1.Investigation and analysis of a brucellosis outbreak in Chongqing in 2021

Objective:To investigate a brucellosis outbreak caused by contact with unquarantined sheep in Chongqing, and provide reference for prevention and control of brucellosis.Methods:In accordance with the requirements of the "Technical Plan of Brucellosis Prevention and Control Project of Chongqing" and the "Working Specification for Epidemic Disposal of Human Brucellosis" (DB50/T 946-2019), a self-designed brucellosis case questionnaire (version 2021) was used to carry out case investigation and laboratory tests on cases reported by medical institutions, and the data were analyzed descriptively.Results:According to "Working Specification for Epidemic Disposal of Human Brucellosis" (DB50/T 946-2019) in Chongqing, the brucellosis outbreak that occurred in December 2021 was determined to be a cluster outbreak, with a total of two confirmed cases of brucellosis, and a incidence rate of 2/9. The reason of the outbreak was the rearing and slaughtering of unquarantined sheep.Conclusion:We should strengthen the inspection and quarantine of livestock such as cattle and sheep, crack down on informal trade of livestock, and reduce the risk of brucellosis.
2.Association between school bullying and the early age at menarche among girls

Objective:
To analyze the correlation between school bullying and the age at menarche (AAM) in girls, so as to provide references for promoting the healthy growth of girls in puberty.
Methods:
In April 2022, a total of 987 middle school girls with onset of menarche in Rongchang District of Chongqing were selected by using a stratified random cluster sampling method to carry out a questionnaire survey and physical examination. The t tests,variance analysis and Dunnett t tests were conducted to analyze the differences between individuals who experienced different types of school bullying and AAM. Multivariate Logistic regression analysis was conducted to analyze the association between school bullying and the early age at menarche.
Results:
The average AAM of 987 girls was (12.13±1.03) years, and 22.90% of them had early AAM. The AAM of those who did not experience bullying events (12.18±0.96) varied significantly with those who experienced bullying events (11.86±1.44) ( t=3.71, P <0.01). The average AAM of individuals who experienced 1, 2, 3 or more school bullying events was (12.08±1.38, 11.74±1.07, 11.61± 1.63 ) years old, respectively. There was a statistically significant difference in AAM between girls who did not experience school bullying and those who experienced 1, 2, 3 or more types of school bullying ( F=6.99, P <0.01). Multivariate Logistic regression analysis found that after adjusting confounding factors, experiencing school bullying ( OR=2.71, 95%CI =2.04-4.27), being deliberately excluded from collective activities or being isolated ( OR=2.58, 95%CI =1.69-4.67), being kicked, pushed or locked in the house ( OR= 2.85 , 95%CI =1.39-4.92), being teased due to physical defects or appearance ( OR=2.74, 95%CI =1.77-5.02), experiencing one school bullying event ( OR=2.33, 95%CI =1.52-4.23), and experiencing two school bullying events ( OR=3.36, 95%CI = 1.82 -7.36), and experiencing three or more school bullying events ( OR=2.89, 95%CI =1.74-5.71) were associated with the early age at menarche ( P <0.05).
Conclusion
School bullying is related to the earlier AAM among girls. Strengthening school anti bullying education might be helpful for promoting girls healthy growth and development in adolescence.
3.Levofloxacin-induced acute hemolytic anemia: A clinical case report

【Objective】 To study the clinical characteristics and treatment of patients with acute hemolytic anemia crisis induced by levofloxacin. 【Methods】 The levofloxacin antibody was detected by the microcolumn gel method and scored by the Naranjo Adverse Drug Reaction Probability Scale(APS). Patients suffered from acute hemolysis anemia induced by levofloxacin was rescued by whole blood exchange(WBE) and therapeutic plasma exchange(TPE). 【Results】 The patient was diagnosed as acute hemolysis induced by levofloxacin as levofloxacin antibody was yielded, and 7 points scored by the Naranjo APS. After WBE and TPE treatment, the patient′s clinical symptoms and signs improved rapidly. Supplemented with immunoglobulin and hormone therapy, the patient was discharged on d 10 after treatment. 【Conclusion】 Levofloxacin can induce acute hemolytic anemia crisis, and WBE and TPR are effective rescue methods.
4. Distribution of pathogenic bacteria and drug resistance analysis in patients with acute exacerbation of chronic obstructive pulmonary disease

