and long operation time (P = 0.021) were associated with RAS. Logistic regression showed that female (OR = 1.745, 95%CI: 1.148 - 3.846, P = 0.024), small radial artery diameter (OR = 4.028, 95%CI: 1.264 - 12.196, P = 0.008), diabetes (OR = 2.148, 95%CI: 1.579 - 7.458, P = 0.019) and unsuccessful access at first attempt (OR = 1.468, 95%CI: 1.212 - 2.591, P = 0.032) were independent predictors of RAS. Follow-up at (28 +/- 7) days after the procedure showed that, compared with non-spasm patients, the RAS patients had higher portion of pain (11.8% vs. 6.2%, P = 0.043). The occurrences of hematoma (7.3% vs. 5.6%, P = 0.518) and radial artery occlusion (3.6% vs. 2.6%, P = 0.534) were similar.
CONCLUSIONSThe incidence of RAS during transradial coronary procedure was 7.8%. Logistic regression analysis showed that female, small radial artery diameter, diabetes and unsuccessful access at first attempt were the independent predictors of RAS.

Objecttve To explore the value of electrocardiographic(ECG)Cornell criteria for detecting left ventricular hypertrophy(LVH)in elderly Chinese men.Methods Since 1990,244 autopsies were performed in our hospital in elderly men,LVH was determined in these autopsy hearts and correlated to EGG LVH signs recorded within 3 months before death according to Comer(Sv3+RIvL)and Sokolow-Lyon criteria(Svl+Rvs or RV6).The reference value of Comell criteria was obtained based on valUes from autopsied healthy hearts,the sensitivity and specificity of Cornell and Sokolow-Lyon criteria for detecting left ventricular hypertrophy in these eldedy men were talculated.Results There were significantly correlations between QRS amplitudes of Cornell and Sokolow-Lyon criteria and autopsy left ventricular wall thickness in tllese hearts.The reference value of Comer criteria(SV3+RaVL)was 2.9 mV.The sensitivity of Sokolow-Lyon and Cornell criteria for detecting LVH Wag 25.4%and 34.3%(P<0.05 vs Sokolow-Lyon criteria),respectively.Condusion Voltage(SV3+R.RavL)≥2.9 mV might be a suitable diagnostic value for detecting left ventricular hypertrophy in Chinese elderly men.
10.Effects of montelukast on atherosderosis and monocyte chemoattractant protein-1 expression in a hypercholesterolemic rabbit model

Objective To investigate the effects of montelukast on atherescleresis and monocyte chemoattractant protein-1 expression in a hyperchulesterolemic rabbit modeL Methods Thirty four male New Zealand white rabbits were randomized into four groups including normal control group (n=6), placebo in normal control group were fed a high cholesterol diet for 12 weeks. Serum lipids were measured at 0, 8 and 12 weeks after intervention. The intima/media ratio, percentages of macrophages or smooth muscle cells in intima and the expression of MCP-1 mRNA were examined. Results Atheresclerosis was evidenced in placebo group and atorvastatin or montelukast treatment significantly reduced neointima (0.32±0.12 and 0.34±0.10 vs. 1.12±0.36,P<0.05) and macrophage content [(9.8±4.6) % and(11.2±3.7) % vs. (34.6±8.8)%,P<0.05], increased SMC content [(18.6±6.9)% and(19.2±8.6)% vs. (5.2±2.3) %, P<0.05] and inhibited expression of MCP-1 mRNA (0.42±0.08 and 0.40±0.06 vs. 2.36±0.48 ,P<0.01). Montelukast had similar anti-atherogenetic effects as atorvastatin but had no influence on plasma lipids. Conclusions Montelukast could attenuate atherosclerosis in this hyperchulesterolemic rabbit model which might be attributed to its anti-inflammatory effects.
