1.Effect of closed-chain exercise training on hemiplegic shoulder pain and shoulder joint stability in stroke patients:a study with ultrasound

Objective To investigate the effect of closed-chain exercise training on hemiplegic shoulder pain and shoulder joint sta-bility in stroke patients,and to explore the relationship between hemiplegic shoulder pain and shoulder joint sta-bility.Methods A total of 52 stroke patients with hemiplegic shoulder pain in Jiangsu Province Hospital from October,2020 to January,2023 were selected,and were randomly divided into control group(n=26)and experimental group(n=26).Both groups received conventional rehabilitation therapy,while the experimental group additionally under-went closed-chain exercise training for shoulder joint.Pain severity and motor function were assessed using Visu-al Analog Scale(VAS)and Fugl-Meyer Assessment-Upper Extremities(FMA-UE)before and after intervention.Musculoskeletal ultrasound was used to measure acromion-greater tuberosity distance(AGT),acromion-lesser tu-berosity distance(ALT),acromiohumeral distance(AHD)and supraspinatus thickness(SST)to evaluate shoulder joint stability.Correlation analysis was conducted on the improvements in shoulder pain and shoulder joint stabil-ity in the experimental group.Results Two cases in the control group and two in the experimental group dropped down.Both groups showed signifi-cant improvements in VAS and FMA-UE scores after intervention(|t|>5.214,P<0.001),and the scores im-proved more in the experimental group than in the control group(|t|>2.087,P<0.05).The experimental group also showed significant improvements in AGT,ALT,AHD and SST(|t|>4.187,P<0.001),with AGT,ALT and AHD being superior to those in the control group(|t|>2.155,P<0.05).The difference of VAS score in the exper-imental group was correlated with the difference of FMA-UE,AGT and ALT after intervention(r>0.434,P<0.05).Conclusion Closed-chain shoulder exercise training can significantly improve shoulder joint stability while alleviating shoulder pain,and effectively enhancing upper limb function in stroke patients with hemiplegic shoulder pain.Hemiplegic shoulder pain is correlated with shoulder joint stability.
2.Effects of local vibration therapy on fine motor function of upper extremity and surface electromyography characteristics in children with spastic hemiplegic cerebral palsy

Objective To investigate the effect of local vibration therapy(LVT)on fine motor function of upper extremity and surface electromyography characteristics in children with spastic hemiplegic cerebral palsy(SHCP).Methods Sixty children with SHCP were divided into the observation group and the control group using random number table method,with 30 cases in each group.The control group was given conventional rehabilitation treatment(physical therapy,occupational therapy,sensory integration training,physiotherapy and rehabilitation therapy of traditional Chinese medicine),while the observation group received LVT for the affected upper extremity in addition to conventional rehabilitation treatment.Both groups were treated for 12 weeks.Fine motor function of upper extremity on the hemiplegic side[Peabody Developmental Motor Scale-Fine Motor(PDMS-FM),Fine Motor Function Measure(FMFM)]and root mean square(RMS)value of the biceps surface electromyography were compared between the two groups as well as before and after treatment.Results Compared with those before treatment,both groups showed increases in PDMS-FM scores for grasping energy range,visual-motor integration energy range,fine motor development quotient,FMFM scores for upper extremity joint mobility ability,grasping ability,operation ability,hand-eye coordination ability and total scores,but a decrease in RMS value of biceps brachii after treatment(P<0.05).After treatment,PDMS-FM scores for grasping energy range and visual-motor integration energy range,FMFM scores for upper extremity joint mobility ability and grasping ability and total score were higher in the observation group than those in the control group.The RMS value of biceps brachii was lower in the observation group than that of the control group(P<0.05).Conclusion LVT can enhance the effect of conventional rehabilitation on fine motor function of upper extremity and electromyography characteristics in children with SHCP.

5.Resistance to deltamethrin and its association with mutation sites in the sodium iron channel domain III gene in Rhipicephalus microplus in Huaihua City of Hunan Province