OBJECTIVE: To investigate the distribution of pathogenic bacteria and drug resistance analysis in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD), so as to guide clinical medication. METHODS: A retrospective study was conducted in Shanxi Dayi Hospital. Totally 1173 cases of AECOPD patients, hospitalized with infection from September 2013 to August 2018, were included. Then relevant laboratory data was collected, including sputum culture, drug sensitivity test, and some commonly used laboratory results. The data was analyzed. RESULTS: A total of 231 strains were isolated in 1173 AECOPD patients,the mian pathogenic bacteria were Gram-negative bacteria(115, 49.7%), fungi(66, 28.6%), Gram-positive bacteria(48, 20.8%),other bacteria(2, 0.9%). Drug-resistant strains of producing ESBLs mainly included Acinetobacter(88.9%), Escherichia coli(87.5%),Pseudomonas aeruginosa(70.6%) and Klebsiella pneumoniae(19.4%). The drug-resistance of Acinetobacter to carbapenems and quinolones was more than 70%. The drug-resistance of Escherichia coli to quinolones was 87.5%. The drug-resistance of Streptococcus pneumoniae to penicillin and first and second generation cephalosporins was 66.7%. The drug-resistance of Staphylococcus to penicillin and first and second generation cephalosporins, quinolones and methicillin was 92.3%, 76.9% and 46.2%, respectively. In all Gram-positive bacteria, drug-resistant strains for Vancomycin and linezolid can't be found. The positive rate of bacteria in the group of PaO_2≤60 mmHg(37.3%) was higher than that in the group of PaO_2> 60 mmHg(8.8%). The detection rate of bacteria in the WBC≥10×10~9/L group(18.7%), NLR≥7.3 group(19.7%)and PCT>0.5 ng/mL group(50.7%),which in turn was higher than that in the WBC<10×10~9/L group(12.7%), NLR<7.3 group(12.0%) and PCT≤0.5 ng/mL group(23.9%). The detection rate of bacteria in the Neu%≤75% group, Neu%(76%~90%) group and Neu%>90% group was 10.6%, 16.9% and 30.1%, respectively. The results of trend chi-square test showed that the detection rate of bacteria increased with the decrease of PaO_2 and the increase of WBC, Neu%, NLR and PCT. CONCLUSION: The distribution and drug resistance of pathogenic bacteria in sputum culture of AECOPD are serious, and the distribution of pathogens in AECOPD patients with different levels of PaO_2, WBC, Neu%, NLR and PCT was different. Therefore, we should pay more attention to sputum culture and some commonly used laboratory results in AECOPD patients, so as to provide the basis for the clinical medication.
5.Seasonal distribution of patient hospitalization due to asthma exacerbation in 7 geographic areas in China.

Objective: To understand the seasonal distribution of patient hospitalization due to asthma exacerbation in 7 geographic areas in China. Methods: This was a retrospective study which involved patients hospitalized for asthma exacerbation in 29 hospitals throughout 7 geographic areas in the mainland of China (northeast, north, central, east, south, northwest and southwest). The numbers of asthmatic patients and total inpatients of the respiratory department of each hospital were recorded. The monthly ratio of asthmatic patients to the total inpatients in every area was calculated and compared. Results: During the study period, 6 480 patients were admitted for asthma exacerbation, accounting for 3.14% of all the 206 135 patients admitted to the respiratory departments in the 29 hospitals. The ratio of asthmatic patients to total inpatients in the northeast area (5.61%) was highest, and the ratio in east area was lowest (1.97%). Statistical analysis showed that the difference among different areas was significant (P<0.000 1). In most areas, both the number and proportion of hospitalized asthmatic patients peaked in spring (February-April) and autumn (September-October). In the northeast area, east area and south area, the peaks in spring were more obvious, while in the north area and southwest area, the peaks in autumn were more obvious. In the northwest area the peaks occurred in winter (December-January) and summer (June-August), respectively. The differences in hospitalization due to asthma among different months were significant in the northeast, north, and southwest areas (P<0.005). Conclusion: The number of patients hospitalized for asthma exacerbation fluctuated with season in different areas in China. In most areas, more asthmatic patients were admitted to hospitals in spring and autumn.

