INTRODUCTION
Acute coronary syndrome (ACS) and end-stage renal disease (ESRD) are both prevalent globally. The diagnosis and management of ACS in ESRD is difficult because the interplay of cardiovascular and renal disease is complicated. The guidelines for ACS may not be applicable to the ESRD population because the trials from which these are drawn mostly excluded ESRD patients.
OBJECTIVETo determine the clinical profile and outcomes of CKD patients on dialysis admitted for ACS in the Philippine General Hospital (PGH).
METHODSWe did a retrospective cohort study and employed a retrospective review of electronic medical records among ESRD patients presenting with ACS in PGH from May 2021 to November 2023. The collected data was analyzed using univariate and bivariate statistics using PRISM software.
RESULTSA total of 48 patients with ESRD were admitted for ACS in this study – 8 with STEMI and 40 with NSTEMI. The mean age was 61 years old and 33 (68.8%) were male. Among those with STEMI, six (75%) presented with Kilip II or more. While among those with NSTEMI, 17 (42.5%) had a GRACE score >140 and 27 (67.5%) had an NSTEMI TIMI risk score >2. On average, the patients were on hemodialysis for 31 months prior to admission. The most common comorbidities were hypertension (91.7%) and heart failure (83.3%). On admission, 18 (37.5%) presented with SBP >160, 7 (14.6%) patients presented with shock, and 4 (8.3%) patients presented with cardiac arrest. 38 (79.2%) patients had anemia on admission. 21 (43.8%) patients had left ventricular hypertrophy on electrocardiogram while 34 (70.8%) patients had cardiomegaly on chest radiography. The average left ventricular ejection fraction on echocardiogram was 46% and 27 (90%) patients had segmental wall motion abnormalities. The most common angiographic finding was 3-vessel coronary artery disease seen in 50% of patients. Almost all patients received dualantiplatelet therapy, high dose statin, and beta-blocker. The mortality rate was high at 43.8% with cardiovascular causes being the most common cause of death.
CONCLUSIONThis study demonstrates the high mortality rate among patients with ESRD presenting with ACS. Our study portrays that patients with ESRD present with higher risk features including abnormalities in vital signs, laboratories, imaging, high prognostications score, and high in-hospital morbidity.

2.Case report: A rare case of a giant left main coronary artery aneurysm in an adult male with two-vessel coronary artery disease.

INTRODUCTION
The existence of a coronary artery aneurysm (CAA) can pose significant risk for death. It can cause thrombosis, dissection, rupture or myocardial infarction. An exceedingly rare involvement of the left main coronary artery (LMCA), particularly giant-sized is even more catastrophic, a finding seen in only 0.1% of patients. Furthermore, co-existence with significant stenotic coronary artery disease (CAD) portends grim survival. Owing to the rarity of this combination, no data is available locally and only limited case reports are documented internationally. Hence, no consensus guidelines have been published yet. This paper aims to contribute to the sparse medical knowledge on the treatment approach and management of LMCA aneurysm with concomitant CAD.
CASE PRESENTATIONA 62-year-old male, Filipino, hypertensive and hyperlipidemic sought consult due to one-year exertional chest pain. Coronary angiogram revealed the LMCA to be a diffusely aneurysmal, large-sized vessel measuring 9.7 mm x 7.9 mm with a significant two-vessel CAD affecting the proximal left anterior descending (LAD) and right coronary artery (RCA). As per multidisciplinary decision, the patient underwent surgical revascularization via cardiopulmonary bypass graft (CABG) addressing the CAD and LMCA aneurysm managed conservatively through guideline-directed medical therapy. The patient’s course of treatment was uneventful. He returned for follow-ups for three months post-surgery and remained symptom-free.
DISCUSSIONGiant coronary artery aneurysms (GCAA) are vessel dilatations that exceed 4x the diameter of a normal adjacent artery. The patient had a unique case of GCAA involving the LMCA combined with two-vessel CAD. Few studies have documented a medical or surgical approach and long-term outcomes are unknown. Without sufficient evidence-based guidelines, the multidisciplinary decision was to perform CABG and manage the LMCA aneurysm conservatively.
CONCLUSIONDue to extremely limited information available on the giant LMCA aneurysm natural history, definitive management remains controversial. A multidisciplinary team approach is highly recommended for patient-specific needs to achieve favorable outcome and ensure survival.

