Extensive epigenetic reprogramming involves in memory CD8+ T-cell differentiation. The elaborate epigenetic rewiring underlying the heterogeneous functional states of CD8+ T cells remains hidden. Here, we profile single-cell chromatin accessibility and map enhancer-promoter interactomes to characterize the differentiation trajectory of memory CD8+ T cells. We reveal that under distinct epigenetic regulations, the early activated CD8+ T cells divergently originated for short-lived effector and memory precursor effector cells. We also uncover a defined epigenetic rewiring leading to the conversion from effector memory to central memory cells during memory formation. Additionally, we illustrate chromatin regulatory mechanisms underlying long-lasting versus transient transcription regulation during memory differentiation. Finally, we confirm the essential roles of Sox4 and Nrf2 in developing memory precursor effector and effector memory cells, respectively, and validate cell state-specific enhancers in regulating Il7r using CRISPR-Cas9. Our data pave the way for understanding the mechanism underlying epigenetic memory formation in CD8+ T-cell differentiation.

2.The relationship between blood glucose variability, disease severity and prognosis of the patients with acute pancreatitis

Objective To explore the relationship between blood glucose variability, disease severity and prognosis of the patients with acute pancreatitis. Methods Total of 242 patients with acute pancreatitis admitted to the department of emergency from January 2019 to December 2019 were enrolled. The organ failure was evaluated according to Marshall's score, the severity of the disease was evaluated according to Atlanta's score, and the blood glucose indexes of three groups of patients with mild acute pancreatitis, moderate severe acute pancreatitis and severe acute pancreatitis were compared within seven days after admission. The relationship between blood glucose index and disease severity in different patients with acute pancreatitis was analyzed. Taking whether a puncture was performed at admission, whether the patient was admitted to the intensive care unit (ICU), and whether the patient died as endpoint events as classification factors, the relationship between blood glucose indicators and disease prognosis of patients with acute pancreatitis was analyzed using the One-Way ANOVA, Kruskal-Wallis test, Mann-Whitney U test, receiver operating characteristic curve (ROC curve), etc. Results Of the 242 patients, 70 cases (28.9%) were mild acute pancreatitis, 71 cases (29.3%) with moderate severe acute pancreatitis, 101 cases (41.7%) with severe acute pancreatitis. There was no statistically significant difference in the coefficient of variation of blood glucose among the three groups within 7 days of admission. The mean, standard deviation, maximum, minimum value and difference between maximum and minimum value of venous blood glucose in severe acute pancreatitis group were higher than those in moderate severe acute pancreatitis group, while those in moderate severe acute pancreatitis group were higher than those in mild acute pancreatitis group. The mean value of blood glucose of invasive operation group (IOP) (n=55) was higher than that of non-invasive operation (NOP) group(n=187). Conclusions The blood glucose level and fluctuation range of patients with acute pancreatitis within seven days after admission, are of great significance for the judgment of the severity and prognosis of the disease.
3.Hand eczema among health care workers and its association with hand hygiene

OBJECTIVE To investigate the current status of prevalence of hand eczema(HE)among the health care workers,explore the influencing factors and analyze the association with hand hygiene.METHODS A questionnaire survey was conducted for the health care workers.The health care workers who had HE were followed up by the dermatology department and were completed the assessment of severity.Logistic regression analysis was per-formed for the influencing factors.RESULTS The questionnaires were distributed to the health care workers of Shanghai First Peoples Hospital in Jul.2024,and 659 valid questionnaires were recycled.The prevalence rate of HE by self-report was 37.03%in recent almost one year.The finger and dorsal hand were the predilection sites of skin damage,with the symptom dominated by itching;dryness,desquamation and vesicles were the primary man-ifestations of skin damage,most of which(64.81%)were moderate.Wearing gloves in work environment and contact with hand sanitizer and disinfectants could make the HE symptoms ever more severe.The use of hand san-itizer/frequent hand washing with soap lye and prolonged glove-wearing were the leading causes of HE.The nurses were dominant among the health care workers with HE(P=0.003),and the proportion of those with allergic his-tory was even higher(P<0.001).As for the health care workers who washed hands for surgeries,high volume of daily surgical procedures(OR=1.325,95%CI:1.121 to 1.567)and glove-wearing duration more than 120 min(OR=3.177,95%CI:1.087 to 9.281)were the risk factors for HE.As for the common hand-washing health care workers,daily hand washing more than 15 times(OR=3.199,95%CI:1.014 to 10.137),glove-wearing dura-tion more than 120 min(OR=2.216,95%CI:1.117 to 4.391)and use of powdered latex glove(OR=1.591,95%CI:1.058 to 2.390)were the risk factors for HE.CONCLUSION The prevalence rate of HE is high among the health care workers.It is necessary to attach great importance to the hand skin health of the health care work-ers and take comprehensive intervention measures for prevention of HE such as optimization of hand hygiene pro-cedure,enhancement of barrier protection and stress on occupational health education so as to achieve the dual goals of infection control and skin health.
4.Effects of transcranial alternating current stimulation combined with sertraline on cognitive function in patients with depressive disorder

