1.Reconstruction of anterior cruciate ligament using a large-diameter autograft of 8-strand semitendinosus/gracilis

Objective:To evaluate the clinical outcomes of a large-diameter autograft of 8-strand semitendinosus/gracilis (8S-ST/G) in reconstruction of anterior cruciate ligament (ACL).Methods:A retrospective analysis was conducted of the 56 patients with primary ACL injury who had been treated by the 8S-ST/G autograft technique at Sports Medicine Service, Beijing Jishuitan Hospital, Capital Medical University between January 2019 and January 2022. The cohort included 42 males and 14 females, with an age of (28.3±7.3) years and a body mass index of (24.4±3.3) kg/m 2. There were 28 left and 28 right knees involved. The preoperative posterior tibial slope measured 14.8°±2.8°. The diameter of 8S-ST/G autograft was (9.3±0.7) mm. The postoperative outcomes documented were knee stability (assessed via KT-1000 arthrometer side-to-side difference, Lachman test, and pivot-shift test), graft maturity, graft roof impingement, and complications. Results:The patients were followed up for (2.4±0.3) years postoperatively. The side-to-side difference decreased significantly from preoperative 6.0 (5.0, 8.0) mm to postoperative 2.0 (0.0, 3.0) mm ( P<0.001). The positive rate of high-grade pivot shift (2+, 3+) reduced significantly from 50.0% (28/56) preoperatively to 3.6% (2/56) postoperatively ( P<0.001). The ACL graft signal grades revealed by the MRI at the final follow-up were grade Ⅰ in 45 cases, grade Ⅱ in 7 cases, and grade Ⅲ in 4 cases. After the 15 patients with their preoperative MRI from other hospitals (showing ACL graft signal of grade I in all) were excluded, the anterior subluxation of lateral compartment (ASLC) and anterior subluxation of medial compartment (ASMC) in the other 41 patients at the final follow-up [(4.8±3.1) mm and 3.0 (2.0, 4.9) mm, respectively] showed no significant differences from the preoperative values [(5.9±3.2) mm and 3.9 (0.0, 6.3) mm, respectively] ( P>0.05). However, in the 30 patients with well-preserved ACL graft signal (grade I), both ASLC and ASMC at the final follow-up [(3.9±2.5) mm and 3.0 (1.5, 4.7) mm] showed significant improvements compared to the preoperative values [(5.5±3.1) mm and 5.1 (2.7, 6.2) mm] ( P<0.05). At the final follow-up, 19.6% (11/56) of the patients showed ACL graft laxity; graft failure occurred in 8.9% (5/56) of the patients, one of whom underwent revision surgery. Second-look arthroscopy revealed concomitant cartilage injuries in 12.5% (7/56) of the patients, cyclops lesions in 3.6% (2/56), and ACL graft impingement in 7.1% (4/56). Conclusion:In the treatment of patients with ACL injury, the 8S-ST/G autograft configuration can significantly improve the stability of the knee joint and graft maturity, but it can not effectively correct the ASLC and ASMC for a subset of patients.
2.Global anterior tibial subluxation after anterior cruciate ligament injury:an incidence and risk factor analysis

