1.Related factors of prognosis in patients with primary Sjogren' s syndrome complicated with renal damage

Objective To analyze the related factors of prognosis in patients with primary Sjogren’s syndrome (SS) complicated with renal damage, and to provide reference for clinical development of personalized prevention and treatment measures. Methods A total of 508 patients with primary SS complicated with renal damage in the First Affiliated Hospital of Xinjiang Medical University from February 2020 to February 2022 were enrolled as study subjects. According to the prognosis status within 3 years, the enrolled patients were divided into good prognosis group (n=426) and poor prognosis group (n=82). Univariate and logistic multivariate regression analyses were adopted to analyze the influencing factors of poor prognosis. Results There were significant differences in hypertension, anemia, renal interstitial chronicity grading, and levels of globulin (GLO), immunoglobulin G (IgG) and hemoglobin between the two groups (P<0.05). Logistic multivariate analysis showed that concurrent hypertension, anemia, increased grade of renal interstitial chronicity, and elevated GLO and IgG levels were risk factors of poor prognosis in patients with primary SS complicated with renal damage, while hemoglobin level was a protective factor (OR: 1.962, 95%CI: 1.056-3.645; OR: 2.467, 95%CI: 1.153-5.278; OR: 17.796, 95%CI: 5.157-61.419; OR: 3.655, 95%CI: 1.812-7.372; OR: 5.732, 95%CI: 2.632-12.480; OR: 0.325, 95%CI: 0.165-0.640, P<0.05). Conclusion Patients with primary SS complicated with renal damage have a higher risk of poor prognosis, which is affected by factors such as hypertension, anemia, and GLO, IgG and hemoglobin levels. Clinically, it is necessary to take active prevention and treatment measures to improve the prognosis of patients.

3.Application effect of multimodal quantitative rehabilitation exercise in patients with ankylosing spondylitis

Objective:To explore the application effect of multimodal quantitative rehabilitation exercise in patients with ankylosing spondylitis and to provide reference for patients′ rehabilitation exercise.Methods:The quasi-experimental study method was used to select 78 patients with ankylosing spondylitis admitted to Rheumatology and Immunology Department of First Affiliated Hospital of Xinjiang Medical University from February 2021 to February 2022 as the research objects. The 39 patients admitted from February 2021 to August 2021 as the control group, and 39 patients admitted from September 2021 to February 2022 as the experimental group. The control group adopted conventional rehabilitation exercise program, and the experimental group adopted multimodal quantitative rehabilitation exercise program. The Bath ankylosing spondylitis disease activity index, Bath ankylosing spondylitis function index, inflammatory factors after 6 months of intervention were compared between the two groups.Results:The Bath ankylosing spondylitis disease activity index in the experimental group after 6 months of intervention was (2.35 ± 0.81) points, and that in the control group was (3.47 ± 1.04) points, with a statistically significant difference ( t = 4.02, P<0.05). The Bath ankylosing spondylitis disease activity index in the two groups were analyzed by repeated measurement variance. The differences of time effect, inter group effect and interaction effect were statistically significant ( Fgroup = 11.27, Ftime = 62.05, Finteraction = 5.47, all P<0.05). The Bath ankylosing spondylitis function index in the experimental group after 6 months of intervention was (2.11 ± 0.32) points, and that in the control group was (3.07 ± 0.58) points, with a statistically significant difference ( t = 4.03, P<0.05). The Bath ankylosing spondylitis function index in the two groups were analyzed by repeated measurement variance. The differences in time effect, inter group effect and interaction effect were statistically significant ( Fgroup = 21.44, Ftime = 42.25, Finteraction = 16.67, all P<0.05). After 6 months of intervention, C-reactive protein, interleukin-6, transforming growth factor β, tumor necrosis factor-α were (43.15 ± 2.21) mg/L, (3.28 ± 0.85) mg/L, (41.67 ± 9.04) ng/L, (176.63 ± 20.15) ng/L respectively in the experimental group, and (50.12 ± 1.67) mg/L, (5.27 ± 0.68) mg/L, (48.65 ± 8.96) ng/L, (194.56 ± 19.45) ng/L respectively in the control group. There was a statistically significant difference in the content of inflammatory factors between the two groups ( t values were 2.05-4.45, all P<0.05). Conclusions:Multimodal quantitative rehabilitation exercise can improve the physiological function of ankylosing spondylitis patients′spine, reduce the disease activity of patients, and promote the rehabilitation of patients.
4.Association of Perceived Stress With Depression Among Vaccinated Healthcare Workers During the SARS-CoV-2 Variant Outbreak: The Mediating Role of Compassion Fatigue

