1.Contribution and interaction of various cells in bone marrow microenvironment to exosomal circular RNA associated with multiple myeloma bone disease

BACKGROUND:Multiple myeloma bone disease is a serious complication of multiple myeloma,and its pathogenesis is closely related to the dysregulation of the bone marrow microenvironment.Peripheral blood exosomes are secreted by various cell types and can reflect a variety of information such as the tumor microenvironment.Exosomal circular RNA is gradually becoming the focus of liquid biopsy due to its high stability,high abundance,high specificity,and high conservation.In multiple myeloma bone disease,there is still a lack of relevant research on the role of exosomal circular RNAs.With the development of single-cell RNA sequencing technology,clarifying the contribution of each cell type in the bone marrow microenvironment to multiple myeloma bone disease-related exosomal circular RNAs and the interaction between each cell type will help provide new biomarkers for the diagnosis and treatment of multiple myeloma bone disease.OBJECTIVE:To preliminarily determine which cells in the bone marrow microenvironment the multiple myeloma bone disease-related peripheral blood exosomal circular RNAs originated from,and explore the effects of cell interactions in the bone marrow microenvironment on multiple myeloma bone disease at the single-cell level.METHODS:Peripheral blood exosomes from six multiple myeloma bone disease patients and five healthy controls were collected for high-throughput sequencing,differently expressed circular RNAs were screened,and GO and KEGG enrichment analyses were performed.The bone marrow single-cell RNA sequencing dataset GSE271107 was obtained from the GEO database.The data of four multiple myeloma bone disease patients were integrated.After quality control and filtering,the batch effect was removed by Harmony method.Subgroup clustering was performed by UMAP,and cell groups were manually corrected after automatic annotation by SingleR.CellChat was used to infer and visualize the intercellular communication in single-cell RNA sequencing data,and analyze the interaction between ligands and receptors.RESULTS AND CONCLUSION:(1)Compared with healthy controls,the expression levels of 1 265 circular RNAs in peripheral blood exosomes of multiple myeloma bone disease patients were significantly upregulated.(2)GO and KEGG analyses suggested that the parent genes of differently expressed circular RNAs were mainly enriched in pathways in nervous system development,pathways in cancer,PI3K-Akt signaling pathway,Hippo signaling pathway,arrhythmogenic right ventricular cardiomyopathy,dilated cardiomyopathy,etc.,which were closely related to the proliferation,adhesion,migration,and homing of myeloma cells,and multiple myeloma-related complications(such as multiple myeloma bone disease,peripheral neuropathy,and cardiac events).(3)The parent genes of multiple myeloma bone disease-related differential circular RNAs were mainly derived from T cells/natural killer cells,B cells and monocytes/macrophages in the bone marrow microenvironment,which affected osteoclast function by regulating the secretion of multiple cytokines.(4)Monocytes/macrophages,as osteoclast precursor cells,interacted with tumor cells and other immune cells.The MIF pathway was the main pathway involved.The above data preliminarily identified the source cells of multiple myeloma bone disease-related peripheral blood exosomal circular RNA in the bone marrow microenvironment,and found that the ligand-receptor interaction of the MIF pathway between monocytes/macrophages and tumor cells and immune cells may be an important factor in the occurrence of multiple myeloma bone disease.
2.Circulating inflammatory proteins and myocardial hypertrophy:large sample analysis of European populations from GWAS Catalog and FinnGen databases