Objective To explore the incidence of global anterior tibial subluxation(ATS)after an-terior cruciate ligament(ACL)injury and analyze its risk factors.Methods The weight-bearing magnet-ic resonance imaging(MRI)scans of 280 consecutive patients with ACL injury in year 2022 and 2023 were retrospectively analyzed.According to the relative relationship between the tibial plateau and the femoral condyle,all patients were divided into three groups of the ATS-negative group(no ATS in neither the medial nor the lateral tibial plateau),the mild ATS-positive group(only ATS of the lateral tibial plateau),and the global ATS-positive group(ATS of both the medial and lateral tibi-al plateau).Moreover,the posterior tibial slope(PTS),the continuity of anterolateral ligament(ALL),the integrity of meniscus,the time from injury to surgery(TFIS)and results of physical ex-aminations were detected to find the risk factors of global ATS.Results Among the 280 consecutive pa-tients,41 patients were finally confirmed to have global AT,reaching an incidence of 14.6%.More-over,there were 110(39.3%)patients of mild ATS and 129(46.1%)without ATS.Meanwhile,the PTS,as well as the incidence of combined medial meniscus injury and TFIS of the global ATS-posi-tive group were significantly higher than the other two groups(P<0.05).In addition,the incidence of combined ALL and lateral meniscus injury,grade of pivot-shift test and KT-1000 side-to-side differ-ence of the global ATS-positive group were significantly higher than the ATS-negative group(P<0.05),without significant differences from the mild ATS-positive group(P>0.05).Furthermore,results of logistic multi-factor regression analysis showed that PTS>12 degrees,TFIS>6 months and combined medial meniscus injury were all independent risk factors of global ATS after ACL injury.Conclusion In this study,the incidence of global ATS after ACL injury was 14.6%.Clinically,ACL-injured pa-tients with PTS>12 degrees,TFIS>6 months or combined medial meniscus injury should be warned against the global ATS.
3.Application value of flexible traction suspension strategy in pure single-incision laparoscopic distal gastrectomy

Objective:To investigate the application value of flexible traction suspension (FTS) strategy in pure single-incision laparoscopic distal gastrectomy (PSILDG).Methods:The retrospective and descriptive study was conducted. The clinicopathological data of 12 patients who underwent PSILDG in The Second Affiliated Hospital of Army Medical University from November 2021 to March 2024 were collected. There were 8 males and 4 females, aged (53±14)years. Observation indicators: (1) intraoperative conditions; (2) postoperative conditions; (3) follow-up. Measurement data with normal distribution were represented as Mean± SD, and measurement data with skewed distribution were represented as M( Q1, Q3). Count data were described as absolute numbers. Results:(1) Intraoperative conditions. All 12 patients underwent PSILDG with FTS strategy successfully, with the operation time of (260±31) minutes, the volume of intraoperative blood loss of 103.3(37.5,150.0)mL, the length of surgical incision of (3.9±0.6)cm. There was no intra-operative special circumstance or complication. (2) Postoperative conditions. Results of histopatho-logical examination showed that among the 12 patients, there were 10 cases of adenocarcinoma, 1 case of signet ring cell carcinoma, and 1 case of high-grade intraepithelial neoplasia. The distance of the proximal tumor margin was 2.8(2.0,3.4)cm, the distance of distal margin was 5.9(5.0,7.5)cm, the tumor diameter was (2.3±1.0)cm, and the number of lymph node dissected was 34±10. On the post-operative first day, all 12 patients had a visual analog score of 1.0. The time to postoperative removal of gastric tube was 1.25(1.00,1.75)days, the time to postoperative first intake of liquid food was 2.00(1.00,2.00)days, the time to postoperative first out-of-bed activity was 1.67(1.00,2.00)days, the time to postoperative first flatus was 2.40(2.00,3.00)days, the time to postoperative first bowel movement was 3.50(2.00,5.00)days, the duration of postoperative hospital stay was (7.10±1.40) days, and the satisfaction score for the abdominal wall incision was 20.6±2.7. No patient experien-ced postopera-tive complications. (3) Follow-up. All 12 patients completed a 30-day follow-up after surgery, with no complication or need for secondary surgery.Conclusion:Application of FTS strategy in PSILDG is safe and feasible.