Objective:
Depression was common during coronavirus disease-2019 (COVID-19) pandemic, while the association of perceived stress with depression among vaccinated healthcare workers has not been investigated. This study aimed to address this issue.
Methods:
We included a total of 898 fully vaccinated healthcare workers during the outbreak of severe acute respiratory syndrome coronavirus 2 Delta variant in Nanjing, 2021. Depression was ascertained by Patient Health Questionnaire-9, with a cut-off score of ≥5 indicative of mild-to-severe depression. Perceived stress, resilience, and compassion fatigue were assessed by Perceived Stress Scale-10, Resilience Scale-25, and Professional Quality of Life Scale version-5, respectively. Logistic regression analyses were used to estimate the odds ratio (OR) and 95% confidence interval (CI), along with subgroup and mediation analyses.
Results:
The prevalence of mild-to-severe depression was 41.1% in vaccinated healthcare workers. The odd of mild-to-severe depression was increased with higher perceived stress. Compared with vaccinated healthcare workers with the lowest tertile of perceived stress, those with the highest tertile had increased odds of mild-to-severe depression by 120% (OR 2.20, 95% CI 1.46 to 3.31) after multivariable-adjustment. However, perceived stress was not associated with mild-to-severe depression in vaccinated healthcare workers with strong resilience, but was in those with weak resilience (pinteraction=0.004). Further analysis showed that compassion fatigue mediated the relationship between perceived stress and mild-to-severe depression, with a mediating effect of 49.7%.
Conclusion
Perceived stress was related to an increased odd of mild-to-severe depression in vaccinated healthcare workers during COVID-19 pandemic, and this relationship might be explained by compassion fatigue.
5.Incidence and characteristics of benign liver space-occupying mass in 17 721 patients with chronic hepatitis B: a color Doppler ultrasound-based case-control study.

OBJECTIVE:
To analyze the incidence and risk factors of benign liver space-occupying mass in patients with chronic hepatitis B (CHB) and the ultrasound features that differentiate these masses from small hepatocellular carcinoma.
METHODS:
We retrospectively analyzed the color Doppler and clinical data of 17 721 patients with CHB treated in the Hepatology Unit of Nanfang Hospital between January, 2016 and December, 2017. The data were compared with those of 21629 healthy control subjects undergoing routine physical examination in the Center of Heath Management of Nanfang Hospital during the same period.
RESULTS:
Compared with the control subjects, the patients with CHB had significantly higher incidences of hepatic cysts (11.8% 8.7%, < 0.05), hepatic hemangioma (8.2% 1.6%, < 0.05) and hepatic cirrhosis nodules (20.6% 2.4%, < 0.05). The incidences of hepatic cysts and cirrhosis nodules increased with age and was significantly higher in male than in female patients ( < 0.001). The highest incidence of hepatic hemangioma was found in CHB patients aged 30-49 years without a gender difference (>0.05). Sonographically, the benign liver masses commonly showed homogeneous echo within the lesion with clear boundaries and regular shape. Hepatic hemangioma was distinctively hyperechoic in 83.32% (1579/1895) of the patients, while small hepatocellular carcinoma presented with weaker peripheral and internal blood flow signals with a lower flow velocity in the arteries and a higher flow velocity in the portal vein. Liver cirrhosis nodules mostly showed a mixture of strong and weak echoes (79.60%; 7637/9595) without blood flow signal within or around the nodule; an increased volume of the nodule accompanied by heterogeneous echoes within the nodule indicated an increased probability of malignant lesion. Hepatic cysts often displayed no echo within the lesion, but the echo could be enhanced posteriorly.
CONCLUSIONS
The patients with CHB are at a significantly higher risk of developing hepatic cysts, hepatic hemangiomas and hepatic cirrhosis nodules than the control population, and an older age and the male gender are associated with a higher incidence of hepatic cysts or cirrhosis. The differences in the sonographic and hemodynamic features can help to differentiate hepatic benign mass from malignant lesions, and kinetic changes in sonography can be used to monitor potential malignant transformation of the cirrhotic lesions.
6.Antiviral and antifibrotic therapies reduce occurrence of hepatocellular carcinoma in patients with chronic hepatitis B and liver fibrosis: a 144-week prospective cohort study.

