1.End-of-life resuscitation decision-making preferences and their influencing factors among terminally ill patients with malignant tumors

Objective To explore the preferences and influencing factors of terminal malignant tumor patients,and to provide a basis for optimizing hospice care.Methods A convenience sampling method was employed to select 485 patients with terminal malignant tumors from the oncology departments of two tertiary,class-A hospitals in Sichuan Province between September 2023 and November 2024.Data on demographic characteristics,decision-making-related information,preferences regarding resuscitation,and cultural belief levels were collected using a structured questionnaire.Results In critical situations,39.4%of patients opted for active rescue,33.6%chose to forgo treatment,and 27.0%preferred to maintain current treatment.Acceptance rates for nine specific interventions ranged from 28.9%to 61.2%.Multivariate logistic regression analysis revealed that stronger cultural beliefs,younger age,having a spouse,being the primary caregiver,employee medical insurance coverage,and use of invasive tubes were significantly associated with a tendency toward active or continued treatment(P<0.05).Prioritizing quality of life or concerns about economic burden were significantly associated with the decision to forgo life-sustaining interventions(P<0.05).Conclusions Patients with terminal malignant tumors exhibit diverse preferences regarding end-of-life resuscitation,significantly shaped by cultural beliefs,demographic factors,and family support.Clinicians should integrate patients'values,cultural backgrounds,and individual needs to provide tiered decision-making support,thereby facilitating medically appropriate and value-concordant care choices.
2.Relationship between spleen volume and non-alcoholic fatty liver disease by three-dimensional computed tomography reconstruction

Objective To investigate the association of spleen volume with the risk of non-alcoholic fatty liver disease(NAFLD)as well as their causal relationship.Methods We included 90 NAFLD cases and 47 healthy controls who had received contrast-enhanced computed tomography(CT)scan of the abdomen at the Second Affiliated Hospital of Xi'an Jiaotong University from November 2022 to November 2023.We conducted three-dimensional reconstruction of the spleen through a deep learning network model using a two-stage coarse-to-fine segmentation approach.We compared the two groups using the two-sample t test or Mann-Whitney U test for continuous data and using the chi-square test for categorical data;evaluated the correlation between spleen volume and liver function indicators through Pearson correlation or Spearman rank correlation analyses;determined the factors influencing the development of NAFLD through multivariable Logistic regression analysis;and further assessed the casual relationship between spleen volume and NAFLD using the inverse variance-weighted two-sample Mendelian randomization(IVW-MR)method.Results Spleen volume was significantly larger in NAFLD cases than in controls(272.93±104.16 vs 204.37±81.20 cm3,P<0.001).The Spearman rank correlation analysis showed that spleen volume was positively correlated with the hepatic steatosis index(rs=0.422,P<0.001)and gamma-glutamyl transferase levels(rs=0.211,P=0.047)in patients with NAFLD.The multivariable Logistic regression analysis indicated that spleen volume was an independent risk factor for the development of NAFLD(odds ratio[OR]=1.01,95%confidence interval[CI]:1.00-1.02,P=0.049).The IVW-MR analysis detected a causal relationship between spleen volume and NAFLD(OR=1.16,95%CI:1.05-1.28,P=0.005).Conclusion Increased spleen volume may be a risk factor for the development and progression of NAFLD.Further studies are still needed to investigate the specific mechanism.
3.Tiaojing Zhuyun formula combined with Dai meridian moxibustion method improves endometrial fibrosis in rats with uterine adhesion by regulating M2 type macrophage polarization

