1.Visual analysis of the current status of research on traditional Chinese medicine treatment for allergic rhinitis

Objective To analyze the hotspots and frontiers of traditional Chinese medicine treatment for allergic rhinitis(AR)in China,aiming to provide reference for the treatment of AR patients in China.Methods Literatures related to traditional Chinese medicine treatment of AR published by CNKI,Wanfang Data and VIP database from January 2004 to December 2023 were searched,and CiteSpace software was used for visual analysis of the number of literatures,key words,research institutions and authors.Results A total of 1604 literatures were included,and the annual number of literatures generally showed an upward trend.The research frontiers in the last 5 years include medication patterns,immune function,quality of life,data mining,and thumbtack needle.The top five institutions in terms of publications were Beijing University of Traditional Chinese Medicine,Tianjin University of Traditional Chinese Medicine,Nanjing University of Traditional Chinese Medicine,the First Affiliated Hospital of Beijing University of Traditional Chinese Medicine,and Chengdu University of Traditional Chinese Medicine.The core authors were Yuan Weiling,Yan Daonan,Zhao Jiping,Zhang Qinxiu,and Feng Weiyun.Conclusion The application of traditional Chinese medicine in treatment of AR has always been in a high research heat,and giving full play to the characteristics of traditional Chinese medicine can be the development direction of AR research in the future.
2.Analysis of the availability of bronchodilators listed in the medical insurance catalog for treatment of chronic obstructive pulmonary disease in community health service centers in Shanghai

ObjectiveTo assess the availability of bronchodilators for treatment of chronic obstructive pulmonary disease (COPD) in community health service centers (CHCs) in Shanghai. MethodsOn the basis of previous research, the questionnaire was updated, and surveys were conducted from April to May 2023 in CHCs in Shanghai, with a focus on the availability of medications for COPD treatment. According to the National Basic Medical Insurance, Work Injury Insurance, and Maternity Insurance Drug List (2023 Edition), a total of 24 types of bronchodilators for COPD treatment were identified. The availability rates were used to assess the accessibility of specific drugs in CHCs, and the dispensing rates were used to evaluate the variety of these medications in CHCs. ResultsA total of 248 CHCs responded, with a response rate of 100.0%. Among them, a total of 232 CHCs (93.5%) were equipped with bronchodilators. In terms of availability rates, the availability rates for β2 adrenoreceptor agonists, muscarinic antagonists, combination drugs, and xanthines drugs were 86.3%, 52.0%, 52.4%, and 85.1%, respectively. Regional differences were observed, in that salbutamol/levalbuterol and budesonide-formoterol were less available in suburban CHCs, ipratropium bromide had lower availability in urban CHCs, and aminophylline was more available in suburban CHCs. Regarding the dispensing rates of the 24 types of bronchodilators for COPD treatment, the median of types equipped by CHCs was 5, with a total of 200 CHCs (80.6%) equipped with fewer than 7 types of bronchodilator drugs. ConclusionThe proportion of CHCs in Shanghai equipped with bronchodilators for COPD is relatively high. However, there exist problems such as limited variety of stocked medications, significant differences in the availability of different drugs, and regional imbalances in drug stocking. To improve the standardized diagnosis, treatment, and management of COPD patients, it is recommended to enhance the availability of COPD treatment medications in CHCs.
3.Safety analysis of blinatumomab in the treatment of pediatric B-cell acute lymphoblastic leukemia

Objective:To analyze the safety profile of blinatumomab in children with B-cell acute lymphoblastic leukemia (ALL).Methods:Demographic and clinical data of 33 pediatric B-cell ALL patients treated with blinatumomab in the Women and Children′s Hospital, Qingdao University from January 2022 to November 2024 were retrospectively collected. Demographic data included gender and age, while clinical data comprised leukemia risk stratification, minimal residual disease (MRD) status before blinatumomab use, treatment duration (14-day or 28-day courses), and safety outcomes included drug-related fever, cytokine release syndrome (CRS), tachycardia, blood pressure abnormalities, elevated transaminases, immune effector cell-associated neurotoxicity syndrome (ICANS), oral mucositis, rash, and infections. Patients were stratified by CRS occurrence and transaminase elevation for comparative analysis of demographic/clinical characteristics.Results:A total of 33 children with B-cell type ALL who received blinatumomab treatment were included. Among them, 21 were male and 12 were female; the age was 5.2 (4.7, 7.0) years, ranging from 1.7 to 10.0 years. Risk stratification included low (2 cases), intermediate (23 cases), and high (8 cases) risk. Pre-treatment MRD was negative in 16 and positive in 17 patients. Eight patients received a 14-day blinatumomab course, while 25 cases received a 28-day course. The overall adverse events (AEs) rate was 81.8% (27/33). Among the 27 patients who experienced AEs, there were 5 cases (18.5%) of severe adverse events (all grade 3). The specific adverse events that occurred in the 33 patients included drug-related fever in 21 cases (63.6%) [including 16 cases (48.5%) of CRS], elevated transaminases in 10 cases (30.3%), infectious symptoms in 5 cases (15.2%), rash in 4 cases (12.1%), tachycardia in 3 cases (9.1%), ICANS in 2 cases (6.1%), and oral mucositis in 1 case (3.0%). No statistically significant differences were observed in gender, age, risk stratification, pretreatment MRD status, and treatment duration between the CRS and non-CRS groups, transaminase-elevated and normal groups (all P>0.05). Conclusions:In pediatric B-cell ALL, the most common AEs related to blinatumomab are CRS and elevated transaminases, but most reactions are mild, with rapid recovery and favorable tolerability.