5.Severe Premature Coronary Artery Disease in Homozygous Familial Hypercholesterolemia with Marked Lipid Reduction After Inclisiran Therapy

Introduction:
Homozygous familial hypercholesterolemia (HoFH) is a
rare inherited disorder characterized by markedly elevated
low-density lipoprotein cholesterol (LDL-C) from birth,
childhood xanthomas, and accelerated atherosclerotic
cardiovascular disease. Achieving LDL-C targets remains
difficult despite statins, ezetimibe, and lipoprotein apheresis,
especially in patients with treatment interruption, poor
adherence, or limited access to specialized lipid services.
We report a female with genetically confirmed HoFH who
developed severe premature coronary artery disease and
showed marked lipid reduction after inclisiran therapy.
Case:
A 28-year-old Malay female with LDL receptor mutationconfirmed HoFH, diagnosed at age 7, had a strong family
history of premature cardiovascular death. She underwent
biweekly lipoprotein apheresis from ages 8 to 15 years,
but later defaulted on follow-up. During pregnancy in
2021, weekly then biweekly apheresis was reintroduced
for severe hypercholesterolemia, but she disengaged
postpartum. In 2025, she re-presented with exertional chest pain, orthopnea, and palpitations. Examination
showed widespread xanthomas. Electrocardiography
demonstrated sinus tachycardia with inferolateral ST
depression. Echocardiography revealed a left ventricular
ejection fraction of 40%, anterior and septal akinesia, severe
mitral regurgitation, moderate tricuspid regurgitation, and
pulmonary hypertension. Her lipid profile showed total
cholesterol 18.1 mmol/L and LDL-C 14.9 mmol/L. Coronary
angiography demonstrated triple-vessel disease with left
main stem involvement and critical right coronary ostial
stenosis. Coronary artery bypass grafting was advised but
declined. She was treated with high-dose rosuvastatin,
ezetimibe, dual antiplatelet therapy, and inclisiran 284 mg.
After one dose of inclisiran, total cholesterol fell to 6.78 mmol/L
and LDL-C to 4.71 mmol/L, a reduction of more than 60%.
Conclusion
This case highlights the aggressive natural history of inadequately controlled HoFH and the importance of sustained
lifelong therapy. Inclisiran may provide additional LDL-C
reduction in selected HoFH patients, although long-term
cardiovascular outcome data remain limited.

6.Clinical profiles and in-patient outcomes of patients with myocardial bridging versus obstructive coronary artery disease: A single center retrospective study

BACKGROUND
Myocardial bridging (MB) is a congenital coronary anomaly characterized by an epicardial coronary artery taking an intramuscular course, causing systolic compression of the tunneled segment. In comparison to coronary artery disease (CAD), myocardial bridges have been uncommonly associated with acute coronary syndromes and sudden cardiac death.Evidence of accelerated atherosclerotic plaque formation proximal to the bridged segment may increase the risk for future adverse cardiac events in these patients.
METHODOLOGYThis Single–Center Retrospective Study included 323 adult in-patients who underwent coronary angiography for suspected myocardial ischemia in 2022. Clinical information and in-hospital outcomes were obtained by review of medical records.
RESULTSMyocardial bridging was observed in 31 out of 323 patients (9.60%), with the majority in the mid-left anterior descending artery (87.10%). MB was more prevalent in females (56.62%), and these patients were younger than patients with obstructive CAD (56.9 versus 63.6 years). Chronic Coronary Syndrome was more prevalent in the MB group (82.62%). The coronary segment proximal to the area with MB showed the concurrent presence of obstructive CAD in 16.12% and non-obstructive CAD in 29.03% of cases. In-hospital mortality occurred in 4.44% of the studied population. However, there were no mortalities in the MB group.
CONCLUSIONAmong patients admitted for suspected myocardial ischemia, 9.6% had MB. These patients were younger and, more often, female. Obstructive and non-obstructive CAD were noted in bridged vessels. Although patients with obstructive CAD have a higher risk of experiencing in-hospital death and cardiac complications, evidence of increased atherosclerotic plaque formation in bridged vessels has important implications for future adverse cardiac events and repeat hospitalizations in the MB population. Aggressive risk factor modification, emphasis on long-term follow-up, and the establishment of clinical practice guidelines are therefore necessary for patients with MB.

