1.Clinical Observation on Prevention of Recurrence of Common Bile Duct Stones After ERCP with Yuyin Lidan Granules

ObjectiveTo observe the clinical efficacy and safety of Yuyin Lidan granules (YYLD) in preventing the recurrence of common bile duct stones (CBDS) in patients with liver and gallbladder dampness-heat syndrome following endoscopic retrograde cholangiopancreatography (ERCP). MethodsThis randomized, parallel, controlled trial enrolled postoperative CBDS-ERCP patients who met the inclusion and exclusion criteria. Sixty-four patients were randomly assigned to an observation group or a control group, with 32 cases in each. Both groups received conventional Western medical treatment after ERCP, while the observation group additionally received YYLD for 8 weeks. The follow-up period lasted for 1 year. The efficacy indicators included bile bilirubin levels, traditional Chinese medicine (TCM) syndrome scores, clinical efficacy rate, pancreatitis and inflammation markers, postoperative liver function, and CBDS recurrence rate at 1-year follow-up, which were used to jointly evaluate the clinical efficacy and safety of both groups. ResultsA total of 56 patients completed the study and were included in the final analysis, i.e., 29 in the observation group and 27 in the control group. Baseline characteristics were comparable between the two groups. Compared with pre-treatment and with the control group after treatment, the bile bilirubin level in the observation group significantly decreased (P<0.05). After treatment, the clinical cure and marked improvement rates were higher in the observation group than in the control group, showing a statistically significant difference in overall clinical efficacy (P<0.05). Compared with pre-treatment, the primary and secondary symptoms in the observation group, as well as the primary symptom and the secondary symptom of nausea and vomiting in the control group (weeks 4 and 8), were significantly reduced (P<0.05). Compared with the control group after treatment, the observation group showed significant reductions in the primary symptom of loose stools/constipation (day 5 and week 4) and in three secondary symptoms, i.e., bitter taste and sticky dry mouth, abdominal distension and poor appetite (throughout the treatment period), and general heaviness and fatigue (day 5 and week 4), with statistical differences (P<0.05). Compared with pre-treatment, both groups showed decreased lipase and urinary amylase levels (P<0.05). However, no significant between-group differences were observed in pancreatitis or inflammation-related indices after treatment. Compared with pre-treatment, all liver function indicators in the observation group and alanine aminotransferase ( ALT ), γ-glutamyl transferase ( γ-GT ), alkaline phosphatase (ALP), and conjugated bilirubin in the control group significantly decreased at weeks 4 and 8 (P<0.05). Compared with the control group after treatment, only serum total bilirubin and unconjugated bilirubin were significantly reduced in the observation group during the treatment period (P<0.05). ConclusionYYLD combined with conventional Western medical treatment can effectively regulate bilirubin metabolism (in bile and serum), improve TCM clinical symptoms, and prevent CBDS recurrence after ERCP in patients with liver and gallbladder dampness-heat syndrome. This regimen is safe and effective and is worthy of further clinical research and promotion.
2.Clinical Experience of Professor TU Jinwen in Staged Differentiating and Treating Cancer-Related Insomnia Based on the Qi and Fire

This paper summarizes professor TU Jinwen's clinical experience in treating cancer-related insomnia (CRI) based on different qi and fire. It is believed that the pathogenesis of CRI can be divided into three stages. At the initial stage, qi movement is constrained, while the strong fire begins to stir, and the sovereign fire is unsettled, when qi is abundant, but the fire is not excessive. For this, Sanhua Jieyu Anshen Decoction (三花解郁安神汤) is suggested, which can move qi and vent constraint, clear and diffuse strong fire, calm the heart and spirit. At the progressive stage, strong fire becomes intense and burning, and qi transformation weakens, with toxin fire harassing the spirit. This is the stage where both qi and fire are excessive, for which Huanglian Jiedu Anshen Decoction (黄连解毒安神汤) can be used to clear and dissipate strong fire, drain fire and resolve toxin, clear heart and calm spirit. At the terminal stage, strong fire subsides, and consumption of qi damages healthy qi, with failure of nourishment of heart spirit, when both qi and fire deplete. Correspondingly, Erren Yangxin Anshen Decoction (二仁养心安神汤) is used to boost qi and nourish yin, restore interaction between the heart and the kidney, nourish the heart and calm spirit.
3.Development of a new paradigm for precision diagnosis and treatment in traditional Chinese medicine

The development of traditional Chinese medicine (TCM) diagnosis and treatment has undergone multiple paradigms, evolving from sporadic experiential practices to systematic approaches in syndrome differentiation and treatment and further integration of disease and syndrome frameworks. TCM is a vital component of the medical system, valued alongside Western medicine. Treatment based on syndrome differentiation embodies both personalized treatment and holistic approaches; however, the inconsistency and lack of stability in syndrome differentiation limit clinical efficacy. The existing integration of diseases and syndromes primarily relies on patchwork and embedded systems, where the full advantages of synergy between Chinese and Western medicine are not fully realized. Recently, driven by the development of diagnosis and treatment concepts and advances in analytical technology, Western medicine has been rapidly transforming from a traditional biological model to a precision medicine model. TCM faces a similar need to progress beyond traditional syndrome differentiation and disease-syndrome integration toward a more precise diagnosis and treatment paradigm. Unlike the micro-level precision trend of Western medicine, precision diagnosis and treatment in TCM is primarily reflected in data-driven applications that incorporate information at various levels, including precise syndrome differentiation, medication, disease management, and efficacy evaluation. The current priority is to accelerate the development of TCM precision diagnosis and treatment technology platforms and advance discipline construction in this area.
4.Sounding the call for functional cure of hepatitis B and opening a new era of combination therapy

