1.Effect of Acupuncture at Neiguan (PC6) on Improving Autism by Promoting Myelination Through The METTL14/m⁶A/PTEN Axis Based on “Xuanfu-Suiqiao” Theory

ObjectiveTo clarify whether METTL14 mediates the core role of acupuncture at Neiguan (PC6) in promoting myelination and improving behavior in young autistic rats through gene intervention technology. MethodsThe ASD model was established by intraperitoneal injection of valproic acid (VPA) in pregnant rats. Male offspring were intracerebroventricularly injected with adenovirus-packaged METTL14 shRNA (sh-METTL14) or its control (sh-NC) on postnatal day 1, with a model group set as well. Subsequently, the juvenile rats were divided into model group, acupuncture group, acupuncture+sh-NC group, and acupuncture+sh-METTL14 group. The acupuncture group received acupuncture at Neiguan (PC6) from postnatal day 7, once daily for 21 consecutive days. Neurobehavioral changes were evaluated by behavioral tests; METTL14 knockdown efficiency and the expression of METTL14, METTL3, and PTEN were detected by quantitative real-time PCR (qRT-PCR) and Western blot (WB); PTEN m6A levels were measured by RNA immunoprecipitation-qPCR (RIP-qPCR); myelin ultrastructure, expression of myelin basic protein (MBP) and neurofascin 155 (NF155), and dendritic spine density were observed using transmission electron microscopy (TEM), enzyme-linked immunosorbent assay (ELISA), immunofluorescence, qRT-PCR, and primary neuron culture. ResultsBehaviorally, knockdown of METTL14 significantly counteracted the beneficial effects of acupuncture in improving self-grooming, open field exploration, three-chamber social interaction, and Morris water maze learning and memory (P<0.05, P<0.01). Compared with the acupuncture+sh-NC group, the acupuncture+sh-METTL14 group showed significantly decreased mRNA and protein expression of hippocampal METTL14 (P<0.01), and the upregulating effects of acupuncture on METTL3 and PTEN expression were reversed (P<0.01). Meanwhile, knockdown of METTL14 significantly inhibited the acupuncture-induced increase in PTEN m6A levels (P<0.01). Morphologically, knockdown of METTL14 attenuated the improvement of myelin structure by acupuncture, reversed the downregulation of MBP and upregulation of NF155 induced by acupuncture, and blocked the increase in dendritic spine density (P<0.05, P<0.01). ConclusionMETTL14 is a key molecule mediating the therapeutic effect of acupuncture at Neiguan. Acupuncture at Neiguan upregulates METTL14, thereby enhancing m6A methylation modification of PTEN mRNA to stabilize its expression, ultimately promoting myelin development and improving behavioral symptoms in ASD juvenile rats. This preliminarily reveals the modern biological connotation of “opening Xuanfu and dredging myelin”.
2.Evaluation of the quality of Jingangteng capsules based on UPLC fingerprinting combined with multi-component content determination

