Objective To propose an evaluation and screening indicator for the reliability of emergency ventilators. Methods Firstly,a regression model was used to clean the data and remove noise to ensure the accuracy of regression analysis. Then, four groups of highly correlated data combinations, including inspiratory tidal volume-minute expiratory volume, peak airway pressure-minute expiratory volume, peak airway pressure-inspiratory tidal volume and positive end-expiratory pressure(PEEP)-mean airway pressure, were determined with the methods of curve fitting and transfer function, and the difference between the theoretical output and the actual output of the data combinations was regarded as an indicator to judge whether the ventilator functioned well or not;finally, the indicator proposed was applied to single and multiple ventilators, and the feasibility of the indicator was determined by the proportion of the ventilators functioning well. Results The evaluation results with a single ventilator showed the four groups of data combinations had the actual output fitted well with the theoretical output, and all the differences between the theoretical output and the actual output were lower than the threshold;the results with multiple ventilators indicated there were 71.49%ventilators functioning well, which was very close to the actual result that 72%ventilators behaved well. Conclusion The evaluation and screening indicator for emergency ventilators has high feasibility, and theoretical support is provided for reliability assessment and selection of series of emergency treatment equipment.[Chinese Medical Equipment Journal,2024,45(7):8-16]

Objective To apply a COMSOL-based finite element analysis method to investigating the electric field effects produced by the human lower limb musculoskeletal system under the synergistic effects of stress field and electromagnetic field. Methods Firstly,a 3D human body model was constructed by Maxon Cinema 4D R21 software, and then imported into COMSOL 6.1 software in STL format.Secondly,an electromagnetic field intervention and stress loading model for the left lower limb of the human body was designed and constructed,in which 15 Hz quasi-pulse group current signals were used for electromagnetic field excitation and the stress field was realized by applying a vibration load with an average compressive force of about 90 N/cm2 to the left foot of the human body. Finally, the electromagnetic properties of human tissue were simulated by numerical simulation, and then the effects of stress field or elecromagnetic field or combined stress field and electromagnetic field on human bioelectric field were compared. Results Simulation results showed that the electric field intensity peaked at the leg joints under both electromagnetic and stress fields acting alone or synergistically, the bioelectric field intensity generated by the human body was related to the distance from the exogenous excitation loading location, and the electric field generated under synergistic action was equivalent to the linear superposition of the bioelectric field in the tissue induced by the electromagnetic field and the stress field acting alone. Conclusion Data supplement is provided for predicting bioelectric field changes within the musculoskeletal tissue, and theoretical foundation is laid for the development and application of multi-physics field synergistic intervention therapy for treating the disorders of the lower limb musculos-keletal system.[Chinese Medical Equipment Journal,2024,45(9):21-26]

Objective:To evaluate the early clinical outcomes of modified suspended fixation with triple plates and double beams through a limited incision in the treatment of acroxoclavicular dislocation. Methods:A retrospective study was conducted to analyze the 23 patients with acromioclavicular dislocation who had been treated at Department of Orthopedics, The People's Hospital of Northern Jiangsu from June 2021 to August 2023 by modified suspended fixation with triple plates and double beams through a limited incision. There were 9 males and 14 females with an age of (52.2±10.6) years. The incision length, operation time, intraoperative bleeding and complications were recorded. The visual analogue scale (VAS) pain score, Constant-Murley shoulder score, American Shoulder and Elbow Surgeons (ASES) score, range of shoulder motion (anterior flexion, abduction, lateral rotation) were recorded and compared between preoperation and the last follow-up. The coracoclavicular distance (CCD), clavicle tract width and coracoid tract width were measured and compared between postoperative day 2 and the last follow-up.Results:The patients were followed up for 12 (12, 16) months. The incision length was 5 (4, 5) cm, the operation time 50 (40, 60) min, and the intraoperative bleeding 30 (20, 30) mL. No such complications occurred as incision infection, fracture, or failure of internal fixation. The VAS pain score, Constant-Murley shoulder score, ASES score, anterior flexion, abduction and lateral rotation of the shoulder at the last follow-up were, respectively, 0 (0, 0) point, (92.8±4.9) points, (90.4±5.7) points, 175 °(170°, 180°), 175°(170°, 175°), and 85°(80°, 85°), significantly improved compared with the preoperative values [6 (5, 6) points, (40.4±7.2) points, (39.3±7.1) points, 85°(80°, 85°), 85°(80°, 90°), and 35°(35°, 40°)] ( P<0.05). The CCD was (6.1±1.3) mm at postoperative day 2 and (7.5±1.7) mm at the last follow-up, showing a significant difference ( P<0.05). Compared with postoperative day 2, the CCD widening at the last follow-up was <4mm in all patients, giving an excellent rate of 100% (23/23). The medial clavicle tract widening and the medial coracoid tract widening at the last follow-up were both <2 mm in all patients, giving an excellent rate of 100% (23/23). The lateral clavicle tract widening at the last follow-up was >2 mm in 2 patients, giving an excellent rate of 91% (21/23). Conclusions:In the treatment of acroxoclavicular dislocation, our modified suspended fixation with triple plates and double beams through a limited incision can lead to reliable reduction fixation and good recovery of shoulder function, showing advantages of short surgical time, easy surgical operation, high safety, and minimal postoperative widening of bone tracts.

Objective To explore the effects of oral voriconazole(VRC)on the pharmacokinetics of tacrolimus(TAC)in renal transplant patients. Methods Renal transplant patients who had taken TAC orally for more than 2 days and achieved steady-state plasma concentration before taking VRC.The trough concentration of TAC was measured on the 3rd,5th and 10th days after VRC 200 or 400 mg·d-1 administration. The trough concentration(C0)of TAC was determined by high performance liquid chromatography. The genotypes of TAC were determined by polymerase chain reaction and the pharmacokinetics of TAC after combined use of VRC were compared. Results After the use of VRC,the TAC C0 of 11 renal transplant patients was 3-8 μg·L-1,and the concentration of TAC ranged from 50.00%to 87.50%of the original dose.Additionally, the impact of VRC on TAC varied significantly among individuals. The mean TAC C0 value after VRC administration was significantly higher than the value before VRC[(12.14±3.89)vss(5.20±2.79)μg·L-1].Eleven renal transplant patients were grouped according to cytochrome P450(CYP)2C19-CYP3A5 gene polymorphism, under the condition of combined administration, the C0/dose of TAC in the slow metabolizer group was higher than that in the fast metabolizer group on the 3rd,5th and 10th days[(582.10±252.30)vs(439.03±166.08),(873.71±449.22)vs(666.60±168.00),(852.10±505.73)vs(261.50±81.98)μg·L-1·mg-1·kg;all P<0.01].Conclusion TAC pharmacokinetics was significantly affected by the VRC in renal transplant recipients, and the principle that TAC dose needed to be reduced by one-third of the original dose was no longer applicable, which may be related to the pharmacokinetics of the VRC itself and the gene polymorphism of CYP2C19/CYP3A5 enzyme. It is recommended to regularly monitor the concentration of TAC when VRC and TAC are used in combination.
