1.Analysis of Influencing Factors on TCM Syndromes of Chronic Gastritis from the Perspective of Five Circuits and Six Qi

Objective To explore the correlation between traditional Chinese medicine(TCM)syndromes of chronic gastritis and the indicators of five circuits and six qi,revealing the influence of different congenital qi endowments on TCM syndrome differentiation and providing evidence for individualized treatment.Methods Medical records meeting inclusion/exclusion criteria were collected from the Hospital Information System(HIS)of Rui Bao Street Community Health Service Center in Haizhu District,Guangzhou between January 20,2017 and January 20,2023.Data included age,birth date,and syndrome differentiation.Yearly circuit(Suiyun),celestial control-terrestrial effect(Sitian-Zaiqian)qi,three patterns of five circuits(Wuyun-Sanji),and syndrome patterns were statistically analyzed using Excel for database construction,with SPSS 23.0 for x2 tests and Logistic regression.Results(1)Among 1 882 chronic gastritis patients,spleen-stomach weakness(25.3%,477/1 882)was the most prevalent syndrome,followed by liver-stomach disharmony type,damp-heat in the spleen-stomach type,stomach-yin deficiency type,and stasis obstruction of stomach collateral type(x2=69.950,P=0.000).(2)Significant differences existed in Suiyun(Taijiao,Taigong,Taiyu,Taizhi,Shaogong,Taishang),Sitian-Zaiqian(Shaoyin-Yangming,Taiyin-Taiyang,Yangming-Shaoyin,Taiyang-Taiyin),and Wuyun-Sanji(excessive/insufficient earth/metal/water circuit)distributions among syndromes(P<0.05 or P<0.01).(3)For Suiyun-syndrome relationships:Birth in Taijiao/Shaogong years was the high-risk factor for developing chronic gastritis of liver-stomach disharmony type(P<0.01);Taishang years were associated with stomach yin deficiency type(P<0.05);Taijiao/Shaogong/Taiyu years were protective against damp-heat in the spleen-stomach type(P<0.05 or P<0.01).(4)For Sitian-Zaiqian-syndrome relationships:Jueyin-Shaoyang/Yangming-Shaoyin years linked to spleen-stomach weakness type(P<0.05);Shaoyang-Jueyin years were correlated with damp-heat in the spleen-stomach type(P<0.05).(5)For Wuyun-Sanji-syndrome relationships:Insufficient earth/water circuit years were associated with liver-stomach disharmony type(P<0.05 or P<0.01);excessive/balanced metal circuit years linked to stomach yin deficiency type(P<0.05 or P<0.01);insufficient metal circuit years were correlated with damp-heat in the spleen-stomach type(P<0.05).Conclusion TCM syndromes of chronic gastritis are influenced by environment-body interactions,disordered visceral function and yin-yang imbalance.Clinical treatment should integrate the varied indicators of five circuits and six qi variations for personalized regimens to improve symptoms and quality of life.
2.Effects of harmonizing needle-knife therapy on joint function and lower limb mechanics in rats with knee osteoarthritis

Objective This study aimed to observe the effects of Yin-Yang balancing acupotomy intervention on knee-joint function and lower limb biomechanics in a rat model of knee osteoarthritis(KOA),and to explore the mechanisms of acupotomy when treating KOA.Methods Forty SD rats were randomly divided into a blank group,a model group,an acupotomy group,and a medication group.Except for the blank group,KOA models were established by injecting a mixed solution of 4%papain and 0.03 mol/L L-cysteine into the left knee-joint cavity.The acupotomy group received Yin-Yang balancing acupotomy interventions targeting the medial/lateral collateral ligaments and patellar ligament.The medication group received daily oral celecoxib(10 mg/(kg·d)).Interventions began on day 7 post-modeling,and occurred once weekly for 4 weeks.All rats were assessed pre-and post-intervention using the modified Lequesne MG knee-joint grading system and rotarod fatigue test.Post-intervention,in vivo DR imaging was used to measure joint space width.Cartilage morphology was evaluated via HE and safranin O-fast green staining.Ligament biomechanical tensile testing was performed.Serum and cartilage tissues were analyzed by ELISA and Western Blot for matrix metalloproteinase-13(MMP-13)expression.Results(1)Compared with the blank group,the model group showed increased modified Lequesne MG scores,reduced rotarod endurance time,and narrowed joint space(P<0.01).(2)Compared with the model and medication groups,the acupotomy group exhibited lower Lequesne MG scores,prolonged rotarod endurance time(P<0.05),and expanded joint space(P<0.05).(3)The elastic modulus of ligaments in the acupotomy group showed no significant difference from those in the model group but was higher than those in the medication group.Yield strength,maximum strain,and yield-to-tensile strength ratio in the acupotomy group were higher than those in the model and medication groups(P<0.05).(4)HE and Safranin O-Fast green staining revealed minimal inflammatory infiltration in the acupotomy group compared with the model group.Cartilage surfaces in the acupotomy group were smoother than those in the medication group.(5)ELISA showed reduced serum MMP-13 levels in the acupotomy group versus the model group(P<0.01),and no significant differences between levels in the drug and acupotomy groups.(6)Cartilage MMP-13 expression in the acupotomy group was significantly lower than that in the model group(P<0.01)and lower than that in the medication group(P<0.05).Conclusions Acupotomy intervention enhances knee joint stability,improves lower limb mechanical alignment,and suppresses MMP-13 expression in KOA rats.