BACKGROUND:Myocardial hypertrophy often leads to severe cardiovascular diseases and is difficult to diagnose due to its early stages being hard to detect.Circulating inflammatory proteins have been found to be significantly associated with cardiovascular diseases,yet the specific mechanisms linking them to myocardial hypertrophy remain unclear.OBJECTIVE:To investigate the relationship between circulating proteins and myocardial hypertrophy using multiple Mendelian randomization approaches.METHODS:Utilizing data from 91 circulating inflammatory proteins in the GWAS Catalog database and the latest myocardial hypertrophy data from the R11 FinnGen database,we employed bidirectional two-sample Mendelian randomization,multivariate Mendelian randomization,and Genome-Wide Association Studies co-localization to investigate the causal relationship between circulating inflammatory proteins and myocardial hypertrophy.The accuracy of the results was verified through sensitivity tests including MR-PRESSO,Cochran's Q test,MR-Egger intercept assessment,leave-one-out analysis,and funnel plot analysis.RESULTS AND CONCLUSION:In the results of two-sample Mendelian randomization,the primary method used for evaluation was the Inverse Variance Weighting(IVW)approach.It was found that the level of T-cell surface glycoprotein CD6 isoform(IVW:P=0.046,OR=0.74,95%Cl:0.66-1.00),level of slit chemokine(IVW:P=2.1×10-2,OR=0.74,95%CI:0.556-0.95),level of Delta and Notch-like epidermal growth factor-related receptor(IVW:P=3.7×10-4,OR=0.66,95%CI:0.49-0.87),level of interleukin-2(IVW:P=3.8×103,OR=0.667,95%CI:0.50-0.88),and sulfotransferase 1A1(IVW:P=1.42×102,OR=0.80,95%CI:0.67-0.96)had a unidirectional causal effect on cardiac hypertrophy.(2)Among the findings in multivariate Mendelian randomization,the levels of the CD6 isoform of T-cell surface glycoprotein(IVW:P=1.39×102,OR=0.81,95%CI:0.69-0.96)and the levels of Delta and Notch-like epidermal growth factor-related receptor(IVW:P=3.7×10-2,OR=0.73,95%CI:0.55-0.98)were positive,indicating that the results remained significant after excluding the effects of other circulating inflammatory proteins that had an impact on myocardial hypertrophy.(3)In colocalization,T-cell surface glycoprotein CD6 isoform levels had H3+H4=0.96,with the most significant single nucleotide polymorphism being rs59570070,suggesting an intrinsic link between T-cell surface glycoprotein CD6 isoform levels and myocardial hypertrophy.(4)Sensitivity results showed no abnormalities,indicating no heterogeneity or pleiotropic effects influencing the results.(5)These results verified that T cell surface glycoprotein CD6 isoforms,Slit chemokine,Delta and Notch-like epidermal growth factor-related receptors,interleukin-2,and sulfotransferase 1A1 had a unidirectional causal effect on myocardial hypertrophy.T cell surface glycoprotein CD6 isoforms and Delta and Notch-like epidermal growth factor-related receptors had the deepest impact,suggesting that there may be related pathways between T cell surface glycoprotein CD6 isoforms and myocardial hypertrophy.Mendelian randomization studies require large amounts of clinical data and therefore often use European samples from international databases for analysis.Since this analytical method has significant advantages in causal inference,precision medicine,and cross-population validation,its research results still hold great significance for the medical development in China.As Mendelian randomization research deepens,it also promotes the collection and analysis of clinical data in China to some extent.In the future,we can further analyze key protein mechanisms,combine multiomics and clinical validation,develop an inflammatory marker monitoring system and novel anti-inflammatory therapies,thereby promoting the prevention and control of cardiovascular diseases and the development of personalized medicine.
3.Clinical Application of Harmonizing and Regulating Pivot Method in Pulmonary Fibrosis:Based on the Characteristics of Essence-Attribute-Function

Based on the theory of "shaoyang(少阳) resembling the pivot" and collateral diseases, this article proposes that pulmonary fibrosis (PF) can be divided into three stages including wind bi (痹), constraint bi, and atrophy bi. The core pathogenesis of PF is the obstruction of the pivot and pulmonary collateral obstruction. In terms of treatment, the basic principles are to harmonize and regulate the pivot, and to promote the circulation of the lung collaterals. Depending on the different characteristics of the "essence-attribute-function", treatment methods such as harmonizing and regulating the pivot, resolving phlegm and removing stasis, supplementing deficiency and harmonizing collaterals are suggested. This approach ensures the regulation of the pivot, smooth circulation of qi and blood, unblocking of the lung collaterals and nourishing the lung body, achieving the goals of balancing the ascending and descending of qi, removing phlegm and stasis, and relieving cough and wheezing.
4.Research progress on the intervention of traditional Chinese medicine in lipid metabolism for obesity treatment via cAMP signaling pathway

Obesity, a global chronic disease, is associated with adipose tissue dysfunction, which is one of the contributing factors to obesity. The cyclic adenosine monophosphate (cAMP) signaling pathway, a key regulator of lipid metabolism, plays a pivotal role in obesity development. Various of traditional Chinese medicine monomers, such as flavonoids, lignans, phenols, and terpenoids, as well as traditional Chinese medicine compound formulas like Xiaoyao powder, Shengmai powder, and Zexie decoction, can maintain energy homeostasis, balance adipose tissue function, regulate glucose metabolism, improve insulin resistance, and suppress inflammatory responses through cAMP signaling pathway regulation, thereby intervening in lipid metabolism for obesity treatment. Although a substantial amount of basic research has preliminarily elucidated the potential mechanisms by which traditional Chinese medicine intervenes in obesity through the cAMP signaling pathway, clinical translational research remains inadequate. There is an urgent need for large-sample, high-quality randomized controlled trials to validate these findings.

