AIM: To observe the preoperative and postoperative clinical efficacy of comprehensive dry eye treatment in cataract patients complicated with diabetes mellitus, with a focus on comparing the differences between the two groups in terms of postoperative dry eye symptoms, tear film function, visual recovery, and complication rates, aiming to provide an effective protocol for the perioperative management and long-term prognosis improvement in this patient population.METHODS:Patients diagnosed with both cataract and diabetes mellitus and presenting with varying degrees of dry eye symptoms, scheduled for cataract surgery at the hospital from May 2023 to December 2024, were enrolled as study subjects. They were divided into two groups using a random number method: the control group received sodium hyaluronate eye drops alone, and the experimental group underwent comprehensive preoperative dry eye treatment(sodium hyaluronate eye drops+warm compress+meibomian gland cleaning and massage+Chinese herbal fumigation+health guidance). Tear film breakup time(BUT), corneal fluorescein staining(FL), Ocular Surface Disease Index(OSDI)score, tear meniscus height(TMH), and non-invasive first tear film breakup time(NIBUT)were compared between the two groups before and after 4 wk of treatment. Meibomian gland loss, tear film lipid layer thickness, and basic ocular symptoms were also assessed.RESULTS:This study included 60 eyes of 60 patients, with a control group of 30 eyes of 30 patients(aged 56.24±10.24 y, 13 males and 17 females)and an experimental group of 30 eyes of 30 patients(aged 58.01±9.79 y, 15 males and 15 females). After 4 wk of preoperative treatment, the BUT in the experimental group increased from 4.09±1.13 s to 10.35±1.46 s, and from 4.15±1.05 s to 8.26±1.36 s in the control group, showing a significant intergroup difference(t=5.737, P<0.001). The FL score in the experimental group decreased from 6.83±0.46 points to 2.86±0.38 points, whereas in the control group it decreased from 6.79±0.39 points to 5.32±0.43 points(t=23.480, P<0.001). After 4 wk of treatment, the NIBUT in the experimental group increased from 5.19±1.12 s to 9.36±1.47 s, compared to an increase from 5.21±1.04 s to 7.18±1.25 s in the control group(t=6.188,P<0.001). The proportion of patients with a thin tear film lipid layer was significantly higher in the experimental group than in the control group(all P<0.01). Ocular clinical symptoms decreased after treatment in both groups, with the experimental group showing lower scores than the control group(all P<0.001).CONCLUSION:Preoperative comprehensive dry eye treatment can multi-dimensionally improve dry eye symptoms and tear film stability in cataract patients with diabetes mellitus, providing an effective strategy for the perioperative management of cataract patients.

Prostate cancer is one of the most common malignant tumors of male genitourinary system. Prostate cancer has the following characteristics: insidious onset, early asymptomatic or not obvious symptoms, complex etiology and pathogenesis, long incubation period and so on. Therefore, the realization of its early diagnosis and treatment is of great significance to the prognosis of patients. Prostate-specific membrane antigen (PSMA) is a type 2 transmembrane glycoprotein that is highly expressed on the membrane of almost all primary and metastatic prostate cancer cells, and is an ideal target for prostate cancer imaging and treatment. In recent years, with the approval of urea-containing small molecule PET (positron emission computed tomography) radiopharmaceutical based on PSMA (68Ga-PSMA-11, 18F-PSMA-1007), PET-CT (positron emission computed tomography/computed tomography) has shown new potential for early diagnosis and accurate staging of prostate cancer patients. This review mainly summarizes the research progress of urea-containing PSMA PET imaging agents and finds that they have defects such as uptake in non-target tissues like the kidneys, lacrimal glands, and salivary glands. Thus, further optimizing their structure to reduce the uptake in non-target tissues, providing provide convenience for the labeling of therapeutic radiopharmaceuticals, thereby achieving the goal of integrated diagnosis and treatment, is an important development direction in this field.

