1.Hemodynamic effects of nalbuphine combined with etomidate or propofol for painless colonoscopy in elderly hypertensive patients

Objective To compare the effects of nalbuphine combined with etomidate and nalbuphine combined with propofol in painless colonoscopy for elderly patients with hypertension. Methods Elderly patients with hypertension who underwent painless colonoscopy in the Department of Anesthesiology of Affiliated Huaian No.1 People’s Hospital of Nanjing Medical University from August 2022 to February 2023 were selected and randomly divided into three groups: nalbuphine combined with etomidate group (E group), nalbuphine combined with propofol group (P group), and the nalbuphine combined with etomidate and propofol group (EP group). The primary endpoint was the incidence of hypotension during anesthesia. Secondary endpoints included mean arterial pressure (MAP), systolic blood pressure (SBP), diastolic blood pressure (DBP), and heart rate (HR) at five time points: before anesthesia (T0), before colonoscopy (T1), 5 minutes after colonoscopy (T2), at the end of the examination (T3), upon leaving the post-anesthesia care unit (PACU, T4). Additionally, examination time, awakening time, Ramsay score, and adverse reactions were compared among the three groups. Results A total of 180 patients were included in the analysis, with 60 cases in each group. The incidence of hypotension was 11.7%, 55.0%, and 33.3% in the E group, P group, and EP group, respectively. The incidence of hypotension in the E group was lower than that in the P and EP groups (P<0.05). At T1, T2, T3, and T4, MAP, SBP, and DBP in the E group were significantly higher than those in the P and EP groups (P<0.05). There were no statistically significant differences among the three groups in awakening time, Ramsay score, or incidence of adverse reactions. Conclusions Compared with nalbuphine combined with propofol, nalbuphine combined with etomidate for painless colonoscopy in elderly patients with hypertension resulted in a lower incidence of hypotension, less impact on hemodynamics, and higher safety.
2.Teaching reform of obstetrics and gynecology course in preventive medicine based on integration of medical care and prevention

Under the Healthy China strategy, the training of preventive medicine professionals should conform to the development of the times and the needs of public health. The article analyzed the problem of "disconnection between medical care and prevention" in teaching the course of Obstetrics and Gynecology for students majoring in preventive medicine, and proposed to strengthen the strategy of "integration of medical care and prevention". Guided by the concepts of "Comprehensive Health", with the cornerstone of cultivating morality and talents, the reform focused on enhancing the post competency of preventive medicine talents. Course content was optimized by reinforcing prevention-focused elements, student-centered teaching method innovation was advocated, and simulation-based training teaching system was established. Moreover, projects integrating science and education were used to improve faculty teaching capacity with tiered and diversified approaches. Course assessment methods were revised and "ideological and political education" was incorporated to establish the education concept of "emphasis on prevention and integration of medical care and prevention". The survey showed that these reform measures effectively improved the comprehensive clinical literacy of preventive medicine students in obstetrics and gynecology, which is of great significance for building a "integration of medical care and prevention" public health talent training model. In the future, the long-term effectiveness of course reform will be ensured from the perspectives of teaching resources, personnel investment, and policy support.
3.Establishment of reference intervals of serum immunoglobulin and complement by Hoffmann and refineR algorithms and evaluation of their feasibility

Objective:To calculate the reference intervals of immune globulinG (IgG), IgA, IgM, complement 3 (C3) and C4 for adults using Hoffmann and refineR methods, and to compare them with the reference interval reported by WS/T 645 of the People′s Republic of China, exploring the clinical application value of the indirect methods.Methods:Cross-sectional study. The physical examination data were collected at Peking Union Medical College Hospital from 2013 to 2023. Box-Cox was used to normalize the data distribution, and the Tukey method was used to remove outliers. Mann-Whitney test and Kruskal-Wallis test were used to compare the differences between gender and age, respectively. Quantile regression analysis was used to examine the effects of age and gender. Hoffmann and refineR methods were employed to calculate RIs of the five parameters.Results:There were 82 251, 82 483, 49 236, 20 027 and 19 942 test results for IgG, IgA, IgM, C3 and C4, respectively. Gender was significantly correlated with IgG, IgA, IgM, C3, and C4 levels, and age was significantly correlated with IgA, IgM, C3, and C4 levels. Reference intervals were calculated using both Hoffmann and refineR methods, yielding comparable results: IgG (7.58-16.85 g/L; 7.54-16.88 g/L), IgA (0.97-4.26 g/L; 0.97-4.34 g/L), C3 (0.73-1.43 g/L; 0.73-1.44 g/L), and C4 (0.11-0.32 g/L). Gender-specific intervals were established for IgM: women (0.44-2.57 g/L; 0.44-2.62 g/L) and men (0.33-1.91 g/L; 0.31-1.87 g/L). Gender and/or age-specific reference intervals were further calculated according to the bias ratio values. The reference intervals calculated by Hoffmann and refineR are in good agreement. The upper and lower limits of reference intervals for IgA and C3, the lower limits of IgG, the upper limits of IgM and C3 calculated by the indirect methods have good comparability with those reported by standards.Conclusions:The total and gender/age-specific reference intervals of IgG, IgA, IgM, C3 and C4 in our hospital population were established by Hoffmann and refineR methods, and the reference intervals established by the two indirect methods were comparable. The reference intervals calculated using these indirect methods were also comparable with those established using the direct method reported in current health standards.
4.Research progress on the mechanism of mitochondria-associated endoplasmic reticulum membranes in diabetic kidney disease

