1.Mechanism of KLF4 in regulating ferroptosis in diabetic nephropathy

ObjectiveTo investigate the role of Krüppel-like factor 4 (KLF4) in type 1 diabetic nephropathy (DN) and to elucidate its underlying mechanisms. MethodsSixteen male Sprague-Dawley (SD) rats were selected and randomly divided into control group and model group, with 8 rats in each group. Rats in model group were intraperitoneally injected with a single dose of 55 mg/kg streptozotocin (STZ) to establish a diabetic nephropathy (DN) model, while those in control group were injected with an equal volume of sodium citrate buffer at the same time. After successful model establishment, the serum levels of blood urea nitrogen (BUN) and serum creatinine (SCR) were determined. Hematoxylin-eosin (HE) staining was performed on renal tissues to observe pathological changes, and immunofluorescence staining was conducted to detect the expression of KLF4 in renal tissues. Lipid peroxidation levels were evaluated by measuring malondialdehyde (MDA), Fe²⁺, and lipid peroxidation products in rat kidneys. A high glucose (HG)-induced cell injury model was established in HK-2 cells, with mitochondrial membrane potential assessed using 5,5',6,6'-tetrachloro-1,1',3,3'- tetraethylbenzimidazolylcarbocyanine iodide (JC-1) staining. Lipid peroxidation levels (MDA, Fe²⁺, and lipid peroxides) were measured in HK-2 cells.KLF4-overexpressing HK-2 cells were then constructed, followed by repeated JC-1, MDA, Fe²⁺, and lipid peroxidation assays. Western blot was performed to evaluate the expression of ferroptosis-related proteins including glutathione peroxidase 4 (GPX4), nuclear factor erythroid 2-related factor 2 (NRF2), and Kelch-like ECH-associated protein 1 (Keap1), in renal tissues, HK-2 cells, and KLF4-overexpressing HK-2 cells. ResultsCompared with the control group, DN rats exhibited elevated serum BUN and SCR levels, glomerular hypertrophy, renal interstitial fibrosis, and decreased KLF4 expression. Additionally, MDA, Fe²⁺, and lipid peroxidation levels increased, indicating enhanced ferroptosis in renal tissues, accompanied by reduced GPX4 and NRF2 expression and elevated Keap1 levels. Similarly, HG-treated HK-2 cells showed decreased KLF4 expression, increased MDA, Fe²⁺ and lipid peroxidation, elevated ferroptosis, and dysregulated GPX4/NRF2/Keap1 expression. However, KLF4 overexpression reversed these alterations induced by high glucose treatment. ConclusionIn the renal tissues of type 1 diabetic rats, the expression of KLF4 decreases the level of ferroptosis increases, and KLF4 overexpression could alleviate HG-induced HK-2 cell injury.
2.Relationship among occupational stress, work-related rumination, and sleep quality in emergency medical dispatchers in some areas of China

Background The increasing demand for emergency services coupled with the special working environment has exacerbated occupational stress and work-related rumination among emergency medical dispatchers, which is noteworthy for its impact on dispatchers' sleep quality. Objective To explore the relationship among occupational stress, work-related rumination, and sleep quality of emergency medical dispatchers, so as to provide reference for improving sleep quality and maintaining physical and mental health of this occupational group. Methods A total of 386 emergency medical dispatchers from 16 provinces and municipalities including Beijing, Shanghai, Tianjin, Inner Mongolia, Zhejiang, Shanxi, Jiangxi, Anhui, Hubei, Hebei, Henan, Sichuan, Guizhou, Yunnan, Fujian, and Hainan of China were investigated with the Chinese version of Effort-Reward Imbalance Questionnaire, Work-Related Rumination Questionnaire, and Insomnia Severity Index. Spearman correlation was used to analyze the association among occupational stress, work-related rumination, and sleep quality. A structural equation model was constructed, with occupational stress as independent variable, the two dimensions of work-related rumination as mediating variables, and sleep quality as dependent variable, respectively. Bootstrap testing was then used to verify potential mediating effect of work-related rumination on the relationship between occupational stress and sleep quality among the emergency medical dispatchers. Results Among the enrolled emergency medical dispatchers, the effort-reward imbalance (ERI) index was 1.03, the score of affective rumination was 15.35±5.26, the score of problem-solving rumination was 17.64±4.63, and the total score of sleep quality was 21.10±6.53. Their ERI index was positively correlated with affective rumination scores (r=0.636, P<0.01), but not with problem-solving rumination scores (P>0.05). Their ERI index, affective rumination scores, and problem-solving rumination scores were positively correlated with sleep quality scores (P<0.05). The direct effect size of occupational stress on sleep quality was 0.627, the indirect effect size of affective rumination was 0.124, and the mediating effect of affective rumination accounted for 16.4% of the total effect (0.755), while the problem-solving rumination had no mediating effect on the relationship between occupational stress and sleep quality. Conclusion Occupational stress and affective rumination in emergency medical dispatchers can predict their sleep quality. Occupational stress can directly affect sleep quality, and indirectly affect it through affective rumination. Managers should pay attention to and evaluate the affective rumination level of emergency medical dispatchers, so as to take corresponding intervention measures to reduce their occupational stress and improve their sleep quality.
3.Risk factors for cough variant asthma and its relationship with changes in indoor environment