OBJECTIVE:
To compare the efficacy and safety of different antiviral and antifibrotic regimens in patients with chronic hepatitis B (CHB) and hepatic fibrosis and the incidence of hepatocellular carcinoma (HCC) associated with these therapies.
METHODS:
A total of 840 patients with CHB and concurrent hepatic fibrosis, who received antiviral therapy in Nanfang Hospital between June, 2010 and June, 2018, were enrolled in this follow-up cohort study. The patients were assigned to 3 cohorts matched for gender, age (difference≤5 years), HBeAg status and liver stiffness measurement (LSM) for treatment with one of the 3 antiviral drugs, namely entecavir, tenofovir dipivoxil and adefovir dipivoxil; each cohort was divided into 2 groups, with one of the groups having a combined treatment with Fufang Biejiaruangan tablet. The cumulative negative conversion rate of HBV DNA, normalization rate of ALT, hepatic fibrosis regression and the incidence of HCC were compared among the 3 cohorts and across the 6 groups at 144 weeks.
RESULTS:
A total of 749 patients were available to follow-up at 144 weeks. Compared with the baseline data, the cumulative negative conversion rate of HBV DNA increased gradually and the abnormal rate of ALT decreased significantly over time during the treatment in all the 6 groups (all < 0.001). Compared with the any of the antiviral drugs used alone, the combined treatments all resulted in significantly better antifibrotic effects (χ=11.345, χ=10.160, χ=6.358; all < 0.05). At 144 weeks, the incidence of HCC were 2.2%, 1.7%, 1.7% and 3.3% in enecavir group, enecavir with Biejiaruangan tablet group, adefovir group, and adefovir with Biejiaruangan tablet group, respectively, showing no significant difference between the two cohorts (4 groups; χ=6.813, =0.138). None of the patients in the 2 groups with tenofovir treatment had HCC by the end of the observation.
CONCLUSIONS
Antiviral therapy combined with antifibrotic therapy can effectively reverse hepatic fibrosis and reduce the incidence of HCC in patients with CHB; among the 3 antiviral drugs, tenofovir dipivoxil can be a better option for reducing the incidence of HCC in these patients.

7.Bibliometric analysis of domestic nursing related to ankylosing spondylitis based on Note Express

Objective? To find out the current research status, research hotspots and development trend of ankylosing spondylitis (AS) nursing in China, discover the relevant problems in this field in time, so as to provide reference for AS nursing. Methods? Bibliometric analysis was conducted on the published journals about AS nursing in China. By Note Express v3.2.0, Wanfang Database, CNKI and VIP were searched online from the published articles on AS related nursing in China published from January of 2000 to December of 2018, the statistical tool SATI 3.2 for bibliographic information analysis was used to extract and analyze the published year, periodicals, author's institutions, fund, and key words and retrieved relational matrix table on research topics through frequency analysis and saved in the form of Excel, and the Netdraw function of Ucinet software was used to generate keyword visualization network knowledge map. Results? A total of 305 articles were retrieved from AS nursing journals, and the number of articles was increasing year by year; 33 articles were published in the top 8 core nursing journals, accounting for 10.61%; the largest number of articles was from the Third Affiliated Hospital, Sun Yat-sen University; 88 papers were supported by funds, accounting for 28.85%;the main structure of research was formed by the research hotspots around AS, including nursing, quality of life, health education and quality of sleep, however, researches on the transitional nursing care for the patients after discharge was limited. Conclusions? Although AS nursing has drawn attention from nurses, the quality of research on AS nursing should be improved, the scientific research fund should be increased in order to promote the development of AS nursing to a deeper level.
8.Correlation between post traumatic growth, social support and resilience in patients with breast cancer