Objective:To explore the improvement effect of Tiaojing Zhuyun formula combined with Dai meridian moxibustion on endometrial fibrosis in rats with intrauterine adhesions(IUA)by regulating M2 type macrophage polarization.Methods:SD rats were selected to establish an IUA model using a dual injury method of mechanical curettage and lipopolysaccharide infection,which were randomly separated into model group,Tiaojing Zhuyun formula group,Dai meridian moxibustion group,Tiaojing Zhuyun formula+Dai meridian moxibustion group and Bujiale group,with 10 rats in each group,another 10 rats were regarded as a sham operation(control)group,after grouping and treatment,the number of embryo implantation of rats in each group after cage closure was detected;HE staining was applied to detect pathological morphology of uterine tissue in rats in different groups,and their endometrial thickness and glandular number were compared;Masson staining was applied to detect endometrial fibrosis in rats of each group,and the collagen volume fraction(CVF)was compared;ELISA was applied to detect serum levels of profibrotic factor TGF-β1,pro-inflammatory factors IL-1β and IL-6,and levels of procollagen type Ⅰ carboxy-terminal peptide(PⅠNP)and procollagen Ⅲ N-terminal peptide(PⅢNP)of rats in each group;immunofluorescence staining was applied to detect expressions of M2 type macrophage markers CD163 and CD68 in endometrium of rats in each group.Results:Compared with control group,model group showed obvious pathological damage to uterine tissue,thickness of endometrium was thinner,the number of glands and embryo implantation were obviously reduced(P<0.05),endometrial CVF,levels of serum PⅠNP,P Ⅲ NP,TGF-β1,IL-1β and IL-6,and the number of CD163 and CD68 positive cells in endometrium were increased(P<0.05);compared with model group,pathological damage to uterine tissue of rats in Tiaojing Zhuyun formula group,Dai meridian moxibustion group,Tiaojing Zhuyun formula+Dai meridian moxibustion group and Bujiale group was reduced,thickness of endometrium,the number of glands and embryo implantation,and the numbers of CD163 and CD68 positive cells in endometrium were increased(P<0.05),endometrial CVF and levels of serum PⅠNP,P Ⅲ NP,TGF-β1,IL-1β and IL-6 were reduced(P<0.05);compared with Tiaojing Zhuyun formula group and Dai meridian moxibustion group,pathological damage to uterine tissue of rats in Tiaojing Zhuyun formula+Dai meridian moxibustion group and Bujiale group was alleviated,thickness of endometrium,the number of glands and embryo implantation,and the number of CD163 and CD68 positive cells in endometrium were increased(P<0.05),endometrial CVF and levels of serum PⅠNP,P Ⅲ NP,TGF-β1,IL-1β and IL-6 were reduced(P<0.05);there was no obvious change in all indicators between Tiaojing Zhuyun formula+Dai meridian moxibustion group and Bujiale group(P>0.05).Conclusion:Combination of Tiaojing Zhuyun formula and Dai meridian moxibustion can promote the polarization of M2 type macrophages,thereby inhibiting inflammation and endometrial fibrosis in IUA rats,and improving their symptoms of uterine adhesions.
4.Intervention effect of health nutrition model based on digital management on patients with gestational diabetes mellitus

Objective:To explore the intervention effect of health nutrition model based on digital management on patients with gestational diabetes mellitus, and to provide reference for blood sugar control and pregnancy outcome improvement of gestational diabetes mellitus.Methods:Using a randomized controlled study method, totally 80 cases of gestational diabetes mellitus admitted to Langfang People's Hospital from October 2021 to March 2023 were prospectively selected and randomly divided into control group and experimental group according to the random number table method, with 40 cases in each group. The control group was given routine management, and the experimental group was combined with the health nutrition model based on digital management on this basis. Both groups began to intervene after the diagnosis of gestational diabetes mellitus, and the intervention lasted until one week before the patient's expected delivery. Follow-up to delivery. The delivery mode, weight gain during pregnancy, insulin use during pregnancy, pregnancy outcome and neonatal outcome of the two groups were statistically analyzed. The blood glucose level of the two groups before and after intervention were compared.Results:The age of the control group was 20-31 (26.25 ± 1.98) years old, while the age of the experimental group was 20-33 (25.93 ± 2.11) years old. The cesarean section rate in the experimental group was 32.50% (13/40), which was lower than that in the control group (57.50%, 23/40) ( χ2=5.05, P<0.05). The weight gain during pregnancy in the experimental group was (6.03 ± 0.89) kg, which was lower than (7.93 ± 1.36) kg in the control group ( t=7.39, P<0.05). After intervention, the levels of serum glycosylated hemoglobin, fasting blood glucose and 2 h postprandial blood glucose in the two groups were lower than those before intervention, and those in the experimental group were (5.19 ± 0.54) %, (4.98 ± 0.49) mmol/L, (6.04 ± 1.15) mmol/L, which were lower than (5.67 ± 0.75) %, (5.37 ± 0.54) mmol/L, (7.16 ± 1.36) mmol/L in the control group ( t=3.28, 3.42, 3.96, all P<0.05). The proportion of macrosomia in the experimental group was 2.50% (1/40), which was lower than 20.00% (8/40) in the control group ( χ2=4.51, P<0.05). The experimental group had no abnormal pregnancy outcome and neonatal outcome respectively as 92.50% (37/40) and 92.50% (37/40), which were 70.00% (28/40) and 57.50% (23/40) in the control group, their difference was statistically significant ( χ2=6.65, 13.07, both P<0.05). Conclusions:The application of health nutrition model based on digital management to intervene in patients with gestational diabetes could effectively improve their blood glucose control effect, improve pregnancy outcome and neonatal outcome, and hada good intervention effect.
5.A multi-center cross-sectional study on the status quo and influencing factors of moral distress of pediatric nurses