4.Effect of sling exercise with Tuina therapy on kinesiophobia in old patients with lumbar disc herniation:a ran-domized controlled trial based on concept of brain-bone axis

Objective To explore the effect of sling exercise with Tuina therapy on kinesiophobia in old patients with lumbar disc herniation,and analyze the mechanism based on brain-bone axis. Methods A total of 56 old patients with chronic lumbar disc herniation and kinesiophobia were selected from the Reha-bilitation Hospital of the Second Affiliated Hospital of Shandong University of Traditional Chinese Medicine from September,2022 to December,2023;and randomly divided into control group(n=28)and experimental group(n=28).The control group accepted conventional exercise therapy,while the experimental group accepted sling exercise with Tuina therapy,for four weeks.They were assessed with simplified Chinese version of Tampa Scale of Kinesiophobia(TSK),Japanese Orthopaedic Association score(JOA)and Visual Analogue Scale for pain(VAS)before and after treatment,while the bone mineral density(BMD)was tested,the levels of osteoprote-gerin(OPG),norepinephrine(NE)and corticosteroids(Cor)in serum were measured,and the median frequency(MF)of weak-link erector spinae was detected with surface electromyography. Results Two cases dropped off in the control group,and one in the experimental group.The scores of all the assessment improved in both groups after treatment(|t|>14.168,P<0.001),as well as the serum levels of OPG,NE and Cor(|t|>2.103,P<0.05),BMD(|t|>2.726,P<0.05),and MF of erector spinae(|t|>14.736,P<0.001);all of them were better in the experimental group than in the control group(|t|>2.154,P<0.05). Conclusion Sling exercise with Tuina therapy can improve the pain and kinesiophobia of lumbar disc herniation in the old adults,which may promote the recovery of physical and mental function through regulating the levels of hor-mones and neurotransmitters related to the brain-bone axis.
5.Establishment of a novel composite rat model of chronic skeletal muscle injury

Objective To establish a novel rat model of chronic skeletal muscle composite injury in the lumbar region,to explore its mechanism and treatment method in depth.Methods Twenty-eight SD rats were randomly divided into control group(n = 10),Hitting injury model(HIM)group(n = 9),Composite damage model(CDM)group(n = 9)(vertical blow combined with forced prolonged sitting).Open field test,HE staining and PCR sequencing were performed 3 days after modeling.Results The open field distance of rats in HIM group was significantly shorter than that in the CDM group,and both groups had shorter distances than the control group.HE staining showed multiple muscle fiber breaks,significant widening of muscle gaps,uneven distribution of muscle nuclei,and visible congestion in the model group of simple blow injury;In the CDM group,muscle fibers were ruptured,the muscle gaps were slightly widened,and inflammatory cell infiltration was observed between the muscles,especially around the muscle fiber breakage,with muscle fiber nuclei gathering.A volcano plot of the sequencing result suggested significant differences in the Pf4 gene.GO analysis of muscle tissue in model group rats showed upregulation of oxygen binding and carrier activity pathways.Conclusions This study attempted to establish a composite skeletal muscle injury model to study the pathological changes of chronic skeletal muscle injury in the low back pain and evaluate the efficacy of treatment method.It has practical application value in animal experiments for chronic non-specific low back pain.
6.ZHUANG Lixing's Experience in Differentiating and Treating Levodopa-Induced Dyskinesia in Parkinson's Disease

This paper summarized Professor ZHUANG Lixing's clinical experience in differentiating and treating levodopa-induced dyskinesia (LID) in Parkinson's disease. It is believed that the fundamental pathogenesis of LID lies in the disharmony or malnourishment of tendons and vessels. Based on the clinical manifestations, peak-dose LID is differentiated into two syndromes: syndrome of hyperactive liver yang causing wind and syndrome of deficiency of both liver and kidney. For the syndrome of hyperactive liver yang causing wind, the treatment focuses on calming the liver to stop the wind, and relaxing the tendons to stop tremors. The main prescription used is Zhengan Xifeng Decoction (镇肝熄风汤) with the addition of Shijueming (石决明) and Zhenzhumu (珍珠母). For the syndrome of deficiency of both liver and kidney, the treatment focuses on nourishing the liver and kidneys, and replenishing yin to stop the wind. The main prescription used is Dabuyin Pills (大补阴丸) with modification. LID in the acoustic phase is differentiated into syndrome of phlegm-damp blocking middle jiao and syndrome of deficiency of both qi and yin. For the syndrome of phlegm-damp blocking middle jiao, the treatment focuses on dissipating phlegm and eliminating dampness, and nurturing tendons and vessels. Wendan Decoction (温胆汤) or Erchen Decoction (二陈汤) with modification is used. For the syndrome of deficiency of both qi and yin, the treatment focuses on replenishing qi and nourishing blood, and nurturing tendons and vessels. The main prescriptions used are Buzhong Yiqi Decoction (补中益气汤) or Bazhen Decoction (八珍汤) or Shenling Baizhu Powder (参苓白术散) with modification. Biphasic LID is differentiated as the Shaoyang pivot disadvantageousness, and the treatment focuses on harmonizing Shaoyang and regulating the pivot. The main prescription used is Xiaochaihu Decoction (小柴胡汤) with modification.