BackgroundCognitive function is closely related to an individual's quality of life and social functioning, with approximately 20%~35% of patients with depressive disorder experiencing some degree of cognitive impairment even after clinical symptom remission. Existing evidence suggests that tACS can improve specific cognitive domains, such as memory function, while its effects on other cognitive dimensions, such as executive functioning, attention, and information processing speed, remain unclear. ObjectiveTo explore the effects of tACS on the multidimensional cognitive functions and emotional problems of patients with depressive disorder, thus to provide references for the treatment of depressive disorder. MethodsForty-nine patients with depressive disorder who were hospitalized in the Fourth People's Hospital of Wuhu from November 2022 to October 2024 and met the diagnostic criteria for depressive disorder outlined in the Diagnostic and Statistical Manual of Mental Disorders, fifth edition (DSM-5), were selected as study participants. Subjects were randomly divided into study group (n=23) and control group (n=26) based on Microsoft Excel. Both groups received sertraline treatment. The initial dose was 50 mg/day, which gradually titrated upward based on individual variability, drug tolerance, and therapeutic response, with a maintenance dose ranging from 100 to 200 mg/day. In addition, the study group underwent tACS therapy for 4 weeks, with 5 sessions per week, each lasting 20 minutes. The control group received sham stimulation, in which the stimulus was interrupted after the first 30 seconds. At baseline, the 4th week, and the 12th week of treatment, patients were assessed using the Hamilton Depression Scale-17 item (HAMD-17), Hamilton Anxiety Scale (HAMA), and MATRICS Consensus Cognitive Battery (MCCB). ResultsRepeated measures analysis of variance indicated that both the time effect and the time×group interaction effect for HAMD-17 scores were statistically significant between the two groups (F=260.437, 25.309, P<0.01). At week 12 of treatment, the HAMD-17 score in the study group was lower than that in the control group (t=4.236, P<0.01). For HAMA scores, the time effect, group effect, and time×group interaction effect were all statistically significant between the two groups (F=248.082, 4.506, 9.500, P<0.05 or 0.01). At weeks 4 and 12, study group reported lower HAMA scores compared with control group (t=4.580, 2.608, P<0.05 or 0.01). Regarding the MCCB scores for attention/vigilance, verbal learning, and overall composite, the time effect, group effect, and time×group interaction effect were all statistically significant between the two groups (F=70.331, 27.882, 51.679, 5.560, 10.948, 7.860, 8.490, 3.874, 5.025, P<0.05 or 0.01). After intervention, the study group showed significantly higher MCCB scores for attention/vigilance, verbal learning, and overall composite at both week 4 (t=-2.149, -3.530, -2.740, P<0.05) and week 12 (t=-3.534, -3.576, -3.838, P<0.01) when compared to the control group. ConclusionThe combined tACS and sertraline therapy may demonstrate superior efficacy to pharmacotherapy alone in the short term for improving attention/vigilance, verbal learning, overall cognitive function, and anxiety symptoms in patients with depressive disorders. Based on the 12-week outcomes, the combined tACS and sertraline therapy not only sustaine its previously observed advantages in improving cognitive domains and anxiety symptoms, but also demonstrate potentially superior efficacy over monotherapy in alleviating depressive symptoms. 6.Transcriptome sequencing reveals molecular mechanism of seed dormancy release of Zanthoxylum nitidum.

The transcriptome sequencing based on Illumina Novaseq 6000 Platform was performed with the untreated seed embryo(DS), stratified seed embryo(SS), and germinated seed embryo(GS) of Zanthoxylum nitidum, aiming to explore the molecular mechanism regulating the seed dormancy and germination of Z. nitidum and uncover key differentially expressed genes(DEGs). A total of 61.41 Gb clean data was obtained, and 86 386 unigenes with an average length of 773.49 bp were assembled. A total of 29 290 DEGs were screened from three comparison groups(SS vs DS, GS vs SS, and GS vs DS), and these genes were annotated on 134 Kyoto Encyclopedia of Genes and Genomes(KEGG) pathways. KEGG enrichment analysis revealed that the plant hormone signal transduction pathway is the richest pathway, containing 226 DEGs. Among all DEGs, 894 transcription factors were identified, which were distributed across 34 transcription factor families. These transcription factors were also mainly concentrated in plant hormone signal transduction and mitogen-activated protein kinase(MAPK) signaling pathways. Further real-time quantitative polymerase chain reaction(RT-qPCR) validation of 12 DEGs showed that the transcriptome data is reliable. During the process of seed dormancy release and germination, a large number of DEGs involved in polysaccharide degradation, protein synthesis, lipid metabolism, and hormone signal transduction were expressed. These genes were involved in multiple metabolic pathways, forming a complex regulatory network for dormancy and germination. This study lays a solid foundation for analyzing the molecular mechanisms of seed dormancy and germination of Z. nitidum.

Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.