7.Comparison of 600 mg versus 300 mg clopidogrel loading dose for patients with ischemic heart disease: A meta-analysis of randomized controlled trials

INTRODUCTION
While a 600 mg loading dose (LD) of clopidogrel has demonstrated superior inhibition of platelet function compared to 300 mg LD, the clinical evidence supporting this superiority is limited. The debate centers on whether higher clopidogrel LD regimen in percutaneous coronary intervention (PCI) outperforms the standard 300 mg LD, with potential benefits being more pronounced in higher-risk patients. Balancing enhanced platelet inhibition to reduce ischemic events against the associated risk of increased bleeding remains a critical consideration in determining the optimal loading dose of clopidogrel for patients with ischemic heart disease.
METHODSA systematic literature search for randomized clinical trials (RCTs) was performed comparing 600 mg with 300 mg LD of clopidogrel using PubMed, MEDLINE, Embase, Cochrane, Clinicaltrials.gov and HerdinPH. Studies included those between 2010 and 2023 involving human subjects. The primary efficacy endpoint was a 1-month rate of major adverse cardiac event (MACE) and the primary safety outcome was bleeding adverse effects.
RESULTSNine RCTs involving 29,827 patients were included in the efficacy analysis. Mean duration of follow-up was 30 days. Only eight studies were eligible for safety analysis. Compared with standard LD clopidogrel, high LD significantly reduced the incidence of overall MACE (OR: 0.82, 95% CI: 0.74-0.91, p = 0.0002), nonfatal myocardial infarction (OR: 0.56; 95% CI: 0.32-0.99, p = 0.15) and target vessel revascularization (OR: 0.63; 95% CI: 0.41-0.95, p = 0.03), without significant difference in terms of cardiac death (OR: 0.89; 95% CI: 0.76-1.04, p = 0.15) and stroke (OR: 0.92; 95% CI: 0.67-1.26, p = 0.61). However, major bleeding risk was higher in the 600 mg LD (1.9%; 261/13288) compared with 300 mg LD (2.4%; 328/13242) [OR: 1.27; 95% CI: 1.08-1.49, p = 0.005] without significant difference in minor bleeding (OR: 1.05; 95% CI: 0.94-1.17, p = 0.35).
CONCLUSIONThe administration of 600 mg clopidogrel LD reduces the overall risk of MACE with associated increased risk of major bleeding.

BACKGROUND OF THE STUDY
Coronary artery disease (CAD) poses a significant global health challenge. In the Philippines, despite increased availability of coronary artery bypass grafting (CABG), cost remains a barrier to access. This research evaluates key factors such as all-cause death, cardiovascular death, repeat revascularization and quality of life post-CABG under the Philippine Health Z Benefit Package (PZBP). Its findings provide critical insights for shaping clinical practices, policymaking and advocating for broader implementation of the PZBP to improve healthcare access and quality of life for post-CABG patients.
METHODSThis retrospective cross-sectional study explores the outcomes of post-coronary artery bypass grafting (CABG) patients enrolled under the PZBP. The study, conducted at Perpetual Succour Hospital in Cebu City from December 2018 to September 2023, included patients diagnosed with CAD based on ACC/AHA 2021 guidelines. Using a complete enumeration strategy, the research employed descriptive statistics for demographic and clinical profiling, and measured quality of life using the World Health Organization Quality of Life Brief Version (WHOQOL-BREF) tool.
RESULTSThe study analyzed 29 patients who underwent CABG through the PZBP, presenting detailed demographic and clinical data. Most patients (41.4%) fell within the 51-60 age range, with males comprising 75.9%. Diabetes mellitus type II was prevalent (58.6%) and 86.2% had three-vessel diseases. The quality of life assessed with WHOQOL-BREF yielded positive scores across physical, mental, social and environmental domains, indicating favorable outcomes. Notably, no cardiovascular deaths, all-cause deaths, or repeat revascularizations were reported during the study, highlighting the effectiveness of CABG under PZBP.
CONCLUSIONThe study on post-CABG patients under the PZBP shows promising outcomes with no all-cause death, cardiovascular death, or repeat revascularization. This indicates the program's effectiveness in delivering accessible, high-quality healthcare, enhancing long-term survival rates and overall well-being. However, addressing underutilization is crucial, highlighting the importance of raising awareness and utilization to further improve post-CABG patients' outcomes and quality of life.