OBJECTIVE To establish the UPLC fingerprint and the method for multi-component content determination in Jingangteng capsules, and to evaluate its quality by combining chemical pattern recognition analysis. METHODS An UPLC method was established. Separation was performed on a Zorbax SB-C 18 Rapid Resolution HD column, with acetonitrile-0.1% formic acid as the mobile phase for gradient elution.Using the Similarity Evaluation System for Chromatographic Fingerprints of Traditional Chinese Medicines (2012 edition), UPLC fi ngerprints were established for 10 batches of Jingangteng capsules, and similarity was evaluated. SPSS 22.0 and SIMCA 14.1 software were used to perform hierarchial-cluster analysis and orthogonal partial least squares discriminant analysis (OPLS-DA), respectively. The same UPLC method was employed to determine the contents of chlorogenic acid, 3,5-dihydroxy-2-methylbenzoic acid-3- O -glucoside (M1), caffeic acid, astilbin, oxyresveratrol, quercitrin and resveratrol in the 10 batches of samples. RESULTS A total of 17 common peaks were identified in UPLC fingerprints of the 10 batches of samples, of which 7 were identified as chlorogenic acid, M1, caffeic acid, astilbin, oxyresveratrol, quercitrin, and resveratrol. The similarities of 10 batches of samples ranged from 0.820 to 0.985. The results of hierarchial-cluster analysis showed that 10 batches of samples were grouped into four categories: S1-S4 formed one group, S5 and S6 formed another, S7, S8 and S10 formed a third, and S9 formed a fourth, consistent with the OPLS-DA results; the variable importance projection values for peaks 7, 10, 2, 16 (resveratrol), 13 (oxyresveratrol), 11, 6 (caffeic acid), 5 (M1) and 15 (quercitrin) were >1. Quantitative analysis results showed that the contents of chlorogenic acid, M1, caffeic acid, astilbin, oxyresveratrol, quercitrin, and resveratrol were 1.650 8-4.213 7, 0.636 2-2.161 7, 0.031 0-0.086 5, 0.239 1-1.069 3, 0.211 9-1.104 0, 0.488 8-2.399 2, and 0.164 0-0.699 8 mg/g, respectively. CONCLUSIONS UPLC fingerprint and content determination methods established in this study are simple to operate, accurate, reliable and reproducible; when combined with chemical pattern recognition analysis, they can be used to evaluate the quality of Jingangteng capsules. Nine components, such as resveratrol, oxyresveratrol, caffeic acid, M1 and quercitrin, may serve as markers of quality variation.
3.Perioperative immune dynamics and clinical outcomes in patients undergoing on-pump cardiac surgery

Objective: To characterize perioperative dynamic changes in immune-cell phenotypes and inflammatory cytokines in patients undergoing CPB (cardiopulmonary bypass) cardiac surgery, and to explore their associations with postoperative outcomes. Methods: In this prospective cohort study, 120 adult patients who underwent elective cardiac surgery under CPB at West China Hospital from May 2022 to March 2023 were enrolled. Perioperative immune-cell phenotypes and concentrations of 40 inflammation-related cytokines were measured. The primary outcomes were the sequential organ failure assessment (SOFA) score at 24 h after surgery and ΔSOFA (the peak SOFA score within 48 h after surgery minus the preoperative SOFA score). Secondary outcomes included major adverse cardiovascular events (MACE), acute kidney injury (AKI), respiratory failure, severe liver injury, and infection. Results: The mean age of enrolled patients was 57±10 years. Of these, 52% (62/120) were male and 90% (108/120) underwent valve surgery. Plasma analysis showed that multiple pro-inflammatory cytokines increased during CPB and remained elevated up to 24 h after surgery; five chemokines and the anti-inflammatory cytokine IL-10 peaked at the end of CPB. The SOFA score increased from 1 (1, 2) preoperatively to 7 (5, 10) at 24 h after surgery, with a ΔSOFA of 6 (4, 8). Within 30 days after surgery, 48 patients (40.0%) developed AKI, 17 (14.2%) developed infection, 4 (3.3%) developed severe liver injury, 3 (2.5%) developed respiratory failure, and 3 (2.5%) experienced MACE. During the 2-year follow-up, 8 patients (6.7%) experienced MACE and 5 (4.2%) died. Conclusion: Multi-organ dysfunction is common after cardiac surgery under CPB (median ΔSOFA, 6), accompanied by perioperative activation of multiple immune-cell subsets and upregulation of pro-inflammatory, anti-inflammatory, and chemotactic mediators. This study provides data-driven evidence and research clues for further investigation of the associations between CPB-related immune perturbations and postoperative organ dysfunction and clinical outcomes.
4.Multi-label fundus disease classification using dual-branch deep learning: an intelligent diagnosis framework inspired by traditional Chinese medicine Five Wheels theory