PURPOSE:
The secondary damage of spinal cord injury (SCI) starts from the collapse of the blood spinal cord barrier (BSCB) to chronic and devastating neurological deficits. Thereby, the retention of the integrity and permeability of BSCB is well-recognized as one of the major therapies to promote functional recovery after SCI. Previous studies have demonstrated that activation of hypoxia inducible factor-1α (HIF-1α) provides anti-apoptosis and neuroprotection in SCI. Endogenous HIF-1α, rapidly degraded by prolylhydroxylase, is insufficient for promoting functional recovery. Dimethyloxalylglycine (DMOG), a highly selective inhibitor of prolylhydroxylase, has been reported to have a positive effect on axon regeneration. However, the roles and underlying mechanisms of DMOG in BSCB restoration remain unclear. Herein, we aim to investigate pathological changes of BSCB restoration in rats with SCI treated by DOMG and evaluate the therapeutic effects of DMOG.
METHODS:
The work was performed from 2022 to 2023. In this study, Allen's impact model and human umbilical vein endothelial cells were employed to explore the mechanism of DMOG. In the phenotypic validation experiment, the rats were randomly divided into 3 groups: sham group, SCI group, and SCI + DMOG group (10 rats for each). Histological analysis via Nissl staining, Basso-Beattie-Bresnahan scale, and footprint analysis was used to evaluate the functional recovery after SCI. Western blotting, TUNEL assay, and immunofluorescence staining were employed to exhibit levels of tight junction and adhesion junction of BSCB, HIF-1α, cell apoptosis, and endoplasmic reticulum (ER) stress. The one-way ANOVA test was used for statistical analysis. The difference was considered statistically significant at p < 0.05.
RESULTS:
In this study, we observed the expression of HIF-1α reduced in the SCI model. DMOG treatment remarkably augmented HIF-1α level, alleviated endothelial cells apoptosis and disruption of BSCB, and enhanced functional recovery post-SCI. Besides, the administration of DMOG offset the activation of ER stress induced by SCI, but this phenomenon was blocked by tunicamycin (an ER stress activator). Finally, we disclosed that DMOG maintained the integrity and permeability of BSCB by inhibiting ER stress, and inhibition of HIF-1α erased the protection from DMOG.
CONCLUSIONS
Our findings illustrate that the administration of DMOG alleviates the devastation of BSCB and HIF-1α-induced inhibition of ER stress.

Compounds isolated from Epimedium include the total flavonoids of Epimedium , icariin, and its metabolites (icaritin, icariside I, and icariside II), which have similar molecular structures. Modern pharmacological research and clinical practice have proved that Epimedium and its active components have a wide range of pharmacological effects, especially in improving sexual function, hormone regulation, anti-osteoporosis, immune function regulation, anti-oxidation, and anti-tumor activity. To date, we still need a comprehensive source of knowledge about the pharmacological effects of Epimedium and its bioactive compounds on the male reproductive system. However, their actions in other tissues have been reviewed in recent years. This review critically focuses on the Epimedium , its bioactive compounds, and the biochemical and molecular mechanisms that modulate vital pathways associated with the male reproductive system. Such intrinsic knowledge will significantly further studies on the Epimedium and its bioactive compounds that protect the male reproductive system and provide some guidances for clinical treatment of related male reproductive disorders.

