1.Mechanism of Qingre antai decoction in improving pregnancy outcomes of threatened abortion rats with blood heat syndrome based on JAK2/STAT3 and PI3K/AKT dual signaling pathways

OBJECTIVE To explore the mechanism by which Qingre antai decoction improves pregnancy outcomes of threatened abortion rats with blood heat syndrome. METHODS The pregnant rats were randomly divided into normal group, model group, dydrogesterone group (0.002 g/kg), and Qingre antai decoction group (44.1 g/kg), with 13 rats in each group. Except for normal group, other groups were given warming-yang Chinese medicine and corresponding drugs intragastrically, once a day, for 12 consecutive days. On the 13th day of pregnancy, a single intragastric administration of mifepristone (5 mg/kg) was performed to establish a model of threatened abortion with blood heat syndrome. On the 14th day of pregnancy, the abortion rate and uterine coefficient were calculated; the pathological morphology of pregnant uterine was observed; the serum levels of 3,5,3′-triiodothyronine (T3), thyroid hormone (T4), thyroid stimulating hormone (TSH), as well as the levels of vascular endothelial growth factor (VEGF) and nitric oxide (NO) in the pregnant uterus were all determined; the expressions of mRNA and protein related to Janus kinase 2 (JAK2)/signal transducer and activator of transcription 3 (STAT3) and phosphatidylinositol 3-kinase (PI3K)/protein kinase B (AKT) pathways were detected. RESULTS Compared with normal group, the model group exhibited endometrial tissue damage, a reduced number of decidual cells, and a significant presence of blood stasis within the uterus; abortion rate, the serum levels of T3, T4 and TSH, the mRNA expressions of JAK2, STAT3 and suppressor of cytokine signaling 3 (SOCS3) as well as protein expressions of p-JAK2, p-STAT3 and SOCS3 in the pregnant uterus were increased significantly ( P <0.05); uterine coefficient, the levels of VEGF and NO in pregnant uterus, mRNA expressions of VEGFR2, PI3K, AKT and endothelial nitric oxide synthase(eNOS), protein expressions of VEGFR2, PI3K and eNOS as well as phosphorylation level of AKT in the pregnant uterus were significantly reduced ( P <0.05). Compared with model group, the endometrial tissue damage and congestion in the Qingre antai decoction group were significantly improved, and the levels of the aforementioned quantitative indicators were significantly reversed ( P <0.05). CONCLUSIONS Qingre antai decoction can improve the pregnancy outcomes in rats with threatened abortion of blood heat syndrome, the mechanism of which may be associated with inhibiting JAK2/STAT3 pathway and activating PI3K/AKT pathway.
2.Arsenic trioxide preconditioning attenuates hepatic ischemia- reperfusion injury in mice: Role of ERK/AKT and autophagy.

BACKGROUND:
Arsenic trioxide (ATO) is indicated as a broad-spectrum medicine for a variety of diseases, including cancer and cardiac disease. While the role of ATO in hepatic ischemia/reperfusion injury (HIRI) has not been reported. Thus, the purpose of this study was to identify the effects of ATO on HIRI.
METHODS:
In the present study, we established a 70% hepatic warm I/R injury and partial hepatectomy (30% resection) animal models in vivo and hepatocytes anoxia/reoxygenation (A/R) models in vitro with ATO pretreatment and further assessed liver function by histopathologic changes, enzyme-linked immunosorbent assay, cell counting kit-8, and terminal deoxynucleotidyl transferase-mediated dUTP nick-end labeling (TUNEL) assay. Small interfering RNA (siRNA) for extracellular signal-regulated kinase (ERK) 1/2 was transfected to evaluate the role of ERK1/2 pathway during HIRI, followed by ATO pretreatment. The dynamic process of autophagic flux and numbers of autophagosomes were detected by green fluorescent protein-monomeric red fluorescent protein-LC3 (GFP-mRFP-LC3) staining and transmission electron microscopy.
RESULTS:
A low dose of ATO (0.75 μmol/L in vitro and 1 mg/kg in vivo ) significantly reduced tissue necrosis, inflammatory infiltration, and hepatocyte apoptosis during the process of hepatic I/R. Meanwhile, ATO obviously promoted the ability of cell proliferation and liver regeneration. Mechanistically, in vitro studies have shown that nontoxic concentrations of ATO can activate both ERK and phosphoinositide 3-kinase-serine/threonine kinase (PI3K-AKT) pathways and further induce autophagy. The hepatoprotective mechanism of ATO, at least in part, relies on the effects of ATO on the activation of autophagy, which is ERK-dependent.
CONCLUSION
Low, non-toxic doses of ATO can activate ERK/PI3K-AKT pathways and induce ERK-dependent autophagy in hepatocytes, protecting liver against I/R injury and accelerating hepatocyte regeneration after partial hepatectomy.

