1.Prognostic analysis of laparoscopic simultaneous radical cystectomy and nephroureterectomy.

OBJECTIVE:
To investigate the safety and prognostic factors influencing the treatment of upper urinary tract urothelial carcinoma (UTUC) combined with bladder cancer (BCa) by laparoscopic simultaneous radical cystectomy and nephroureterectomy (RCNU).
METHODS:
The clinical data of patients admitted to Peking University Third Hospital for laparoscopic RCNU surgery from January 2009 to September 2023 were analyzed retrospectively. Based on the same gender, age (±5 years), history of uroepithelial tumors, underlying diseases, T-stage, N-stage, M-stage, American Society of Anesthesiologists (ASA) score, Charlson comorbidity index, and body mass index (BMI) (±5), 34 patients with RCNU were matched 1 ∶1 with patients with bladder cancer who underwent laparoscopic radical cystectomy (RC) alone. Kaplan-Meier survival analysis was used to calculate patient survival, and Cox proportional regression risk model was used to analyze clinical factors affecting prognosis.
RESULTS:
Of the 68 patients enrolled, the follow-up rate was 100% with a median follow-up time of 27.0 (11.7, 60.2) months. Comparison of intraoperative conditions (including operation time, estimated intraoperative bleeding, intra-operative blood transfusion, etc.) between the two groups of patients showed no significant difference (P>0.05). Comparison of preoperative creatinine and postoperative creatinine between the two groups of patients showed significant differences (P < 0.05). The perioperative Clavien grade Ⅲ-Ⅳ complication rates were 2.9% (1/34) in the RC group and 5.9% (2/34) in the RCNU group. There was no significant difference in terms of perioperative complications between the two groups. Overall survival was significantly lower in the patients receiving RCNU compared with the matched group receiving RC alone (P < 0.05). Cox regression analysis suggested that two factors, high N stage and high postoperative creatinine, were independent risk factors affecting the prognosis of patients in the 2 groups (P < 0.05).
CONCLUSION
The overall survival prognosis of patients undergoing RCNU surgery was worse compared with laparoscopic RC surgery alone during the same period. There was no clinically significant difference between the two groups in terms of operation time, intraoperative bleeding, and perioperative complications, and there were clinically significant differences in preoperative renal function and post-operative renal function.

2.Parkin inhibits iron overload-induced cardiomyocyte ferroptosis by ubiquitinating ACSL4 and modulating PUFA-phospholipids metabolism.

Iron overload is strongly associated with heart disease. Ferroptosis is a new form of regulated cell death indicated in cardiac ischemia-reperfusion (I/R) injury. However, the specific molecular mechanism of myocardial injury caused by iron overload in the heart is still unclear, and the involvement of ferroptosis in iron overload-induced myocardial injury is not fully understood. In this study, we observed that ferroptosis participated in developing of iron overload and I/R-induced cardiomyopathy. Mechanistically, we discovered that Parkin inhibited iron overload-induced ferroptosis in cardiomyocytes by promoting the ubiquitination of long-chain acyl-CoA synthetase 4 (ACSL4), a crucial protein involved in ferroptosis-related lipid metabolism pathways. Additionally, we identified p53 as a transcription factor that transcriptionally suppressed Parkin expression in iron-overloaded cardiomyocytes, thereby regulating iron overload-induced ferroptosis. In animal studies, cardiac-specific Parkin knockout mice (Myh6-CreER T2 /Parkin fl/fl ) fed a high-iron diet presented more severe myocardial damage, and the high iron levels exacerbated myocardial I/R injury. However, the ferroptosis inhibitor Fer-1 significantly suppressed iron overload-induced ferroptosis and myocardial I/R injury. Moreover, Parkin effectively protected against impaired mitochondrial function and prevented iron overload-induced mitochondrial lipid peroxidation. These findings unveil a novel regulatory pathway involving p53-Parkin-ACSL4 in heart disease by inhibiting of ferroptosis.