Objective To analyze the risk factors for cough variant asthma (CVA) and investigate the influence of indoor environment on CVA. Methods From July 2021 to August 2024, 315 patients admitted to the hospital due to CVA were selected as the CAV group. Meanwhile, 100 healthy individuals were selected as the control group. Clinical data of all subjects were collected. Univariate analysis and multivariate logistic regression analysis were used to identify the influencing factors of CVA. Results The proportions of subjects with family history and recurrent respiratory tract infection in the CVA group were higher than those in the control group (P<0.05). The CVA group had significantly higher rates of indoor renovation, mold growth, infrequent bedding cleaning, opening window for ventilation once to 3 times a week, keeping pets, and passive smoking compared to the control group within one year, and the differences were statistically significant (P<0.05). Multivariate logistic regression analysis results showed that indoor-decoration in the past year (OR=2.282, 95%CI: 1.1454.549), mould growth (OR=2.036, 95%CI: 1.228-3.376), opening window for ventilation once to 3 times a week (OR=1.895, 95%CI: 1.253-2.865), seldom cleaning bedding (OR=2.257, 95%CI: 1.132-4.499), keeping pets (OR=2.071, 95%CI: 1.146-3.743), and passive smoking (OR=2.208, 95%CI: 1.377-3.541) were independent risk factors for CVA (P<0.05). Conclusion There is a significant correlation between the occurrence of CVA and indoor environment. Changes in indoor environment include indoor-decoration in the past year, mould growth, low frequency of opening window for ventilation, low frequency of bedding cleaning, keeping pets and passive smoking which are risk factors for the occurrence of CVA.
4.Effects of combined use of active ingredients in Buyang Huanwu Decoction on oxygen-glucose deprivation/reglucose-reoxygenation-induced inflammation and oxidative stress of BV2 cells.