Objective:
To develop a dual-branch deep learning framework for accurate multi-label classification of fundus diseases, addressing the key limitations of insufficient complementary feature extraction and inadequate cross-modal feature fusion in existing automated diagnostic methods.
Methods:
The fundus multi-label classification dataset with 12 disease categories (FMLC-12) dataset was constructed by integrating complementary samples from Ocular Disease Intelligent Recognition (ODIR) and Retinal Fundus Multi-Disease Image Dataset (RFMiD), yielding 6 936 fundus images across 12 retinal pathology categories, and the framework was validated on both FMLC-12 and ODIR. Inspired by the holistic multi-regional assessment principle of the Five Wheels theory in traditional Chinese medicine (TCM) ophthalmology, the dual-branch multi-label network (DBMNet) was developed as a novel framework integrating complementary visual feature extraction with pathological correlation modeling. The architecture employed a TransNeXt backbone within a dual-branch design: one branch processed red-green-blue (RGB) images to capture color-dependent features, such as vascular patterns and lesion morphology, while the other processed grayscale-converted images to enhance subtle textural details and contrast variations. A feature interaction module (FIM) effectively integrated the multi-scale features from both branches. Comprehensive ablation studies were conducted to evaluate the contributions of the dual-branch architecture and the FIM. The performance of DBMNet was compared against four state-of-the-art methods, including EfficientNet Ensemble, transfer learning-based convolutional neural network (CNN), BFENet, and EyeDeep-Net, using mean average precision (mAP), F1-score, and Cohen's kappa coefficient.
Results:
The dual-branch architecture improved mAP by 15.44 percentage points over the single-branch TransNeXt baseline, increasing from 34.41% to 44.24%, and the addition of FIM further boosted mAP to 49.85%. On FMLC-12, DBMNet achieved an mAP of 49.85%, a Cohen’s kappa coefficient of 62.14%, and an F1-score of 70.21%. Compared with BFENet (mAP: 45.42%, kappa: 46.64%, F1-score: 71.34%), DBMNet outperformed it by 4.43 percentage points in mAP and 15.50 percentage points in kappa, while BFENet achieved a marginally higher F1-score. On ODIR, DBMNet achieved an F1-score of 85.50%, comparable to state-of-the-art methods.
Conclusion
DBMNet effectively integrates RGB and grayscale visual modalities through a dual-branch architecture, significantly improving multi-label fundus disease classification. The framework not only addresses the issue of insufficient feature fusion in existing methods but also demonstrates outstanding performance in balancing detection across both common and rare diseases, providing a promising and clinically applicable pathway for standardized, intelligent fundus disease classification.

6.Using catheter "bare-support" technique under local anesthesia via transradial artery approach to treat severe vertebrobasilar artery stenosis:single center clinical experience

