Extensive evidence has demonstrated that glucagon-like peptide-1 receptor agonists (GLP-1RAs) can ameliorate obesity. Our previous studies revealed that (Ex-4)2-Fc, a long-acting GLP-1RA we developed, depends on the leptin pathway to treat obesity. However, the mechanisms linking (Ex-4)2-Fc and leptin resistance remain largely unclear. To address this question, we explored the mechanism of GLP-1RAs from the perspective of the gut microbiota, as increasing evidence indicates an important link between the gut microbiota and obesity. This study aimed to explore the potential role of the gut microbiota in the treatment of GLP-1RAs. We found that (Ex-4)2-Fc treatment reshaped obesity-induced gut microbiota disturbances and substantially increased the abundance of Akkermansia muciniphila (Am). In addition, (Ex-4)2-Fc did not respond well in antibiotic-treated (ATB) Obese mice. Subsequent studies have shown that this defect can be overcome by gavage with Am. In addition, we found that Am enhanced (Ex-4)2-Fc therapy by producing the metabolite inosine. Inosine regulates the macrophage adenosine A2A receptor (A2A) pathway to indirectly reduce leptin levels in adipocytes Thus, elucidating the role of metabolites in regulating the leptin pathway will provide new insights into GLP-1RAs therapy and may lead to more effective strategies for guiding the clinical use of antidiabetic agents.
2.Expression and significance of serum SBSN in patients with colorectal cancer

Objective To investigate the level and significance of serum suprabasin(SBSN)in patients with colorectal cancer(CRC).Methods A total of 200 patients with CRC in Nanjing Medical University Af-filiated Cancer Hospital from October 2023 to October 2024 were selected as the CRC group,200 patients with colorectal polyps were selected as the benign bowel disease(BCD)group,and 177 healthy people were selected as the control group.In addition,the serum SBSN levels of 84 CRC patients 7-10 d after surgery were collect-ed.The levels of SBSN in each group were compared,and the relationship between serum SBSN level and clin-icopathological characteristics of CRC patients was analyzed.Multivariate Logistic regression was used to ana-lyze the influencing factors of CRC.The receiver operating characteristic(ROC)curve was used to analyze the diagnostic efficacy of serum SBSN alone and in combination with carcinoembryonic antigen(CEA)and carbo-hydrate antigen 19-9(CA19-9)for CRC.Results The serum SBSN level in CRC group was significantly high-er than that in BCD group and control group(P<0.001).Serum SBSN level in CRC patients was associated with TNM stage and lymph node metastasis(both P=0.001),but not with other clinicopathological features(P>0.05).Serum SBSN,CEA,CA19-9 were independent risk factors for CRC(P<0.001).ROC curve anal-ysis showed that the area under the curve(AUC)of SBSN alone in the diagnosis of CRC was 0.734,which was higher than that of CEA(0.611)and CA19-9(0.669)alone,and the AUC of the three combined diagno-sis of CRC was 0.816.The sensitivity and specificity were 70.0%and 81.4%,respectively.The serum SBSN level of CRC patients decreased after operation,and the difference was statistically significant compared with that before operation(P<0.05).Conclusion SBSN is associated with the occurrence,metastasis and progno-sis of colorectal cancer,and has a certain clinical value in the diagnosis and prediction of colorectal cancer.Ser-um SBSN is a potential biomarker for the auxiliary diagnosis of colorectal cancer.
3.Comparison of the therapeutic effects of combination regimens based on omadacycline or tigecycline in the treatment of carbapenem-resistant acinetobacter baumannii pneumonia

