1.Analysis of the effect of parenting care map of premature infants on parenting competence and parenting stress of primiparas

Objective:To analyze the effects of parenting care map of premature infants on parenting competence and parenting stress of primipara, with a view to providing a theoretical basis for the development, implementation and modification of clinical interventions.Methods:Convenient sampling method was used to select primigravid women who gave birth in Lishui Maternal and Child Health Hospital from January 2023 to January 2024 for randomized controlled study, which was divided into the observation group and the control group according to the random number table. The control group received routine nursing, and the observation group guided the parents of premature infants to draw the child care map together on the basis of the control group. The scores of Chinese version of parenting sense of competence scale ( C-PSOC ) and Chinese version of Simplified Parenting Stress Index ( PSI-SF ) before and after intervention were compared.Results:A total of 120 cases of primiparous women who met the inclusion and exclusion criteria were enrolled, 60 cases in the control group and 60 cases in the observation group. The age of the control group was (26.25 ± 3.93) years, and the age of the observation group was (25.86 ± 4.22) years. Before the intervention, the difference between the C-PSOC dimension scores and total scores, PSI-SF dimension scores and total scores of the 2 groups was not statistically significant (all P>0.05). After the intervention, the maternal C-PSOC efficacy, satisfaction dimension scores and total scores of primiparous women in the observation group were (32.05 ± 6.27), (40.29 ± 8.19), and (72.23 ± 9.39) points, respectively, which were higher than those of the control group, which were (28.46 ± 6.47), (37.08 ± 8.06), and (65.53 ± 10.24) points, and the differences between the 2 groups were all statistically significant ( t = 3.09, 2.16, 3.74, all P<0.05). After the intervention, the PSI-SF scores and total scores of primiparous women in the observation group on the dimensions of parental distress, personal interaction disorder, and difficult child were (24.43 ± 5.44), (27.51 ± 6.53), (29.47 ± 5.82), and (81.54 ± 10.38) points, respectively, which were lower than those of the control group (28.57 ± 5.52), (30.19 ± 7.02), (32.78 ± 5.37), and (90.97 ± 10.12) points, and the differences between the 2 groups were all statistically significant ( t = 4.41, 2.17, 3.23, 5.04, all P<0.05). Conclusions:Premature child care map can effectively improve the parenting sense of competence of primiparas and reduce their parenting pressure. It is a convenient and effective nursing intervention tool.

3.Application of LEARNS model based on Triangle stratified model on the management of out-patients with type 2 diabetes mellitus

Objective:To analyze the effects of stratified follow-up intervention combined with LEARNS model on self-management, glucose metabolism, compliance and satisfaction in patients with type 2 diabetes mellitus (T2DM).Methods:This study was a quasi-experimental study. A total of 212 patients with T2DM who visited the DM nursing clinic of Beijing Tiantan Hospital, Capital Medical University from June 2020 to May 2021 were selected and divided into control group (102 cases) and intervention group (110 cases) according to the treatment time. All the patients received the conventional medical treatment and health education guidance for 6 months, at the same time, the intervention group received management of the triangle stratified model combined with LEARNS mode. The glucose metabolism, self-management behavior, satisfaction and compliance were compared before and after the intervention.Results:Before the intervention, the baseline data of the 2 groups had no significant difference(all P>0.05) and were comparable. Three and six months after the intervention, the HbA1c values of the intervention group were (6.18 ± 0.57)% and (6.40 ± 0.47)%, which were significant better than those in the control group, (6.74 ± 0.90)% and (6.99 ± 0.91)%, the differences were significant ( t=8.05 and 9.12, both P<0.01). Three and six months after the intervention, the SDASC values in the intervention group were (46.51 ± 5.80) and (44.41 ± 5.61), which were higher than those in the control group, (43.45 ± 4.20) and (42.08 ± 3.67), the difference were significant ( t=6.56 and 5.32, both P<0.01). Three and six months after the intervention, the compliance and patient satisfaction of intervention group were better than those in the control group ( χ2=5.52 and 7.26, t=7.02 and 15.75, all P<0.05). Conclusions:Triangle hierarchical management combined with LEARNS model can effectively improve the blood glucose levels of T2DM patients as well as their self-management levels, satisfaction and compliance, which can help nurses reasonably allocate follow-up time.
4.Effect of dynamics of instantaneous flow rate on the quantification of the severity of degenerative mitral regurgitation using M-mode proximal isovelocity surface area