3.Effect of Go-Ichi-Ni-San complex subunit 1 on disease progression and chemotherapy resistance in hepatocellular carcinoma

ObjectiveTo investigate the role and mechanism of Go-Ichi-Ni-San complex subunit 1 (GINS1) in the progression of hepatocellular carcinoma (HCC) and the development of chemotherapy resistance. MethodsThe tumor database GEPIA2 was used to analyze the differential expression of GINS1 between HCC patients and healthy individuals, and pathological tissue samples were collected from 40 HCC patients who were admitted to The Affiliated Tumor Hospital of Xinjiang Medical University and the First Affiliated Hospital of Xinjiang Medical University from May 2017 to January 2021. Immunohistochemical staining was used to measure the difference in the expression of GINS1 between HCC tissue and corresponding adjacent tissue, and the correlation between the expression level of GINS1 and the clinical TNM stage of HCC was analyzed. Western blot was also used to measure the difference in the expression of GINS1 between HCC Huh7/Hep3B/Li-7/MHCC97H cell lines and normal human QSG7701 hepatocytes. The method of lentivirus transfection was used to establish the MHCC97H cell line with stable GINS1 knockdown and its negative control cell line. CCK-8 assay and colony formation assay were used to measure cell proliferative capacity; scratch assay was used to measure cell migration ability; Transwell assay was used to measure cell invasion ability; cells were treated with oxaliplatin to measure their sensitivity to chemotherapy drugs. Nude mice were used to establish a tumor-bearing model and observe the effect of GINS1 knockdown on the growth of HCC in vivo. Western Blot was used to measure the expression levels of the proteins associated with the Notch pathway and the JAK/STAT pathway. The cells were treated with the Notch receptor agonist Jagged-1 to analyze the association between GINS1 and the Notch/JAK/STAT pathway. The independent-samples t test was used for comparison of continuous data between two groups; a one-way analysis of variance was used for comparison between multiple groups, and the least significant difference t-test was used for further comparison between two groups. ResultsThe expression of GINS1 was upregulated in HCC patients, HCC tissue, and HCC cell lines (all P<0.05), and the expression level of GINS1 was positively correlated with the clinical TNM stage of HCC (r=0.822, P=0.011). Compared with the negative control cells, the GINS1-knockdown MHCC97H cells showed significant reductions in proliferation, migration, and invasion activities (all P<0.01) and a significantly enhanced sensitivity to oxaliplatin (P<0.01). Compared with the nude mice in the control group, GINS1 knockdown caused significant inhibition of tumor weight and volume in vivo in nude mice (all P<0.001). Compared with the negative control cells, the GINS1-knockdown MHCC97H cells showed significant reductions in the expression levels of Notch1, Notch3, p-JAK2, and p-STAT3 (all P<0.05), while there were no significant differences in the overall expression levels of JAK2 and STAT3 (P>0.05). After Jagged-1 treatment, the GINS1-knockdown MHCC97H cells showed significant increases in proliferation, migration, and invasion activities and a significant reduction in sensitivity to oxaliplatin, as well as significant increases in the levels of p-JAK2 and p-STAT3 (all P<0.05). ConclusionGINS1 is upregulated in HCC and can promote HCC progression and chemotherapy resistance through the Notch/JAK2/STAT3 pathway.
4.Analysis on the evaluation and analysis for the operation quality of the equipment of urological department based on the pressure-state-response model