Objective:To compare the therapeutic effects of combination regimens based on omadacycline or tigecycline in the treatment of patients with carbapenem-resistant acinetobacter baumannii (CRAB) pneumonia.Methods:A retrospective analysis was performed on the case data of 92 patients diagnosed with CRAB pneumonia who were admitted to the Intensive Care Unit (ICU) of Jinhua Municipal Central Hospital between January 2022 and March 2024. Among these cases, 49 patients received combination therapy with omadacycline as the foundation (hereafter referred to as the omadacycline group),A total of 43 cases were treated with tigecycline-based combination therapy (hereafter referred to as the tigecycline group). The demographic and clinical data of the two patient groups, including pre- and post-treatment improvements in inflammatory markers, bacterial clearance rates, ventilator weaning success rates, and 28-day survival rates, were compared.Results:No significant differences were observed in the baseline characteristics between the two patient groups.The body temperature, white blood cell count, C-reactive protein levels, and procalcitonin concentrations in both groups of patients showed significant improvement after 72 hours of treatment.The differences observed were statistically significant ( P<0.05).Following the completion of the treatment regimen, the bacterial clearance rates in the omadacycline and tigecycline groups were 69.4% and 44.2%, respectively ( P = 0.015). Moreover, the success rates for weaning patients from ventilator support were 71.4% and 51.2%, respectively ( P = 0.046).However, no statistically significant difference was observed in the 28-day survival rate.No serious adverse drug reactions were observed in either of the groups. Conclusions:Both omadacycline- and tigecycline-based combination therapies have demonstrated significant efficacy in improving inflammatory markers in patients with CRAB pneumonia. There was no statistically significant difference in the 28-day survival rate between the two patient groups. Furthermore, combination therapy utilizing omadacycline can significantly enhance the efficacy of promoting negative conversion of the etiological agent,minimize the duration of mechanical ventilation.
4.Reliability and validity of General Burnout Scale in multiple populations

Background As one of the common tools for job burnout assessment, Maslach Burnout Inventory-General Survey (MBI-GS), its reliability and validity across different populations in China have not been examined yet. Objective To evaluate the reliability and validity of General Burnout Scale (GBS) by multiple occupational groups through the translation and preliminary test of MBI-GS. Methods Based on the Special Project of Occupational Hazards in Key Populations led by the Institute of Occupational Health and Poison Control, China CDC, key occupational groups in five typical industries were selected by multi-stage stratified cluster sampling, including teachers, firefighters, manufacturing workers, medical staff, and traffic police. Confirmatory factor analysis (CFA) was used to evaluate the construct validity of GBS by single-factor, two-factor, and three-factor structure models. The model fitness was assessed by ratio of the chi-square statistic to the respective degrees of freedom (χ2/ν), root mean square error of approximation (RMSEA), goodness-of-fit index (GFI), comparative fit index (CFI), and parsimony-adjusted non-normed fit index (PNFI). Spearman correlation analysis was used to calculate the criterion validity of GBS with occupational stress, depressive symptoms, and sleep disorders. Cronbach's α coefficient and composite reliability (CR) coefficient were used to evaluate the internal consistency reliability of GBS. Results A total of 3485 subjects were surveyed in this study, 3375 valid questionnaires were recovered with a valid response rate of 96.84%. The results of CFA showed that in the adjusted three-factor structure model of GBS (exhaustion, cynicism, and professional efficacy), the χ2/ν < 4, the RMSEA ranged from 0.032 to 0.069, the GFI > 0.90, the CFI > 0.90, and the PNFI > 0.70, which illustrated a good fitness than that of the single- or the two-factor structure models in different occupational groups. To revise GBZ 188 Technical Specification for Occupational Health Surveillance based on national laws, regulations, standards, specifications and legal documents of occupational disease, and combination with the actual situation in China. The main modifications are as follows: the occupational health surveillance for workers exposed to toluene (xylene may implement by reference), bromopropane, methyl iodide, ethylene oxide, chloroacetic acid, indium and its compounds, coal tar, coal tarasphalt, asphalt, β-naphthylamine, dust of metal and its compounds(tin, iron, antimony, barium and its compounds), hard metal dust, erionite dust, low temperature, laser, tick-borne encephalitis virus, Borrelia burgdorferi, and human immunodeficiency virus, for scraper or grind operators, and underground workers using squatting or kneeling position, crawling position, side-lying position, or shoulder position for a long period of time are included. The emergency health screening for workers exposed to arsenic, fluorine and its inorganic compounds, and acrylamide are included. The occupational medical examination (OME) for workers exposed to amino and nitro compounds of benzene, phosgene, monomethylamine, organic fluorine and dimethyl sulfate has been adjusted and made mandatory, with corresponding assessments required upon leaving the job. The special occupational health surveillance for workers exposed to mycobacterium tuberculosis and hepatitis virus is removed. The OME conclusion of reexamination is removed, and standardize recheck/additional inspection requirements. The optional items in OME performed before, during and after leaving post are removed, but the optional items in emergency medical examination are retained. Additional OME items are added. The Guideline for OME Summary Reports is added as informative appendix, and so on. The revised GBZ 188 Technical Specification for Occupational Health Surveillance is more scientific and practical.

