1.Effects of the expanded lateral thoracic artery perforator flap in the reconstruction of breast scar contracture deformity after burns in minor females

Objective:To investigate the effects of the expanded lateral thoracic artery perforator flap in the reconstruction of breast scar contracture deformity after burns in minor females.Methods:The study was a retrospective observational study. From July 2018 to October 2023, 8 female children aged 4 to 12 years and with breast scar contracture deformity after burns, who met the inclusion criteria, were admitted to the Department of Burns and Plastic Surgery of Guangzhou Red Cross Hospital of Jinan University. The skin and soft tissue expander (hereinafter referred to as expander) was placed in the first stage. The contracture scar was removed and released in the second stage, and the wound formed after the scar was removed measured between 9 cm×8 cm and 15 cm×10 cm. The expanded lateral thoracic artery perforator flap was designed and transferred to repair the wound with resected flap area of 10 cm×9 cm to 16 cm×11 cm, and the wound at the flap donor area was directly sutured. The complications such as incision infection, hematoma, and expander exposure were observed after stage Ⅰ surgery. After stage Ⅱ surgery, the survival of the flap and the wound healing at the flap donor area were observed. During the 1-year follow-up after the stage Ⅱ surgery, the breast development was evaluated according to tanner staging performance of female pubertal breast development, the aesthetic effect of the affected breast was evaluated by using the aesthetic effect evaluation standard after breast surgery, the Vancouver scar scale (VSS) was used to score the scar condition at the flap donor and recipient areas, and the satisfaction of the children's families with the surgical outcomes was investigated by using a self-made scale.Results:After stage Ⅰ surgery, no incision infection, hematoma, expander exposure, or other complications occurred in 8 children. After stage Ⅱ surgery, only one child had tissue necrosis at the distal end of the flap with a size of about 2 cm×1 cm, which healed after dressing change, and the flap in other children had good blood supply, soft texture, moderate thickness, and similar color to the skin at the recipient area. The wounds at all flap donor areas healed well. During the 1-year follow-up after stage Ⅱ surgery, 7 children had normal breast development, with their breast volume, height, and shape being almost the same as or similar to the healthy side, with the aesthetic effect of all being grade Ⅰ; the breast in one child had not yet developed, and these indicators were not evaluated. The locations of nipple areola complex in 8 children were almost the same as or similar to those in the healthy side, and their skin color, integrity, texture, and elasticity of the partial breast repaired by the transferred flap were similar to those in the healthy side, with the aesthetic effect of all being grade Ⅰ. The shapes of nipple and areola in 5 children were inconsistent with those in the healthy side because of the original scar, with the aesthetic effect of all being grade Ⅱ, and the shapes of nipple and areola in the other 3 children were consistent with those in the healthy side, with the aesthetic effect of all being grade Ⅰ. The VSS score of the scar at the flap recipient area was 2-5, and the VSS score of the scar at the flap donor area was 1-3. Seven children's families were satisfied with the surgical effect, and one child's family was basically satisfied with the surgical effect.Conclusions:For the breast scar contracture deformity of minor females after burns, the expanded lateral thoracic artery perforator flap is used for reconstruction before puberty, which results in fewer postoperative complications, good breast shape, and hidden scar at the flap donor area. It is beneficial for the normal development of adolescent breasts, and is one of the safe and effective methods for the treatment of breast scar contracture deformity in minor females after burns.

