1.The value of iSEND immune score combined with LIPI in assessing the prognosis of non-small cell lung cancer following immunotherapy

Objective:To investigate the value of iSEND(inhaled,self-immunoregulatory,extracellular nanovesicle-based delivery)immune score combined with lung immune prognostic index(LIPI)in evaluating the prognosis of non-small cell lung cancer(NSCLC)patients undergoing immunotherapy.Methods:A retrospective analysis was conducted on the clinical data of 100 patients with advanced NSCLC who received immunotherapy from February 2018 to February 2023.The iSEND immune score and LIPI data of these patients were collected.Patients were divided into 3 groups(poor,moderate,and good)according to their iSEND and LIPI scores.Kaplan-Meier survival curves were polotted to analyze progression-free survival(PFS)among all patients and within different groups.Cox regression analysis was used to identify the risk factors affecting patient prognosis.Results:After immunotherapy,the objective response rate(ORR)was 42.00%(42/100)and the disease control rate(DCR)was 82.00%(82/100)among NSCLC patients.The ORR and DCR were lowest in the poor groups and highest in the good groups for both iSEND immune score and LIPI score,with significant differences among groups(all P<0.01).The mPFS for all 100 NSCLC patients was 7.63 month(95%CI[7.23,8.05]).The mPFS for the poor,moderate,and good groups in terms of iSEND immune score was 4.69,6.58,and 8.99 months,respectively,with the good group having the longest PFS,followed by the moderate group,and the poor group(χ2=125.391,P<0.000 1).Similarly,the mPFS for the poor,moderate,and good groups in terms of LIPI was 4.54,6.39,and 8.49 months,respectively,with the good group having the longest PFS,followed by the moderate group and poor group(χ2=115.707,P<0.000 1).Cox multivariate analysis identified ECOG PS>1,distant metastasis,iSEND immune score≥2,and LIPI≥2 as independent risk factors affecting patient prognosis.Conclusion:iSEND immune score and LIPI can serve as valuable prognostic indicators for NSCLC patients undergoing immunotherapy,demonstrating certain clinical value.
2.Improved YOLOv5s based method for immunohistochemically positive cell counting

Objective To propose a novel method for immunohistochemically positive cell counting based on the improved YOLOv5s.Methods Regarding the small target characteristics of positive cells,a small target detection layer was added to refine feature extraction.Then,a bidirectional weighted feature pyramid network was used to replace path aggregation network(PANet)in the neck network for realizing multi-scale feature fusion.Additionally,the method used coordinate attention mechanism to make the model pay more attention to small target characteristics,and replaced the original GIoU with EIoU loss function for enhancing the detection performance.Results The model was trained on the self-built immunohistochemical image dataset.The average accuracy of the improved model was 89.3%,which was 4.0%higher than the original model and surpassed mainstream target detection models.The 5-year survival prediction model constructed with the method achieved an average accuracy of 76.8%and an average area under the curve of 0.81,demonstrating its superior prediction ability.Conclusion The proposed model can quickly detect the number of immunohistochemically positive cells and effectively assist doctors in survival prediction.

The polyfoliate perforator flap is a new type of flap that was developed on the basis of the traditional polyfoliate myocutaneous flap, polyfoliate fascial flap and perforator flap. It overturns the traditional idea that the deep fascial vascular network is the fundamental for a survival of the flap, and enables the flaps to achieve the best profile and function of the recipient areas with minimal damage to the donor area. In order to improve the understanding of the polyfoliate perforator flap and further standardise its clinical application, this paper forms a consensus on the definition, classification, indications, operative points and precautions of the polyfoliate perforator flap, so as to provide references in diagnosis and treatment process and practical application for the surgeons.

In the USA, there were about 1 806 590 new cancer cases in 2020, and 606 520 cancer deaths are expected to have occurred in 2021. Lung cancer has become the leading cause of death from cancer in both men and women (Siegel et al., 2020). Clinical studies show that the five-year survival rate of lung cancer patients after early diagnosis and treatment intervention can reach 80%, compared with that of patients having advanced lung cancer. Thus, the early diagnosis of lung cancer is a key factor to reduce mortality.

