Neurocritical care involves complex pathophysiological mechanisms, and its incidence is higher, injuries are more severe, and treatment is more challenging in high-altitude environments. This consensus, based on the latest domestic and international evidence-based medical data, establishes a standardized, goal-oriented framework for neurocritical care management applicable in high-altitude regions and nationwide. The consensus was developed following international standards for evidence quality assessment and underwent two rounds of Delphi expert consultation, resulting in 32 recommendation statements covering three parts: management systems, monitoring and assessment, and core strategies. Key updates include: advocating for the establishment of independent neurocritical care units and implementing precise tiered diagnosis and treatment based on the "Five Differences in Critical Care" concept; constructing a "trinity" multimodal brain monitoring system centered on cerebral blood flow, cerebral oxygenation, and brain function, emphasizing routine bedside transcranial Doppler ultrasound, cerebral oximetry, and continuous electroencephalography monitoring; shifting management strategies from mild hypothermia therapy to targeted temperature management, and defining the "446" target management pathway for the supercritical stage; emphasizing the assessment of static and dynamic cerebrovascular autoregulation functions through multimodal methods to achieve individualized optimal mean arterial pressure management; elevating cerebrospinal fluid management goals to the level of "glymphatic system" function maintenance; implementing a multidisciplinary collaborative, whole-process management model focusing on patients' long-term neurological functional outcomes; de-escalation criteria include multidimensional indicators such as recovery of brain structure, restoration of cerebrovascular autoregulation, improvement in cerebrospinal fluid dynamics, and reduction in biomarker levels; and integrating cutting-edge technologies like artificial intelligence into post-critical care management and rehabilitation planning. This consensus systematically integrates the entire process of neurocritical care management, reflecting the modern connotation of goal-oriented, dynamic, and multimodal integration in neurocritical care medicine. It aims to adapt to new trends such as deepening understanding of pathophysiological mechanisms, the integration of medicine and engineering, and the empowerment of artificial intelligence, thereby further advancing the discipline of critical care medicine.

A molecularly imprinted electrochemical sensor for rapid detection of tenuazonic acid(TeA)was developed based on the Au-MoS2/MOF(Fe2+/Fe3+)high-efficiency catalytic cycle amplification strategy, using p-aminobenzoic acid(PABA)as the functional monomer, and TeA as the template molecule. The molecularly imprinted polymer(MIP)was prepared on the surface of Au-MoS2/MOF(Fe2+/Fe3+)modified electrode through electropolymerization.By introducing flower-like MoS2 nanoflakes(MoS2 NFs)as a co-catalyst into a mixed-valence structured Fe-MOF(Fe2+/Fe3+),the H2O2 electrochemical signal of the MIP/Au-MoS2/MOF(Fe2+/Fe3+)/GCE was significantly enhanced.Under optimal conditions, the sensor exhibited good selectivity and high sensitivity toward TeA.A linear relationship(R2=0.992)was observed between the electrochemical response and TeA concentration in the range of 0.001-10 μg/kg,with a detection limit of 0.3 ng/kg.The developed method was successfully applied to determination of TeA in fruit samples, with recoveries ranging from 90.8%to 110.8%,and relative standard deviations from 1.9%to 8.4%.

