1. Primary thymic mucosa-associated lymphoid tissue lymphoma: 7 clinical cases report and a review of the literature

Objective:
To reveal clinical features, pathological diagnosis, treatment and prognosis of primary thymic mucosa-associated lymphoid tissue (MALT) lymphoma and review literatures.
Methods:
The clinical characteristics, pathological diagnosis, laboratory texts, treatment and prognosis of 7 cases of primary thymic MALT lymphoma identified at the First Affiliated Hospital of Nanjing Medical University from November 2017 to January 2019 were collected and analyzed.
Results:
Of 7 primary thymic MALT lymphoma cases, six were female. Patients were often asymptomatic and were found mediastinal mass by chest CT. After mediastinal mass resection, pathologist reported a primary thymic MALT lymphoma. Laboratory tests showed all patients were positive for anti-nuclear antibody, anti-Ro52 antibodies and anti-Sjogren’s syndrome A antibodies, and increased erythrocyte sedimentation rate (ESR) . Four were diagnosed with Sjogren’s syndrome (SS) . After surgery, the patients were given the positron emission tomography computed tomography (PET-CT) scans. All cases received "watch and wait" approach. Up to now, all cases showed good prognoses and none of them relapsed.
Conclusion
Primary thymic MALT lymphoma was rare, and it was often associated with autoimmune diseases. Such patients who usually had good prognoses should be followed up closely and avoided excessive treatments if there were no indications of intervention.
2.Characteristics and clinical significance of nailfold capillaroscopy in patients with systemic lupus erythematosus

Objective:To summarize the characteristics of nailfold capillaroscopy (NC) in patients with systemic lupus erythematosus (SLE) and explore its clinical significance.Methods:NC examination was performed in 162 SLE patients. The clinical data of SLE patients was collected. Tianniu NC scoring standard was used. The t test was applied to analyze the measurement data, the χ2 test was applied to analyze the counting data. the Pearson or Spearman test was used to evaluate the correlative factors of NC in patients with SLE. Results:NC abnormalities were seen in 87.7%(142/162) of SLE patients, and the incidence of mild, moderate and severe abnormalities was 29.0%(47 cases), 45.1%(73 cases) and 13.6%(22 cases) respectively. The most common NC abnormal manifestation in SLE patients was decreased blood flow velocity (86.4%). In patients with moderate to severe NC abnormalities, the proportions of patients with Raynaud's phenomenon (37.9% vs 23.9%, χ2=2.955, P=0.043) and interstitial lung disease (8.0% vs 0, χ2=5.213, P=0.023), and the level of D-Dimer [(1 992±2 279) μg/L vs (1 248±1 721) μg/L, t=-1.624, P=0.013] were significantly higher than those in the groups with normal/mild NC abnormalities. Correlation analysis demonstrated that Raynaud's phenomena, interstitial lung disease, pulmonary hypertension and D-Dimer were positively correlated with the NC abnormality. Conclusion:NC abnormalities are common in SLE patients. Decreased blood flow velocity is the most frequent manifestation. SLE patients with moderate to severe NC abnormalities should be actively screened for pulmonary hypertension and interstitial lung disease.
3. Time course of attention bias in face recognition of patients with depression

Objective:
To explore the characteristics of cognitive processing over time in recognition of different emotional faces in patients with depression.
Methods:
Totally 29 patients with depression(patient group) and 30 normal control (control group)were selected.Subjects freely viewed positive, sad, threatening and neutral facial stimuli.Eye movement data were collected during the process of watching, and the differences of attention bias in time when the two groups of subjects recognized the four emotional faces were compared.The temporal variation trend of attention between the two groups was analyzed.
Results:

4.Effects of femtosecond laser-assisted anterior capsulotomy versus manual continuous curvilinear capsulorhexis on intraocular lens centration