9.Association of metabolic dysfunction-associated fatty liver disease with coronary artery calcification among Filipino patients in a tertiary hospital in Cebu City

BACKGROUND:
Non-alcoholic fatty liver disease (NAFLD), now known as Metabolic Dysfunction-Associated Fatty Liver Disease (MAFLD), is linked to cardiovascular disease. This renaming emphasizes the role of metabolic problems. Coronary artery calcification (CAC) reflects early coronary artery disease, but data on the MAFLD-CAC link is limited.
OBJECTIVE:
To demonstrate the association between metabolic dysfunction-associated fatty liver disease (MAFLD) based on its criteria and coronary artery calcification, as measured by CT CAC score.
METHODS:
This single-center retrospective study involved adult Filipino patients who underwent CT CAC scoring between January 2021 and January 2023. Clinical and laboratory data were obtained via review of electronic records.
RESULTS:
This study involved 147 patients with an average age of 62 years, primarily females (57.14%), and mostly falling into the Obese-Class I category (31.29%). The most common comorbidities were hypertension (95.24%), dyslipidemia (62.59%), and diabetes mellitus (38.1%). In terms of CAC scores using the CT Agatston method, majority (30.61%) had low calcium buildup (Stage 2 with scores between 1-99). Approximately 26.53% had higher liver fat content with liver HU below 40, while 73.47% had lower liver fat content with HU equal to or greater than 40. Furthermore, 25.17% of patients with fatty livers and other risk factors were diagnosed with MAFLD, while 74.83% were not. The p-value indicated a significant difference in proportions, suggesting a lower proportion of MAFLD among those who had undergone CT CAC scoring. However, the Pearson Chi-Square statistic (4.051) and the p-value (0.256) indicated no statistically significant association between MAFLD and CT CAC.
CONCLUSION
The study found a notably lower proportion of MAFLD diagnoses in patients who underwent CT CAC scoring. Additionally, there was no statistically significant link between MAFLD and CT CAC.

10.Association between MLPH gene hypermethylation in peripheral blood and coronary heart disease.

OBJECTIVES:
To investigate the association between methylation levels of tumor suppressing subtransferable candidate 1 (TSSC1) and melanophilin (MLPH) genes in peripheral blood and coronary heart disease (CHD) in Chinese population.
METHODS:
This case-control study was conducted in 86 CHD patients and 95 healthy individuals, whose methylation levels of TSSC1 and MLPH genes in peripheral blood were determined using mass spectrometry. Mann-Whitney U test was used to compare the methylation levels in different subgroups. The correlation of TSSC1 and MLPH gene methylation levels with age and gender were evaluated using Spearman correlation coefficient and contingency coefficient, respectively.
RESULTS:
Compared with the healthy individuals, the CHD patients showed a significant correlation between MLPH hypermethylation and myocardial infarction (MI) (MLPH_CpG_2.7: P=0.045; MLPH_CpG_3/cg06639874: P=0.049; MLPH_CpG_5: P=0.019), and this correlation was even stronger in individuals below 65 years of age (MLPH_CpG_2.7: P=0.014; MLPH_CpG_4: P=0.001) and in male subjects (MLPH_CpG_2.7: P=0.004; MLPH_CpG_3/cg06639874: P=0.044). The methylation level of TSSC1 gene in peripheral blood was not found to correlate with CHD or its subtypes.
CONCLUSIONS
Our findings suggest a correlation of MLPH hypermethylation in peripheral blood with CHD and MI in Chinese population, especially in individuals below 65 years and in male individuals.