Cemental tear is a rare and indetectable condition unless obvious clinical signs present with the involvement of surrounding periodontal and periapical tissues. Due to its clinical manifestations similar to common dental issues, such as vertical root fracture, primary endodontic diseases, and periodontal diseases, as well as the low awareness of cemental tear for clinicians, misdiagnosis often occurs. The critical principle for cemental tear treatment is to remove torn fragments, and overlooking fragments leads to futile therapy, which could deteriorate the conditions of the affected teeth. Therefore, accurate diagnosis and subsequent appropriate interventions are vital for managing cemental tear. Novel diagnostic tools, including cone-beam computed tomography (CBCT), microscopes, and enamel matrix derivatives, have improved early detection and management, enhancing tooth retention. The implementation of standardized diagnostic criteria and treatment protocols, combined with improved clinical awareness among dental professionals, serves to mitigate risks of diagnostic errors and suboptimal therapeutic interventions. This expert consensus reviewed the epidemiology, pathogenesis, potential predisposing factors, clinical manifestations, diagnosis, differential diagnosis, treatment, and prognosis of cemental tear, aiming to provide a clinical guideline and facilitate clinicians to have a better understanding of cemental tear.

4.Mitochondria and myocardial ischemia/reperfusion injury: Effects of Chinese herbal medicine and the underlying mechanisms.

Ischemic heart disease (IHD) is associated with high morbidity and mortality rates. Reperfusion therapy is the best treatment option for this condition. However, reperfusion can aggravate myocardial damage through a phenomenon known as myocardial ischemia/reperfusion (I/R) injury, which has recently gained the attention of researchers. Several studies have shown that Chinese herbal medicines and their natural monomeric components exert therapeutic effects against I/R injury. This review outlines the current knowledge on the pathological mechanisms through which mitochondria participate in I/R injury, focusing on the issues related to energy metabolism, mitochondrial quality control disorders, oxidative stress, and calcium. The mechanisms by which mitochondria mediate cell death have also been discussed. To develop a resource for the prevention and management of clinical myocardial I/R damage, we compiled the most recent research on the effects of Chinese herbal remedies and their monomer components.
5.Research progress and challenges of molecular recognition techniques in the screening of active ingredients in Traditional Chinese medicine.

Traditional Chinese medicine (TCM) has become an important treasure trove of natural resources for the development of new medicines due to their diverse compositions, significant therapeutic effects, and few side effects. The screening of active ingredients in TCM represents a crucial step in elucidating the material basis and mechanism of action of TCM. At present, efficient and precise molecular recognition techniques based on intermolecular interactions have been extensively employed for the identification of active ingredients in TCM. This paper presents a review of the fundamental principles underlying solution-phase/affinity ligand fishing, solid-phase/affinity ligand fishing, molecular imprinting and molecular docking techniques, with a particular focus on their applications in the screening of active ingredients in TCM. Furthermore, the paper compares the advantages and disadvantages of the various techniques and identifies the limitations of existing techniques. In conclusion, the paper identifies the prospective trajectory of molecular recognition techniques in the domain of TCM research. This paper not only provides theoretical references for the development of new methods of active ingredient screening but also helps to promote the modernization and internationalization of TCM.
6.Diagnostic technique based on firefly algorithm for fault of MR coil of MRI equipment with superconductivity

Objective:To explore a method that can improve the detecting accuracy for faults of magnetic resonance(MR)through diagnostic technique based on firefly algorithm for fault of MR coil of magnetic resonance imaging(MRI)equipment with superconductivity,so as to enhance the performance and reliability of MRI equipment.Methods:A diagnosis technique for fault of MR coils in MRI equipment with superconductivity was proposed.Wavelet basis function was used to decompose initial vibration signals,and they were separated into resonance components with high-frequency and that with low-frequency,so as to enhance signal purity.The signal entropy value of deconvolution became the minimum value through iteratively adjusted the filter's coefficients,and identified the optimal system function of inverse filter,thus,to effectively recover the resonance characteristic of initially high and low frequency.A multi-objective function,which integrated entropy value of low-resonance component,smoothness and the differentiation of that with high-resonance components,was constructed.An improved firefly algorithm was introduced to combine linearly decreasing inertia weight factor,and simulate the process of swarm intelligence optimization,so as to realize global search and precise identification for faults'characteristics.Finally,the optimally diagnostic result was outputted.Results:The diagnostic technique based on firefly algorithm for fault of MR coil of MRI equipment with superconductivity can effectively decompose and enhance the original signal,and significantly suppress the interference of background noise.The algorithm can approach the true solution of the fault,which only need 298 iterations through global search for the combination of faults'features and diagnostic parameters.It can realize accurate identification for the faults of MR coil.Conclusion:The diagnostic technique,which based on firefly algorithm for fault of MR coil of MRI equipment with superconductivity,appears obvious advantages on diagnostic accuracy and convergence speed.It can provide a feasible approach for rapid diagnosis for fault of MRI equipment with superconductivity.
7.The clinical study of azacitidine and lenalidomide combination in myelodysplastic neoplasm patients with TP53 mutations