7.Advances in polymorphisms of genes associated with oxaliplatin-induced peripheral neurotoxicity

Peripheral neurotoxicity represents one of the most severe dose-limiting adverse reactions associated with oxaliplatin, with genetic polymorphisms playing a significant role in oxaliplatin-induced peripheral neuropathy (OIPN). OIPN can be categorized as acute or chronic based on onset timing. The former presents clinically as sensory abnormalities or even motor disorders, while the latter presents clinically as limb sensory disorders that persist, numbness or pain in the hands and feet. The transporter genes OCT2, OCTN2, and NHE1 may be implicated in OIPN; drug-metabolizing enzyme gene GSTP1 Ile105Val, DPYD rs1801265, voltage-gated sodium channel (NaV) gene SCN4A rs2302237, SCN9A rs6746030, SCN10A rs12632942, and other associated genes such as HLA-G rs1610696, rs371194629 and CCNH rs2230641, rs3093816 are associated with severe OIPN. Conversely, DNA repair-related gene XRCC1 rs23885, NaV gene SCN9A rs3750904, rs12478318 and rs6754031 are associated with reduced OIPN risk. In the future, the genetic research findings on OIPN can be translated into clinical applications, ultimately achieving individualized precision medicine for patients.
8.Optimization of Quality Standards for Polygalae Radix Based on Characteristic Chromatograms and QAMS for Xanthones

ObjectiveTo address the limitations of the current quality standard for Polygalae Radix(PR), which relies on a single component for quality assessment and struggles to holistically control its intrinsic quality, by constructing a comprehensive quality evaluation system integrating "macro-characterization of chemical profile, synchronous quantification of multiple index components, and quantitative analysis of multi-components by single marker(QAMS) for key component groups". This study aims to facilitate the scientific revision of the quality standard for PR. MethodsHigh performance liquid chromatography(HPLC) characteristic chromatograms were established for 11 batches of PR medicinal materials(YZ), 10 batches of PR decoction pieces(YP), and 10 batches of licorice-processed PR decoction pieces(ZYZ), followed by similarity evaluation and identification of common peaks. HPLC-QAMS was developed for xanthones(sibiricaxanthone B, polygalaxanthone Ⅺ, polygalaxanthone Ⅲ) in the characteristic chromatograms. Simultaneously, the external standard method(ESM) was used to determine the contents of the corresponding xanthones and 3,6'-disinapoyl sucrose in YZ, YP, and ZYZ, followed by multivariate statistical analysis and Spearman correlation analysis. ResultsThe similarity between the characteristic chromatograms of 31 batches of PR samples and the reference chromatogram was>0.9. A total of 13 common peaks were identified, and 10 of these peaks were characterized through reference standard comparison. The successfully constructed QAMS method showed that the relative correction factors(RCFs) of sibiricaxanthone B and polygalaxanthone Ⅺ to polygalaxanthone Ⅲ were 0.76 and 0.88, and their relative retention times(RRTs) were 0.85 and 0.97, respectively. The results calculated by the QAMS method showed no significant difference from those obtained by the ESM. According to the limit standard for polygalaxanthone Ⅲ in the 2020 edition of the Pharmacopoeia of the People's Republic of China(hereinafter referred to as the Chinese Pharmacopoeia), the pass rate of 31 batches of samples was only 19.35%. Multivariate statistical analysis indicated certain compositional differences between different batches of YZ and YP, as well as between YP and ZYZ, with 3,6'-disinapoyl sucrose identified as the main differentiating component. Furthermore, correlation analysis revealed that the content of polygalaxanthone Ⅲ was positively correlated with the contents of sibiricaxanthone B and polygalaxanthone Ⅺ, but showed no association with the content of 3,6'-disinapoyl sucrose. ConclusionIt is recommended that the content limit for polygalaxanthone Ⅲ in YZ,YP and ZYZ be revised to not less than 0.07%, or the total content of polygalaxanthone Ⅲ, sibiricaxanthone B and polygalaxanthone Ⅺ be not less than 0.18%. The newly established triple quality control model of "holistic control via characteristic chromatograms, precise quantification of oligosaccharide esters, and efficient detection of xanthones by QAMS" provides a systematic and precise solution for quality evaluation of PR and similar Chinese herbal medicines.
9.Academic Characteristics of Contemporary Chinese Medicine Masters in Treating Diabetic Kidney Disease Based on SrTO