Objective:To construct an evaluation system for the operation quality of the equipment of the urological department based on the pressure-state-response(PSR)model,so as to explore it's application effect in the evaluation for the operation quality of the equipment of urological department.Methods:Based on the PSR model,the analytic hierarchy process was used to determine the weights of index,and the fuzzy evaluation method was adopted to evaluate the management efficiency for equipment.A total of 160 devices of clinical use in the Urological Department of The First Affiliated Hospital of Xi'an Jiaotong University from January 2022 to January 2024 were selected.According to different management modes,the random number table was adopted to equally distribute these devices into conventional management mode(80 devices)and management mode with evaluation system(model evaluation)for operation quality of the equipment of the urological department(80 devices).A self-made questionnaire was used to investigate the satisfaction of the usage managers in the urological department and operation room,department of medical equipment,and department of managing infection for the clinical services of the two kinds of management modes.The infection situations,management quality of equipment operation,and the score of operation quality in using equipment between two kinds of management modes were compared.Results:The average repair rate of equipment failure,risk rate of infection,inefficiency rate of cleaning,and problem rate of quality inspection in the management mode with model evaluation were respectively(1.58±0.69)%,(1.98±0.69)%,(1.10±0.78)%and(1.01±0.70)%,all of which were lower than those in the conventional management mode,and the differences of them were statistically significant(t=7.879,6.663,7.863,14.601,P<0.05).In the 120 patients who adopted the management mode with model evaluation,the infection rates of using equipment in laparoscopic minimally invasive surgery,endoscopic surgery for stone,resectoscopic surgery and flexible ureteroscope were respectively 2.50%,3.33%,4.17%and 3.33%,all of which were lower than those in the conventional management mode,and the differences between the two kinds of modes were statistically significant(x2=6.696,6.916,5.154,7.101,P<0.05).The scores of the standardization of using equipment,effectiveness of cleaning and disinfection,recognition of emergency management,and qualification of controlling infection in the management mode with model evaluation were significantly higher than those in the conventional management mode,and the differences were statistically significant(t=22.198,11.806,9.385,18.071,P<0.05).The satisfaction scores of the managers involved in using equipment in the operation room of urology,department of medical equipment and department of managing infection for the management mode with model evaluation were all higher than those of the conventional management mode,and the differences were statistically significant(t=14.426,14.916,14.707,P<0.05).Conclusion:The quality evaluation system of equipment operation of the urological department based on the PSR model can reduce the failure rate of equipment,and improve the usage efficiency of equipment,and ensure the medical safety of patients.
5.Investigations into the mechanisms underlying the regulatory effect of EPHA2 on keratinocyte proliferation and differentiation via ERK pathway in psoriasis

Objective:To investigate the expression of ephrin type-A receptor 2 (EPHA2) in psoriatic lesions and its effect on the proliferation and differentiation of normal human epidermal keratinocytes (NHEKs) .Methods:The GDS4602 dataset from the Gene Expression Omnibus (GEO) database was analyzed to determine EPHA2 gene expression changes in psoriatic lesions. Skin tissue samples were collected from 3 psoriasis patients and 3 healthy controls, and EPHA2 expression was determined in the skin tissues by immunofluorescence staining. Twelve female BALB/c mice were randomly divided into 3 groups (4 mice in each group) : a normal control group (receiving no treatment), an imiquimod group (topically treated with 62.5 mg of imiquimod 5% cream), and an imiquimod + ALWⅡ-41-27 group (topically treated with 62.5 mg of imiquimod 5% cream, followed by intraperitoneal injections of the EPHA2 inhibitor ALWⅡ-41-27 at a dose of 20 mg·kg -1·d -1) ; after 6 days of treatment, dorsal skin samples were harvested for hematoxylin-eosin (HE) staining, immunofluorescence staining was performed to determine the expression of EPHA2 and phosphorylated extracellular signal-regulated kinase 1/2 (p-ERK1/2), and real-time fluorescence-based quantitative PCR (qPCR) was conducted to determine the mRNA expression of the nuclear proliferation antigen Ki67, involucrin (Ivl), loricrin (Lor), and keratin 10 (Krt10). In vitro cultured NHEKs were divided into a control group (receiving no treatment), an M5 group (treated with 10 ng/ml M5 cytokines [including interleukin-17A, interleukin-22, interleukin-1α, oncostatin M and tumor necrosis factor-α]), an ALWⅡ-41-27 group (treated with 1 μmol/L ALWⅡ-41-27), and an M5 + ALWⅡ-41-27 group (treated with 10 ng/ml M5 and 1 μmol/L ALWⅡ-41-27) ; after 24 hours of treatment, the 5-ethynyl-2′-deoxyuridine (EdU) assay was performed to assess cellular proliferative activity, Western blot analysis to determine the expression of EPHA2, ERK and their phosphorylated proteins, and qPCR to determine the mRNA expression of KI67, IVL, LOR, and KRT10. One-way analysis of variance, Dunnett's T3 test, two-independent-sample t test, and paired t test were used for statistical analysis. Results:GEO database analysis revealed upregulated EPHA2 expression in psoriatic lesions compared with normal skin tissues from healthy controls ( t = 21.07, P < 0.001). Immunofluorescence staining showed increased EPHA2 expression in skin tissues from psoriasis patients and mouse models of psoriasis compared with those from healthy controls and normal control mice, respectively (both P < 0.01). In the animal experiments, the imiquimod group showed thicker epidermis, increased Ki67 mRNA expression, decreased mRNA expression of Ivl, Lor, and Krt10, and elevated p-ERK1/2 expression compared with the normal control group and imiquimod + ALWⅡ-41-27 group (all P < 0.05). In the cell experiments, the M5 group showed an increased proportion of EdU-positive cells (35.61% ± 1.18% vs. 24.83% ± 0.60% and 12.49% ± 1.52%, t = 8.12, 12.00, P = 0.015, 0.001, respectively), increased KI67 mRNA expression, and decreased mRNA expression of IVL, LOR, and KRT10 compared with the control group and M5 + ALWⅡ-41-27 group (all P < 0.05) ; Western blot analysis revealed that the expression levels of EPHA2, p-EPHA2, and p-ERK1/2 in NHEKs were significantly higher in the M5 group than in the control group and M5 + ALWⅡ-41-27 group (all P < 0.05), but there was no significant difference in the ERK1/2 protein expression among groups ( P > 0.05) . Conclusion:EPHA2 expression was upregulated in psoriatic lesions, which may promote keratinocyte proliferation and inhibit its differentiation, possibly via the ERK pathway.