Objective To explore the safety and efficacy of using the"bare-support"technique via radial artery approach under local anesthesia for endovascular treatment of severe vertebral-basilar artery stenosis.Methods Patients were retrospectively and continuously enrolled from June 2021 to March 2023,admitted to the Department of Neurology at Zhongnan Hospital of Wuhan University,and underwent endovascular treatment for severe vertebral-basilar artery stenosis under local anesthesia via the radial artery approach using the"bare-support"technique.Baseline and clinical data,including sex,age,hypertension,diabetes,hyperlipidemia,coronary heart disease,atrial fibrillation,history of stroke,smoking history,preoperative the National Institutes of Health stroke scale(NIHSS)score,and preoperative modified Rankin scale score were collected from the patient.Surgical-related data were also collected,including site of stenosis(vertebral artery V4 segment,basilar artery,vertebral artery V4 segment+basilar artery),stenosis rate(70%-90%,>90%-99%),target lesion vessel diameter,lesion vessel length,vertebral-subclavian angle,vertebral-subclavian distance,use of distal access catheter,anesthesia method(local anesthesia,general anesthesia),presence of isolated vertebral artery(opposite vertebral artery occlusion),whether the distal access catheter was shaped,position of the distal access catheter in the vertebral artery(vertebral artery V2 segment,V3 segment,V4 segment),endovascular treatment method(simple balloon dilation,balloon dilation+self-expanding stent,balloon dilation stent),intraoperative cerebral ischemia symptoms,severe intraoperative cerebral ischemia symptoms,and whether the surgical approach was changed.Gather clinical outcomes and follow-up status,including the surgery success rate(surgery completed via radial artery access with a residual stenosis rate of the target vessel less than 30%after endovascular treatment,and modified thrombectomy in cerebral infarction[mTICI]grade 3),neurological function impairment within 72hours after surgery,perioperative complications(disabling stroke,nondisabling stroke,puncture site complications),in-stent restenosis within six months post-surgery(≥50%narrowing reoccurring at the location or within 5 mm diameter of stent after previous stent placement or angioplasty)and stroke relapses(another stroke event after the first stroke).Base on whether the distal access catheter was shaped,the patients were divided into a shaping group and a non-shaping group.Compare the baseline and clinical data,as well as the vertebral-subclavian angle and distance between the groups.Results A total of 33 patients who underwent endovascular treatment for severe vertebral-basilar artery stenosis using the"bare stent"technique via the radial artery approach were included,comprising 29 males and 4 females,aged between 42 to 76 years,with an average age of(62±9)years.Among them,13 were in the shaping group,20 was in the non-shaping group.(1)No significant differences in clinical or baseline data were observed between the two groups(all P>0.05).Compared with the non-shaping group,patients from the shaping group had significantly smaller vertebral-subclavian angle([62.80±21.57]° vs.[109.57±28.63]°,P<0.01),and significantly longer vertebral-subclavian distance([13.58±7.35]mm vs.[6.13±4.31]mm,P=0.002).(2)Among the 33 cases,30(90.9%)were completed under local anesthesia,while 3 cases(9.1%)with isolated vertebral arteries experienced severe ischemic intolerance during surgery and were switched to general anesthesia.The success rate of endovascular treatment for severe vertebrobasilar artery stenosis via transradial artery approach was 93.9%(31/33),with only 2 cases switched to transfemoral approach due to difficulty in establishing the radial artery access also succeeded in completing the surgery.Neurological function impairment occurred in 5 cases(15.2%)within 72 hours postoperatively.The perioperative complication rate was 9.1%(3/33),including 1 case(3.0%)of disabling stroke and 2 cases(6.1%)of non-disabling stroke.No puncture site complications had occurred.During the 6-month follow-ups,in-stent restenosis occurred in 2 cases(6.1%),with no recurrence of stroke.Conclusions Endovascular treatment of severe vertebrobasilar artery stenosis using the"bare-support"technique via the radial artery under local anesthesia is safe and feasible.Larger prospective randomized controlled trials are needed to validate these findings.
7.Clinical characteristics of patients with pregnancy-related chronic pain visiting pain clinic

Objective:To analyze the clinical characteristics of patients with pregnancy-related chronic pain visiting the pain clinic.Methods:The number of pregnant patients who completed a pregnancy registration at the Obstetrics and Gynecology Hospital Affiliated to Nanjing Medical University from 2022 to 2024 was collected. The medical records were reviewed to identify the patients who visited the department of pain of our hospital due to chronic pain related to pregnancy. The clinical characteristics such as the visiting situation, gestational weeks, age and types of pain were analyzed.Results:From 2022 to 2024, the total number of registered pregnant patients was 64, 818, of which, 2, 224 cases visited the pain clinic, and the annual proportions of pregnancy-related chronic pain visits were 2.540%, 3.836% and 3.889% respectively. Among the patients who attended the clinic, 77.97% were pregnant (5.82% in early pregnancy, 41.93% in mid-pregnancy, and 52.25% in late pregnancy), and 21.03% were postpartum patients. A total of 83.72% were aged 20-34 yr. The types of pain were pelvic girdle pain (40.96%), limb joint pain (28.82%), low back pain (14.16%), cervical spondylosis (3.64%), peripheral nerve entrapment syndrome (3.42%), headache (2.97%), chest and back pain (2.79%), pelvic and perineal pain (1.66%), neuralgia (0.94%) and other pains (0.63%).Conclusions:From 2022 to 2024, the proportion of registered pregnant women at our hospital who visited to the pain clinic due to pregnancy-related chronic pain increases year by year. The common types of pain are pelvic girdle pain, limb joint pain and low back pain.
8.Wound infection caused by Mycobacterium senegalense:one case report and literature review

