1.Distribution and drug resistance variation of pathogens isolated from sterile body fluids in a three-A hospital of Jiangxi Province from 2016 to 2023

OBJECTIVE To investigate the distribution and drug resistance of the pathogens isolated from sterile body fluids in a hospital so as to provide reference for clinical treatment of the infections.METHODS The sterile body fluids specimens that were cultured positive for pathogens were retrospectively collected from a three-A hos-pital of Jiangxi Province between 2016 and 2023.The distribution and drug resistance of the isolated pathogens were observed.RESULTS Totally 16,658 strains of pathogens were isolated from the sterile body fluid speci-mens in 2016-2023,among which gram-negative bacteria accounted for 51.74%.The species of bacteria that ranked the top 5 places were as follows:Escherichia coli,Klebsiella pneumoniae,Staphylococcus epidermidis,Staphylococcus hominis,and Staphylococcus aureus.The isolation rates of methicillin-resistant Staphylococcus aureus and methicillin-resistant coagulase-negative Staphylococcus were 28.32%and 74.94%,respectively;the strains maintained high sensitivity to linezolid and vancomycin,with the drug resistance rates no more than 2.50%.The drug resistance rates of Enterococcus faecium and Enterococcus faecalis to linezolid and vancomycin were always less than 4.20%.Among the species of Enterobacter,the drug resistance rates of the E.coli strains to meropenem declined from 17.65%to 5.94%;conversely,the drug resistance rate of the K.pneumoniae strains to carbapenems reached high up to 54.36%,showing an upward trend on the whole.Among the non-fermenting gram-negative bacteria,the drug resistance rate of the A.baumannii strains to carbapenems remained high(about 80.00%);while the drug resistance rates of the P.aeruginosa strains to meropenem dropped from 60.71%down to 24.32%.CONCLUSIONS The gram-negative bacteria are dominant among the pathogens isolated from the ster-ile body fluids in the hospital between 2016 and 2023.Although some major species of drug-resistant pathogens are effectively under control,the pathogens isolated from the sterile body fluid specimens maintain varying degree of drug resistance to various types of antibiotics.
2.Comparative Analysis of Two Chemiluminescent Immunoassay Systems for the Detection of Serum HBsAg Mutants

Objective To analyze and compare the ability of two chemiluminescent immunoassay systems for detecting HBsAg mutant strains.Methods The two chemiluminescent immunoassay systems,I3000 and Liaison XL,were used to detect nine HBsAg mutant strains with known HBsAg mutation sites respectively,and to compare the detection ability of these two systems for the nine HBsAg mutant strains.The clinical serum samples that were negative for HBsAg by the I3000 system were reviewed by the Liaison XL system for HBsAg,qRT-PCR amplification primers were designed for the HBsAg mutant strains with positive results by the Liaison XL system,and fluorescence quantitative PCR and Hepatitis B serologic tests were performed on the clinical samples that had inconsistent negative-positive results between the two aforementioned systems.Results The Liaison XL detected all nine HBsAg mutant strains with known HBsAg mutation sites with positive results,whereas the I3000 detected six of them with positive but three with negative results.Using the Liaison XL system to test 1 520 samples from hospitalized patients who were negative for HBsAg with the I3000 detection system,four positive results were found,including one positive result by the fluorescence quantitative PCR assay,which belonged to the HBsAg mutant of the mutant strain P124L-F/Y143H-D144E-G145R and the patient's test values for HBeAb and HBcAb were high,suggesting that the patient was a minor triple positive.Conclusion Liaison XL can detect the P124L-F/Y143H-D144E-G145R HBsAg mutant in clinical samples,and it is very important to select HBsAg test reagents with strong HBsAg mutant recognition and capture ability.
3.Health economic analysis and medical cost analysis of children with severe hepatitis B in China: A retrospective study from 2016 to 2022

Background::Hepatitis B poses a heavy burden for children in China, however, the national studies on the distributional characteristics and health care costs of children with severe hepatitis B is still lacking. This study aimed to analyze the disease characteristics, health economic effects, and medical cost for children with severe hepatitis B in China.Methods::Based on patient information in the Hospital Quality Monitoring System, cases with severe hepatitis B were divided into four groups according to age, and the etiology and symptoms of each group were quantified. The cost of hospitalization was calculated for cases with different disease processes, and severity of disease. The spatial aggregation of cases and the relationship with health economic factors were analyzed by Moran’s I analysis. Results::The total number of children discharged with hepatitis B from January 2016 to April 2022 was 1603, with an average age of 10.5 years. Liver failure cases accounted for 43.48% (697/1603) of total cases and cirrhosis cases accounted for 11.23% (180/1603). According to the grouping of disease progression, there were 1292 cases without associated complications, and the median hospitalization cost was $818.12. According to the spatial analysis, the aggregation of cases was statistically significant at the prefectural and provincial levels in 2019, 2020, and 2021 (all P <0.05). The number of severe cases was negatively correlated with gross domestic product (Moran’s I <0) and percentage of urban population (Moran’s I <0), and positively correlated with the number of pediatric beds per million population (Moran’s I >0). Conclusion::The number of severe hepatitis B cases is low in areas with high gross domestic product levels and high urban population ratios, and health care costs have been declining over the years.
4.Research progress in the relationship between periodontitis and anxiety disorders/depression disorders