Journal of Geriatric Cardiology 2025;22(7):678-689
BACKGROUND:
Medical informatics accumulated vast amounts of data for clinical diagnosis and treatment. However, limited access to follow-up data and the difficulty in integrating data across diverse platforms continue to pose significant barriers to clinical research progress. In response, our research team has embarked on the development of a specialized clinical research database for cardiology, thereby establishing a comprehensive digital platform that facilitates both clinical decision-making and research endeavors.
METHODS:
The database incorporated actual clinical data from patients who received treatment at the Cardiovascular Medicine Department of Chinese PLA General Hospital from 2012 to 2021. It included comprehensive data on patients' basic information, medical history, non-invasive imaging studies, laboratory test results, as well as peri-procedural information related to interventional surgeries, extracted from the Hospital Information System. Additionally, an innovative artificial intelligence (AI)-powered interactive follow-up system had been developed, ensuring that nearly all myocardial infarction patients received at least one post-discharge follow-up, thereby achieving comprehensive data management throughout the entire care continuum for high-risk patients.
RESULTS:
This database integrates extensive cross-sectional and longitudinal patient data, with a focus on higher-risk acute coronary syndrome patients. It achieves the integration of structured and unstructured clinical data, while innovatively incorporating AI and automatic speech recognition technologies to enhance data integration and workflow efficiency. It creates a comprehensive patient view, thereby improving diagnostic and follow-up quality, and provides high-quality data to support clinical research. Despite limitations in unstructured data standardization and biological sample integrity, the database's development is accompanied by ongoing optimization efforts.
CONCLUSION
The cardiovascular specialty clinical database is a comprehensive digital archive integrating clinical treatment and research, which facilitates the digital and intelligent transformation of clinical diagnosis and treatment processes. It supports clinical decision-making and offers data support and potential research directions for the specialized management of cardiovascular diseases.

ObjectiveTo explore the effects of Kaixuan Jiedu core prescription (KXJD) on sphingolipid metabolism in the mouse model of imiquimod-induced psoriasis-like skin lesions. MethodsThirty-seven male C57BL/6J mice were randomly assigned into five groups: healthy control (n=11), model (n=11), methotrexate (MTX, n=5), low-dose (15.21 g·kg-1) KXJD (n=5), and high-dose (30.42 g·kg-1) KXJD (n=5). Psoriasis-like skin lesions were induced in mice with 62.5 mg 5% imiquimod cream applied on the back. The KXJD groups and MTX group were treated with 0.2 mL corresponding decoction and MTX, respectively, by gavage daily, while the other groups were given an equal volume of normal saline by the same way. After 5 days of treatment, back skin lesions were collected. Firstly, healthy control and model mice were selected for tandem mass tag (TMT) quantitative proteomics (control vs model=3 vs 3) and targeted lipid metabolomics (control vs model=11 vs 11). Then, the binding degree between core components and target proteins was predicted via network pharmacology and molecular docking. Finally, an animal experiment was performed to decipher the specific regulation mechanism of KXJD on sphingolipid metabolism. Immunohistochemistry was employed to determine the expression level of sphingosine-1-phosphate (S1P), and Western blot was employed to determine the expression levels of sphingosine kinase 2 (SPHK2) and monocyte chemotactic protein-1 (MCP-1). ResultsTMT proteomics and targeted lipid metabolomics suggested that sphingolipid metabolism was active in the psoriatic skin, and key proteases [serine palmitoyltransferase, long chain base subunit 2 (SPTLC2), SPHK2, delta(4)-desaturase sphingolipid 1 (Degs1), and ceramide synthase 4 (CerS4)] and 8 sphingolipid metabolites (including ceramides, sphingol, sphingomyelin, and glycosphingolipid) expressed abnormally (P<0.05) compared with those in the healthy skin. The molecular docking results indicated that the binding energy between the active components (quercetin, kaempferol, and luteolin) in KXJD and key proteins involved in sphingolipid metabolism was less than-8 kal·mol-1. Further experimental verification showed elevated expression levels of SPHK2, S1P, and MCP-1 in psoriatic skin compared with healthy skin (P<0.05), and KXJD down-regulated the expression levels of SPHK2, S1P, and MCP-1 compared with the model group (P<0.05). ConclusionThis study indicates that there is an imbalance in sphingolipid metabolism in psoriatic skin lesions. KXJD may reduce psoriasis-like lesions in mice by regulating sphingolipid metabolism via the SPHK2/S1P/MCP-1 pathway.