Protein self-assemblies at the micro- and nano-scale are of great interest because of their morphological diversity and good biocompatibility. High-throughput screening of protein self-assembly at different scales and morphologies using protein crystallization screening conditions is an emerging method. When using this method to screen protein self-assembly conditions, some apparently transparent droplets are often observed, in which it is not clear whether self-assembly occurs. We explored the interaction between β-lactoglobulin and the protein crystallization kit Index™ C10 and observed the presence of micro- and nano-scale protein self-assemblies in the transparent droplets. The diverse morphology of the micro- and nano-scale self-assemblies in the transparent droplets formed by mixing different initial concentrations of β-lactoglobulin and Index™ C10 was further investigated by scanning electron microscope. Self-assembly process of fluorescence-labelled β-lactoglobulin was monitored continuously by laser confocal microscope, allowing real-time observation of the liquid-liquid phase separation phenomenon and the morphology of the final self-assemblies. The internal structure of the self-assemblies was gradually ordered over time by in-situ X-ray diffraction. This indicates that the self-assembly phenomenon within transparent droplets, observed in protein self-assembly condition screening experiments, is worthy of further in-depth exploration.

8.Retrospect and prospect of clinical pathway management in China

The main work and achievements of clinical pathway work in China since 2009 were systematically reviewed in the paper. It analyzed the problems existing in the implementation of clinical pathway management in China, and suggested on such management in the future. 9. Clinical effect and safety of 144-week treatment with entecavir capsules in treatment-naïve HBeAg-positive patients with chronic hepatitis B

Objective:
To investigate the clinical effect and safety of entecavir capsules in the treatment of treatment-naïve HBeAg-positive patients with chronic hepatitis B (CHB).
Methods:
A total of 158 HBeAg-positive CHB patients were given oral entecavir capsules at a dose of 0.5 mg/time once a day for 144 weeks. Clinical outcome and safety were evaluated at baseline and at 24, 48, 72, 96, 120, and 144 weeks of treatment respectively. The Fisher’s exact test was used for the analysis of categorical data.
Results:
After 144 weeks of treatment, 90.91% of all patients achieved virologic response (< 69 IU/ml), the normalization rate of alanine aminotransferase was 88.18%, the clearance rate of HBeAg was 33.33%, and the seroconversion rate of HBeAg was 24.07%. Of all patients, 2 dropped out due to adverse events and 5 experienced serious adverse reactions.
Conclusion
Entecavir capsules can inhibit viral replication and have good safety in treatment-naïve HBeAg-positive CHB patients.
10.Analysis of the factors associated with treatment outcomes in spinal cord decompression sickness

Objective To analyze the factors associated with treatment outcomes of dive-related spinal cord decompression sickness (DCS).Methods A retrospective analysis was made in 95 patients with dive-related spinal cord DCS (with an age range of 43 ± 11).Such medical data as dive data,clinical medical data,spinal cord magnetic resonance imaging (MRI),electrophysiological data,latency of DCS symptoms upon completion of diving,the relationship between symptom onset and the interval from the start of HBO therapy to the termination of HBO therapy were collected for analysis.The severity and treatment outcomes of spinal cord DCS were assessed by the Boussuges grade system during the acute stage and 3 months after treatment.The treatment outeomes of spinal cord DCS were classified into complete recovery and incomplete recovery.All the patients were treated with the widely-recognized treatment protocol.Results Fifty-six patients (59%) had complete recovery,3 months after DCS seizure,and 39 patients (41%) had incomplete recovery following 3 months of treatment.Complete recovery rate in the young patients was higher than that in the old ones.The complete recovery rate of the patients with normal spinal cord MRI detection results was higher than that of the patients with abnormal MRI detection results,and the complete recovery rate of the patients with normal electrophysiological detection results was higher than that of the patients with abnormal electrophysiological detection results.The complete recovery rate of the patients with Boussuges scores ≤7 at onset time was higher than that of the patients with Boussuges scores of over 7.The complete recovery rate of the patients without back pain was higher than that of the patients with acute back pain.The complete recovery rate of the patients with over 50-minute symptom latency upon surfacing was higher than that of the patients with less than 50-minute symptom latency.The complete recovery rate of the patients with less than 4.5-hour time interval between symptom onset and HBO therapy was higher than that of the patients with over 4.5-hour time interval.Conclusions The patients with younger age,normal MRI detection results,normal electrophysiological data,Boussuges scores ≤7,no complaint of back pain after surfacing,longer symptom latency upon dive completion,and shorter time interval between symptom onset and HBO therapy would have better treatment outcomes.