9.The role of self-evaluation in non-suicidal self-injury among adolescents with depressive disorder

Objective:To explore the internal mechanism by which self-evaluation influences non-suicidal self-injury (NSSI) in adolescent patients with depressive disorder.Methods:Clinical data from 214 adolescent patients with depressive disorder hospitalized at Suzhou Guangji Hospital from March 2022 to January 2024 were prospectively collected. According to the DSM-5 diagnostic criteria for NSSI, participants were divided into an NSSI group (158 cases [38 males, 120 females, age 12-17 (14.2±1.5) years]) and a non-NSSI group (56 cases [20 males, 36 females, age 12-18 (14.5±1.8) years]). A self-developed basic information questionnaire was used to collect demographic data. Standardized tools including the Self-rating Depression Scale (SDS), Inventory of Parent and Peer Attachment (IPPA), Self-acceptance Questionnaire (SAQ), and Perceived Stress Scale-10 (PSS-10) were used to assess their depression level, parental attachment level, self-evaluation/acceptance level, perceived stress level, and other relevant psychological characteristics. Differences in psychological characteristics between the two groups were compared. Logistic regression, correlation analysis, and mediation effect models were used to explore the relationships between variables and NSSI and their mechanisms.Results:The NSSI group had significantly higher total scores on the SDS (41.3±7.7 vs. 34.4±9.3) and PSS-10 (25.5±6.1 vs. 21.3±6.5) than the non-NSSI group ( F=29.12, F=18.17, respectively; all P<0.001). Conversely, the NSSI group had significantly lower total scores on the SAQ (31.2±8.8 vs. 35.9±8.9) and IPPA (56.3±13.6 vs. 63.4±13.8) compared to the non-NSSI group ( F=11.24, F=10.84, respectively; all P<0.001). Stepwise logistic regression analysis identified depression level (SDS total score, OR=1.12, 95% CI: 1.05-1.19), self-evaluation (SAQ subscale score, OR=1.17, 95% CI: 1.04-1.31), and perceived stress (PSS-10 total score, OR=1.11, 95% CI: 1.01-1.22) as predictors of NSSI (all P<0.05). Chain mediation analysis showed that self-evaluation had a significant positive direct effect ( β=0.025, P<0.01) and a negative indirect effect ( β=-0.038, P<0.001) on NSSI, with a negative total effect ( β=-0.012, P<0.05). The indirect effect was realized through three pathways: a single mediation pathway of self-evaluation via perceived stress ( β=-0.016), a single mediation pathway of self-evaluation via depression ( β=-0.011), and a chain mediation pathway of self-evaluation via perceived stress and depression ( β=-0.011) (all P<0.05). Conclusion:Self-evaluation influences NSSI behavior through a dual mechanism involving both direct and indirect effects. The indirect protective effect is primarily achieved by reducing perceived stress and depression levels.
10.Active screening of bacteria from neonates of different sources in a tertiary general hospital in Shanghai

Objective To analyze the active screening results on bacteria from neonates from different sources in the neonatal ward of a tertiary general hospital in Shanghai,provide a basis for the prevention and control of neo-natal infections.Methods The monitoring results of active screening on all neonates admitted to the neonatal ward of the hospital from 2017 to 2023 were collected retrospectively.Changes in bacterial detection among neonates ad-mitted from the obstetrics department and the community were compared and analyzed.Results From 2017 to 2023,a total of 4 265 neonates were admitted to the hospital,including 3 339 from department of obstetrics(obste-trics source group)and 926 from community(community source group).Active screening showed that 490 and 572 neonates were detected with bacteria,respectively.Bacterial detection rate of neonates in obstetrics source group was lower than community source group(14.68%vs 61.77%,P<0.001);detection rates of bacteria from pha-rynx and umbilical site of neonates in community source group were both higher than obstetrics source group(both P<0.001);detection rate of bacteria from neonates'umbilical site was higher than that from pharynx of both ob-stetrics source group and community source group(both P<0.001).A total of 1 348 bacterial strains were detec-ted,detection rates of Escherichia coli,Staphylococcus epidermidis,Enterococcus faecalis,Streptococcus agalac-tiae,and Acinetobacter baumannii in the obstetrics source group were all higher than community source group(all P<0.05),detection rates of Staphylococcus aureus and Enterobacter cloacae in the obstetrics source group were both lower than community source group(both P<0.05).66 neonates were detected multidrug-resistant organism(MDRO),14 were from obstetrics source group and 52 from community source group,63 neonates were detected methicillin-resistant Staphylococcus aureus(MRSA).The detection rates of MDRO and MRSA from community source group were both higher than obstetrics source group(both P<0.001);detection rate of MDRO from neo-nates'umbilical site was higher than that from the pharynx of the community source group(P<0.001).Conclusion Bacteria detected in the obstetrics source group is mainly Escherichia coli,while that detected from community source group is mainly Staphylococcus aureus.MRSA is the main MDRO from neonates,and bacterial screening of neonates admitted from the community should be strengthened to prevent and control the spread of MDRO,espe-cially community-acquired MRSA in neonatal wards.
