Objective To explore the effect of 3D printing auxiliary stent on the operation of laryngeal microsurgery instrument. Methods Auxiliary stent of suspension laryngoscope was design and 3D printed.30 standardized training residents as experimental participants were randomly divided into conventional group and auxiliary stent group(15 in each group). The pig's larynx was used as a specimen, and the vocal folds were marked with localized staining.Participants performed operation on the stained areas of the vocal folds under suspension laryngoscope(60 times of grasping in 1 h).The conventional group performed operation unaided; The auxiliary stent group performed operation with the auxiliary stent as a support. Two senior chief physicians scored the participants'stability and accuracy. Results The operational stability scale score of the conventional group and the auxiliary stent group were(5.03±4.33)and(8.99±3.42),respectively;The operation accuracy rates were 58.4%(526/900)and 78.9%(710/900),respectively.The operation accuracy rate, operation stability of the auxiliary stent group were significantly higher than those in the conventional group, and the differences were significant(P<0.01).Conclusion The auxiliary stent can significantly improve the stability and accuracy of operation, which has highly application value.

Objective:To observe the effect of point injection at Zusanli(ST36)plus abdominal point application on gastrointestinal dysfunction after laparoscopic surgery. Methods:A total of 204 patients with gastrointestinal dysfunction after laparoscopic surgery were recruited and divided into four groups using the random number table method, with 51 cases in each group. The control group received conventional postoperative intervention. In addition to the treatment in the control group, the point injection group was given point injection at Zusanli(ST36),the application group was offered abdominal point application, and the integrated group received point injection at Zusanli(ST36)and abdominal point application. The treatment lasted 3 consecutive days in all four groups. The recovery time of gastrointestinal function indicators and the incidence rate of postoperative nausea and vomiting(PONV)were observed and recorded. Before and after treatment, the visual analog scale(VAS)was used to assess abdominal pain intensity, the venous blood type 1 helper T cells/type 2 helper T cells(Th1/Th2)was determined, the serum levels of interleukin(IL)-6 and interferon(IFN)-γ were detected using the enzyme-linked immunosorbent assay, and the plasma levels of motilin and gastrin were measured using radioimmunoassay. Results:Compared to the control group, the first exhaust time, the first defecation time, and the time of restoring fluid diet came earlier in the other three groups(P<0.05)and were earlier in the integrated group than in the point injection and application groups(P<0.05).The point injection, application, and integrated groups had a lower PONV incidence rate than the control group, and the integrated group was lower than the point injection and application groups(P<0.05).The intra-group comparisons showed that the VAS score and the levels of IL-6 and INF-γ decreased after treatment in all four groups(P<0.05);the point injection, application, and integrated groups were lower than the control group(P<0.05),and the integrated group was lower than the point injection and application groups(P<0.05).The intra-group comparisons also demonstrated that the levels of Th1/Th2,motilin,and gastrin increased after the intervention in the four groups(P<0.05);the point injection, application, and integrated groups were higher than the control group(P<0.05),and the integrated group was higher than the point injection and application groups(P<0.05).Conclusion:Point injection at Zusanli(ST36)plus abdominal point application can encourage postoperative exhaust, defecation, and the recovery of diet fluid, alleviate postoperative abdominal pain, reduce PONV,balance Th1/Th2,and regulate the secretion of motilin and gastrin in patients with gastrointestinal dysfunction after laparoscopic surgery.

Objective Summarize the clinical application and elevating the caudal end of suspension laryngoscope to deal with severe glottis exposure difficulties.Method A retrospective analysis was conducted on the clinical data of 25 patients with severe dystotic glottic exposure(Cormack-Lehane grade Ⅲ or Ⅳ)who underwent laryngoscopy support surgery from January 2021 to January 2024,then analyze the intraoperative management and outcomes. Results In all the patients, after the angle of the suspension laryngoscope was adjusted to the maximum and the glottis could not be exposed satisfactorily, the caudal end of the suspension laryngoscope stent was lifted to obtain a larger tilt angle and field of view. After the caudal end of the stent was elevated by(11.50±6.31)cm,the glottis was exposed satisfactorily in 23 cases and the operation was performed successfully; In 2 cases, the laryngeal mask airway was used in conjunction with a fibrolaryngoscope and laryngeal forceps were used to extract the lesion step by step.Pharyngeal mucosal tearing and oozing of blood occurred in 24 cases, and the bleeding stopped after electrocoagulation;one case had loose and dislodged teeth. There were no significant changes in blood pressure and heart rate of patients before and after stent lifting(P>0.05).Conclusion For the severe difficulty of glottis exposure during suspension laryngoscopic surgery,it can be dealt with by lifting the caudal end of the stent, which is a simple and effective method that does not cause serious complications and is worth clinical application.

A growing body of evidence has indicated the important role of autophagy receptors in directing ubiquitinated or non-ubiquitinated cargos towards autophagy. Autophagy receptors bind to LC3 (microtubule-associated protein 1 light chain 3) on phagophore and autophagosome membranes, and recognize signals on cargoes in the delivery system of autophagy. However, the diverse domains in the receptor structures determine that their roles would never be limited to autophagy. Up to date, increasing numbers of the receptor proteins have been demonstrated to serve as a molecular link or switch participating in autophagic degradation, apoptosis or cell survival signals. Here, we highlight the non-autophagic roles of these receptor proteins to draw attention to this growing research topic.

Objective To evaluate the clinical diagnosis and treatment of acute renal infarction. Methods Two cases (3 sides) of acute renal infarction were reported. The patients were 1 male and 1 female, with the age of 62 and 54 years.Case 1 presented acute left flank pain, and enhanced CT showed a non-enhanced area in the upper and mid pole of the left kidney. The diagnosis of focal renal infarction was made and treated with low-molecular heparin (6000 U ).Case 2 presented acute both right abdominal and flank pain, and enhanced CT showed right renal artery embolism and right renal complete infarction.Digital subtraction angiography (DSA) and catheter thrombolytic therapy was applied.4 months later, the patient presented acute left flank pain, and enhanced CT showed a low density area in left kidney without enhanced by contrast, and DSA and catheter thrombolytic therapy was applied again. Results In case 1,contrastenhanced MRI showed a still low signal area like enhanced CT after 2 days of treatment. The renal function remained normal in the follow-up of 36 months. In case 2,the right kidney resorted to moderate blood flow but became atrophy later. In the follow-up of 4 months,a recurrent focal infarction was confirmed in left kidney by enhanced CT.The left kidney also resorted to moderate bloodflow after DSA and catheter thrombolytic therapy. The renal function became normal after follow-up of 10 months and no new infarction was observed. Conclusions The diagnosis of acute renal infraction could be made by enhanced CT or MRI.Early diagnosis and location of the infraction renal artery is critical for recovery of the impaired renal function.Acute renal infraction should be suspected in patients with unexplained persistent and steady flank or abdominal pain in emergence department.