Objective:To investigate the effect of instantaneous flow rate on the consistency of diagnostic accuracy of severe degenerative mitral regurgitation (DMR) using proximal isovelocity surface area (PISA).Methods:From June 2019 to June 2021, 75 patients with DMR who underwent echocardiography in Department of Echocardiography of Zhongshan Hospital, Fudan University were prospectively enrolled. The instantaneous flow rate of DMR during the systolic phase was calculated using M-mode PISA(PISA M-mode), and a time-integrated curve was plotted. Regurgitant volume (RVol) and effective regurgitant orifice area (EROA) were calculated by traditional PISA (PISA max), pair PISA (PISA pair), and PISA M-mode, respectively. RVol acquired from cardiac magnetic resonance (CMR) volumetric method in 22 patients of the enrolled patients. The correlation and consistency of RVol acquired between the three PISA methods and CMR were compared. Agreement of diagnostic accuracy of severe mitral regurgitation (sMR) acquired between the three PISA methods and multi-parameter algorithm by American Society of Echocardiography (ASE) was analyzed using Cohen′s Kappa analysis. Results:The curve of instantaneous flow rate of DMR showed unimodal pattern with the peak at mid-late systolic phase. The correlation of RVol acquired between PISA methods and CMR was moderate for PISA max and PISA pair ( r=0.77, 0.80, both P<0.001), whereas PISA M-mode presented strong correlation with CMR ( r=0.87, P<0.001). RVol acquired from PISA max was larger than that of CMR[(69.1±37.1) ml vs (49.0±29.0)ml, P=0.002]. Both PISA max and PISA pair were shown moderate agreement of diagnostic accuracy of sMR with ASE multi-parameters algorithm (RVol: κ=0.496, 0.525, both P<0.001; EROA: κ=0.570, 0.578, both P<0.001), while PISA M-mode presented strong agreement (RVol: κ=0.867 and EROA: κ=0.802, both P<0.001). Conclusions:Based on the unimodal pattern of instantaneous flow rate in patients with DMR, PISA max may significantly overestimate RVol, exposing a significant proportion of patients with DMR to unnecessary MR surgery. PISA M-mode presents better correlation and consistency with CMR on the quantification of RVol compared with PISA max and PISA pair, and may improve the diagnostic accuracy of quantification of sMR using PISA.
5.Intervention effect of logotherapy on anxiety and depression in patients with chronic glomerulonephritis

Objective:To observe and analyze the intervention effect of logotherapy on anxiety and depression in patients with chronic glomerulonephritis (CGN).Methods:Selected 102 CGN patients from March 2019 to May 2020 in Beijing Tiantan Hospital Affiliated to Capital Medical University as the research objects, the general information of the patients was collected, and the patients were randomly divided into the observation group and the control group with 51 cases in each group. The control group was given routine nursing intervention for CGN patients, and the observation group was given logotherapy therapy intervention based on the intervention of the control group. The differences in the scores of the Self-rating Anxiety Scale (SAS) and Self-rating Depression Scale (SDS) between the two groups before and after 3 months of intervention.Results:There were no significant differences in SAS and SDS scores before and after intervention in the control group ( P>0.05). The SAS and SDS scores in the observation group after 3 months of intervention were (40.67 ± 4.17), (48.74 ± 5.01) points, which were significantly lower than (51.34 ± 5.04), (55.19 ± 5.49) points in the control group ( t=8.37, 3.32, both P<0.01). Conclusions:The positive psychological intervention of patients with logotherapy can effectively improve the anxiety and depression of patients with CGN.
6.Meta synthesis of qualitative research on dignity cognition and experience of the elderly in nursing homes