6.Meta-analysis of correlation between assisted reproductive technology and postpartum breastfeeding outcomes

Objective:To evaluate the impact of assisted reproductive technology (ART) on postpartum breastfeeding outcomes.Methods:This paper electronically retrieved the China Biology Medicine disc, China National Knowledge Infrastructure, VIP, WanFang Data, PubMed, Embase, Web of Science, and Cochrane Library. The search period was from database establishment to March 15, 2023. After independent literature search, screening, data extraction, and quality evaluation by two researchers, Meta-analysis was conducted using R 4.2.2 software.Results:A total of 11 articles were included. Meta-analysis showed that compared with naturally conceived mothers, the rates of exclusive breastfeeding at 1th week postpartum ( RR=0.84, 95% CI: 0.73-0.97), exclusive breastfeeding at 6th months postpartum ( RR=0.77, 95% CI: 0.61-0.98), and the incidence of breastfeeding for >6 months postpartum ( RR=0.71, 95% CI: 0.53-0.96) were decreased, and the rate of artificial feeding at 12th months postpartum ( RR=1.09, 95% CI: 1.02-1.17) was increased. However, there were no statistically significant differences in the rate of artificial feeding at 8th months postpartum, the incidence of breastfeeding duration >12 months, and the incidence of breastfeeding difficulties ( P>0.05) . Conclusions:ART reduces the rate of exclusive breastfeeding in postpartum 1th week and 6th months, and the incidence of postpartum breastfeeding duration>6 months, and increases the artificial feeding rate in postpartum 12th months. However, the impact of ART on the incidence of breastfeeding difficulties is not yet clear and still needs to be further demonstrated by high-quality studies.
7.Protective effect and mechanisms of neostigmine in combination with anisodamine against pulmonary oxygen toxicity

Objective Pulmonary oxygen poisoning resulting from hyperbaric oxygen,frequently occurs in specialized operations,without any current effective prevention or treatment measures.To elucidate the impact and mechanism of neostigmine(NEO)in combination with anisodamine(ANI)(neoscopolamine)on pulmonary oxygen toxicity.Methods The animal model of pulmonary oxygen poisoning was established.C57BL/6 mice were exposed to 2.5 ATA 99.9%oxygen for 6 h.The control group mice were injected with normal saline ip,while the treatment group mice received injections of ANI(25 mg/kg,ip)and NEO(50 μg/kg,ip).Lung tissues were collected and stained with HE to observe any pathological injuries after exposure.Evans blue stain was utilized to identify lung permeability,wet/dry lung ratio,and protein concentration in the bronchoalveolar lavage fluid(BALF)to assess the lung injury's severity.The modifications in inflammatory factors,oxidative stress indicators,and iron content in lung tissue were assessed.Results The results showed that the 2.5 ATA 99.9%oxygen-exposed group experienced a significant worsening of lung injury,as well as increased lung permeability,lung wet/dry ratio,and protein content in alveolar lavage fluid when compared to the control group.Moreover,mRNA levels of pro-inflammatory cytokines IL-1β,IL-6,TNF-α,and IFN-γ in the lung tissue of the model group were significantly elevated,while the levels of anti-inflammatory cytokines IL-4 and TGF-β were significantly reduced.The oxidative index MDA also significantly increased,while the antioxidant index GSH significantly decreased.Additionally,the expression of GPX4,a marker of ferroptosis,increased with an increase in iron content.Neoscopolamine treatment successfully reversed those effects.Conclusion The combined use of ANI and NEO had a protective effect on pulmonary oxygen poisoning.Neoscopolamine may inhibit inflammation and oxidative stress by activating the cholinergic anti-inflammatory pathway,thereby reducing the content of free iron in lung tissue and finally inhibiting cell ferroptosis.
8.Integration of Traditional Chinese and Western Medicine in the Treatment of Liver Transplant Patient with Severe COVID-19: A Case Report