7.Study on Species Characteristics of Chinese Patent Medicines for Antitussive Effect

This study was aimed to reveal the species characteristics of Chinese patent medicines for antitussive ef-fect and provide references for developing new drugs. This research targeted Chinese patent medicines for antitussive effect which were included in the Pharmacopoeia of the People's Republic of China and the New National Chinese Patent Medicines as well as those characterized by keywords such as cough cure, cough alleviating, antitussive effect, cough, persistent cough. The analysis was made on the species characteristics, such as the number of Chinese patent medicines for antitussive effect, license number, ethnomedicine patent medicines, drugs for children use, protection of varieties of traditional Chinese medicine, the number of drugs, the generic names of drug, and drug forms. The results showed that 684 Chinese patent medicines for antitussive effect collected in this research had ac-counted for 8.60% of the total 7 260 of Chinese patent medicines. A total of 7 450 license numbers were approved, and 33% of the Chinese patent medicines shares one license number. One Chinese patent medicine owns 16.6 li-cense numbers on average. Ethnomedicine patent medicines had 3 Tibetan prescriptions such as the Shiwuwei Chenxiang pill and 4 Mongolian prescriptions, such as the Siwei Tumuxiang powder. Drugs for children accounted for 14%, including 9 forms. The type of the generic names of drug reached 16 and most of them originate from abbrevia-tions of the main drug in prescription. The number of drugs in prescription ranges from 8 to 16. Chinese patent medicines for antitussive effect involved 16 forms, of which the proportion of the use of solid preparation was higher than the liquid preparation. It was concluded that Chinese patent medicines for antitussive effect were characterized by such advantages such as a variety of species, various forms, the reasonable number of drugs, considerable medicine retail market share and drug for children use which can meet the clinical needs, and meanwhile some prob-lems, such as a lack of criteria for the generic names of drug, the homogenization of fierce competition, and inade-quacy of ethnomedicine patent medicines.
8.Clinical features and removal strategies of intracranial foreign bodies

Objective To investigate clinical features,diagnosis,clearance strategies and prognosis of intracranial foreign bodies.Methods Twenty patients with intracranial foreign bodies were analyzed retrospectively,together with review of the related literatures.Results Twenty patients underwent craniotomy for intracranial foreign body removal under guidance of preoperative CT and X-ray localizations and intraoperative C-arm X-ray machine and ultrasound localizations.A total of 35 foreign bodies were removed.One patient underwent second surgical resection after the incomplete removal due to displacement of intracranial foreign bodies.According to Glasgow outcome score (GOS) at discharge,the outcomes were good (GOS =4-5 points) in 16 patients,poor (GOS =2-3 points) in three and death (GOS =1 point) in one.Conclusions CT and X-ray locations before surgery and C-ann X-ray machine and ultrasound locations in operation avail the removal of foreign bodies by craniotomy.In the meantime,prognosis is satisfactory.
9.Effect of Matteuccia struthiopteris polysaccharides on systemic lupus erythematosus-like syndrome induced by Campylobacter jejuni in BALB/c mice

Matteuccia struthiopteris is a nature plant, which contains a lot of potential active components. In the present study, we investigated the effect of polysaccharides extracted from Matteuccia struthiopteris on lupus-like syndrome induced by Campylobacter jejuni CJ-S131 in BALB/c mice. Mice were randomly divided into normal, model control, SLE model (vehicle treated), Matteuccia struthiopteris polysaccharides treated (30 and 15 mg x kg(-1)) groups and prednisone 5 mg x kg(-1) treated groups. The effect of Matteuccia struthiopteris polysaccharides (Ms) on weight and organ index of BALB/c mice was detected. Autoantibodies and total IgG production were measured by enzyme linked immunosorbent assay. Proteinuria was measured and kidneys were examined by light microscopy. Compared with SLE model group, treatment with Matteuccia struthiopteris polysaccharides 30 and 15 mg x kg(-1) reduced weight loss and Matteuccia struthiopteris polysaccharides 15 mg x kg(-1) reduced spleen swelling (P < 0.05). The increased production of autoantibodies and total immunoglobulin G (IgG) were also significantly inhibited. Matteuccia struthiopteris polysaccharides protected kidney against glomerular injury in BALB/c mice with reduced immunoglobulin deposition and lowered proteinuria (P < 0.01). Matteuccia struthiopteris polysaccharides had a protective effect on lupus-like syndrome induced by CJ-S131 in BALB/c mice.