Functional cure (i.e., clinical cure) of chronic hepatitis B (CHB) is a core research objective in the field of hepatology. With the revision of the Expert opinions on the technical guiding Principles for clinical trials of drugs in the treatment of chronic hepatitis B virus infection, this article concisely reviews the evolving concept of functional cure for hepatitis B, research progress on the dynamic trajectories and threshold of key serological markers (such as HBsAg/anti-HBs), strategies for safety evaluation of treatment discontinuation, and the importance of building a collaborative innovation ecosystem involving scientific research, industry, healthcare, policy, and societal forces. Through multidimensional technological breakthroughs and interdisciplinary collaboration, the functional cure rate of chronic hepatitis B infection is expected to improve significantly over the next 5-10 years, laying the foundation for achieving the World Health Organization (WHO)'s goal of eliminating viral hepatitis as a public health threat by 2030.
5.Clinical utility of a robotic intelligent endoscope transportation system in the digestive endoscopy center

Objective:To evaluate the clinical utility of an intelligent endoscope transportation system in the digestive endoscopy center.Methods:A parallel-group controlled trial was conducted at Digestive Endoscopy Center of Qilu Hospital of Shandong University from June 1st to December 31st 2024, comparing robotic intelligent endoscope transport (experimental group) versus manual transport (control group). Performance metrics, including response time, transportation speed, labor efficiency, contamination prevention, closed-loop traceability, and nursing staff satisfaction, were statistically analyzed. Full-time equivalent (FTE) was introduced to quantify the operational efficiency of the experimental group.Results:The study included a total of 60 206 instances of intelligent endoscope transportation and 60 485 instances of manual transportation data. The robotic group demonstrated significantly shorter response times versus manual group for initial dispatch (51.08±14.97 seconds VS 54.44±13.61 seconds, t=35.8, P<0.001) and recovery response time (32.52±11.26 seconds VS 40.20±11.40 seconds, t=103.93, P<0.001). During the 148 days operational period, the success rate was 99.83% (60 104/60 206) and the failure rate was 0.17% (102/60 206) for robotic transports. Primary failure causes were wireless disconnection, pathfinding errors, and mechanical faults, averaging 1.05 malfunctions/month with no adverse events. The success and failure rate was 99.26% (60 043/60 485) and 0.74% (442/60 485) respectively for manual transports. Staff satisfaction was significantly higher for robotic transport in endoscopic transportation (4.65±0.55 scores VS 3.97±0.98 scores, t=96.5, P<0.001) and delivery process (4.71±0.59 scores VS 3.90±1.04 scores, t=210.3, P<0.001). and workload intensity was significantly lower (4.06±0.77 scores VS 4.48±0.63 scores, t=59.9, P=0.025). The system reduced labor requirements by 3.68 FTE, yielding annual savings of ¥657 000. Conclusion:The robotic intelligent endoscope transport system improves work efficiency, reduces nursing labor costs and physical workload, enhances job experience and satisfaction, and enables full-process smart traceability, providing a validated solution for endoscopy center logistics.
6.Investigation on the Oligomeric Status and Thermal Stability Properties of Pathological Mutations of KDSR in Progressive Symmetrical Erythematokeratosis

Progressive symmetric erythrokeratodermia(PSEK)is a rare hereditary skin disease charac-terized by symmetrical erythema,hyperkeratosis and multiorgan lesions.Its clinical phenotypes are highly heterogeneous and may be accompanied by symptoms such as thrombocytopenia,which can be fatal in se-vere cases.The genotype-phenotype association mechanism of PSEK is extremely complex.Currently,it is known that mutations in multiple genes such as GJB3,KDSR,and KRT83 can cause the disease.A-mong them,3-ketodihydrosphingosine reductase(KDSR)has been found to harbor nearly 20 clinical mu-tations.These mutations interfere with the de novo ceramide synthesis pathway,disrupt the homeostasis of the skin barrier,and cause platelet production disorders and multi-organ lesions,making it a current research hotspot in the molecular mechanism of PSEK.The pathogenic mutations of KDSR are widely and uniformly distributed throughout the entire protein,rather than being limited to the traditionally recog-nized active center,suggesting that the impairment of the KDSR enzymatic activity is not the only cause of PSEK.In view of this,this study selected four typical mutants of KDSR(KDSRQG55-56R,KDSRn38C,KDSRY186F,KDSRG182S),and first used recombinant expression technology to prepare pure and homoge-neous mutant proteins.Subsequently,thermal stability experiments as well as oligomerization analysis were conducted on these four mutant proteins.The results showed that the Tm values of the four mutants were significantly lower than that of the wild type.Particularly,KDSRF138C and KDSRQG55-56R were nearly completely denatured at physiological temperature.This result was perfectly consistent with the further Rosetta energy analysis.In conclusion,this study took several pathological mutations of the PSEK patho-genic factor KDSR as the research object and discovered that the conformational stability of KDSR might be closely related to the occurrence of PSEK pathogenicity,indicating that the imbalance of conformation-al homeostasis is very likely to be one of the common contributing factors of many genetic diseases,inclu-ding PSEK.This provides a new theoretical basis and reference for explaining the molecular mechanism of genotype-phenotype heterogeneity in many genetic diseases.
7.Influencing factors and interaction mechanisms of information quality in medical record home page: a grounded theory study