Liver transplant patients require lifelong immunotherapy, and if they are infected by SARS-Cov-2, their immune function will face double whammy. This report described the integrated approach of traditional Chinese and western medicine in the treatment of a liver transplant patient with severe COVID-19. The treatment was involved with western medicine such as antiviral, immunosuppressive, focusing on maintaining immune balance. Traditional Chinese medicine was given based on the differentiation of syndromes, targeting at the core pathogenesis and using methods such as promoting qi circulation, clearing heat and resolving dampness, draining lung and relieving panting. Following the treatment, the patient exhibited notable improvement in clinical symptoms and liver function, leading to the effective cessation of disease progression and a shortened recovery period.
9.Intervention Effect and Mechanism of Celastrol on NASH Induced by Methionine-choline Deficiency Diet in Mice

OBJECTIVE:To study the intervention eff ects and pot ential m echanism of celastrol on non-alcoholic steatohepatitis (NASH)induced by methionine-choline deficiency (MCD)diet. METHODS :Male C 57BL/6J mice were randomly divided into normal control group ,model group ,celastrol low-dose and high-dose groups [ 0.5,1 mg/(kg·d)],with 7 mice in each group. The normal control group was given a methionine-choline sufficient diet ,while the model group and administration groups were fed an MCD diet to induce NASH model. At the same time ,normal control group and model group were given polyoxyethylene castor oil intragastrically;administration groups were given relevant drugs intragastrically ;the volume of gavage was 0.1 mL/g,once a day , for consecutive 4 weeks. The liver morphology was observed ,and the pathological changes of liver tissue were observed by HE staining and oil red O staining. The levels of serum liver enzymes (AST,ALT),and the levels of lipid indexes (TC,TG)in serum and liver tissue were detected by enzyme method. The protein expression of NF-κB p65,TNF-α and IL-6 in liver tissue were determined by Western blotting assay. RESULTS :Compared with normal control group ,the volume of the liver was reduced and the color was yellow ,and the surface was rough in model group ;inflammatory cell infiltration ,fat vacuoles and lipid droplets aggregation were found in the liver tissue ;the serum levels of TC and TG were significantly decreased ,the levels of serum liver enzymes and protein expression of NF-κB p65,TNF-α and IL-6 in liver tissue were significantly increased (P<0.01). Compared with model group ,the liver surface of each administration group was ruddy and smooth without brown spots ,the inflammatory cells and fat vacuoles in liver tissue were reduced ,and the coverage area of lipid droplets was reduced ;the levels of serum TC and TG were significantly increased ,the levels of serum liver enzymes ,the levels of TG and protein expression of NF-κB,TNF-α and IL-6(except for celastrol low-dose group )in liver tissue were significantly decreased (P<0.05 or P<0.01). CONCLUSIONS : Celastrol can improve the liver injury of NASH model mice induced by MCD diet ,which is related to the reduction of TG accumulation in liver tissue and inhibition of the expression of inflammatory related factors.
10.Clinical practice and discovery of selective coronary venous bypass grafting in the treatment of diffuse coronary artery disease

Objective To summarize the clinical features of patients with diffuse coronary artey diseases,and evaluate the clinical efficacy of off-pump coronary artery bypass grafting(OPCABG) combined with selective coronary venous bypass grafting (SCVBG).Methods Retrospectively analyzed the clinical data of 61 patients with diffuse right coronary stenosis undergoing operation of OPCABG + SCVBG from January 2007 to December 2013,and couducted the comparative study of the patients who underwent OPCABG during the same period based on propensity score.Patients were divided into SCVBG group(61 cases,underwent OPCABG + SCVBG) and control group(60 cases,matched by propensity score and underwent OPCABG without SCVBG).Results Compared with control group,the rate of myocardial infarction in SCVBG group was higher (67.2% vs.46.7c%,P <0.05),the heart rate was faster[(69.92 ± 15.82) bpm vs.(64.48 ± 13.72) bpm,P < 0.05],the low density lipoprotcin and triglyceride were higher[(2.67 ± 0.78) mmol/L vs.(2.37 ± 0.78) mmol/L (1.84 ± 0.79) mmol/L vs.(1.36 ± 0.60) mmol/L,both P < 0.05] and the troponin I was higher in the first postoperative day [0.85 (0.29,3.15)μg/L vs.5.09 (2.02,13.03)μg/L,P < 0.05].The perioperative(postoperative) mortality(1.6% vs.0) and the long-term survival curve difference had no statistically significance(P >0.05).Conclusion Patients with coronary artery disease should pay more attention to the control of heart rate and blood lipids,poorly controlled heart rate and high blood lipids are the important factors for the development of coronary heart disease.The exact efficacv of selective coronary vein arterialization for diffuse coronary artery disease is confirmed through the small sample comparative study.
