Objective: To explore the application effectiveness of healthcare failure mode and effect analysis (HFMEA) in optimizing intelligent blood logistics transport pathways for safety assurance. Methods: Data from 1 851 cases of intelligent blood logistics transport were collected between September 2023 and March 2025. Based on the implementation phases of HFMEA measures, the cases were divided into a control group (n=120), observation group 1 (n=219), and observation group 2 (n=1 512). Through systematic analysis of the transport processes, hazard scoring and decision tree analysis were conducted for each process, and phased optimization measures were implemented for high-risk failure modes. Results: The transport duration of intelligent blood logistics was 35.5 (20.8, 71.1) min in the control group, 25.1 (10.9, 40.7) min in observation group 1, and 9.9 (4.2, 44.5) min in observation group 2. Observation group 2 exhibited significantly shorter transport time compared to both observation group 1 and the control group, with statistically significant differences between groups (P<0.000 1). Conclusion: The implementation of HFMEA-driven measures significantly reduced intelligent blood logistics transport duration, thereby fostering the evolution of smart hospital ecosystems while enhancing healthcare service quality and operational efficiency.

Objective:To investigate dietary patterns under social jet lag in pregnant women with gestational diabetes mellitus (GDM) and analyze its relationship with blood glucose control and delivery outcomes, to provide a reference for comprehensive management of pregnant women with GDM.Methods:A cross-sectional study method was used to select pregnant women with GDM during their stay in the obstetrics ward of the Obstetrics and Gynaecology Hospital of Nanjing Medical University from October 2022 to December 2023 as the study population. Social jet lag and dietary information were collected using Simplified Food Frequency Questionnaire and Self-assessment Questionnaire on Social Jet Lag. Spearman correlation analysis was used to analyze the correlation between dietary patterns and blood glucose control as well as delivery outcomes. Receiver operating characteristic curve was used to analyze the predictive performance of social jet lag, dietary patterns and fasting blood glucose for cesarean section. Results:A total of 350 pregnant women with GDM were included, 140 pregnant women with social time lag, aged (31.97 ± 3.88) years old; 210 pregnant women without social time lag, aged (32.32 ± 3.91) years old. The plant-based dietary pattern factor, fast food dietary pattern factor, and the intakes of protein, fat, and carbohydrates of pregnant women with social jet lag were -0.06 (-0.25, 0.32) and 0.08 (-0.43, 0.66), (119.57 ± 89.77) g, (82.69 ± 59.48) g, (228.77 ± 140.18) g, compared with pregnant women without social jet lag [-0.21 (-0.39, 0.08), -0.24 (-0.57, 0.07), (86.29 ± 47.75) g, (57.31 ± 37.86) g, (170.79 ± 85.56) g], the differences were statistically significant ( Z=-2.39, -2.86, t=81.43, 90.78, 88.34, all P<0.05). The social jet lag of GDM women was positively correlated with plant-based dietary patterns, fast food dietary patterns, fasting blood glucose, glycated hemoglobin and cesarean section ( r values were 0.19-0.24, all P<0.05). The plant-based dietary pattern of pregnant women with GDM was negatively correlated with fasting blood glucose and cesarean section ( r=-0.17, -0.24, both P<0.05), while the fast food dietary pattern was positively correlated with fasting blood glucose and cesarean section ( r=0.20, 0.19, both P<0.05). The area under the curve in the combined prediction of multiple factors such as social jet lag, plant-based dietary pattern, fast food dietary pattern and fasting blood glucose for the outcome of cesarean section during pregnancy was 0.742. Conclusions:The dietary structure of GDM women with social jet lag is unreasonable, consume more carbohydrates, sugar, and fats, which are associated with poor glycemic control and increased cesarean section rate. Medical staff should be alerted to social jet lag preferences and dietary intake of GDM and include them in the risk assessment of adverse outcomes of GDM, give timely correction and strengthen management, which can provide a reference for early preventive interventions for GDM with different risk stratification.

