1.Current status of registration of radiopharmaceutical clinical trials

Objective:To analyze the status of registration of clinical trials of radiopharmaceuticals in China, and to provide reference for the development and clinical application of radiopharmaceuticals.Methods:By searching the clinical trial registration and information disclosure platform of the Center for Drug Evaluation of the National Medical Products Administration (NMPA), the data on clinical trials of radiopharmaceuticals registered from 2014 to 2024 were collected and analyzed for trial design, administered dose, common indications, and geographical distribution.Results:A total of 77 clinical trials were included. The Compound Annual Growth Rate for the number of projects from 2014 to 2024 was 40%. Diagnostic radiopharmaceuticals were predominantly based on 18F and 99Tc m, while therapeutic radiopharmaceuticals primarily utilized 177Lu and 90Y. All indications were concentrated in the field of oncology. Regarding trial design, non-randomized (71.4%), open-label (89.6%), and single-arm (66.2%) trials accounted for the highest proportions. Geographical distribution showed Beijing (29 trials), Shanghai (18 trials), and Jiangsu province (14 trials) as the regions with the highest concentration of clinical trials. Conclusions:Radiopharmaceutical clinical trials in China have shown rapid growth. However, research remains predominantly focused on oncology, with a relatively high proportion of early-stage trials. In order to fully utilize the potentials of radiopharmaceuticals and improve the quality of clinical trials, nuclear medicine researches should broaden therapeutic applications, implement prudently administerd dose in clinical trials, and implement optimized radiation protection procedures across all clinical trial centers.
2.The value of whole exome sequencing in the etiological diagnosis and treatment of urolithiasis

Objective:To evaluate the role of whole exome sequencing(WES)in the etiological diagnosis and precision medicine management of patients with urolithiasis.Methods:We conducted a retrospective review of 21 patients with urolithiasis and pathogenic gene mutations identified by WES at The Second Affiliated Hospital of Zhengzhou University between April 2019 and March 2025. The cohort included 13 males and 8 females,with a mean age of(18.9 ± 11.1)years;18 patients were under 25 years old. Clinical presentations included nephrocalcinosis(8 patients)and urinary tract calculi(13 patients),with five patients exhibiting extra-renal manifestations such as renal tubular acidosis and hyperaldosteronism. Stone composition analysis identified calcium oxalate(16 patients),cystine(4 patients),and carbonate apatite(1 patient). Metabolic abnormalities were prevalent,including hypocitraturia(11 patients),hyperoxaluria(8 patients),and hypercalciuria(7 patients),with eight patients presenting two or more concurrent disorders. All patients underwent WES and comprehensive metabolic evaluation. Sequencing was performed on an Illumina Hiseq4000 platform,achieving a mean depth of > 100× and coverage of > 98% in target regions. Variants were classified according to the American College of Medical Genetics and Genomics(ACMG)guidelines.Results:WES identified 12 distinct genes across autosomal recessive(9 genes: AGXT, GRHPR, ATP6V1B1, SLC12A1, KCNJ1, SLC3A1, SLC7A9, SLC34A3, WFS1),autosomal dominant(2 genes: CASR, ADCY10),and X-linked recessive(1 gene: CLCN5)inheritance patterns. Genotype-phenotype correlations revealed mutations associated with primary hyperoxaluria(8 patients),hypercalciuria(7 patients),and renal malformation due to a WFS1 mutation(1 patient). A positive genetic diagnosis was achieved in 100% of patients with either urinary oxalate > 1 000 μmol/24 h or cystine stones. 8 patients received a diagnosis of hereditary stone disease at their first presentation(non-delayed group),while 13 experienced a mean diagnostic delay of(9.6 ± 3.9)years. The delayed diagnosis group had a significantly older age at initial stone onset[(10.2 ± 5.3)years vs.(6.8 ± 3.1)years, P = 0.03]and a higher incidence of impaired renal function(6 patients vs. 1 patient, P = 0.04). Analysis of diagnostic delay by gene subgroup showed delays in 2/4 patients with cystinuria[ SLC3A1/ SLC7A9;(8.2 ± 3.5)years],5/8 with primary hyperoxaluria[ AGXT/ GRHPR;(10.5 ± 4.1)years],5/7 with hypercalciuria-related genes[ CASR/ ADCY10/ SLC12A1/ KCNJ1/ SLC34A3;(9.8 ± 3.8)years],and 1/2 with other genes[ ATP6V1B1/ WFS1/ CLCN5;(7.6 ± 2.2)years]. Among 32 mutation sites detected,21 were classified as pathogenic/likely pathogenic and 11 as variants of uncertain significance. Four novel mutations were identified: ATP6V1B1(presenting with renal tubular acidosis,nephrocalcinosis,and hypocitraturia), WFS1(presenting with renal malrotation,hydronephrosis,and stones without metabolic abnormalities), SLC12A1(presenting with Bartter syndrome type 1,chronic renal insufficiency,hypercalciuria,hypocitraturia,alkalosis,and hyperaldosteronism),and SLC3A1(presenting with bilateral renal stones and cystinuria). Conclusions:WES is crucial in identifying the underlying etiology of urolithiasis and can guide targeted treatment. We recommend early WES for patients with an initial stone presentation before age 25,those with nephrocalcinosis,or those with abnormal metabolic workups to facilitate precise diagnosis and preventive care.

