1.Observation on the efficacy of preoperative comprehensive dry eye treatment in cataract patients with diabetes

AIM: To observe the preoperative and postoperative clinical efficacy of comprehensive dry eye treatment in cataract patients complicated with diabetes mellitus, with a focus on comparing the differences between the two groups in terms of postoperative dry eye symptoms, tear film function, visual recovery, and complication rates, aiming to provide an effective protocol for the perioperative management and long-term prognosis improvement in this patient population.METHODS:Patients diagnosed with both cataract and diabetes mellitus and presenting with varying degrees of dry eye symptoms, scheduled for cataract surgery at the hospital from May 2023 to December 2024, were enrolled as study subjects. They were divided into two groups using a random number method: the control group received sodium hyaluronate eye drops alone, and the experimental group underwent comprehensive preoperative dry eye treatment(sodium hyaluronate eye drops+warm compress+meibomian gland cleaning and massage+Chinese herbal fumigation+health guidance). Tear film breakup time(BUT), corneal fluorescein staining(FL), Ocular Surface Disease Index(OSDI)score, tear meniscus height(TMH), and non-invasive first tear film breakup time(NIBUT)were compared between the two groups before and after 4 wk of treatment. Meibomian gland loss, tear film lipid layer thickness, and basic ocular symptoms were also assessed.RESULTS:This study included 60 eyes of 60 patients, with a control group of 30 eyes of 30 patients(aged 56.24±10.24 y, 13 males and 17 females)and an experimental group of 30 eyes of 30 patients(aged 58.01±9.79 y, 15 males and 15 females).After 4 wk of preoperative treatment, the BUT in the experimental group increased from 4.09±1.13 s to 10.35±1.46 s, and from 4.15±1.05 s to 8.26±1.36 s in the control group, showing a significant intergroup difference(t=5.737, P<0.001). The FL score in the experimental group decreased from 6.83±0.46 points to 2.86±0.38 points, whereas in the control group it decreased from 6.79±0.39 points to 5.32±0.43 points(t=23.480, P<0.001). After 4 wk of treatment, the NIBUT in the experimental group increased from 5.19±1.12 s to 9.36±1.47 s, compared to an increase from 5.21±1.04 s to 7.18±1.25 s in the control group(t=6.188,P<0.001). The proportion of patients with a thin tear film lipid layer was significantly higher in the experimental group than in the control group(all P<0.01). Ocular clinical symptoms decreased after treatment in both groups, with the experimental group showing lower scores than the control group(all P<0.001).CONCLUSION:Preoperative comprehensive dry eye treatment can multi-dimensionally improve dry eye symptoms and tear film stability in cataract patients with diabetes mellitus, providing an effective strategy for the perioperative management of cataract patients.
2.Research progress of urea-containing PET tracers targeting prostate specific membrane antigen

Prostate cancer is one of the most common malignant tumors of male genitourinary system. Prostate cancer has the following characteristics: insidious onset, early asymptomatic or not obvious symptoms, complex etiology and pathogenesis, long incubation period and so on. Therefore, the realization of its early diagnosis and treatment is of great significance to the prognosis of patients. Prostate-specific membrane antigen (PSMA) is a type 2 transmembrane glycoprotein that is highly expressed on the membrane of almost all primary and metastatic prostate cancer cells, and is an ideal target for prostate cancer imaging and treatment. In recent years, with the approval of urea-containing small molecule PET (positron emission computed tomography) radiopharmaceutical based on PSMA (68Ga-PSMA-11, 18F-PSMA-1007), PET-CT (positron emission computed tomography/computed tomography) has shown new potential for early diagnosis and accurate staging of prostate cancer patients. This review mainly summarizes the research progress of urea-containing PSMA PET imaging agents and finds that they have defects such as uptake in non-target tissues like the kidneys, lacrimal glands, and salivary glands. Thus, further optimizing their structure to reduce the uptake in non-target tissues, providing provide convenience for the labeling of therapeutic radiopharmaceuticals, thereby achieving the goal of integrated diagnosis and treatment, is an important development direction in this field.
3.Study on protection of cerebral ischemia-reperfusion injury by HSYA activated neuronal autophagy based on SIRT1

