The intestinal mucosal barrier is closely associated with the occurrence and development of type 2 diabetes mellitus(T2DM).It plays an important role in maintaining the homeostasis of the internal environment and resisting pathogenic invasion by regulating the metabolic homeostasis,immune response,and microbial-host interactions in the human body.According to recent studies,intestinal mucosal barrier dysregulation may constitute a key link in the pathogenesis of T2DM.Herein,we systematically reviewed the mechanisms by which intestinal mucosal barrier damage contributes to the onset of T2DM and the latest advances in modern medical intervention strategies.We also discussed and summarized the latest research findings on using traditional Chinese medicine(TCM)to regulate the intestinal mucosal barrier in the management of T2DM.Currently,the causal relationship and mechanisms linking intestinal mucosal barrier damage to the onset of T2DM remain unclear,and systematic research on the synergistic regulatory mechanisms of approaches integrating TCM and Western medicine is lacking.The approach to manage T2DM through the integration of TCM and Western medicine can provide insights and basis from multiple aspects,such as TCM syndrome differentiation and treatment,balancing drug properties,and multi-targeted therapy.In the future,we should fully leverage the advantages of integrated TCM and Western medicine in treating T2DM and conduct in-depth research focused on the intestinal mucosal barrier,thereby informing evidence-based clinical applications.

Background Hypertension is one of the chronic diseases with the highest prevalence in China, and a history of hypertension may potentially exacerbate hearing loss. Investigating the association between long-term blood pressure trends and hearing thresholds could contribute to hearing protection efforts for occupationally noise-exposed populations. Objective By investigating hearing thresholds and blood pressure levels among occupationally noise-exposed workers in an urban rail transit enterprise, and conducting a comprehensive analysis of the association between long-term blood pressure changes and hearing thresholds, to provide data references for health management strategies targeting occupationally noise-exposed workers. Methods Workers exposed to occupational noise at a rail transit enterprise were enrolled as study subjects and underwent pure-tone audiometry. Group-based trajectory modeling was employed to identify blood pressure trajectories. Categorical data were compared using chi-square tests, while normally distributed continuous variables were analyzed via t-tests and analysis of variance (ANOVA). Generalized linear mixed models (GLMMs) were subsequently applied toexamine associations between these trajectory groups and high-frequency hearing thresholds. Results Among 2 002 occupationally noise-exposed workers, the median (P₂₅, P₇₅) age was 32 (28, 35) years, with a median (P₂₅, P₇₅) working tenure of 7 (3, 10) years. In 2019, the positive hypertension rate was 9.04%, with a mean systolic blood pressure (SBP) of (122.97±11.60) mmHg and a mean diastolic blood pressure (DBP) of (76.37±9.02) mmHg. The hearing loss prevalence was 10.1%, showing bilateral high-frequency average hearing thresholds of (17.18±8.71) dB and speech-frequency average thresholds of (13.79±3.46) dB. Three distinct trajectory groups were identified for both SBP and DBP. Compared with other trajectory groups, the high-stable DBP group exhibited significantly higher hearing loss prevalence (χ²=6.34, P=0.042) and elevated high-frequency hearing thresholds (all Ps<0.05). Specifically, within the 30-39 age subgroup, the moderate-stable DBP group demonstrated 1.96 dB lower high-frequency thresholds than the high-stable group [β(95%CI): −1.96 (−3.61, −0.32), P=0.020]. Conclusion Among occupationally noise-exposed workers in a municipal rail transit enterprise, DBP trajectories demonstrated a positive association with high-frequency hearing thresholds. Notably, in young and middle-aged occupationally noise-exposed populations, DBP may exert a more critical influence than SBP on the progression of hearing loss.