Diabetic kidney disease (DKD) is one of the major microvascular complications of diabetes. With the increasing prevalence of diabetes, it has become an important cause of end-stage renal disease, which seriously threatens the life and health of patients and aggravates the medical burden of society. The dysfunction of mitochondria and endoplasmic reticulum (ER) plays a key role in the progression of DKD, and the mitochondria-associated endoplasmic reticulum membrane (MAM) is the core hub of the dynamic interaction between the two. Mitochondrial dynamics and apoptosis affect the pathological process of DKD. This article systematically reviews the multiple molecular mechanisms of MAM in the occurrence and development of DKD, reveals its involvement in the fine regulatory network of kidney injury by regulating calcium ion (Ca2+) balance, glucose metabolism, inflammatory response and autophagy, and clarifies how MAM dysfunction drives the transformation of DKD into end-stage renal disease. In addition, this article deeply explores the potential of MAM-related biomarkers in early diagnosis, as well as innovative therapeutic strategies such as drug intervention and gene repair targeting MAM, which provides theoretical references for basic mechanism research and clinical practice of DKD.

6.Three-dimensional radiographic characteristics of 18 odontogenic myxoma

Objective:To analyze the there-dimensional radiographic characteristics of odontogenic myxoma(OM).Methods:Clinical records,histopathological reports,SCT or CBCT images of 18 patients with OM were retrospectively collected,and radiograph-ic features including lesion site,shape,size,expansion,intralesional septa and relationship with surrounding tissues were analyzed.The lesions were classified into types by the forms of septa.Results:18 patients(8 males and 10 females,aged 9-39 years old)were included.Among them,12 lesions(66.7%)were round-like in shape,7(38.9%)were located in the posterior maxilla,and 8(44.4%)in the posterior mandible.According to the septa map,the lesions included 4(22.2%)without septum(unilocular type),4 with(22.2%)sporadic septum on the periphery of the tumor(peripheral type),4(22.2%)with the septum from the periphery towards the center of the tumor(centripetal type)and 6(16.7%)with multiple septa interwoven as grid-like trabeclations(criss-crossing tup).Labial/buccal cortex expansion was present in 11 lesions(61.1%),and palatal cortex expansion in 12 lesions(66.7%).Moreover,cortex discontinuity occurred in 14 lesions(77.8%),impacted tooth was present in 9 lesions(16.7%),and root resorption was present in 8 lesions(44.4%).Conclusion:Most OMs occur in the posterior jaw,with multilocular and discontinued cortex.OMs can be classified into unilocular,perpheral,centripetal and cirss-crossing type by the forms of septa in the lesion.

10.Clinical characteristics of ABCB4 gene variant-associated cholestatic liver disease in adults

Objective:To investigate the clinical manifestations, pathological, and gene mutation characteristics of ABCB4 gene variant-associated cholestatic liver disease in adults. Methods:Eight adult cases of ABCB4 gene variant-associated cholestatic liver disease who were hospitalized in the Department of Hepatology, Fifth Medical Center of the People's Liberation Army General Hospital from May 2010 to December 2022 were enrolled in this study. The clinical manifestations, pathological features, gene variant features, and prognostic conditions were analyzed. Patient gene testing and biological information analysis were performed using whole-exome next-generation sequencing. SPSS 19.0 software was used to conduct descriptive analysis. Results:Among the eight adult cases of the ABCB4 gene variant, there were three males and five females, with a median age of onset of 24 (20, 37) years. There were three cases with a compound heterozygous variant in ABCB4, and the clinical phenotypes included two cases of progressive familial intrahepatic cholestasis type 3 and one case of intrahepatic cholestasis of pregnancy overlapping with low-phospholipid-associated cholelithiasis syndrome. There were five cases with a single heterozygous variant in ABCB4, and the clinical phenotypes included two cases of intrahepatic cholestasis of pregnancy overlapping with drug-induced liver injury and three cases of low-phospholipid-associated cholelithiasis syndrome. Imaging of all eight cases showed liver fibrosis, and six cases already had cirrhosis. All patients underwent liver histopathological examination, which mainly showed cholestasis and portal fibrosis in eight cases, small bile duct hyperplasia in seven cases, copper deposition in three cases, and cirrhosis in five cases. ABCB4 screening revealed 11 different mutations, including eight new mutations. The pathogenicity assessment showed that c.2394+82C>T (intron) was a benign mutation, and the rest were deleterious mutations. Ursodeoxycholic acid was the treatment for all patients, with a follow-up time of 7.5 (0.5, 12.7) years. One case died of end-stage liver disease, two cases developed cholestatic cirrhosis, and five cases were in stable condition. Conclusion:The adult ABCB4 gene variant-associated cholestatic liver disease are mostly single heterozygous mutations, the clinical phenotypes are diverse and overlapping, the disease is more severe in those who carried non-functional mutations.