Objective:To investigate effect and mechanism of hydroxysafflor yellow A(HSYA)activating neuronal autophagy on cerebral ischemia-reperfusion injury through a combination of in vitro and in vivo experiments.Methods:SD rat MCAO/R model was established by improved suture method.Rats were randomly divided into sham surgery(Sham)group,MCAO/R group and MCAO/R+HSYA group,following indicators were detected to determine extent of cerebral ischemia-reperfusion nerve damage:Z-Longa neu-rological function score was detected,TTC staining to measure cerebral infarction area,and TUNEL staining to measure cell apopto-sis;Western blot was used to detect protein expressions of autophagy related markers LC3,Beclin1,P62 and SIRT1 in rat brain tis-sue;immunofluorescence staining was used to observe expression of LC3 co-localization with neurons.OGD/R injury model of SH-SY5Y cells was established and randomly divided into Normal group,OGD/R group,OGD/R+HSYA group,OGD/R+SIRT1 inhibitor(EX-527)group and OGD/R+EX-527+HSYA group.Western blot was used to detect protein expressions of LC3,Beclin1,P62 and SIRT1.Results:Compared with Sham group,model group rats showed impaired neurological function,significantly increased neu-robehavioral scores,widespread cerebral infarction,significantly increased neuronal cell apoptosis,significantly increased autophagy related protein Beclin1 expression and LC3-Ⅱ/LC3-Ⅰ,significantly decreased P62 expression,significantly increased LC3/NeuN co-stained cells,and decreased SIRT1 expression;compared with model group,HSYA intervention group showed a significant decrease in neurological functional scores,a significant reduction in cerebral infarction area,a significant decrease in neuronal cell apoptosis,a further increase in Beclin1 expression and LC3-Ⅱ/LC3-Ⅰ,a further decrease in P62 expression,number of LC3/NeuN and P62/NeuN co-stained cells also increased,and SIRT1 expression significantly increased.Expression trends of Beclin1,LC3-Ⅱ/LC3-Ⅰ,P62 and SIRT1 of cells between normal group,model group and HSYA intervention group were same as animal experiment;compared with model group,expressions of SIRT1,Beclin1 and LC3-Ⅱ/LC3-Ⅰ in OGD/R+EX-527 group were significantly reduced,while expression of P62 was significantly increased;compared with OGD/R+EX-527 group,there was no significant change in SIRT1 expression in OGD/R+EX-527+HSYA group,LC3-Ⅱ/LC3-Ⅰ and Beclin1 expression were significantly increased,and P62 expres-sion was significantly decreased.Conclusion:HSYA can significantly improve neurological deficits in rats after cerebral ischemia-reperfusion,reduce cerebral infarction area,and decrease neuronal cell apoptosis rate,whose neuroprotective effect may be related to its activation of SIRT1,which significantly enhances neuronal autophagy.
4.Model of cardiovascular metabolic risk intervention for obese students based on the operating mechanism of vice president of health

To explore an effective health management model for obese students,a comprehensive intervention was carried out for obese students with cardiovascular and metabolic risks,and the effectiveness of this intervention model was evaluated.From Jan to Apr 2024(excluding the winter vacation),300 students were selected from 6 primary schools in Qibao Community,Minhang District,Shanghai,to participate in the study(28 students dropped out during the study period).The study subjects were divided into two intervention groups(pilot intervention group:equipped with a health vice principal;general intervention group:not equipped with a health vice principal)and a control group.The intervention group received comprehensive intervention measures such as science popularization,diet,exercise and psychology,while the control group received daily health management.The post-intervention results showed that the intervention group had significant improvements in healthy diet,scientific exercise and positive psychology,with significant differences compared to the control group(P<0.05).At the same time,the intervention group had a reduced detection rate of obesity(BMI≥P95),and a decreased detection rate of abnormal metabolic indicators such as blood pressure,fasting blood glucose and triglyceride,especially significant differences in fasting blood glucose and triglyceride compared with the control group(P<0.001).In addition,the pilot intervention group under the operation of health vice principal showed better effects in changing healthy behaviors and improving some metabolic indicators compared with the general intervention group.The implementation of this project provided a scientific basis for the promotion of a comprehensive intervention model for student health under the oprtation of health vice principle.

The goal of medical talent training under the new medical science concept is to comprehensively enhance the quality of medical education and cultivate high-quality medical talents with interdisciplinary integration, innovation spirit, and an international perspective to meet the demands of the new era. To achieve this goal, the joint education program between Chongqing Medical University and the University of Leicester integrates comprehensive quality development into the undergraduate medical education system, introducing the cutting-edge international course "Respiratory and Cancer Precision Medicine". This course centers on precision medicine, spanning respiratory diseases and oncology, and uses a clinical-basic cyclical integrated course design with a highly-informationized blended teaching model. Students have gained a close integration of clinical practice and research capabilities, significantly enhancing their inquiry spirit, critical thinking, independent learning, and higher-order cognitive skills. They also engage in innovative thinking and interdisciplinary collaboration within an international context to better address complex medical issues. 6.The expression of the threonine and tyrosine kinase gene in eyelid basal cell carcinoma and its impact on the biological behavior of malignant tumor cells