ObjectiveTo explore the academic characteristics of contemporary renowned Chinese medicine masters in treating diabetic kidney disease (DKD) from the perspectives of principles, methods, formulas, and medications. MethodsIn strict accordance with the Systematic Review of Text and Opinion (SrTO) process developed by the Joanna Briggs Institute (JBI), an Australian evidence-based healthcare center, the databases including China National Knowledge Infrastructure (CNKI), VIP Database, Wanfang Data, and China Biomedical Literature Service System (SinoMed) were searched. Based on predefined inclusion and exclusion criteria, text information extraction, quality evaluation, and text information synthesis were conducted sequentially. The data were analyzed and presented in the form of text and figures. ResultsA total of 215 articles related to 43 contemporary renowned experts in the fields of Chinese medicine nephrology and endocrinology were included. The study found that the academic thoughts of these masters in the treatment of DKD are extensive, involving multiple levels such as disease understanding, therapeutic strategies, formula application, and medication use. In terms of disease understanding, the primary pathogenesis is characterized by deficiency in the root and excess in the manifestation. It is emphasized that internal factors, such as congenital endowment deficiency, interact with external factors such as improper diet, emotional disturbances, invasion of exogenous pathogens, and delayed or inappropriate treatment, to jointly induce the disease. This further gives rise to various pathogenetic theories, including obstruction of renal collaterals by blood stasis, toxin-induced damage to renal collaterals, latent wind disturbing the kidney, and internal heat leading to mass formation. In terms of therapeutic strategies and medication use, the principal treatment method is to replenish Qi and nourish Yin. Stage-based and syndrome-differentiated treatments are advocated. Flexible use of insect-derived drugs and wind-dispelling drugs is emphasized, along with proficiency in applying classical formulas and drug pairs. Integrated internal and external treatments, as well as the combined application of multiple therapeutic approaches, are commonly employed for comprehensive management. Meanwhile, the concept of "preventive treatment of disease" is upheld, and individualized long-term management of patients is advocated. ConclusionThrough the SrTO process, the academic thoughts of contemporary renowned Chinese medicine masters in the treatment of DKD have been systematically and standardly synthesized, providing a scientific and standardized basis for future theoretical exploration.

Based on predefined inclusion and exclusion criteria, text information extraction, quality evaluation, and text information synthesis were conducted sequentially. The data were analyzed and presented in the form of text and figures. ResultsA total of 215 articles related to 43 contemporary renowned experts in the fields of Chinese medicine nephrology and endocrinology were included. The study found that the academic thoughts of these masters in the treatment of DKD are extensive, involving multiple levels such as disease understanding, therapeutic strategies, formula application, and medication use. In terms of disease understanding, the primary pathogenesis is characterized by deficiency in the root and excess in the manifestation. It is emphasized that internal factors, such as congenital endowment deficiency, interact with external factors such as improper diet, emotional disturbances, invasion of exogenous pathogens, and delayed or inappropriate treatment, to jointly induce the disease. This further gives rise to various pathogenetic theories, including obstruction of renal collaterals by blood stasis, toxin-induced damage to renal collaterals, latent wind disturbing the kidney, and internal heat leading to mass formation. In terms of therapeutic strategies and medication use, the principal treatment method is to replenish Qi and nourish Yin. Stage-based and syndrome-differentiated treatments are advocated. Flexible use of insect-derived drugs and wind-dispelling drugs is emphasized, along with proficiency in applying classical formulas and drug pairs. Integrated internal and external treatments, as well as the combined application of multiple therapeutic approaches, are commonly employed for comprehensive management. Meanwhile, the concept of "preventive treatment of disease" is upheld, and individualized long-term management of patients is advocated. ConclusionThrough the SrTO process, the academic thoughts of contemporary renowned Chinese medicine masters in the treatment of DKD have been systematically and standardly synthesized, providing a scientific and standardized basis for future theoretical exploration.