Objective:To systematically review the qualitative research on dignity cognition and experience of the elderly in nursing homes.Methods:Qualitative research on the dignity cognition and experience of the elderly in nursing homes was retrieved by computer in PubMed, Web of Science, Cochrane Library, Embase, PsycINFO, CINAHL, China National Knowledge Infrastructure, Wanfang Data, VIP and China Biology Medicine disc. The retrieval time limit was from the establishment of the database to May 31, 2021. The quality of the article was assessed using the quality evaluation criteria for qualitative research of the Joanna Briggs Institute Evidence-Based Health Care Center.Results:A total of 10 articles were included, and 30 research results were extracted. The research results were grouped into 8 categories, and 4 integrated results were synthesized, namely, the elderly's cognition of dignity, the impact of impaired dignity on the elderly, the factors affecting the maintenance of dignity, and the elderly's dignity maintenance strategies.Conclusions:Nursing homes should attach importance to the dignity of the elderly, provide dignity care, protect the autonomy of the elderly, and strengthen the social support system to promote the realization of the elderly with dignity.
7.Study of early recovery of left ventricular rotation and torsion after percutaneous coronary intervention using two dimensional speckle tracking echocardiography

Objective To evaluate left ventricular rotation and torsion and its early recovery after percutaneous coronary intervention (PCI) in patients with coronary heart disease and normal left ventricular ejection fraction (LVEF) using two dimensional speckle tracking echocardiography.Methods Twenty three consecutive patients with coronary heart disease and normal LVEF were divided into group B (with coronary stenosis <70%) and group C (with coronary stenosis >70% and with PCI).Along with 11 healthy controls(group A),indices including basal rotation (BR),apical rotation (AR),left ventricular torsion (LVT) and normalized time to peak were compared among groups,correlative analysis was made between LVT and each indices mentioned above,indices of group C before and 24 hours after PCI were compared.Results AR,LVT in group B and C reduced relative to group A (P <0.05),meanwhile time to peak of BR in group C shortened relative to other groups.BR,AR and normalized time to peak of BR were correlated to LVT respectively.BR and LVT in group C increased after PCI(P <0.05).Conclusions AR was sensitive to ischemia,the reduction of time to peak of BR in group C might be restriction and compensation.Sensitive to early recovery of left ventricular function after reperfusion,BR could be a predictive index of early effect of percutaneous coronary intervention.
8.The effects of Tangzhiping granules on blood glucose and IGF-Ⅰ in T2DM rats

Objective To explore the effects of Tangzhiping granules on the blood glucose and IGF-Ⅰ in T2DM rats.Methods 40 SPF grade Wistar rats whose weight were about (200±20)g were selected as the subjects for this study,one half were male and another half were female.After be adapted to the normal food for 1 week,8 rats were selected randomly from all of rats following their weight as the blank group (ordinary feed).The remaining the others were fed with the high-fat and high-sugar food.The 6th week,the other rats would be undergo intraperitoneal injection once in left lower belly with 40 mg/kg streptozotocin (STZ),and enable them to become T2DM rats.The 8th week,we would select successful animal models 32 which were divided randomly into a modle group,a Tangmaikang (TMK) granules group,a Tangzhiping granules (TZP)group.The 12th week,we would execute all of rats and collect the blood serums which were tested for the blood glucose and IGF-Ⅰ.Results () Compared model group with blank group,the levels of the blood glucose of the model group increased to (28.33±0.50)mmol/L,while the IGF-Ⅰ decreased to (3.40 ± 0.83) pg/ml,which showed the model of type 2 diabetic rats was successfully set up.② The levels of the blood glucose of the Tangzhiping granules group and Tangmaikang granules group were lower than that of the model group to (15.06±0.39)mm ol/L,(20.12±0.58)mmol/L,but the levels of the IGF-Ⅰ were higher to (9.39±2.91)pg/ml,(5.58± 1.09)pg/ml,which implied that each group of pharmacological treatment had a good effect.③ The level of the blood glucose and IGF-Ⅰ of the Tangzhiping granules group was also significantly different from that of the Tangmaikang granule group,the therapeutic effect of Tangzhiping granules group was better than that of Tangmaikang granules group.Conclusion Tangzhiping granules can reduce the blood glucose of T2DM rats.And also can increase IGF-Ⅰ of T2DM rats.Tangzhiping granules can improve the various symptoms of diabetes mellitus at the same time.
9.Evaluation of right ventricular global and regional volume and systolic function in patients with atrial septal defect using real-time three-dimensional echocardiography