Objective? To investigate the correlation between post traumatic growth, social support and resilience in patients with breast cancer, and to provide reference for intervention programme on enhancing positive psychological quality. Methods? Totally 180 patients with breast cancer from 4 ClassⅢ Grade A hospitals in Jiangsu Province were selected by convenience sampling method from June to October in 2018. They were investigated with General Information Questionnaire, Post Traumatic Growth Inventory (PTGI), Social Support Rating Scale (SSRS), and Connor-Davidson Resilience Scale (CD-RISC). Results? In breast cancer patients, the total scores of PTGI, SSRS and CD-RISC were (62.52±17.59), (35.15±9.29) and (56.51±18.33) scores. Correlation analysis showed that the total scores of PTGI and its each dimension were positively correlated with the total scores of SSRS, as well as the total scores of CD-RISC and its each dimension (P<0.01); the total scores of SSRS and its each dimension were positively correlated with the total scores of CD-RISC and its each dimension (P< 0.01). Multivariate linear regression analysis showed that resilience, methods of disease detection, and treatment plans were independent influencing factors of PTG in patients with breast cancer. Both social support and resilience could positively predict PTG, whose path coefficients were 0.493 and 0.302, respectively. Resilience was a mediator of social support and PTG. The indirect effect was 0.080. Conclusions? Resilience plays an important role in mediating between social support and PTG in patients with breast cancer. The resilience training could promote patients' PTG ability .
9.Implementation effect of nursesˊ hierarchical training plan based on training outline and nurses′needs

Objective To explore the implementation effect of nurses′ hierarchical training plan based on training outline and nurses′ needs. Methods Nurses′ hierarchical training outline was established in accordance with three theoretical training modules(humanistic accomplishment, professional competence, career planning/development) as well as three skill training modules(basic nursing skills, specialist nursing skills and first aid nursing skills); the training needs of nurses were investigated by adopting a self-made Training Needs Questionnaire, then the hierarchical training plan was developed based on the training outline and nurses′ training needs. Results There were some differences in the training needs of different levels of nurses in the fields of clinical knowledge and skills,nursing management, nursing education, scientific research and new nursing guidelines and progress(F=2.80-22.47,P<0.05 or 0.01). Before the implementation of nurses′ hierarchical training plan based on training outline and nurses′ needs, nurses′ satisfaction about training, theoretical examination result, clinical working ability and patients′satisfaction towards nursing were 89.06%(228/256),94.80%(1 185/1 250), 96.63%(1 060/1 097) and 95.35%(5 470/5 737), respectively. Then, after the implementation, the corresponding values got improved into 96.76%(239/247), 97.44%(1 297/1 331), 98.75%(1 106/1 120) and 99.22%(6 070/6 118),which were statistically significant(χ2=11.11-176.27,P<0.01). Conclusion Integrating nurses′hierarchical training outline with the training needs helps to provide targeted hierarchical training so as to improve the professional quality of nurses and the patients′satisfaction towards nursing.
10.Value of transrectal ultrasound combined with imaging techniques in the diagnosis of benign and malignant prostate nodules

Objective To explore the value of transrectal ultrasound (TRUS) combined with imaging techniques in the diagnosis of benign and malignant prostate nodules.Methods Totally 170 suspected prostate carcinoma patients were selected and treated with TRUS before puncture.Taking pathological results as the gold standard,the accuracy,specificity,sensitivity,positive predictive value and negative predictive value of transrectal power Doppler ultrasound (PDI),transrectal realtime tissue elastography (TRTE) and PDI combined with TRTE in diagnosis of benign and malignant prostate nodules were compared.Results In the diagnosis of prostate cancer by PDI,the sensitivity,specificity,positive predictive value,negative predictive value and accuracy were 72.9%,74.3%,78.7%,67.9% and 73.5% respectively,while for the TRTE were 73.9%,78.4%,81.6%,69.9% and 75.9%,and there were no significant differences (P > 0.05).PDI combined with TRTE was able to increase the sensitivity (94.8%),negative predictive value (90.9%) and accuracy (82.9%) for diagnosis of prostate nodules (P <0.05).Conclusion Transrectal PDI and TRTE can be used as a single indicator of benign and malignant prostate nodules.The combination of the two methods can gain high sensitivity,negative predictive value aud accuracy.