Turning to critical illness is a common stage of various diseases and injuries before death. Patients usually have complex health conditions, while the treatment process involves a wide range of content, along with high requirements for doctor′s professionalism and multi-specialty teamwork, as well as a great demand for time-sensitive treatments. However, this is not matched with critical care professionals and the current state of medical care in China. Telemedicine, which shortens the distance of medical professionals and the gap of disease diagnosis and treatments in various regions through electronic information, can effectively solve the current problem. Therefore, there is an urgent need to develop a standardized, high-quality visualization telemedicine round system .Therefore, experts have been organized to search domestic and foreign literature on telemedicine round for critically ill patients and to form this consensus based on clinical experiences so as to further improve the level of critical care treatments in regions.
7.Signaling Pathways Related to Polycystic Ovary Syndrome and Regulation by Traditional Chinese Medicine: A Review

Polycystic ovary syndrome (PCOS) is one of the most prevalent gynecological diseases, and its incidence is increasing year by year, seriously affecting the physical and mental health of female patients. The pathogenesis of this disease is complex and has not been fully clarified. At present, PCOS is mainly treated by Western medicine, which, however, has poor efficacy and induces various adverse reactions. Therefore, developing safe and effective therapies has become a difficult problem that needs to be solved. Studies have confirmed that traditional Chinese medicine (TCM) can regulate phosphatidylinositol 3-kinase/protein kinase B(PI3K/Akt), mitogen-activated protein kinase/extracellular signal-regulated kinase (MAPK/ERK), Toll-like receptor 4/nuclear factor-κB (TLR4/NF-κB), transforming growth factor-β (TGF-β)/Smads, secreted glycoprotein/β-catenin (Wnt/β-catenin), adenosine monophosphate-activated protein kinase (AMPK), and advanced glycation endproduct/receptor for advanced glycation endproducts (AGE/RAGE) signaling pathways to ameliorate insulin resistance, inhibit inflammation and oxidative stress, regulate endocrine hormone disorders, and intervene in apoptosis and autophagy, thus alleviating the symptoms, slowing down the disease progression, and improving the ovarian function. The treatment of PCOS with TCM has demonstrated definite effects and high safety. Therefore, exploring this disease from cellular and molecular perspectives can provide a theoretical basis for its clinical treatment and new drug development. However, there is a lack of systematic reviews on the modulation of relevant signaling pathways by TCM in the treatment of PCOS. This article reviews the research progress in the treatment of PCOS with the active ingredients and compound prescriptions of TCM by regulating relevant signaling pathways in recent years, with the aim of providing evidence to support the promotion of TCM for treating PCOS in the future.
8.Construction of Prediction Models for Hepatic Sinus Obstruction Syndrome and Budd-Chiari Syndrome