4.Effects of cytomegalovirus infection on gene expression and signaling pathways in preauricular somatic cells

Objective:To analyze the effects of cytomegalovirus (CMV) infection on gene expression and signaling pathways in preauricular somatic cells, and to explore potential intervention approaches for neonatal hearing loss caused by CMV infection.Methods:CMV infection data (GSE234062) were retrieved and downloaded from the GEO database. R studio software was used to analyze the differentially expressed genes (DEGs) between the CMV-infected group and the control group. ExpressAnalyst and String tools were employed to analyze the enrichment of DEGs in signaling pathways, biological processes, molecular functions, and protein-protein interactions.Results:After CMV infection of preauricular somatic cells, the expression levels of viral RNA and green fluorescent protein (GFP) RNA increased significantly. Meanwhile, 233 genes were upregulated and 123 genes were downregulated post-infection. Signaling pathway analysis showed that the upregulated genes were mainly enriched in pathways such as calcium ion channels, GABA synapses, morphine addiction, and cGMP-PKG, while the downregulated genes were primarily enriched in pathways including focal adhesion, extracellular matrix (ECM)-receptor interaction, amebiasis, and proteoglycans in cancer. Biological process analysis revealed that the upregulated genes were closely associated with neuron development and cell maturation, whereas the downregulated genes were mainly involved in actin filament-dependent processes, tissue development, and vascular development. Molecular function enrichment analysis indicated that the upregulated genes were mainly clustered in sequence-specific DNA binding, RNA polymerase Ⅱ distal enhancer binding, and G protein-coupled amine receptor activity, while the downregulated genes were closely related to molecular functions such as actin binding, structural molecule activity, and extracellular matrix structure.Conclusions:CMV infection can activate calcium ion channel and GABA synaptic pathways, and impair focal adhesion and ECM-receptor interaction pathways in preauricular somatic cells. This suggests that targeting these related pathways may help alleviate neonatal hearing impairment caused by CMV infection.