Objective To evaluate right ventricular (RV) global and regional volume and systolic function in patients with secondum atrial septal defect (ASD) using real-time three-dimensional echocardiography (RT3DE),and to explore the relationship between parameters derived from RT3DE and parameters measured by conventional echocardiography.Methods RT3DE images were acquired from 22 patients with secundum ASD and 22 normal controls for evaluation and analysis to obtain RV global and regional end-diastolic volume (EDV),end systolic volume (ESV),systolic volume (SV) and ejection fraction (EF) in three compartments (inflow,body and outflow).RV dyssynehrony parameters were calculated as the standard deviation (SD) and maximum difference (dif) of time to minimum systolic volume (Tmsv) and Tmsv corrected by heart rate (Tmsv%) in three RV compartments.Conventional echocardiographic parameters including pulmonary artery systolic pressure (PASP),pulmonary vascular resistance (PVRe) and maximum diameter of ASD (ASD-D) were calculated and recorded.Results RV global and regional EDV,ESV and SV were larger (all P <0.001) and EF was lower (all P <0.05) in the ASD group than in the controls.RV regional volume and systolic function were changed to various degrees in different compartment in the patients group.There were no significant differences in Tmsv,Tmsv% and RV dyssynchrony parameters between the two groups.In patients with ASD,RV global and regional EDV,ESV and SV in the inflow compartment were positively correlated with ASD-D and PASP (r =0.463-0.704,all P < 0.05) ; RV global EF was negatively correlated with PVRe (r =-0.477,P < 0.05).Conclusions In patients with ASD,RV global and regional volume was enlarged and systolic function was impaired with distinct characteristics; RV global volume and regional volume in the inflow compartment were positively correlated with RV pre-load while RV global EF was negatively correlated with RV afterload.
10.Impact of myocardial contrast echocardiography on evaluating the myocardial perfusion in Beagles with ischemic cardiomyopathy and cardiac resynchronization therapy

Objective To investigate the impact of myocardial contrast echocardiography (MCE) on evaluating the myocardial perfusion in beagles with ischemic cardiomyopathy and cardiac resynchronization therapy (CRT),and to estimate the myocardial perfusion by myocardial blood flow (MBF) value.Methods Twelve adult beagles were randomly divided into two groups (CRT group and non-CRT group).Each beagle underwent a ligature in the first diagonal branch and a pacemaker was implanted.The pacing was started in CRT group,but was not started in non-CRT group.MCE and speckle tracking imaging were performed to evaluate the MBF value and circumferential strain (Cir1 2SD),radial strain (R12SD) and longitudinal strain (L12SD) of 12 segments of left ventricle at baseline,before CRT,and 4 weeks after CRT.Results There was no significant difference of LVEF,LVEDV,LVESV,Cir12SD,and R12SD between two groups at baseline or before CRT( P >0.05).After 4 weeks of CRT,LVEF,LVEDV,LVESV,Cir1 2SD,and R 12SD in two groups were ( 58.8 ± 8.2 ) % vs (39.5 ± 8.7 ) %,(28.2 ± 2.9) ml vs (34.2 ± 2.5 ) ml,( 13.9± 2.6 ) ml vs (21.5± 4.7)ml,(29.1 ± 6.6)ms vs (46.5 ± 10.1)ms and (36.1 ± 10.7)ms vs (67.6± 11.2)ms( P <0.05 ).A,β,and MBF value between two groups were ( 13.6 ± 2.2 )dB vs ( 14.9 ± 3.0)dB,(5.1±1.1 )s- 1 vs (4.8 ± 2.1 )s- 1,(67.6 ± 12.1)dB/s vs (72.8 ± 8.6)dB/s( P >0.05) at baseline and were (7.4 ± 1.2)dB vs (7.3±2.7)dB,(3.9±0.9)s-1 vs (2.9±0.9)s-1,(23.4±4.2)dB/s vs (22.2±4.1)dB/s (P>0.05)before CRT.After 4 weeks of CRT,A,β,and MBF value in CRT group were higher than those in non-CRT group (12.1 ± 1.8)dB vs (9.5 ± 1.7)dB,(4.7 ± 0.3)s-1 vs (3.1 ± 0.8)s-1,(47.2 ± 8.6)dB/s vs (29.5 ±4.2)dB/s,all P <0.05).Conclusions In beagles with ischemic cardiomyopathy,CRT can not only improve cardiac synchrony,but also increase myocardial perfusion.