Periodontitis, a common chronic inflammatory disease, progressively damages the supporting tissues of teeth, ultimately resulting in tooth loss. The rising incidence of periodontitis in adults has prompted researchers to observe a frequent co-occurrence of mental health disorders (such as anxiety disorders, depression disorders) in patients undergoing periodontitis onset and treatment. The existence of severe periodontitis can even aggravate the mental and psychological disorders of patients. Furthermore, the long-term fast-paced, high-pressure modern life is easy to cause a series of psychological problems, in turn affecting the occurrence and development of periodontitis. At present, researchers have reported the correlation between periodontitis and anxiety disorders/depression disorders. However, due to the lack of systematic understanding, most of them are clinical investigations or epidemiological statistics without deep mechanism studies. In view of the above problems, this article elucidates the bidirectional relationship between periodontitis and depression/anxiety disorders in recent years by examining recent epidemiological findings, exploring potential bidirectional pathogenic mechanisms, and discussing current treatment strategies. Ultimately, this review seeks to provide new perspectives for improving both oral and mental health outcomes in patients affected by periodontitis and anxiety/depression disorders.
5.Progress on Epidemiological Studies for the Relationship Between Plasma Glucose Indicators and Breast Cancer Risk

Hyperglycemia is a primary biological characteristic of diabetes.It is well known that three key indicators are commonly used in clinical practice to measure plasma glucose levels,such as 2-hour postload glucose(2hPG),fasting plasma glucose(FPG),and glycated hemoglobin(HbA1c).However,the association between these indicators and breast cancer incidence remains unclear.This paper systematically reviews the relationship between plasma glucose levels and breast cancer based on the above three indicators,to provide a reference for the formulation of effective plasma glucose management strategies for the primary prevention of breast cancer.
6.Relationship of RDW and blood lipid metabolism indicators with carotid atherosclerotic plaque stability in elderly patients with ACI

Objective To analyze the relationship of red blood cell distribution width(RDW)and blood lipid metabolism indicators with carotid atherosclerotic plaque stability in elderly patients with acute cerebral infarction(ACI).Methods A total of 110 elderly ACI patients admitted in our hospital from March 2021 to November 2024 were retrospectively recruited,and according to their carotid plaque characteristics,they were divided into stable plaque group(48 cases)and unstable plaque group(62 cases).The RDW,and levels of TC,TG,HDLC,LDL-C and homocysteine(Hcy)were detected.Results The unstable plaque group had significantly higher levels of RDW,TC,TG,LDL-C and Hcy,but lower HDL-C level than the stable plaque group(P<0.01).RDW,TC,TG,HDL-C,LDL-C and Hcy were the influencing factors of carotid atherosclerotic plaque stability in elderly ACI patients(P<0.05,P<0.01).ROC curve analysis suggested that the AUC value of combined detection of RDW,TC,TG,HDL-C,LDL-C and Hcy in evaluating the stability of carotid atherosclerotic plaque was 0.940(95%CI:0.898~0.983),and the combination had bet-ter efficiency than single indicator detection(P<0.05).Conclusion RDW and blood lipid metabo-lism indicators are associated with the stability of carotid atherosclerotic plaque in elderly ACI patients,and they can be used as biochemical evaluation indicators for the stability.

8.Efficacy and safety of hepatic arterial infusion chemotherapy plus bevacizumab with sintilimab vs.atezolizumab in advanced hepatocellular carcinoma