Objective To investigate the relationship between the threonine and tyrosine kinase(TTK)gene and eyelid basal cell carcinoma(BCC).Methods Bioinformatics methods were used to screen the core gene(namely,TTK)associated with the occurrence and development of BCC from the Gene Expression Omnibus(GEO)database.Surgically removed eyelid BCC tissue specimens(BCC cells were divided into BBC Grade Ⅰ,Ⅱ and Ⅲ groups by tumor grade)and be-nign tumor tissue specimens(Control group)were collected from Ziyang Central Hospital for subsequent experiments.Cel-lular immunofluorescence assay(CIA)was used to detect the expression of the TTK gene in benign and malignant eyelid tumor cells.After knocking down TTK in BCC cells through transfection with lentiviruses(the cells transfected with LV-TTK-shRNA were taken as the TTK-shRNA group,and those transfected with LV-BBC-shRNA were taken as the BBC nega-tive control group),CIA was used to detect the expression of key proteins Bcl-2 and Bax in the apoptotic signaling pathway of each group of cells.Results The bioinformatics analysis showed that the TTK gene was the core gene associated with the occurrence and development of eyelid BCC.CIA detection results revealed that the fluorescence signal intensities in the tumor cytoplasm of Control,BCC Grade Ⅰ,Ⅱ,and Ⅲ groups were 1.03±0.07,1.28±0.11,1.58±0.13 and 1.92±0.17,respectively.The fluorescence signal intensity gradually increased,and the difference in fluorescence signal intensity among the four groups was statistically significant(all P<0.05).Compared with that in the Control group(1.02±0.05),the cell fluorescence intensity was increased in the BCC negative control group(1.74±0.12)and decreased in the TTK-shRNA group(1.31±0.09)(P<0.05).The difference in cell fluorescence intensity was significant among the Control,BCC nega-tive control and TTK-shRNA groups(all P<0.05).The fluorescence intensity of the anti-apoptotic protein Bcl-2 was 1.04±0.12 in the Control group,2.12±0.23 in the BCC negative control group,and 1.43±0.15 in the TTK-shRNA group.The fluorescence intensity of the pro-apoptotic protein Bax was 1.02±0.08 in the Control group,0.64±0.11 in the BCC negative control group,and 1.47±0.16 in the TTK-shRNA group.After TTK knockdown,the expression level of BcL-2 in BCC cells decreased,and that of Bax increased.The fluorescence intensities of BcL-2 and Bax were significantly different among the Control,BCC negative control and TTK-shRNA groups(all P<0.05).Conclusion The TTK gene plays a role in the regulation of eyelid BCC cell proliferation,and this effect is closely related to the PI3K-AKT-Bcl-2/Bax signaling path-way.
7.Therapeutic effect of pressing needle therapy on nausea and vomiting in patients after gynecological laparoscopic surgery

Objective To observe the clinical effect of pressing needle in preventing and reducing nausea and vomiting in patients after gynecological laparoscopic surgery.Methods A total of 199 patients undergoing gynecological laparoscopic surgery from May to Nov 2023 at Obstetrics and Gynecology Hospital,Fudan University were randomly divided into research group(n=99)and control group(n=100).The observation group was given Tanzhong,zanzhu and Taichong pressing needles on the basis of the control group.The postoperative nausea and vomiting were observed in the two groups.Results There were significant differences in the incidence and duration of postoperative nausea,the incidence of postoperative vomiting between the two groups(P<0.05),but there was no significant difference in the duration of postoperative vomiting.The incidence of nausea and vomiting in the observation group was lower than that in the control group.Conclusion Pressing needle can effectively prevent the occurrence of nausea and vomiting after gynecological laparoscopic surgery,and reduce the degree of nausea and vomiting,and reduce the duration of nausea.

10.Association between handgrip strength and chronic kidney disease in adult residents in Anhui Province

Objective:To explore the independent association between handgrip strength and chronic kidney disease (CKD) in adult residents in Anhui Province using data from the China Adult Chronic Disease and Risk Factor Surveillance (2023).Methods:A multi-stage stratified cluster random sampling method was used to select residents aged ≥18 years for surveys, physical measurements, and laboratory tests. Relevant covariates were adjusted, and a multivariable logistic regression model was established to infer the association between handgrip strength and CKD, followed by subgroup analysis.Results:A total of 7 295 participants were included in the study, with age of (61.5±13.2) years, and 55.6% of the study participants were women. The results of the multivariate logistic regression analysis showed that with each 1.0 kg increase in handgrip strength, the risk for CKD decreased by 1.3% ( OR=0.987, 95% CI: 0.978-0.997). Compared with those with low handgrip strength, the people with moderate hasdgrip strength ( OR=0.818, 95% CI: 0.694-0.964) and high handgrip strength ( OR=0.729, 95% CI: 0.598-0.989) had lower risk for CKD. In the subgroup analysis, the association between handgrip strength and risk for CKD remained unchanged regardless age, sex, smoking status, and alcohol consumption statuys, and the prevalence of hypertension and hyperlipidemia (interaction P>0.05), except BMI and diabetes. Conclusion:The decline in handgrip strength is associated with an increased risk for CKD in adult residents in Anhui.
