Objective:To evaluate the clinical effect of the position pillows for neuraxial anesthesia. Methods:This was a prospective randomized controlled trial. Four hundred and twelve patients regardless of gender, aged ≥18 yr, of American Society of Anesthesiologists Physical Status classification Ⅰ-Ⅲ, who underwent elective surgery under neuraxial anesthesia at Peking University Third Hospital from February to October 2023, were selected and divided into 2 groups ( n=206 each) using a random number table method: pillow group (P group) and control group (C group). Group C underwent the conventional procedure for neuraxial anesthesia. The patients were placed in a position using the position pillow on the basis of oral education before routine anesthesia in group P. The success rate of puncture at first attempt, puncture time and position placement time were recorded. The adjustment of position, body movement and occurrence of discomfort during the puncture were also recorded. The visual analogue scale score was used to evaluate the level of anxiety before positioning, after positioning and after anesthesia. The visual analogue scale score was used to evaluate the patient′s comfort and the operator′s satisfaction with position after the anesthesia was completed. Results:Compared with group C, the time for positioning was significantly shortened, the anxiety level was decreased after positioning and after anesthesia, the rate of improvement in anxiety was increased, the scores for the patient′s comfort and the operator′s satisfaction with position were increased ( P<0.05), and no significant changes were found in the success rate of puncture at first attempt, puncture time and incidence of body movement during the puncture and incidence of the adjustment of position ( P>0.05). No discomfort was observed in either group during the puncture. Conclusions:This new type of position pillows for the neuraxial anesthesia can not only optimize the effect of position placement, but also improve the patients′ comfort.

Objective:To evaluate the efficacy and safety of triamcinolone acetonide-saline submucosal injection for prevention of esophageal stricture after endoscopic submucosal dissection (ESD) for extensive superficial esophageal neoplasms. Methods:A total of 75 patients who underwent ESD for superficial esophageal neoplasms involving larger than 2/3 of the esophageal circumference at the Department of Gastroenterology, Northern Jiangsu People's Hospital from January 2018 to December 2020 were enrolled. Patients were randomly assigned to triamcinolone acetonide-saline submucosal injection group (group A, n=25), triamcinolone acetonide injections immediately after ESD group (group B, n=25) and the control group undergoing only ESD (group C, n=25). Serial gastroscopy was performed to assess wound healing and esophageal stricture. Endoscopic balloon dilatation (EBD) was performed when patients experienced esophageal stricture. The completion of ESD, time of operation, the amount of triamcinolone acetonide, the incidences of esophageal stricture and the time of EBD treatment of the three groups were compared. Results:All ESD procedures were successfully performed without complications such as intraoperative perforation, massive bleeding or postoperative delayed perforation. The operation time of group A, B and C were 72.87±12.99 min, 94.15±14.22 min and 74.08±11.86 min, respectively, with significant difference ( F=20.925, P<0.001). In pairwise comparison the above indicator in group A and group C was significantly shorter than that of group B (LSD- t=5.759, P<0.001; LSD- t=5.432, P<0.001), but there was no difference between group A and group C (LSD- t=0.327, P=0.745). There was no significant difference in the amount of triamcinolone acetonide between group A and group B (125±15 mg VS 133±19 mg, t=1.673, P=0.101). The rates of wound healing under endoscopy after 1 month of group A, B and C were 76% (19/25), 84% (21/25), and 76% (19/25), respectively, with no significant difference ( χ2=0.636, P=0.728). The esophageal stricture rates were 52% (13/25) in both group A and B, and 84% (21/25) in group C, with significant difference among the three groups ( χ2=7.295, P=0.026), and group A and B showed a significantly lower stricture rate than that of group C ( P=0.015; P=0.015). The median time of EBD treatment of group A, B and C were 4 (range 0 to 9), 5 (range 0 to 13) and 9 (range 0 to 16), respectively, with significant difference ( H=17.58, P<0.001). In pairwise comparison the above indicator in group A and B was significantly less than that of group C ( H=23.96, P<0.001; H=19.00, P=0.002), but there was no significant difference between group A and group B ( H=4.96, P=0.407). Esophageal stricture was observed in all patients with circumferential mucosa resected in the three groups. But the times of EBD treatment were 6.90±1.10 in group A, 10.13±2.42 in group B and 15.29±0.76 in group C with significant difference ( F=57.754, P<0.001). In pairwise comparison the above indicator in group A was less than that in group B (LSD- t=4.294, P<0.001) and this indicator in group B was less than that in group C (LSD- t=6.294, P<0.001). Median EBD times in patients with non-circumferential mucosal defects in the three groups were 0 (range 0 to 9), 0 (range 0 to 6) and 8 (range 0 to10), respectively, with significant difference ( H=19.72, P<0.001). In pairwise comparison, the EBD time in group A and B was less than that in group C ( H=17.93, P<0.001; H=16.62, P<0.001), but there was no statistical difference between group A and B ( H=1.31, P=0.779). Conclusion:Triamcinolone acetonide-saline submucosal injection ESD can safely and effectively prevent esophageal stricture after ESD for large-area superficial esophageal neoplasms, reduce the operation time and time of EBD treatment in patients with circumferential mucosa defect compared with local injections of triamcinolone acetonide after ESD.