Objective To assess the effects of femtosecond laser-assisted anterior capsulotomy versus manual continuous curvilinear capsulorhexis on intraocular lens (IOL) centration by measuring and comparing shape,size and positional parameters.Methods Eighty cases (80 eyes) with age-related cataract from March 2015 to July 2016 in our hospital were collected.Forty patients (40 eyes) in the femtosecond group underwent femtosecond laser anterior capsulotomy and 40 patients (40 eyes) in the annular group were treated with annular continuous capsulorhexis.At postoperative 1 week,1 month,1 year,the slit lamp digital photography and Pentacam anterior segment analyzer were used to collect the anterior segment images,and the parameters include horizontal and vertical diameter,roundness,package and partial centre of IOL were analyzed by image analysis software image-pro-plus 6.0.Using repeated measurement of variance analysis,Fisher exact probability,and Generalized estimation equations (GEE),the differences between parameters of the groups and IOL decentration predictor were analyzed.Results The vertical diameter,roundness,maximum distance,minimum distance,package and horizontal deviation were statistically significant between the femtosecond group and the annular group at 1 week and 1 month after operation (all P < 0.05),and there was no significant difference in horizontal diameter and vertical deviation between two groups (all P > 0.05).At 1 year after operation,the maximum distance,minimum distance,package and horizontal deviation between two groups were statistically significant (all P < 0.05).There was no significant difference in vertical and horizontal diameter,roundness,and vertical deviation between two groups (all P > 0.05).There was no patient with horizontal shift > 0.4 mm in the femtosecond group at 1 week,1 month and 1 year;The ratio of cases with horizontal shift >0.4 mm and <0.4 mm in the annular group at 1 week,1 month and 1 year were 2/38,4/36,8/32,respectively.There was significant difference in the horizontal shift > 0.4 nun between the two groups after 1 year(P < 0.05).In the univariate generalized estimating equation model,the capsulorhexis type was a significant predictor of horizontal shift.Of all the capsulorhexis parameters,the difference of inclusion degree was statistically significant (P =0.001).There was negative correlation between the horizontal shift and package at different time points in the femtosecond group at 1 week,1 month and 1 year (r =-0.89,P < 0.05;r =-0.72,P < 0.05;r =-0.58,P < 0.05);There was negative correlation between the horizontal shift and package at different time points in the annular group at 1 week(r =-0.68,P < 0.05),but no correlation at 1 month and 1 year (r=-0.41,P>0.05;r=-0.33,P>0.05).Conclusion The anterior capsule made by femtosecond laser anterior capsulotomy is more round,centered and accurate,pocket IOL position is more stabled and neutral.

Objective:
To investigate the clinically and genetic characteristics of children with Leigh syndrome.
Method:
Patients with clinically diagnosed Leigh syndrome(LS)in the department of Neurology, Beijing Children′s Hospital from January 2013 to February 2016 underwent the mitochondrial DNA (mtDNA) and nuclear DNA (nDNA) detecting with next generation sequencing (NGS) technology. The clinical data of gene confirmed cases were retrospectively collected and analyzed. The differences in the onset age, clinical manifestations, lactic acid level and MRI results between the mtDNA variation and nDNA variation were compared and analyzed.
6.Prevention and nursing of complications of PFNA internal fixation in elderly patients with femoral trochanteric fracture

Objective To investigate the prevention and nursing of complications of proximal femoral anti-intramedullary nail (PFNA) internal fixation in elderly patients with femoral trochanteric fracture.Methods A total of 78 patients with PFNA internal fixation were randomly divided into control group and experimental group (n =39).The control group was treated with routine nursing,and the patients in the experimental group were treated with personalized quality nursing on the basis of control group.Complications and nursing effect of two group were compared.Results The overall satisfaction of the experimental group was higher (P < 0.05),and the incidence of complications was lower than that that in the control group (P < 0.05).Conclusion The effect of high quality nursing intervention in patients with femoral fractures of PFNA is significant,which can reduce the postoperative complications,reduce the physical and mental pain and improve the quality of life.So it is worthy of promotion.

9.Method of detection of negative terminology in Chinese electronic medical record.

The method for detecting the negative terms in Chinese electronic medical record (EMR) is useful in providing evidence for constructing concept index. In this respect, we adopted an improved method which combined maximum matching with mutual information in order to extract terms in EMRs. This method can overcome the influence of overlay ambiguity. In addition, for the determination of negative semantic, we also adopted an improved method which combined rule-based method with word co-occurrence. This new method can reduce the probability of appearance of false positive terms caused by punctuation input errors. The result showed that the negative predictive value is 7.85% higher than the rule-based method.

10.Study on Information Extraction of Clinic Expert Information from Hospital Portals.

Clinic expert information provides important references for residents in need of hospital care. Usually, such information is hidden in the deep web and cannot be directly indexed by search engines. To extract clinic expert information from the deep web, the first challenge is to make a judgment on forms. This paper proposes a novel method based on a domain model, which is a tree structure constructed by the attributes of search interfaces. With this model, search interfaces can be classified to a domain and filled in with domain keywords. Another challenge is to extract information from the returned web pages indexed by search interfaces. To filter the noise information on a web page, a block importance model is proposed. The experiment results indicated that the domain model yielded a precision 10.83% higher than that of the rule-based method, whereas the block importance model yielded an F₁ measure 10.5% higher than that of the XPath method.