Objective:To assess the efficacy and safety of azacitidine combined with lenalidomide in MDS patients and explore potential mechanisms of therapeutic response.Methods:Sixteen MDS patients with TP53 mutations received azacitidine plus lenalidomide at ZhongDa Hospital, Southeast University (January 2021–June 2025). Efficacy and safety were assessed, and TP53 mutation status was correlated with treatment response. Whole-transcriptome sequencing and bioinformatics were used to explore molecular biomarkers associated with therapeutic efficacy.Results:Sixteen patients (median age 69.5 years, range 52–82; 8 males, 8 females) were enrolled. According to the Molecular International Prognostic Scoring System (IPSS-M), 1, 2, and 13 patients were classified as median low, high, and very high risk, respectively. Among 16 TP53-mutated patients, 11 had biallelic mutations and 5 had monoallelic mutations. Overall response rate was 56.3% (9/16), composite complete remission rate (CRc) was 31.3% (5/16), and hematology improvement rate was 25% (4/16). Among TP53-mutated patients, the response rate was 56.3% (9/16), with variant allele frequency dropping from 65.6% to 16.5% in responders ( P=0.017). In patients with TP53 mutations and complex karyotype, response rate was 53.8% (7/13), with 57.1% (4/7) showing disappearance of CK post-treatment. The most common grade 3–4 nonhematologic adverse events were infections (9/16, 56.3% ), including pneumonia (4/16, 25.0% ), gastrointestinal infections (3/16, 18.8% ), perianal infections (1/16, 6.3% ) and sepsis (1/16, 6.3% ). High CBX8 expression may be linked to treatment response. Conclusion:Azacitidine plus lenalidomide is an effective and safe therapy for MDS, including patients with TP53 mutations and complex karyotypes. Treatment markedly reduces TP53 variant allele frequency in responders, and high CBX8 expression may predict therapeutic response.
8.Strategies for management of trauma orthopedic implant and effect on control of infections in operating rooms

OBJECTIVE To explore the impact of trauma orthopedic implant management on the operating room in-fection control rate and the medical device vendor follow-up rate.METHODS Trauma orthopedic implant manage-ment was implemented at Shaoxing People's Hospital from Mar.2023 to Mar.2024,involving 2 047 trauma or-thopedic surgeries(study group).For comparison,a control group consisting of 2 067 trauma orthopedic surger-ies from Mar.2022 to Feb.2023 under routine management was selected.The impact of trauma orthopedic im-plant management on operating room efficiency,infection control rate and medical device vendor follow-up rate in both groups were analyzed.RESULTS The study group showed satisfaction rates of 78.65%and 91.35%for cir-culating cooperation familiarity and handwashing cooperation familiarity,respectively,both higher than those in the control group(P<0.001).The study group also demonstrated higher rates of standardized use of orthopedic trauma implants(96.43%),accurate item-code correspondence(98.19%)and timely availability(98.19%)than those in the control group,with a lower medical device vendor follow-up rate of 3.57%(P<0.001).The study group's monthly overtime hours,costs,time costs,average cost per re-disinfection and re-cleaning service and monthly re-disinfection re-cleaning frequency were 13.85 hours,235.86 yuan/day,25 233.51 yuan,0.35 yuan/time and 183.81 times/month,respectively,all lower than those in the control group(P<0.001).The study group had higher rates of consumable access compliance(99.22%),accurate charging(98.58%)and timely delivery(97.90%)than the control group(P<0.001).The study group also achieved a 100.00%implant regulatory recovery rate,higher than the control group(P<0.001).Furthermore,the surgical site infection rate in the study group was 1.91%,lower than that in the control group(P<0.001).CONCLUSIONS This study establishes a trauma orthopedic implant management system integrating supply chain management,an identification system and IoT technology.This model effectively addresses core issues such as the difficulty in regulating multiple vendors,blind spots in virtual warehousing and disorder in item-code tracing under the traditional vendor self-management model.It significantly improves operating room efficiency,reduces the surgical site infection rate,and achieves a notable decrease in the medical device vendor follow-up rate.
9.Aeromedical assessment of a helicopter flying personnel with asymptomatic primary hyperparathyroidism: a case report and literature review