Objective To establish a predictive model for hepatic sinusoidal obstruction syndrome(HSOS)and Budd-Chiari syn-drome(BCS),and to evaluate the performance of the model.Methods From April 2016 to February 2024,20 patients with HSOS and 40 patients with BCS who were first discharged from the Beijing Friendship Hospital Affiliated to Capital Medical University hospital infor-mation system(HIS)system were retrospectively collected.The clinical data of all patients were extracted,and the independent risk fac-tors for HSOS were screened by multi-factor Logistic regression method,and the differential diagnosis model was established accordingly,and then the efficacy was evaluated by receiver operating characteristic(ROC)curve.Results Multivariate Logistic regression analysis showed that hepatic vein stenosis or unclear display(OR=39.441,95%CI:5.928-262.429)was an independent risk factor for HSOS,and paravertebral vein opening was an independent protective factor for HSOS(OR=0.026,95%CI:0.002-0.285).Based on the above two parameters,a prediction model for HSOS in patients with hepatic venous outlet tract obstruction was established.The ROC curve showed that the area under the curve(AUC)of the model was 0.922(95%CI:0.864-0.991),with the sensitivity of 85.0%and the specificity of 92.5%.Conclusion Hepatic vein stenosis or unclear display is an independent risk factor for HSOS in patients with hepatic vein outflow tract obstruction during hospitalization,and the model constructed can predict the risk of HSOS.
9.Management of severe glottis exposure difficulty during suspension laryngoscopic surgery

Objective Summarize the clinical application and elevating the caudal end of suspension laryngoscope to deal with severe glottis exposure difficulties.Method A retrospective analysis was conducted on the clinical data of 25 patients with severe dystotic glottic exposure(Cormack-Lehane grade Ⅲ or Ⅳ)who underwent laryngoscopy support surgery from January 2021 to January 2024,then analyze the intraoperative management and outcomes.Results In all the patients,after the angle of the suspension laryngoscope was adjusted to the maximum and the glottis could not be exposed satisfactorily,the caudal end of the suspension laryngoscope stent was lifted to obtain a larger tilt angle and field of view.After the caudal end of the stent was elevated by(11.50±6.31)cm,the glottis was exposed satisfactorily in 23 cases and the operation was performed successfully;In 2 cases,the laryngeal mask airway was used in conjunction with a fibrolaryngoscope and laryngeal forceps were used to extract the lesion step by step.Pharyngeal mucosal tearing and oozing of blood occurred in 24 cases,and the bleeding stopped after electrocoagulation;one case had loose and dislodged teeth.There were no significant changes in blood pressure and heart rate of patients before and after stent lifting(P>0.05).Conclusion For the severe difficulty of glottis exposure during suspension laryngoscopic surgery,it can be dealt with by lifting the caudal end of the stent,which is a simple and effective method that does not cause serious complications and is worth clinical application.

Background::Hepatitis B poses a heavy burden for children in China, however, the national studies on the distributional characteristics and health care costs of children with severe hepatitis B is still lacking. This study aimed to analyze the disease characteristics, health economic effects, and medical cost for children with severe hepatitis B in China.Methods::Based on patient information in the Hospital Quality Monitoring System, cases with severe hepatitis B were divided into four groups according to age, and the etiology and symptoms of each group were quantified. The cost of hospitalization was calculated for cases with different disease processes, and severity of disease. The spatial aggregation of cases and the relationship with health economic factors were analyzed by Moran’s I analysis. Results::The total number of children discharged with hepatitis B from January 2016 to April 2022 was 1603, with an average age of 10.5 years. Liver failure cases accounted for 43.48% (697/1603) of total cases and cirrhosis cases accounted for 11.23% (180/1603). According to the grouping of disease progression, there were 1292 cases without associated complications, and the median hospitalization cost was $818.12. According to the spatial analysis, the aggregation of cases was statistically significant at the prefectural and provincial levels in 2019, 2020, and 2021 (all P <0.05). The number of severe cases was negatively correlated with gross domestic product (Moran’s I <0) and percentage of urban population (Moran’s I <0), and positively correlated with the number of pediatric beds per million population (Moran’s I >0). Conclusion::The number of severe hepatitis B cases is low in areas with high gross domestic product levels and high urban population ratios, and health care costs have been declining over the years.