Objective:To assess the level of moral distress among pediatric nurses in China and explore its influencing factors, providing a reference for future interventions.Methods:A cross-sectional survey was conducted using a convenience sampling method from May 2022 to March 2023. Pediatric nurses meeting the inclusion and exclusion criteria were selected from children′s specialized hospitals, general hospitals, and maternal and child health hospitals across 16 provinces in China. Moral distress was assessed using the Chinese version of the Moral Distress Scale-Revised (MDS-R). Data analysis included descriptive statistics, non-parametric analysis, and multiple linear regression.Results:A total of 509 pediatric nurses participated in the survey, including 10 males and 499 females. Among them, 45 were aged ≤25 years, 268 were aged 26-35 years, 172 were aged 36-45 years, and 24 were aged >45 years.The median moral distress score was 58.00 (26.00, 77.50). Factors independently associated with higher levels of moral distress included having a master′s degree ( t = 2.33), working in a ward with more than 40 beds ( t = 3.38), working in a ward without parental accompaniment ( t = 2.60), being passively assigned to the nursing profession ( t = 2.63), and considering resignation but not leaving the job ( t = 3.85) (all P<0.05). Conclusions:Pediatric nurses in China experience a moderate level of moral distress. Further research is needed to understand its underlying causes and to develop preventive and intervention strategies.

7.Efficacy and Safety of Polatuzumab Vedotin Combined with Chemotherapy in the Treatment of Relapsed and Refractory Diffuse Large B-Cell Lymphoma

Objective:To observe the efficacy and safety of polatuzumab vedotin(pola)combined with chemotherapy in the treatment of relapsed and refractory diffuse large B-cell lymphoma(R/R DLBCL).Methods:A total of 23 patients with R/R DLBCL treated at the First Affiliated Hospital of Zhejiang University and its Liangzhu Branch from April 2023 to March 2024 were retrospectively collected.All patients were treated with pola combined with chemotherapy regimens such as BR,R-GDP,R-CHOP,or other regimens.Results:All 23 patients were evaluable for efficacy,with 10 achieving complete response(CR),7 partial response(PR),3 stable disease(SD),and 3 progressive disease(PD).The most common adverse events included myelosuppression,fever,and pulmonary infection.No severe adverse events resulted in drug withdrawal.Conclusion:Pola combined with chemotherapy demonstrates promising efficacy and a favorable safety profile in the treatment of R/R DLBCL.

Various chromatographic methods were comprehensively applied to study the chemical composition of the ethyl acetate extract from Aucklandiae Radix. The structures of all compounds were identified by analyzing their physicochemical properties and using spectroscopic methods. Two new sesquiterpenoids, named auclappsines A and B(1 and 2) were isolated and identified. Through in vitro high content screening and with the use of a guggulsterone-induced L02 cells, the effects of 1 and 2 on farnesoid X receptor(FXR) protein expression were investigated. The results showed that 1 had a significant FXR activation effect, providing a scientific basis for the development of drugs for the treatment of liver and gallbladder diseases.