Objective To investigate the level of deltamethrin resistance and mutation sites in the sodium iron channel gene in Rhipicephalus microplus in Huaihua City, Hunan Province, and to examine the correlation between deltamethrin resistance and mutation sites in the sodium iron channel gene in Rh. microplus. Methods Rh. microplus was sampled from multiple yellow cattle farms in Huaihua City, Hunan Province from June to September 2022, and the level of resistance to deltamethrin was determined in ticks using the adult immersion test. The sodium iron channel domain III gene was amplified in deltamethrin-resistant and wild-type Rh. microplus using PCR assay. Following sequencing and sequence alignment, mutation sites were detected in bases. The sodium iron channel domain III gene in Rh. microplus was translated, and the signal peptide, transmembrane domain, and phosphorylation and glycosylation sites were detected in amino acid sequences. The tertiary structures of the sodium iron channel domain III protein of deltamethrin-resistant and wild-type Rh. microplus were deduced and compared, and the association be tween mutation sites in bases and resistance to deltamethrin was examined in Rh. microplus according the level of deltamethrin resistance, sequence alignment and protein tertiary structure. Results The median (LC50) and 95% lethal concentrations (LC95) of deltamethrin were 121.39 mg/L and 952.61 mg/L against Rh. microplus, with a resistance factor of 9.24 and level II resistance. The sequence of the sodium ion channel domain III gene was 1 010 bp in size, and mutation sites were detected in two neighboring bases in the sequence of the sodium ion channel domain III gene in deltamethrin-resistant Rh. microplus. Although no signal peptides were found in the sodium iron channel domain III protein of deltamethrin-resistant or wild-type Rh. microplus, 6 trans-membrane domains, 42 phosphorylation sites and 8 glycosylation sites were identified, with a significant difference in the tertiary structure of the sodium iron channel domain III protein between deltamethrin-resistant and wild-type Rh. microplus. Conclusions Level II resistance to deltamethrin is detected in Rh. microplus in Huaihua City, Hunan Province, and two mutation sites that correlate with the emergence of deltamethrin resistance are identified in the sequence of the sodium iron channel domain III gene in deltamethrin-resistant Rh. microplus.
6.Care burden of primary caregivers of maintenance hemodialysis patients and its correlation with psychological resilience and social support

Objective:To explore the care burden of the primary caregivers of maintenance hemodialysis (MHD) patients and its correlation with psychological resilience and social support.Methods:From January 2020 to December 2021, 97 MHD patients and 97 primary caregivers admitted to Zhumadian Central Hospital were selected by convenient sampling. The subjects were investigated with the Zarit Caregiver Burden Interview (ZBI) , Connor-Davidson Resilience Scale (CD-RISC) and Social Support Rating Scale (SSRS) . Pearson correlation analysis was used to explore the correlation between the care burden, psychological resilience and social support of the main caregivers of MHD patients. Model 4 in the PROCESS macro program was used to test the intermediary model, and the significance of the intermediary effect was tested by Bootstrap analysis. A total of 97 questionnaires were distributed, and the effective recovery rate was 100.00%.Results:There were statistically significant differences in the ZBI scores of MHD patients' main caregivers with different ages, with or without other care workers, total care time, daily care time, psychological resilience and social support ( P<0.05) . The total ZBI score of the main caregivers of MHD patients was (42.39±10.61) , of which 1.03% (1/97) had no or very mild burden, 44.33% (43/97) had mild burden, 50.52% (49/97) had moderate burden, and 4.12% (4/97) had severe burden. The total score of CD-RISC of the main caregivers of MHD patients was (53.09±5.69) , of which 79.38% (77/97) of the main caregivers had low-level resilience, and 20.62% (20/97) of the main caregivers had medium-level resilience. The total SSRS score of the main caregivers of MHD patients was (30.33±7.19) , of which 21.65% (21/97) of the main caregivers had low-level social support, 76.29% (74/97) of the main caregivers had medium-level social support, and 2.06% (2/97) of the main caregivers had high-level social support. The care burden of the main caregivers of MHD patients was negatively correlated with psychological resilience and social support, and psychological resilience was positively correlated with social support ( P<0.05) . Social support positively predicted psychological resilience and negatively predicted care burden ( P<0.05) . Psychological resilience negatively predicted care burden ( P<0.05) . The intermediary model (dependent variable was care burden; independent variable was social support; intermediary variable was psychological resilience) had a significant intermediary effect. The indirect effect accounted for 79.23% of the total effect, and the direct effect accounted for 20.77% of the total effect. Conclusions:The main caregivers of MHD patients have moderate care burden, and social support can directly affect the care burden, and also indirectly affect the care burden through the mediation of psychological resilience.
7.Knowledge,attitude and practice related with AIDS of floating population in Hongkou District,Shanghai

Objective To evaluate the knowledge,attitude and practice(KAP)related with AIDS of local floating population.Methods A total of 1 942 floating persons were sampled by multistage cluster sampling and interviewed with questionnaire in the agricultural markets,construction sites,and their habitats within 3 communities in Hongkou District.Results 82.9% of the interviewees were 20 ~49 years old.The average score was 34.77 ±3.52 (maximum score is 44)for knowledge,12.11 ±2.32(maximum score is 19)for attitude,5.50 ±0.95(maximum score is 7)for practice,respectively.The scores increased with educational level significantly.The scores of knowledge were 33.26 ±3.54,34.63 ±3.23,36.56 ±3.39 among the subjects with educational levels of primary school and below,junior high school,senior high school and above.The scores of attitude were 13.77 ±2.27,14.79 ±2.39,15.62 ±2.38,respectively.And the score of practice was 5.40 ±0.90,5.51 ±0.93,5.58 ±1.03,respectively.Conclusion In the present,the KAP relating AIDS of local floating population is poor in Hongkou District.A variety of intervention methods and operational patterns should be developed for the floating population with different educational levels and jobs.