Chronic ulcer of the lower extremities amounts for a grave and serious problem for public health. Western medicine focuses on controlling infection, improving blood circulation, surgical debridement, skin grafting, etc, but there are bottlenecks in the treatment. Traditional Chinese medicine (TCM) has a long history and a legacy of sound clinical efficacy in this area. TCM has developed a unique, effective external theory, and a large number of topical prescriptions and external technology. Through this research, a safe and effective treatment protocol of TCM for chronic ulcer of the lower extremities can be formed. To this end, during China's "Eleventh Five-Year" Plan, special research committees and projects on TCM external treatments and external technologies were established. This study on ulcer of the lower extremities constitutes one of the major research topics.
8.Firebird stent for the treatment of patients with coronary heart disease: short-term clinical outcome

Objective The sirolimus-eluting stent (SES) has dramatically reduced the rate ofrestenosis in comparison to that with the bare-metal stent (BMS).This study aimed to evaluate the short-term efficacy and safety of Firebird stent implantation for patients with coronary heart disease (CHD). Methods From Apri12006 through July 2007, 155 patients (mean age 58.93~10.27 years) with CHD were implanted with Firebird stent or Cypher select stent at Daxing Hospital. Patients were followed up for one year. All-cause mortality, major adverse cardiac events (MACE, including cardiac death, myocardial infarction, recurrence of angina pectoris, heart failure, revascularization, and adverse arrhythmia) and stent thrombosis were compared between the 2 groups. Results Of the 155 consecutive patients, 147 patients were revascularized completely. Of these patients, 48 (with 59 lesions) were treated with Firebird stent, 59 patients (with 75 lesions) with Cypher select stent. The demographic characteristics were similar in the 2 groups. All the angiographic and procedural results were not significantly different between the 2 groups. All-cause mortality, myocardial infarction, recurrence of angina pectoris, MACE and stent thrombosis were almost identical between the 2 groups before discharge, at 6 months and at one year .Conclusion The short-term efficacy and safety of Firebird stent are similar to that of the cypher select stent for the treatment of patients with CHD.
9.Different therapeutic proportion of patients undergone coronary angiography in the era of development in MSCT

Objective To study the different therapeutic proportion of the patient populations undergone coronary angiography (CAG) in the era of development in multislice spiral computed tomography(MSCT).Methods Two hundred and fifty four consecutive patients(mean age 59.24±10.65),who underwent CAG at Daxing Hospital from February 2007 through October 2007,were enrolled,160 patients were male and 94 were female.By evaluating from the coronary angiogram,the patients were not diagnosed to have coronary heart disease(CHD) with less than 50% diameter stenosis of coronary artery;the patients to have CHD with more than or equal to 50% stenosis of coronary artery;the patients were performed the procedure of percutaneous coronary intervention(PCI) with more than or equal to 70% stenosis;the patients were proposed to have coronary aortic bypass graft(CABG) surgery with left main coronary artery lesions or diffuse triple coronary artery lesions.Results In the 254 consecutive patients,59 patients(23.2%) had not been diagnosed to have CHD;195(76.8%)to have CHD,of these patients with CHD,49 patients(19.3%)were not indicated for PCI (including the patients receiving follow-up coronary angiography after stenting),81(31.9%)had been performed the procedure of stent implantation,57(22.4%)proposed to have CABG,8(3.1%)the procedure of PCI had not been successful,or had not been performed because of patients opposing to this therapy.Conclusion Multislice spiral computed tomography can be applied as a non-invasive screening tool to exclude the presence of CHD,to increase the positive proportion of the populations with CHD in all patients receiving coronary angiograhpy,to avoid the use of CAG in a subset of patients.
10.PET imaging of brain function while puncturing the acupoint ST36.

OBJECTIVETo explore the experimental method of obtaining position emission tonogiaphy (PET) imaging evidence of changes in cerebral function by puncturing the Stomach 36 (ST36, Zusanli) acupoint.
METHODSData on changes of cerebral glycometabolism were obtained from six healthy male volunteers with positron emission tomography. Visual experimental evidence, as well as statistical parametric mapping (SPM), was gathered while puncturing the ST36 (Zusanli, right leg) acupoint.
RESULTSThere was increased glycometabolism in the hypothalamus, head of the caudate nucleus, temporal lobe, the sinistral cerebellum, postcentral gyrus, and brain stem while the acupoint ST36 was being punctured.
CONCLUSIONSAcupuncture on ST36 can lead to increase in glycometabolism in the vegetative nerve centers, which is correlated with gastric function. Visual experimental evidence of ST36 acupuncturing on functional gastrointestinal disorder was obtained in our study.