This study aims to explore the effects and action mechanisms of the active ingredients in Buyang Huanwu Decoction(BYHWD), namely tetramethylpyrazine(TMP) and hydroxy-safflor yellow A(HSYA), on oxygen-glucose deprivation/reglucose-reoxygenation(OGD/R)-induced inflammation and oxidative stress of microglia(MG). Network pharmacology was used to screen the effective monomer ingredients of BYHWD and determine the safe concentration range for each component. Inflammation and oxidative stress models were established to further screen the best ingredient combination and optimal concentration ratio with the most effective anti-inflammatory and antioxidant effects. OGD/R BV2 cell models were constructed, and BV2 cells in the logarithmic growth phase were divided into a normal group, a model group, an HSYA group, a TMP group, and an HSYA + TMP group. Enzyme-linked immunosorbent assay(ELISA) was used to detect the levels of inflammatory cytokines such as interleukin-1β(IL-1β), tumor necrosis factor-α(TNF-α), and interleukin-6(IL-6). Oxidative stress markers, including superoxide dismutase(SOD), nitric oxide(NO), and malondialdehyde(MDA), were also measured. Western blot was used to analyze the protein expression of both inflammation-related pathway [Toll-like receptor 4(TLR4)/nuclear factor-kappa B(NF-κB)] and oxidative stress-related pathway [nuclear factor erythroid 2-related factor 2(Nrf2)/heme oxygenase-1(HO-1)]. Immunofluorescence was used to assess the expression of proteins such as inducible nitric oxide synthase(iNOS) and arginase-1(Arg-1). The most effective ingredients for anti-inflammatory and antioxidant effects in BYHWD were TMP and HSYA. Compared to the normal group, the model group showed significantly increased levels of IL-1β, TNF-α, IL-6, NO, and MDA, along with significantly higher protein expression of NF-κB, TLR4, Nrf2, and HO-1 and significantly lower SOD levels. The differences between the two groups were statistically significant. Compared to the model group, both the HSYA group and the TMP group showed significantly reduced levels of IL-1β, TNF-α, IL-6, NO, and MDA, lower expression of NF-κB and TLR4 proteins, higher levels of SOD, and significantly increased protein expression of Nrf2 and HO-1. Additionally, the expression of the M1-type MG marker iNOS was significantly reduced, while the expression of the M2-type MG marker Arg-1 was significantly increased. The results of the HSYA group and the TMP group had statistically significant differences from those of the model group. Compared to the HSYA group and the TMP group, the HSYA + TMP group showed further significant reductions in IL-1β, TNF-α, IL-6, NO, and MDA levels, along with significant reductions in NF-κB and TLR4 protein expression, an increase in SOD levels, and elevated Nrf2 and HO-1 protein expression. Additionally, the expression of the M1-type MG marker iNOS was reduced, while the M2-type MG marker Arg-1 expression increased significantly in the HSYA + TMP group compared to the TMP or HSYA group. The differences in the results were statistically significant between the HSYA + TMP group and the TMP or HSYA group. The findings indicated that the combined use of HSYA and TMP, the active ingredients of BYHWD, can effectively inhibit OGD/R-induced inflammation and oxidative stress of MG, showing superior effects compared to the individual use of either component.

5.Research progress in chemical components and pharmacological activities of different medicinal parts of Citrus changshan-huyou.

As a member of the Citrus genus of the Rutaceae family, Citrus changshan-huyou(CSHY) is mainly produced in Quzhou city, Zhejiang province. Modern research shows that different medicinal parts of CSHY(immature fruit, mature fruit peel, flower buds, leaves, seeds, etc.) are abundant in flavonoids, terpenes, coumarins, phenolic acids, and volatile oils. Their pharmacological activities include respiratory system protection, liver protection, anti-inflammation, anti-hyperlipidemia, anti-hyperglycemia, and antioxidation. Based on the summarization of 374 chemical components in different medicinal parts of CSHY identified in the past 20 years, this study reviewed their pharmacological actions and mechanisms and further analyzed the current status of quality control of different medicinal parts of CSHY, aiming to provide reference for the resource development and exploitation and the quality control research of different medicinal parts of CSHY.

6.Resveratrol promotes mitophagy via the MALAT1/miR-143-3p/RRM2 axis and suppresses cancer progression in hepatocellular carcinoma.

OBJECTIVE:
Resveratrol (Res) is a promising anticancer drug against hepatocellular carcinoma (HCC), but whether its anti-HCC effects implicate mitophagy remains unclear. Therefore, we aimed to explore the specific role of Res in mitophagy and the related mechanisms during the treatment of HCC.
METHODS:
HepG2 cells and tumor-grafted nude mice were used to investigate the effects of low-, middle- and high-dose of Res on HCC progression and mitophagy in vitro and in vivo, respectively. A series of approaches including cell counting kit-8, flow cytometry, wound healing and transwell assays were used to evaluate tumor cell functions. Transmission electron microscopy, immunofluorescence and Western blotting were used to assess mitophagy. Mitochondrial oxygen consumption rate, reactive oxygen species and membrane potential were used to reflect mitochondrial function. After disrupting the expression of metastasis-associated lung adenocarcinoma transcript 1 (MALAT1), miR-143-3p, and ribonucleoside reductase M2 (RRM2), the effects of the MALAT1/miR-143-3p/RRM2 axis on cell function and mitophagy under Res treatment were explored in vitro. Additionally, dual-luciferase reporter and chromatin immunoprecipitation were used to confirm interactions between target genes.
RESULTS:
Res significantly inhibited the proliferation and promoted apoptosis of HCC cells in vitro, while significantly suppressing tumor growth in a dose-dependent manner and inducing mitophagy and mitochondrial dysfunction in vivo. Interestingly, MALAT1 was highly expressed in HCC cells and its knockdown upregulated miR-143-3p expression in HCC cells, which subsequently inhibited RRM2 expression. Furthermore, in nude mice grafted with HCC tumors and treated with Res, the expression of MALAT1, miR-143-3p and RRM2 were altered significantly. In vitro data further supported the targeted binding relationships between MALAT1 and miR-143-3p and between miR-143-3p and RRM2. Therefore, a series of cell-based experiments were carried out to study the mechanism of the MALAT1/miR-143-3p/RRM2 axis involved in mitophagy and HCC; these experiments revealed that MALAT1 knockdown, miR-143-3p mimic and RRM silencing potentiated the antitumor effects of Res and its activation of mitophagy.
CONCLUSION