7.Syndromes and Mechanisms of Depression Induced by Second Hit in Mice

ObjectiveTo explore the syndromes and mechanisms of depression induced by maternal separation (MS) combined with chronic restraint stress (RS) in mice. MethodOn postnatal day 0 (PD0), the offspring mice were randomized into a blank group (NC) and a modeling group. The mouse model of depression was established by MS+RS for 21 days. After removal of female mice on PD21, the modeled mice were randomized into model, Wenyang, Jieyu, Wenyang Jieyu, and fluoxetine groups, with 15 mice in each group. The sucrose preference, tail suspension, and open field tests were carried out to evaluate the anxiety and depression-like behavior in mice. Enzyme-linked immunosorbent assay was used to measure the adrenocorticotrophic hormone (ACTH) and corticosterone (CORT) levels in mouse plasma. High performance liquid chromatography-electrochemical detector was used to determine the content of monoamine neurotransmitters in the hippocampus. Real-time fluorescence quantitative polymerase chain reaction was employed to determine the mRNA levels of genes in the 5-hydroxytryptamine (5-HT) system, hypothalamic-pituitary-adrenal (HPA) axis, and brain-derived neurotrophic factor (BDNF) signaling pathway in the hippocampus. Immunohistochemistry was employed to determine the expression levels of proteins in the 5-HT system and HPA axis in the hippocampus. The Simple Western system was used to determine the protein levels of BDNF and tyrosine kinase receptor B (TrkB) in the hippocampus. ResultCompared with the NC group, the model group exhibited depression-like behavior, which was significantly relieved by Wenyang Jieyu prescription and fluoxetine. Compared with the NC group, the model group showed elevated levels of CORT and ACTH in the plasma (P<0.01), which, however, were lowered by Wenyang Jieyu prescription and fluoxetine (P<0.05, P<0.01). Compared with the NC group, the model group showed inhibited expression of neurotransmitters in the hippocampus (P<0.05, P<0.01), while Wenyang Jieyu prescription and fluoxetine restored the expression of neurotransmitters (P<0.05, P<0.01). Compared with NC group, the model group showed inhibition of the 5-HTergic nerve and abnormal activation of the HPA axis, and Wenyang Jieyu prescription and fluoxetine regulated the abnormal state of the 5-HTergic nerve and HPA axis. Compared with NC group, the modeling down-regulated the mRNA and protein levels of BDNF and TrkB in the hippocampus (P<0.05, P<0.01), which, however, were recovered in Wenyang, Jieyu, Wenyang Jieyu, and fluoxetine groups (P<0.05, P<0.01). ConclusionThe mouse model of depression induced by MS+RS may present the syndrome of Yang deficiency and liver depression. Wenyang Jieyu prescription may increase the content of hippocampal neurotransmitters by regulating the 5-HT system and the BDNF signaling pathway mediated by the HPA axis, thereby alleviating depression-like behavior in mice.
8.Wenyang Jieyu Prescription Regulates Hippocampal Neural Plasticity in Depressed Mice via NLRP3/Caspase-1/IL-1β Pathway