ObjectiveTo investigate the therapeutic effect of Dayuanyin on acute lung injury induced by H1N1 infection and decipher the potential mechanism. MethodThe constituents in Dayuanyin were analyzed by ultra-high performance liquid chromatography-quadrupole-exactive orbitrap mass spectrometry (UHPLC-Q-Exactive Orbitrap MS). Forty-eight female BALB/c mice were randomized into normal, model, oseltamivir (19.5 mg·kg-1), and low-, medium-, and high-dose (2.73, 5.46, 10.92 g·kg-1) Dayuanyin groups. The normal and model groups were administrated with deionized water by gavage, and the other groups were administrated with the corresponding drugs by gavage. On day 3 of drug administration, the normal group received nasal inhalation of normal saline, and the other groups were inoculated intranasally with A/RP/8/34 (H1N1) for the modeling of influenza virus infection. Mice were administrated with drugs continuously for 7 days and weighed daily. Sampling was performed 12 h after the last administration, and the lung tissue was weighed to calculate the lung index. Hematoxylin-eosin staining was performed to observe the pathological and morphological changes of the lung tissue and bronchi. The cytometric bead array (CBA) was used to measure the serum levels of interferon-gamma (IFN-γ), C-X-C motif ligand 1 (CXCL1), tumor necrosis factor-alpha (TNF-α), chemokine ligand 2 (CCL2), interleukin-12p70 (IL-12p70), chemokine ligand 5 (CCL5), interleukin-1β (IL-1β), chemokine (C-X-C motif) ligand 10 (CXCL10), granulocyte-macrophage colony-stimulating factor (GM-CSF), interleukin-10 (IL-10), interferon-beta (IFN-β), interferon-alpha (IFN-α), and interleukin-6 (IL-6). According to the results of mass spectrometry and network pharmacology, we analyzed the mechanism of Dayuanyin in treating acute lung injury caused by H1N1. The protein levels of extracellular signal-regulated kinase 1/2 (ERK1/2), p38 mitogen-activated protein kinase (p38 MAPK), nuclear factor-kappa B (NF-κB), and their phosphorylated forms were determined by Western blot. The mRNA levels of myeloid differentiation factor 88 (MyD88), Toll-like receptor 3 (TLR3), Toll-like receptor 7 (TLR7), and Toll-like receptor 8 (TLR8) in the lung tissue were measured by Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR). ResultA total of 57 compounds, including paeoniflorin and baicalein, were detected in Dayuanyin. Compared with the normal group, the model group showed decreased body weight (P<0.01), lung edema and hemorrhage, increased lung index (P<0.01), and elevated levels of IFN-γ, IL-12p70, CCL5, IL-1β, CXCL10, GM-CSF, IFN-β, and IL-6 (P<0.01). Compared with the model group, Dayuanyin attenuated alveolar wall thickening, capillary congestion, and immune cell infiltration, reduced the alterations in body weight and lung index (P<0.01), and down-regulated the protein levels of IFN-γ, IL-12p70, CCL5, IL-1β, CXCL10, GM-CSF, IFN-β, and IL-6 (P<0.01). A total of 57 key genes were predicted by network pharmacological analysis, of which the MAPK signaling pathway was the main target signaling pathway. Compared with the normal group, the model group showed up-regulation in the protein levels of phosphorylation (p)-ERK1/2, p-p38 MAPK, and p-NF-κB (P<0.01) and the mRNA levels of TLR7, TLR8, MyD88, and TLR3 (P<0.05, P<0.01). Compared with the model group, Dayuanyin lowered the phosphorylation levels of ERK1/2, p38 MAPK, and NF-κB p65 in a dose-dependent manner (P<0.01) and down-regulated the mRNA levels of TLR3, TLR7, TLR8, and MyD88 (P<0.01). ConclusionDayuanyin can prevent and control H1N1 infection-induced acute lung injury by inhibiting the TLR/MAPK/NF-κB signaling pathway.