Nodal marginal zone B-cell lymphoma(NMZL),the least common subtype of marginal zone lymphoma,represents a low-grade malignancy arising from the marginal zone of lymph node follicles,composed of small B-cells with an inert non-Hodgkin lymphoma nature.It accounts for 1.5% to 1.8% of all non-Hodgkin lymphomas and 10% of all marginal zone lymphomas.The low incidence and lack of typical clinical and pathological features pose a challenge to the diagnosis and clinical management of NMZL.In this article,we reported the diagnosis and treatment of a case of NMZL located in the parapharyngeal space of the left neck and reviewed the relevant literature from both domestic and international sources.We summarized the clinical manifestations,histopathological features,immunohistochemical characteristics,imaging features,diagnosis and treatment modalities,and prognosis of NMZL.

Obesity, as a chronic recurrent disease, has become a major public health challenge in China. To implement the requirements of the Healthy China Initiative (2019—2030), under domestic guidelines or consensus statements on overweight and obesity, and in alignment with the latest scientific advances globally, the Quality control protocol for adult overweight and obesity screening in health management (examination) institutions (2025 edition) was developed. This protocol was drafted by the Health Management Center of Shanghai Changzheng Hospital and formulated through multiple rounds of deliberation by experts in China’s health examination quality control field. The protocol establishes unified standards for screening facilities, personnel qualifications, and measurement or testing procedures. It defines specific screening items, outlines a standardized screening pathway, and sets requirements for the final medical review, ensuring the scientific validity, effectiveness, and safety of the screening process. The implementation of this protocol will enhance the consistency of weight management practices for adults across health examination institutions and strengthen the quality control of overweight and obesity screening programs.
9.Improvement effect of hirudin on post-stroke depression in mice and its mechanism