Age-dependent loss of skeletal muscle mass and function is a feature of sarcopenia, and increases the risk of many aging-related metabolic diseases. Here, we report phenotypic and single-nucleus transcriptomic analyses of non-human primate skeletal muscle aging. A higher transcriptional fluctuation was observed in myonuclei relative to other interstitial cell types, indicating a higher susceptibility of skeletal muscle fiber to aging. We found a downregulation of FOXO3 in aged primate skeletal muscle, and identified FOXO3 as a hub transcription factor maintaining skeletal muscle homeostasis. Through the establishment of a complementary experimental pipeline based on a human pluripotent stem cell-derived myotube model, we revealed that silence of FOXO3 accelerates human myotube senescence, whereas genetic activation of endogenous FOXO3 alleviates human myotube aging. Altogether, based on a combination of monkey skeletal muscle and human myotube aging research models, we unraveled the pivotal role of the FOXO3 in safeguarding primate skeletal muscle from aging, providing a comprehensive resource for the development of clinical diagnosis and targeted therapeutic interventions against human skeletal muscle aging and the onset of sarcopenia along with aging-related disorders.

Objective:To develop the Core Occupational Stress Scale (COSS) for key occupational populations, and to assess the reliability and validity of COSS in China.Methods:According to the literature review, in-depth interview and expert evaluation, the item pool of COSS was established. A total of 20 981 employees (3 703 employees from 2018 and 17 178 employees from 2019) of manufacturing, medical, and traffic polices, etc. from Beijing, Tianjin, Shanghai, Chongqing, Jiangsu, Shandong, Zhejiang, Hunan, Guangdong and Hubei were investigated using convenient sampling of those participating in general or occupational health examination of the day. Item differential test and exploratory factor analysis (EFA) were used to screen items from the item pool; confirmatory factor analysis (CFA) was used to test structure validity; criterion and convergent validity were tested by Pearson correlation. Cronbach′s α coefficient was used to test the reliability of the scale. Results:The EFA suggested a four-factor structure for a 17-item version of COSS, which were social support, organization and reward, demand and effort, and control. It explained 62.06% of the total variance and factor loadings ranged from 0.447 to 0.918. The CFA confirmed the hypothesized four-factor model (GFI=0.904, CFI=0.912, RMSEA=0.079). The COSS scores were positively correlated with burnout, depressive symptoms, and effort-reward imbalance scores with rranging from 0.357 to 0.567 ( P<0.05). The total COSS and each dimension of Cronbach′s α coefficients were 0.772-0.896. Conclusions:The COSS has good reliability and validity and can be used as an occupation stress assessment for occupational populations in China.

Objective:
To investigate the high occupational stress and its influencing factors in traffic police in Shanghai.
Methods:
728 traffic police were selected as the study subjects, and the《Occupational Health Questionnaire》was used to investigate and evaluate their job demand-control (JDC) and effort-reward imbalance model (ERI) occupational stress situation respectively. The related influencing factors were analyzed.
Results:
