1.Advances in drug therapy of metastatic castration-resistant prostate cancer

Metastatic castration-resistant prostate cancer (mCRPC) implies highly heterogeneous molecular tumor behavior, rapid disease progression and poorer prognosis, seriously threatening the lives of patients.Therefore, early identification and treatment are significant.Among the current clinical treatment regimens for mCRPC, abiraterone acetate, enzalutamide and docetaxel have demonstrated improvement in overall survival (OS).Furthermore, patients carrying homologous recombination repair mutation (HRRm) are sensitive to poly ADP-ribose polymerase inhibitors (PARPi).For patients with positive prostate specific membrane antigen (PSMA), Lutetium-177 (177Lu)-PSMA-617 radiation ligand therapy is a new and effective treatment method.Radium-223 has also been used as the first-line treatment for patients with bone pain symptoms.However, due to the rapid updates in the field of mCRPC drug therapy, clinicians find it difficult to keep up with the latest developments in this area.Emerging drugs have great potential, such as immune checkpoint inhibitors (ICIs) and antibody-drug conjugates (ADCs), but more evidence is needed, and molecular markers helping predict the efficacy of these drugs should be explored.This article reviews the advances in the drug therapy of mCRPC, with the aim of providing reference for clinical practice.

Purpose/Significance To explore the technical key points and implementation paths of relevant policies,and provide ref-erence for the planning and construction of future smart healthy cities.Method/Process It reviews and analyzes domestic and internation-al policy progress in the field of smart healthy cities,deeply analyzes policy documents,reveals the evolution trajectory,core elements,and driving effects on urban health development.Result/Conclusion Establishing a framework for health informatization,resource net-working,intelligent services,and integrated supervision can effectively address urban health challenges,provide efficient health services,and improve residents'quality of life and hygiene level.Policies such as optimizing the allocation of medical resources,promoting coordi-nation and cooperation among medical institutions,and expanding the health industry will jointly promote the sustained progress of urban health ecosystems.
3.A series case study on anthrax cases in five hospitals in North China

Objective:To conduct a series case study on hospitalized anthrax cases in five hospitals in North China, to share clinical experiences in the diagnosis and treatment of cutaneous and pulmonary anthrax.Methods:A retrospective, multicenter cohort study was conducted on the anthrax patients admitted to five hospitals in North China from August 2018 to March 2022. Forty patients were divided into severe and mild groups. The clinical features, treatment and prognosis of the patients were collected and analysed. Statistical evaluations included independent sample t test, Mann-Whitney U test, and chi-square test. Results:Among the 40 patients with anthrax, 10(25.0%) were severely ill and 30(75.0%) were mildly ill. According to the sites of infection, 40 patients were classified as 39 cutaneous anthrax cases (one case had secondary pulmonary anthrax) and one pulmonary anthrax case. The rates of chills and fever, lymphadenopathy, liver dysfunction and hypoalbuminemia in the severe group were all higher than those in the mild group, with statistically significant differences ( χ2=5.71, 6.54, 4.68 and 9.22, respectively, all P<0.05). The peripheral white blood cell count, neutrophil count, neutrophil/lymphocyte ratio and C-reactive protein were (11.8±4.9)×10 9/L, (9.5±5.1)×10 9/L, 8.6±7.3, 27.9(8.6, 167.7) mg/L, respectively, which were all higher than those in mild disease group ((7.5±2.4)×10 9/L, (5.0±2.1)×10 9/L, 3.2±2.3, 3.5(1.2, 14.7) mg/L), with statistically significant differences ( t=2.66, t=2.71, t=2.32 and Z=-3.01, respectively, all P<0.05). The albumin level in the severe group was (35.5±8.1) g/L, which was lower than that of the mild group ((43.7±3.2) g/L), and the difference was statistically significant ( t=-3.13, P=0.011). The severe cases were more likely to have skin lesions greater than four centimetre in diameter, multiple, vesicular, or edematous, with a significant difference ( χ2=6.01, P=0.014). Among 39 patients with cutaneous anthrax, 28(71.8%) in the mild group were treated with penicillin alone, and nine (23.1%) in the severe group were treated with penicillin, ofloxacin, piperacillin/tazobactam combined with one of linezolid, doxycycline, or clindamycin for anti-infection treatment. The two patients with pulmonary anthrax were treated with closed thoracic drainage for pleural effusion and pneumothorax, and were treated with two bactericidal and one protein synthesis inhibitor antibiotics. All 40 anthrax patients were cured and discharged from hospital. Conclusions:Patients with mild cutaneous anthrax can generally be treated with single penicillin, and patients with severe cutaneous anthrax and pulmonary anthrax should be treated with combined antibiotics.
4.The effect of butylphthalide injection combined with interventional therapy on patients with acute anterior circulation large vessel occlusive stroke