Objective:To discuss the effect of hirudin on post-stroke depression(PSD)in the mice,and to clarify its potential mechanism.Methods:Sixty male C57BL/6 mice were randomly divided into control group,PSD group,PSD+low dose of hirudin group,PSD+medium dose of hirudin group,and PSD+high dose of hirudin group,and there were 12 mice in each group.The stroke model was established by middle cerebral artery occlusion(MCAO),and the depression model was induced by chronic unpredictable mild stress(CUMS)combined with solitary housing.The mice in PSD+low dose of hirudin,PSD+medium dose of hirudin,and PSD+high dose of hirudin groups were intravenously injected with 10,20,and 40 U·kg-1 hirudin,respectively,while the mice in control and PSD groups received equal volumes of saline.The body weights of the mice were recorded on days 0,7,14,and 21 of CUMS.The LONGA neurological function score was calculated.Sucrose preference test,tail suspension test,and forced swimming test were used to detect the sucrose preference rate,immobility time in tail suspension,and forced swimming in various groups,respectively;HE staining was used to observe the histopathological changes in the medial prefrontal cortex(mPFC);biochemical kits were used to detect the levels of malondialdehyde(MDA)and reduced glutathione(GSH)as well as superoxide dismutase(SOD)activity in mPFC tissue of the mice in various groups;2',7'-dichlorodihydrofluorescein diacetate(DCFH-DA)fluorescence probe method was used to detect the reactive oxygen species(ROS)positive rate in mPFC tissue of the mice in various groups;real-time fluorescence quantitative PCR(RT-qPCR)and Western blotting method were used to detect the expression levels of nucleoredoxin(NXN)mRNA and protein in mPFC tissue of the mice in various groups.Results:Compared with control group,the body weight of the mice in PSD group was significantly decreased on days 0,7,14,and 21 of CUMS(P<0.05 or P<0.01).Compared with PSD group,the body weights of the mice in PSD+low dose of hirudin,PSD+medium dose of hirudin,and PSD+high dose of hirudin groups were significantly increased on days 14 and 21 of CUMS(P<0.05 or P<0.01),and the neurological function scores were significantly decreased(P<0.05 or P<0.01).The sucrose preference test,tail suspension test,and forced swimming test results showed that compared with control group,the sucrose preference rate of the mice in PSD group was significantly decreased(P<0.01),while the immobility times in tail suspension and forced swimming were significantly increased(P<0.01).Compared with PSD group,the sucrose preference rates of the mice in PSD+low dose of hirudin,PSD+medium dose of hirudin,and PSD+high dose of hirudin groups were significantly increased(P<0.05 or P<0.01),and the immobility times were significantly decreased(P<0.05 or P<0.01).The HE staining showed normal cell morphology,clear structure,and uniform size distribution in mPFC tissue in control group.In PSD and PSD+low dose of hirudin groups,the number of the cells in mPFC tissue was significantly reduced,with severe vacuolar degeneration and pyknotic nuclei.Compared with PSD group,the numbers of the cells in PSD+medium dose of hirudin and PSD+high dose of hirudin groups were significantly increased,and the vacuolar degeneration and nuclear pyknosis were alleviated.The Biochemical and DCFH-DA fluorescence probe assays results showed that compared with control group,the GSH level and SOD activity in mPFC tissue of the mice in PSD group were significantly decreased(P<0.01),while the MDA level and ROS positive rate were significantly increased(P<0.01).Compared with PSD group,the GSH levels and SOD activities of the mice in PSD+low dose of hirudin,PSD+medium dose of hirudin,and PSD+high dose of hirudin groups were significantly increased(P<0.05 or P<0.01),while the MDA levels and ROS positive rates were significantly decreased(P<0.05 or P<0.01).The RT-qPCR and Western blotting results showed that compared with control group,the expression levels of NXN mRNA and protein in mPFC tissue of the mice in PSD group were significantly decreased(P<0.01).Compared with PSD group,the expression levels of NXN mRNA and protein in mPFC tissue of the mice in PSD+low dose of hirudin,PSD+medium dose of hirudin,and PSD+high dose of hirudin groups were significantly increased(P<0.05 or P<0.01).Conclusion:Hirudin promotes redox balance in mPFC of the PSD mice,repairs neurological damage,and improves PSD.
10.A cisplatin prodrug-based self-assembling ozone delivery nanosystem sensitizes radiotherapy in triple-negative breast cancer.

Lacking therapeutic targets highlights the crucial roles of chemotherapy and radiotherapy in the clinical management of triple-negative breast cancer (TNBC). To relieve the side effects of the chemoradiotherapy combination regimen, we design and develop a self-assembled micelle nanosystem consisting of perfluorocarbon chain-modified cisplatin prodrug. By incorporating perfluorodecalin, this nanosystem can effectively carry ozone and promote irradiation-derived reactive oxygen species (ROS) production. By leveraging the perfluorocarbon sidechain, the nanosystem exhibits efficient internalization by TNBC cells and effectively escapes from lysosomal entrapment. Under X-ray irradiation, ozone-generated ROS disrupts the intracellular redox balance, thereby facilitating the release of cisplatin in a reduction-responsive manner mediated by reduced glutathione. Moreover, oxygen derived from ozone decomposition enhances the efficacy of radiotherapy by alleviating tumor hypoxia. Notably, the combination of irradiation with ozone-loaded cisplatin prodrug nano system synergistically prompts antitumor efficacy and reduces cellular/systemic toxicity in vitro and in vivo. Furthermore, the combo regimen remodels the tumor microenvironment into an immune-favored state by triggering immunogenic cell death and relieving hypoxia, which provides a promising foundation for a combination regimen of immunotherapy. In conclusion, our nanosystem presents a novel strategy for integrating chemotherapy and radiotherapy to optimize the efficacy and safety of TNBC clinical treatment.