ObjectiveTo explore the effects of Wenyang Jieyu prescription (WJP) on neuroinflammation and synaptic plasticity in the mouse model of depression induced by maternal separation combined with restraint stress. MethodThe mice on postnatal day 0 (PD0) were randomized into a control group and a modeling group. Maternal separation combined with restraint stress was employed to establish the mouse model of depression. After the removal of female mice, the modeled mice were randomized into model, Wenyang prescription (5.85 g·kg-1), Jieyu prescription (12.03 g·kg-1), WJP (16.71 g·kg-1), and fluoxetine (2.6 mg·kg-1) groups on the weaning day (PD21), with 15 mice in each group. The mice were administrated with corresponding drugs mixed with the diet from PD21 to PD111. The sucrose preference test, open field test, O-maze test, and novel object recognition test were then carried out to evaluate the depression state, memory, and learning ability of the mice. Immunohistochemistry (IHC) was employed to observe the ionized calcium-binding adapter molecule-1 (Iba-1) in hippocampal microglia. High performance liquid chromatography (HPLC) was employed to measure the content of noradrenaline (NE) and epinephrine (E) in the hippocampus. Enzyme-linked immunosorbent assay (ELISA) was employed to determine the content of interleukin (IL)-18 and IL-1β in the hippocampus. Western blot was employed to determine the protein levels of NOD-like receptor protein 3 (NLRP3), apoptosis-associated speck-like protein containing a CARD (ASC), cysteine aspartate-specific protease-1 (Caspase-1), IL-1β, synaptophysin (Syn), and postsynaptic density 95 (PSD95). ResultCompared with control group, the model group showed decreased sucrose preference rate, time spent in central zone within 5 min, total movement distance, time spent in the open arm, and cognition index (P<0.05, P<0.01). The microglia in the model group presented amoeba-like appearance, the Iba1 increased. Moreover, the model group showed decreased content of NE and E (P<0.01), elevated levels of IL-1β and IL-18 (P<0.01), down-regulated protein levels of PSD95 and Syn (P<0.05, P<0.01), and up-regulated protein levels of NLRP3, ASC, Caspase-1, and IL-1β (P<0.05, P<0.01). Compared with model group, WJP and fluoxetine increased the sucrose preference rate, time spent in central zone within 5 min, total movement distance, time spent in the open arm, and cognition index (P<0.05, P<0.01). They recovered the microglia and the Iba1 decreased. Moreover, the drugs increased the content of NE and E (P<0.05, P<0.01), lowered the levels of IL-1β and IL-18 (P<0.01), up-regulated the protein levels of PSD95 and Syn (P<0.01), down-regulated the protein levels of NLRP3, ASC, Caspase-1, and IL-1β (P<0.05, P<0.01). ConclusionWJP can treat the depressive behavior induced by maternal separation combined with restraint stress in mice, with the performance outperforming Wenyang prescription and Jieyu prescription. It may alleviate the neuroinflammation induced by microglia and improve the synaptic plasticity by regulating the NLRP3 pathway and increasing neurotransmitters in the hippocampus.
9.Wenyang Jieyu Prescription Regulates Hippocampal Neuron Apoptosis and Improves Synaptic Plasticity in Depressed Mice via BDNF/Akt/mTOR Pathway

ObjectiveTo explore the mechanism of Wenyang Jieyu prescription in regulating hippocampal neuron apoptosis and improving synaptic plasticity in the mouse model of depression induced by maternal separation combined with restraint stress. MethodThe mice on postnatal day 0 (PD0) were randomly assigned into a control group (n=10) and a modeling group (n=50). Maternal separation combined with restraint stress was adopted to establish the mouse model of depression, and the modeled mice were randomized into model, Wenyang prescription, Jieyu prescription, Wenyang Jieyu prescription, and fluoxetine groups (n=10) on the weaning day (PD21). From PD21 to PD111, the mice were fed with the diets mixed with corresponding medicines. The sucrose preference test, open field test, O-maze test, and novel object recognition test were then conducted to evaluate the depression, memory, and learning abilities of mice. Immunohistochemistry (IHC) was employed to measure the atomic absorbance (AA) of postsynaptic density protein 95 (PSD95) in the hippocampus. Terminal-deoxynucleoitidyl transferase-mediated nick-end labeling (TUNEL) was employed to detect the apoptosis of hippocampal neurons. Western blot was employed to determine the protein levels of brain-derived neurotrophic factor (BDNF), phosphorylated tyrosine kinase receptor B/tyrosine kinase receptor B (p-TrkB/TrkB), phosphorylated protein kinase B/protein kinase B (p-Akt/Akt), phosphorylated mammalian target of rapamycin/mammalian target of rapamycin (p-mTOR/mTOR), B-cell lymphoma-2 (Bcl-2), Bcl-2-associated X (Bax), cysteinyl aspartate-specific proteinase-3 (Caspase-3), synaptophysin (Syn), and PSD95. ResultCompared with the control group, the modeling decreased the sucrose preference rate, time spent in central zone within 5 min, total movement distance, time spent in the open arm, and cognition index (P<0.01). Furthermore, it decreased the expression of PSD95, increased the neuron apoptosis in the hippocampus (P<0.01), down-regulated the protein levels of BDNF, p-TrkB/TrkB, p-Akt/Akt, p-mTOR/mTOR, Bcl-2, PSD95, and Syn (P<0.01), and up-regulated the protein levels of Bax and Caspase-3 (P<0.05) in the hippocampus. Compared with the model group, Wenyang Jieyu prescription and fluoxetine increased the sucrose preference rate, time spent in central zone within 5 min, total movement distance, time spent in the open arm, and cognition index (P<0.05, P<0.01). Moreover, the drugs increased the expression of PSD95, reduced the neuron apoptosis (P<0.01), up-regulated the protein levels of BDNF, p-TrkB/TrkB, p-Akt/Akt, p-mTOR/mTOR, Bcl-2, PSD95, and Syn (P<0.01), and down-regulated the protein levels of Bax and Caspase-3 (P<0.01). ConclusionWenyang Jieyu prescription outperformed Wenyang prescription and Jieyu prescription in the treatment of the depressive behavior induced by maternal separation combined with restraint stress in mice. It exerted the therapeutic effect by reducing the hippocampal neuron apoptosis and improving the synaptic plasticity via the BDNF/Akt/mTOR pathway.
10.Mechanism of serum exosomal miR-17-5p on regulation of Treg cells in premature ovarian insufficiency