Objective To explore the clinical features,diagnosis,and treatment of Mycobacterium senegalense infection.Methods We reported a case of postoperative wound infection caused by M.senegalense in a patient who underwent spinal intradural tumor surgery at Beijing Tsinghua Changgung Hospital.CNKI,Wanfang,VIP,and PubMed databases were searched from inception to April 1,2024 using keywords Mycobacterium senegalense both in Chinese and English to identify relevant reports.Thirteen eligible articles were retrieved,including 1 in Chinese and 12 in English.The clinical data of the 13 cases of M.senegalense infection were reviewed and analyzed.Results A case of postoperative wound infection caused by M.senegalense was reported in a patient who underwent spinal intradural tumor surgery.The patient was a 53-year-old male.He was previously healthy.The patient was initially treated for schwannoma by resection of an intraspinal space-occupying lesion at another hospital.Two weeks after the operation,the skin surrounding the surgical incision became redness,swelling,and pain.The patient did not have fever.M.senegalense was identified from pus culture.The patient was treated with doxycycline,moxifloxacin,and clarithromycin combination therapy,as well as wound disinfection and dressing changes.The infected wound subsequently healed.Among the 13 patients with M.senegalense infection,30.8%(4/13)were males and 69.2%(9/13)were females.The average age of patients was(51.6±17.7)years.The reported cases included skin and soft tissue infection(7 cases),bloodstream infection(2 cases),artificial joint infection(2 cases),and one case each of osteomyelitis and prosthetic infection.About half(46.2%,6/13)of the patients were immunosuppressed.Most(76.9%,10/13)of the patients were infected after surgery and trauma.Antimicrobial susceptibility test showed that all of the M.senegalense isolates were susceptible to amikacin,ciprofloxacin,clarithromycin and doxycycline.Conclusions M.senegalense infection is rarely reported.It primarily occurs following surgical procedures or trauma.Appropriate and adequate antibiotic combination therapy based on antimicrobial susceptibility testing generally results in favorable outcomes.
9.Patients waiting for a liver transplantation from the perspective of psychological resilience:a qualitative study

Objective To explore the psychological feelings and coping resources of patients waiting for a liver transplantation,in order to provide references for implementing psychological nursing care.Methods Using purposive sampling,15 patients waiting liver transplantation surgery from June to November 2024 at a tertiary hospital in Qingdao were selected as study subjects.Semi-structured in-depth interviews were conducted,and data were analyzed using interpretative phenomenological analysis to extract themes.Results The psychological feelings of patients waiting for a liver transplantation included 2 main themes with 6 sub-themes,namely,the complex and contradictory psychological feelings of liver transplant patients during the waiting period(expectations for transplanta-tion and future outlook,multiple psychological burdens intertwining,introspection and growth),and mobilizing psychological resilience resources to deal with complex psychological feelings(core resources,internal resources,external resources).Conclusion Clinical medical staff should implement psychological care for patients waiting for a liver transplantation based on the complex and contradictory psychological feelings they experience.Also,they should target the coping resources for psychological feelings to construct coping strategies,thereby comprehensively improving patients' psychological health during the waiting period.The coping strategies include promoting patients' health behaviors,improving their psychological status,and providing multidimensional social support for them.
10.Guideline for Adult Weight Management in China

Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.