Background and Aims:In recent years,with the continuous progress of systemic therapy,hepatic arterial infusion chemotherapy(HAIC)combined with immune checkpoint inhibitors and anti-angiogenic agents has demonstrated significant efficacy in the treatment of advanced hepatocellular carcinoma(HCC).However,direct comparisons between different immunotherapeutic targets,such as PD-1 and PD-L1 inhibitors,in terms of clinical benefit and safety remain limited.This study aimed to compare the efficacy and safety of HAIC plus bevacizumab and sintilimab(HAIC-BP1)versus HAIC plus bevacizumab and atezolizumab(HAIC-BPL)in advanced HCC.Methods:A retrospective analysis was conducted on 88 patients with advanced HCC who received first-line HAIC-BP1or HAIC-BPL at Sun Yat-sen University Cancer Center between January 2020 and December 2022.Progression-free survival(PFS),overall survival(OS),objective response rate(ORR),disease control rate(DCR),and adverse events(AEs)were compared between the two groups.Cox regression analysis was performed to identify prognostic factors affecting PFS.Results:A total of 47 patients were included in the HAIC-BP1 group and 41 patients in the HAIC-BPL group,with no statistically significant differences in baseline characteristics between the two groups(all P>0.05).The ORR(59.6%vs.65.9%)and DCR(72.3%vs.80.5%)did not significantly differ between the HAIC-BP1 group and the HAIC-BPL group(both P>0.05).After a median follow-up of 16.3 months,there were no significant differences in median OS(21.3 months vs.22.4 months)or median PFS(6.7 months vs.6.2 months)between the HAIC-BP1 group and the HAIC-BPL group(both P>0.05).The incidence of AEs was similar,and no treatment-related deaths occurred.Multivariate Cox regression analysis identified tumor diameter>10 cm as an independent adverse prognostic factor for PFS(HR=0.48,95%CI=0.27-0.83,P=0.009).Conclusion:Both HAIC-BP1 and HAIC-BPL demonstrated comparable efficacy and favorable safety profiles as first-line treatment options for advanced HCC.Tumor diameter>10 cm was an independent unfavorable prognostic factor for PFS,underscoring the importance of patient stratification in clinical decision-making.
9.Comparative analysis of environmental γ radiation dose rate measurement results of social testing institutions in Jiangsu Province, China

Objective To identify issues and summarize experiences through the analysis and discussion of the current measurement comparison activity, and to improve the radiation detection ability of the social radiation environment detection institutions in Jiangsu Province, China. Methods Fixed measurement points were established. The institutions were assigned to different groups and carried out measurements one-by-one on site. For cosmic ray response value, each institution measured and recorded 50 original data; for γ dose rate in fields, roads, and indoor environments, each reference institution measured and recorded 10 original data. The institutions calculated the monitoring results independently, and the organizer’s expert group checked the results to ensure that there were no calculation errors. Gross errors were eliminated by Grubbs test, and the comparison results were evaluated by Z-score method (median/normalized interquartile range). Results Among the 75 measurement results (excluding the cosmic ray response values), 94.7% were rated as “satisfactory”, 5.3% as “problematic”, and none as “unsatisfactory”. The “problematic” results involved two FH40G γ radiation dose rate meters and one AT1121 γ radiation dose rate meter. Conclusion The cosmic ray response value of the instrument does not necessarily affect the comparison and evaluation of environmental γ radiation dose rate measurement results. AT1121 γ radiation dose rate meter may not be suitable for measuring the air-absorbed dose rate of γ radiation in general environment.
10.Parkin inhibits iron overload-induced cardiomyocyte ferroptosis by ubiquitinating ACSL4 and modulating PUFA-phospholipids metabolism.

Iron overload is strongly associated with heart disease. Ferroptosis is a new form of regulated cell death indicated in cardiac ischemia-reperfusion (I/R) injury. However, the specific molecular mechanism of myocardial injury caused by iron overload in the heart is still unclear, and the involvement of ferroptosis in iron overload-induced myocardial injury is not fully understood. In this study, we observed that ferroptosis participated in developing of iron overload and I/R-induced cardiomyopathy. Mechanistically, we discovered that Parkin inhibited iron overload-induced ferroptosis in cardiomyocytes by promoting the ubiquitination of long-chain acyl-CoA synthetase 4 (ACSL4), a crucial protein involved in ferroptosis-related lipid metabolism pathways. Additionally, we identified p53 as a transcription factor that transcriptionally suppressed Parkin expression in iron-overloaded cardiomyocytes, thereby regulating iron overload-induced ferroptosis. In animal studies, cardiac-specific Parkin knockout mice (Myh6-CreER T2 /Parkin fl/fl ) fed a high-iron diet presented more severe myocardial damage, and the high iron levels exacerbated myocardial I/R injury. However, the ferroptosis inhibitor Fer-1 significantly suppressed iron overload-induced ferroptosis and myocardial I/R injury. Moreover, Parkin effectively protected against impaired mitochondrial function and prevented iron overload-induced mitochondrial lipid peroxidation. These findings unveil a novel regulatory pathway involving p53-Parkin-ACSL4 in heart disease by inhibiting of ferroptosis.