Traditional Chinese medicine (TCM) serves as a treasure trove of ancient knowledge, holding a crucial position in the medical field. However, the exploration of TCM's extensive information has been hindered by challenges related to data standardization, completeness, and accuracy, primarily due to the decentralized distribution of TCM resources. To address these issues, we developed a platform for TCM knowledge discovery (TCMKD, https://cbcb.cdutcm.edu.cn/TCMKD/). Seven types of data, including syndromes, formulas, Chinese patent drugs (CPDs), Chinese medicinal materials (CMMs), ingredients, targets, and diseases, were manually proofread and consolidated within TCMKD. To strengthen the integration of TCM with modern medicine, TCMKD employs analytical methods such as TCM data mining, enrichment analysis, and network localization and separation. These tools help elucidate the molecular-level commonalities between TCM and contemporary scientific insights. In addition to its analytical capabilities, a quick question and answer (Q&A) system is also embedded within TCMKD to query the database efficiently, thereby improving the interactivity of the platform. The platform also provides a TCM text annotation tool, offering a simple and efficient method for TCM text mining. Overall, TCMKD not only has the potential to become a pivotal repository for TCM, delving into the pharmacological foundations of TCM treatments, but its flexible embedded tools and algorithms can also be applied to the study of other traditional medical systems, extending beyond just TCM.

Objective: To establish a radio frequency identification (RFID) technology-based emergency release process of uncrossmatched type O red blood cells for trauma patients, thereby providing a reference method for blood inventory management. Methods: The medical records of uncrossmatched type O red blood cell suspension released from Jan 2023 to Dec 2024 were retrospectively analyzed. The pre-implementation (2023, n=23) and post-implementation (2024, n=19) data of the RFID technology-based emergency uncrossmatched transfusion management method were compared to evaluate its effectiveness. Results: During the study period, 42 patients received a total of 92 units uncrossmatched type O red blood cell suspensions in emergency, with no case of hemolytic transfusion reactions reported. Traffic accidents were the primary cause of emergency uncrossmatched transfusion, accounting for 66.7% (28/42) of cases. The patients in observation group were significantly older than those in the control group (60.58y vs 48y, P<0.05), with a trend toward more critical trauma conditions (84.2% vs 56.5%, P=0.053). Among 29 critically injured patients, the blood release time in the observation group was significantly shorter than that in the control group (2 min vs 4 min, P<0.05). After excluding deceased patients, the observation group showed shorter hospitalization duration compared to the control group (18.5 d vs 35.58 d, P<0.05). Patients treated with a high red blood cell-to-plasma ratio demonstrated a significantly higher resuscitation success rate compared to those with a low ratio (64.3% vs 30%, P<0.05). Conclusion: The implementation of the RFID technology-based emergency uncrossmatched transfusion management method can significantly reduce the blood release time, shorten average hospitalization duration, improve treatment efficiency, and ensure transfusion safety for trauma emergency patients.

Under the Healthy China strategy, the training of preventive medicine professionals should conform to the development of the times and the needs of public health. The article analyzed the problem of "disconnection between medical care and prevention" in teaching the course of Obstetrics and Gynecology for students majoring in preventive medicine, and proposed to strengthen the strategy of "integration of medical care and prevention". Guided by the concepts of "Comprehensive Health", with the cornerstone of cultivating morality and talents, the reform focused on enhancing the post competency of preventive medicine talents. Course content was optimized by reinforcing prevention-focused elements, student-centered teaching method innovation was advocated, and simulation-based training teaching system was established. Moreover, projects integrating science and education were used to improve faculty teaching capacity with tiered and diversified approaches. Course assessment methods were revised and "ideological and political education" was incorporated to establish the education concept of "emphasis on prevention and integration of medical care and prevention". The survey showed that these reform measures effectively improved the comprehensive clinical literacy of preventive medicine students in obstetrics and gynecology, which is of great significance for building a "integration of medical care and prevention" public health talent training model. In the future, the long-term effectiveness of course reform will be ensured from the perspectives of teaching resources, personnel investment, and policy support.