3.HIV-1 subtypes and drug-resistance mutation sites in HIV/AIDS patients with antiretroviral-therapy failure

ObjectiveTo analyze the characteristics of HIV-1 subtypes and drug-resistance mutation sites among HIV-infected patients who received high-efficiency antiretroviral therapy but failed. MethodsA total of 130 plasma samples were collected from the patients who received antiviral treatment for 6 months in Taizhou City of Zhejiang Province in 2021 but failed the treatment and the viral load was ≥1 000 copies·mL-1. Nucleic acid in the samples was extracted, and the pol gene was amplified by nested reverse transcription PCR. After next-generation sequencing, online tools were used to compare and analyze the subtypes and drug-resistant mutation sites. ResultsA total of 110 samples were successfully sequenced. The main HIV-1 subtype was CRF01_AE, accounting for 42.72% (47 cases), followed by CRF07_BC, 35.45% (39 cases); CRF08_BC, 10.00% (11 cases); CRF85_BC , 8.18% (9); and a small number of B subtype, 1.81% (2 cases) and C subtype, 1.81% (2 cases). The online tool comparison showed that there were 67 cases with mutations of drug-resistance sites and 61 cases with drug-resistance. The mutation sites were mainly M184V, K103N, K65R and V181C, and the mutation rates were 20.00% (22 cases), 10.91% (12 cases), 8.18% (9 cases) and 8.18% (9 cases), respectively. These mutation sites caused different degrees of resistance to nucleoside reverse transcriptase inhibitors (NRTI), non- nucleoside reverse transcriptase inhibitors (NNRTI) and protease inhibitors (PI), including 45 cases of NRTI, 61 cases of NNRTI and 2 cases of PI resistance. ConclusionThe HIV infected people who fail the treatment in Taizhou are mainly with the subtypes CRF01_AE and CRF07_BC. The rate of drug-resistance mutation is at a moderate level, mainly due to the mutation of NRTI and NNRTI drug-resistance sites, and a small number of PI drug-resistance sites. Therefore, the antiviral treatment plan for HIV infected people should be reasonably adjusted, and the detection of drug-resistance mutation sites should be strengthened to avoid the generation of transmissible drug-resistance strains.
4.Etiological analysis of acute respiratory tract infection in Taizhou of Zhejiang Province from 2020 to 2021

ObjectiveWe examined the principal respiratory pathogens in patients with acute respiratory tract infection in Taizhou, Zhejiang Province during 2020‒2021 to provide evidence for prevention, diagnosis and treatment of acute respiratory tract infection. MethodsFrom September 2020 to August 2021, a total of 2 831 cases with acute respiratory tract infection were collected from two influenza sentinel surveillance hospitals in Taizhou, which had then received the examination of 22 respiratory pathogens by multiple fluorescence quantitative PCR. ResultsThe total positive rate of respiratory pathogens in 2 831 samples was 14.13%, among which enterovirus (7.77%) and respiratory syncytial virus (1.59%) were the principal pathogens. Except enterovirus, there was no significant difference in the positive rate of pathogens detected by gender(P>0.05). Moreover, there was significant difference in pathogens by age (P<0.05), with the highest positive rate in 0‒4 years(35.21%). There was also significant difference in pathogens by seasons (P<0.05), with the highest positive rate in summer(20.54%). ConclusionThe positive rate of acute respiratory tract infection decreases significantly, compared with that before the COVID-19 epidemic. The differences in the positive rate differ significantly by age and seasons. Comprehensive consideration of diverse factors before diagnosis and the utilization of multiple fluorescent quantitative PCR can quickly and effectively determine the pathogens in the early stage of infection. Our findings may provide certain support for the diagnosis and treatment of acute respiratory infections in the context of COVID-19 in Taizhou.
5.Application of gene screening technology in screening common newborn genetic diseases