Objective To investigate the effect of butylphthalide and sodium chloride injection on patients who received endovascular treatment for acute anterior circulation large vessel occlusive stroke.Methods A total of 173 patients were identified from February 2015 to December 2017 in the Department of Neurology of Jingling Hospital in this retrospective observational study.Propensity score-matching analysis was performed to balance differences in baseline characteristics between patients who received butylphthalide injection (butylphthalide group) and those who did not (control group).The modified Rankin Scale scores at 90 days were compared between the butylphthalide and control groups.Results A total of 144 patients who received endovascular treatment for acute anterior circulation large vessel occlusive stroke were finally analyzed,54 cases in the butylphthalide group and 90 cases in the control group.The proportion of good functional outcome at 90 days in the butylphthalide group was higher than that in the control group (63.0% (34/54) vs 44.4% (40/90);x2=4.633,P=0.031).Thirty-six pairs were matched successfully by the propensity score matching,36 patients in the butylphthalide group and 36 in the control group.There was no statistically significant difference in the 90-day functional outcome between the two groups (66.7% (24/36) vs 44.4% (16/36);x2=3.600,P=0.058).One hundred and fifteen patients were recanalized,47 cases in the butylphthalide group and 68 cases in the control group,and after the propensity score matching,30 pairs were analyzed.The proportion of good functional outcome at 90 days in the butylphthalide group was higher than that in the control group (73.3% (22/30) vs 46.7% (14/30);x2=4.444,P=0.035).Conclusion After propensity score-matching,butylphthalide and sodium chloride injection could improve 90-day functional outcome in patients with acute anterior circulation large vessel occlusive stroke and obtained recanalization by endovascular treatment while could not before propensity score-matching.
5.Prognostic evaluation of high sensitivity-C reactive protein in peripheral T-cell lymphoma

Objective:To investigate the prognostic significance of high sensitivity-C reactive protein (Hs-CRP) in patients with peripher-al T-cell lymphoma (PTCL). Methods:A total of 234 newly diagnosed PTCL patients with a median age of 48 years were analyzed retro-spectively. Serum Hs-CRP levels and other factors, including tumor stage and international prognostic index (IPI), were determined. Af-ter a median follow-up of 23 months, the relationship between Hs-CRP and overall survival (OS) was observed. Results:Serum Hs-CRP level positively correlated with IPI score (r=0.132, P<0.001), tumor stage (r=0.183, P=0.005), B symptoms (r=0.225, P=0.001), and lactic dehydrogenase (r=0.169, P=0.009), but negatively correlated with plasma albumin levels (r=?0.343, P<0.001), hemoglobin concentra-tion (r=?0.239, P<0.001), and platelet count (r=0.131, P=0.045), and is uncorrelated with age (P>0.05), gender (P>0.05), fitness score (P>0.05), and leukocyte count (P>0.05). Patients with serum Hs-CRP levels≤10 mg/L had better OS than patients with serum Hs-CRP levels>10 mg/L. Univariate and multivariate Cox regression models showed that platelet count, Hs-CRP, albumin levels, and IPI score were independent adverse prognostic factors. Conclusion:The baseline Hs-CRP level can serve as a major indicator of prognosis in PT-CL patients.
6.Expression of PRAME gene in acute leukemia and its clinical significance compared with FLT3 gene

Objective To investigate the expression of preferentially expressed antigen of melanoma (PRAME) gene in acute leukemia and its clinical significance.Methods PRAME mRNA levels were detected in bone marrow samples from 119 cases with acute leukemia (out of which 97 cases were acute myeloid leukemia, 22 cases were acute lymphocytic leukemia) by TaqMan based real-time quantitative PCR methods and compared it with the expression of FLT3 gene. Results PRAME mRNA was revealed in 50 of 97 AML (51.5 %), 8 of 22 ALL (36.4 %), and 5 patients were found little expression of PRAME in 11 healthy subjects and 17 non-blood disease patients. However, 17 AML, and 3 ALL patients with PRAME expression had no detectable FLT3. 9 patients with the expression of both PRAME and FLT3 genes were monitored in short follow-up. The lower level of PRAME was detected in all patients when they were in complete remission (CR) after chemotherapy. Conclusion PRAME gene was expressed in acute leukemia patients and will hopefully become a symbolic gene during the course of diagnosis and treatment in acute leukemia.