Objective:To explore the diagnosis and treatment of the flying personnel with asymptomatic primary hyperparathyroidism (aPHPT) and the principles of aeromedical assessments.Methods:The clinical data of a helicopter navigator with aPHPT was retrospectively analyzed, and the related literature was reviewed.Results:A helicopter navigator was diagnosed with hyperplasie parathyroide or parathyroid adenoma in November 2020. In March 2021, the patient had his parathyroid adenoma explored and excised. Postoperatively, levels of parathyroid hormone (PTH) and serum calcium were mildly elevated, but without typical clinical manifestations of primary hyperparathyroidism, so temporary grounding was warranted. In June 2023, the patient returned to the hospital for re-examination, where PTH and serum calcium remained elevated. Parathyroid static imaging showed a focal concentration in the right lower parathyroid region, which was consistent with the manifestation of parathyroid adenoma so that the patient was diagnosed with aPHPT, resulting in temporary grounding. In June 2024, the patient was admitted to the Department of Endocrinology, Air Force Medical Center for aeromedical assessment, where PTH and serum calcium showed mild elevation, without such complications as urinary calculi or osteoporosis. The patient was diagnosed with aPHPT with no indication for surgery. Dynamic follow-up and monitoring of parathyroid-related indicators were recommended. The patient was determined as eligible for flight via aeromedical assessment and has flown safely for 200 h so far.Conclusions:For flying personnl with aPHPT, it is crucial to identify potential risks of disease progression and find out whether parathyroidectomy can improve clinical symptoms and surgical outcomes. The aeromedical assessment of aPHPT flying personnel should involve comprehensive assessments of conditions and dynamic monitoring of biochemical indicators, and be individualized according to the type of aircraft and flight duties.
10.Effect of oral administration of vitamin D3 on intestinal barrier function in patients after gastric cancer surgery

Objective:To investigate the effect of oral administration of vitamin D3 on intestinal barrier function in patients after gastric cancer surgery,and to provide a reference for perioperative nutritional therapy in patients with gastric cancer.Methods:The conve-nience sampling method was used to select 80 patients with gastric cancer who were admitted to Department of Gastrointestinal Surgery in a grade A tertiary hospital in Xiamen,China,from June 2021 to May 2023,and the patients were divided into intervention group and control group using a random number table.The patients in the intervention group were given oral administration of vitamin D3 800 IU/d for 14 consecutive days before surgery.ELISA was used to measure the serum levels of 25-hydroxyvitamin D3[25(OH)D3],intestinal barrier indicators(D-lactate,Zonulin),and inflammatory indicators(C-reactive protein,interleukin-6)within 24 hours after admis-sion and on days 1,4,and 7 after surgery,and the changes in these indicators were compared between the two groups.Results:A total of 71 patients were enrolled finally,with 34 in the intervention group and 37 in the control group.On days 1,4,and 7 after surgery,the intervention group had a significantly lower level of D-lactate than the control group(F=3.978,P=0.026;F=9.649,P=0.005;F=4.389,P=0.021).On day 4 after surgery,the intervention group had a significantly lower level of Zonulin than the control group(F=3.198,P=0.035).Conclusion:Oral administration of vitamin D3 before surgery may accelerate the recovery of intestinal barrier function in pa-tients with gastric cancer.