Objective:To detect the genes of common genetic diseases in newborns with the high-throughput sequencing technology based on target gene capture, to study the incidence rate of such diseases, the carrying rate and variant types of pathogenic mutations related to such diseases, and to explore the application value of the high-throughput sequencing technology in screening genetic diseases of newborns.Methods:The heel blood of 1 793 newborns born in Guangdong province from June 2019 to April 2020 were collected, and the exon regions of 138 common genetic disease-related genes in neonates were detected using the high-throughput sequencing technology based on target gene capture.The pathogenicity of the mutations was interpreted according to the " Classification Criteria and Guidelines for Genetic Variation(2017)" , in which known disease and probable disease were considered as positive mutations.The positive mutations were verified by Sanger sequencing technology, and the test results were analyzed with statistical methods.Results:Among the 1 793 newborns, 978 were male and 815 were female.A total of 158 positive cases were screened(8.81%), and 11 positive diseases were detected.Among the positive diseases, there were 41 cases(2.29%)of autosomal recessive deafness type 1A, 40 cases(2.23%)of Gilbert syndrome or Crigler-Najjar syndrome, and 33 cases(1.84%)of glucose-6-phosphate dehydrogenase deficiency(1.84%), 19 cases(1.06%)of familial hypercho-lesterolemia, 18 cases(1.00%) of sodium taurocholate cotransporter peptide deficiency disease, 2 cases(0.11%)of mitochondrial non-syndromic deafness, 2 cases(0.11%)of Citrin deficiency, 1 case(0.06%)of holocarboxylase synthase deficiency, 1 case(0.06%)of β-thalassemia and 1 case(0.06%)of metachromatic leukodystrophies.Of all studied cases, 972 carried one or more positive mutations, involving 85 kinds of diseases in total.The diseases with a high carrying rate were Gilbert syndrome or Crigler-Najjar syndrome(359 cases, 20.02%), autosomal recessive deafness type 1A(302 cases, 16.84%), and sodium taurocholate cotransport peptide deficiency disease(291 cases, 16.22%). The high-frequency mutation sites were UGT1A1 gene c. 211G> A, GJB2 gene c .109G> A and SLC10A1 gene c. 800C> T. Conclusions:The common genetic diseases detected in neonates from Guangdong province are autosomal recessive deafness type 1A, Gilbert syndrome or Crigler-Najjar syndrome, glucose-6-phosphate dehydrogenase deficiency, familial hypercholesterolemia, and sodium taurocholate cotransport peptide deficiency.There are high-frequency carrying mutation sites in the population.Preliminary genetic screening of common neonatal genetic diseases can accumulate data and experience for the development of newborn genetic screening.
6.Effects of Lentinan on Promotion Heart Function of Carvedilol in Dilated Cardiomyopathy Model in Rats

Objective To study the mechanism of lentinum's promotion on carvedilol's effects on dilated cardiomyopathy's heart function.Methods SD rat model of dilated cardiomyopathy was established with doxorubicin hydrochloride.The rat model was divided into blank control group,carvedilol group,lentinan group,carvedilol + lentinan group.Rats in the blank control group were given 2 mL of 0.9% sodium chloride solution.Rats in the carvedilol group were given 10 mg·kg-1of carvedilol and 2 mL of 0.9% sodium chloride solution.Rats in the lentinan group were given 1 mg·kg-1 of lentinan,and 2 mL of 0.9% sodium chloride solution.Rats in the carvedilol + lentinan group were given10 mg· kg-1 of carvedilol,1 rmg·kg-1 of lentinan,and 2 mL of 0.9% sodium chloride solution.The heart function,irrflammatory factors and T lymphocytes were compared.Results The left ventricular ejection fraction (LVEF) in carvedilol + lentinan group and carvedilol group were significantly higher than that in blank control group and lentinan group (P < 0.05),while Tei index was significantly lower (P < 0.05).The LVEF in carvedilol + lentinan group was significantly higher than that in carvedilol group (P <0.05),while Tei index was significantly lower (P <0.05).The hs-CRP,TNF-α,II-6 and IL-8 in carvedilol + lentinan group and lentinan group were significantly lower than that in blank control group and carvedilol group (P <0.05),while IL-10,CD3+,CD4+ and CD8+ were significantly higher (P < 0.05).The hs-CRP,TNF-α,II-6 and IL-8 in C + L group were significantly lower than in lentinan group (P < 0.05),while IL-10,CD3+,CD4+ and CD8+ were significantly higher (P < 0.05).Conclusion Lentinan could significantly promote carvedilol's effects on dilated cardiomyopathy's heart function through lowering down inflammatory factors and increasing T lymphocytes.