Objective:To investigate the influencing factors of the quality of inpatient medical record home page information in public hospitals under the background of the reform of diagnosis related groups(DRG)-based payment system and their mechanisms of action, so as to provide references for continuous improvement of the quality of medical record home page information.Methods:From July to August 2024, semi-structured interviews were conducted with clinical physicians and personnel engaged in the filling, quality control, review, and management of inpatient medical record home pages in public hospitals in Hangzhou, using purposeful sampling. The interview data were qualitatively analyzed based on the grounded theory to identify the influencing factors of the quality of medical record home page information and construct a theoretical model of the mechanisms of action of these factors.Results:Through open coding of the 17 interview materials, 53 initial concepts and 13 categories were identified. Further induction through axial coding led to the extraction of four main categories: external environment, organizational resources, person-job fit, and work ecology. The theoretical model of the mechanisms of action of the influencing factors on the quality of medical record home page information was as follows: external environmental factors such as policies and regulations played a macroscopic guiding role in the quality of medical record home pages; organizational resource factors such as the hospital′s information construction played a mesoscopic regulating role in the quality of medical record home pages; person-job fit factors such as the professional skills of clinical physicians and medical record-related staff played a direct driving role in the quality of medical record home pages; and work ecology factors such as the workload of clinical physicians and medical record-related staff played an external modulating role in the quality of medical record home pages.Conclusions:Under the background of DRG reform, the quality of inpatient medical record home page information was influenced by multiple factors. To systematically improve the quality of medical record home page information, it is necessary for multiple parties to work together. Relevant government departments should improve the top-level design and optimize the external environment, guide and assist medical institutions in building quality control systems, and strengthen resource allocation. Medical institutions should focus on enhancing the job competence and person-job fit of relevant personnel and also use intelligent means to optimize work processes to reduce workload.
8.A cisplatin prodrug-based self-assembling ozone delivery nanosystem sensitizes radiotherapy in triple-negative breast cancer.

Lacking therapeutic targets highlights the crucial roles of chemotherapy and radiotherapy in the clinical management of triple-negative breast cancer (TNBC). To relieve the side effects of the chemoradiotherapy combination regimen, we design and develop a self-assembled micelle nanosystem consisting of perfluorocarbon chain-modified cisplatin prodrug. By incorporating perfluorodecalin, this nanosystem can effectively carry ozone and promote irradiation-derived reactive oxygen species (ROS) production. By leveraging the perfluorocarbon sidechain, the nanosystem exhibits efficient internalization by TNBC cells and effectively escapes from lysosomal entrapment. Under X-ray irradiation, ozone-generated ROS disrupts the intracellular redox balance, thereby facilitating the release of cisplatin in a reduction-responsive manner mediated by reduced glutathione. Moreover, oxygen derived from ozone decomposition enhances the efficacy of radiotherapy by alleviating tumor hypoxia. Notably, the combination of irradiation with ozone-loaded cisplatin prodrug nano system synergistically prompts antitumor efficacy and reduces cellular/systemic toxicity in vitro and in vivo. Furthermore, the combo regimen remodels the tumor microenvironment into an immune-favored state by triggering immunogenic cell death and relieving hypoxia, which provides a promising foundation for a combination regimen of immunotherapy. In conclusion, our nanosystem presents a novel strategy for integrating chemotherapy and radiotherapy to optimize the efficacy and safety of TNBC clinical treatment.

10.Research Progress in the Relationship Between Hyperhomocysteinemia and Chronic Kidney Disease.

In patients with chronic kidney disease(CKD),the incidence of cardiovascular events and the mortality rate are significantly higher than those in the general population.Approximately 85% of end-stage CKD patients develop hyperhomocysteinemia(HHcy)due to impaired renal function.It is not yet fully determined whether HHcy merely acts as a biomarker of CKD or plays a role in the pathogenesis.Although supplementation with folic acid and other B vitamins(B6 and B12)has been proven to lower the plasma level of total homocysteine,their effectiveness in slowing CKD progression or reducing cardiovascular complications remains uncertain.This article reviews the latest research progress in the relationship between HHcy and CKD,aiming to give new insights into the prevention and treatment of CKD and its complications.