Objective:To explore the involvement of serum exosomal miR-17-5p in the pathogenesis of regulatory T (Treg) cells deficiency in premature ovarian insufficiency (POI) patients.Methods:In a case-control study, 32 Chinese Han women with POI and 32 Chinese Han women with regular menstrual cycles (healthy control, HC) attending the Department of Gynecological Endocrinology, the First Affiliated Hospital of Nanjing Medical University from January 2019 to December 2020 were recruited. The relative expressional level of serum exosomal miR-17-5p was confirmed by reverse transcription quantitative polymerase chain reaction (RT-qPCR). To understand how POI-exosome affects Treg cells, peripheral blood mononuclear cells (PBMC) of healthy volunteer were transfected with miR-17-5p, and cultured with PBS as the control or with exosomes derived from POI women or HC for 72 h. Subsequently, to evaluate the functionality of the miR-17-5p upregulated in POI-exosome, native CD4 + T cells from healthy donors were sorted by magnetic beads and induced into induced Treg (iTreg) cells in vitro, and the induction efficiency, the proliferation and apoptosis rates of iTreg cells were detected after transfection with miR-17-5p mimic/inhibitor. TargetScan, miRDB, miRTarBase and miRWalk were used to predict targets gene of miR-17-5p. Results:The expression level of serum exosomal miR-17-5p was markedly up-regulated in POI compared with HC ( P<0.001). Furthermore, after culture with POI-exosome [(0.93±0.40)%], the frequency of Treg cells was significantly reduced as compared with that culture with PBS [(3.77±0.89)%, P=0.005]. Subsequently, the frequency of Treg cells in miR-17-5p mimic group [(4.30±1.91)%] was significantly lower than that in negative control group [(11.97±1.82)%, P=0.007]. In contrast, both Th1 and Th17 did not be affected (both P>0.05). The induction efficiency of iTreg cells did not be influenced by transfection with miR-17-5p (both P>0.05), however, the iTreg cells apoptosis rate in miR-17-5p mimic group [(16.31±1.27)%] was significantly higher than that in negative control group [(13.01±1.80)%, P=0.035], while apoptosis rate was significantly lower [(7.97±0.71)%] and proliferation rate was higher [(88.83±0.25)%] in miR-17-5p inhibitor group than those in negative control group [(11.42±0.23)%, P=0.001; (87.60±0.70)%, P=0.045]. Transforming growth factor beta receptor 2 was most likely targets gene of miR-17-5p. Conclusion:The exosomal miRNA profile of patients with POI differed from that of HC, and miR-17-5p, which is markedly increased in POI patients, decreased the frequency of Treg cells by inhibiting iTreg cell proliferation and promoting iTreg cells apoptosis. Thus, our study suggested that exosomal miR-17-5p promoted iTreg cells apoptosis and resulted in Treg cells deficiency in POI pathogenesis.