BACKGROUND:
Chronic kidney disease (CKD) is associated with common pathophysiological processes, such as inflammation and fibrosis, in both the heart and the kidney. However, the underlying molecular mechanisms that drive these processes are not yet fully understood. Therefore, this study focused on the molecular mechanism of heart and kidney injury in CKD.
METHODS:
We generated an microRNA (miR)-26a knockout (KO) mouse model to investigate the role of miR-26a in angiotensin (Ang)-II-induced cardiac and renal injury. We performed Ang-II modeling in wild type (WT) mice and miR-26a KO mice, with six mice in each group. In addition, Ang-II-treated AC16 cells and HK2 cells were used as in vitro models of cardiac and renal injury in the context of CKD. Histological staining, immunohistochemistry, quantitative real-time polymerase chain reaction (PCR), and Western blotting were applied to study the regulation of miR-26a on Ang-II-induced cardiac and renal injury. Immunofluorescence reporter assays were used to detect downstream genes of miR-26a, and immunoprecipitation was employed to identify the interacting protein of LIM and senescent cell antigen-like domain 1 (LIMS1). We also used an adeno-associated virus (AAV) to supplement LIMS1 and explored the specific regulatory mechanism of miR-26a on Ang-II-induced cardiac and renal injury. Dunnett's multiple comparison and t -test were used to analyze the data.
RESULTS:
Compared with the control mice, miR-26a expression was significantly downregulated in both the kidney and the heart after Ang-II infusion. Our study identified LIMS1 as a novel target gene of miR-26a in both heart and kidney tissues. Downregulation of miR-26a activated the LIMS1/integrin-linked kinase (ILK) signaling pathway in the heart and kidney, which represents a common molecular mechanism underlying inflammation and fibrosis in heart and kidney tissues during CKD. Furthermore, knockout of miR-26a worsened inflammation and fibrosis in the heart and kidney by inhibiting the LIMS1/ILK signaling pathway; on the contrary, supplementation with exogenous miR-26a reversed all these changes.
CONCLUSIONS
Our findings suggest that miR-26a could be a promising therapeutic target for the treatment of cardiorenal injury in CKD. This is attributed to its ability to regulate the LIMS1/ILK signaling pathway, which represents a common molecular mechanism in both heart and kidney tissues.

Journal of Regional Anatomy and Operative Surgery 2025;34(1):49-54
Objective To investigate the effect of cannulated screw internal fixation via a small incision sinus tarsi approach on calcaneal related angles in patients with calcaneal fractures. Methods A total of 107 patients with calcaneal fractures who were treated in our hospital from June 2020 to July 2023 were selected, and they were divided into the group A(n=56,underwent lateral L-shaped approach combined with steel plate internal fixation) and group B (n=51,underwent cannulated screw internal fixation via a small incision sinus tarsi approach). The Bohler angle,Gissane angle, calcaneal width, calcaneal height,American Orthopaedic Foot and Ankle Association (AOFAS) Maryland foot function score, and visual analogue scale (VAS) score before and after surgery were compared between the two groups. The correlations between the imaging data before and after surgery, their correlation with the AOFAS Maryland foot function score and VAS score, and the correlations between the difference in imaging indexes before and after surgery and the improvement rates of Maryland foot function score and VAS score were analyzed by Pearson method. Results The Bohler angle,Gissane angle, calcaneal width, and calcaneal height after surgery in both groups were significantly improved compared with those before surgery (P<0.05). The group B had smaller Bohler angle 1 month after surgery, and greater Bohler angle 3 months and 6 months after surgery, and Gissane angle and calcaneal width 3 months after surgery compared with those in group A (P<0.05). At 1,3,and 6 months after surgery, the VAS scores of both groups were significantly reduced compared with those before surgery (P<0.05),while the Maryland foot function scores were significantly increased compared with those before surgery (P<0.05). The Maryland foot function scores and VAS scores 1,3,and 6 months after surgery of the group B were significantly better than those of the group A(P<0.05). The Bohler angle, calcaneal width,Gissane angle, and calcaneal height were all correlated before and after surgery (P<0.05);before surgery, the calcaneal width and height were not correlated with the Maryland foot function score or VAS score (P>0.05),while the Bohler angle and Gissane angle were correlated with the Maryland foot function score and VAS score (P<0.05);after surgery, the Bohler angle, calcaneal width,Gissane angle, and calcaneal height were all correlated with the Maryland foot function score and VAS score (P<0.05). The differences in Bohler angle,Gissane angle, calcaneal width, and calcaneal height before and after surgery were correlated with the improvement rates of Maryland foot function score and VAS score (P<0.05). Conclusion The Bohler angle,Gissane angle, and calcaneal height all increase, while the calcaneal width decrease after treated with cannulated screw internal fixation via a small incision sinus tarsi approach in calcaneal fractures, which has better therapeutic effect than that of lateral L-shaped approach combined with steel plate internal fixation. The changes in the Bohler angle,Gissane angle, calcaneal width, and calcaneal height are correlated with the clinical efficacy.