Objective To evaluate the effects of fasting and high-fat diet on the pharmacokinetics of rabeprazole sodium enteric-coated tablets in healthy Chinese subjects.Methods A single-center,randomized,open,two-agent,two-sequence,four-cycle,fully repeated crossover,single-dose trial design was used in this study,healthy subjects were assigned to receive single dose of rabeprazole sodium enteric-coated tablets 0.1 g in either fasting or high-fat diet state,and blood samples were taken at different time points,respectively.The concentrations of rabeprazole sodium enteric-coated in plasma were determined by liquid chromatography-tandem mass spectrometry(LC-MS/MS),the model method of the non-compartmental was used to calculate the pharmacokinetic parameters by Phoenix WinNonlin 8.2.Results The main pharmacokinetic parameters of rabeprazole sodium enteric-coated tablets in fasting state and high-fat diet state were as follows:Cmax were(339.63±156.47)and(318.86±132.13)ng·mL-1;t1/2 were(2.34±0.68)and(3.60±2.40)h;AUC0_t were(556.62±251.65)and(528.50±201.78)ng·mL-1·h;AUC0-∞ were(563.39±255.69)and(535.15±203.24)ng·mL-1·h;tmax were 3.65 and 6.99 h.After high-fat diet,the Cmax and AUC of rapeprazole sodium after high-fat and high-calorie diet decreased,Cmax decreased by 6.12％,AUC0-t decreased by 5.05％,AUC0-∞ decreased by 5.01％,andtmaxwas delayed by about 3.34 h.Cmax,AUC0-t and AUC0-∞ 90％confidence interval were 73.13％-115.10％,83.22％-112.28％and 83.40％-112.13％,respectively.Neither was between 85.00％-125.00％.Conclusion High-fat diet affects the absorption rate and degree of rabeprazole sodium enteric-coated,so it is suitable to be administered on an empty stomach.

Pathological cardiac hypertrophy is an early and significant cardiac structural charac-teristic that contributes to the onset and progression of heart failure(HF).Its mainly structural feature is the abnormally enlarged cardiomyocyte.Effective intervention targets for abnormally en-larged cardiomyocyte remain to be identified.Previous studies have shown that the cellular shape and size can be regulated by the actin related protein 2/3(Arp2/3)complex,which is an actin-binding protein complex involved in the actin nucleation and assembly.However,the roles of the Arp2/3 complex in cardiomyocyte hypertrophy remain unknown.Here our study identifies its no-vel roles in the occurrence and development of cardiomyocyte hypertrophy.We found that mRNA levels of all subunits from the Arp2/3 complex are significantly upregulated(P<0.05)in the an-giotensin II(Ang Ⅱ)-induced neonatal rat primary and H9c2 cardiomyocyte hypertrophy.Fur-ther studies showed that siRNA-directed ARPC2 silencing inhibits the reactivation of fetal genes and enlargement of cardiomyocyte area induced by Ang Ⅱ in neonatal rat primary cardiomyocytes(NRCMs)and H9c2 cells(P<0.05).In addition,the upstream activators of the Arp2/3 com-plex including SH3 protein interacting with Nck,90 kD(SPIN90)and Ras-related C3 botulinum toxin substrate 1(Rac1)/WASp family Verprolin-homologous protein-2(WAVE-2)are upregu-lated(P<0.05)in Ang Ⅱ-induced neonatal rat primary and H9c2 cardiomyocyte hypertrophy,indicating the excessive activation of the Arp2/3 complex.We further show that CK666,a specif-ic Arp2/3 complex inhibitor,prevents the reactivation of fetal genes and the enlargement of car-diomyocyte area induced by Ang Ⅱ in NRCMs and H9c2 cells(P<0.05).Our results reveal that the Arp2/3 complex plays a crucial role in Ang Ⅱ-induced cardiomyocyte hypertrophy,which is beneficial to further studies about the molecular mechanisms by which the Arp2/3 complex regu-lates pathological cardiac hypertrophy.

There is limited evidence to support the use of glucagon-like peptide-1 receptor agonists (GLP-1RAs) in solid organ transplants (SOTs). This systematic review and meta-analysis aimed to assess the safety and efficacy of GLP-1RAs in this population. PubMed, Embase, and Cochrane databases were a thorough literature search until July 2024 for SOTs with pre- and posttransplant diabetes mellitus (DM). Hemoglobin A1c (HbA1c), weight, and body mass index (BMI) were the primary outcomes. We estimated odds ratios and standardized mean difference (SMD) or mean difference (MD) with 95% confidence interval (CI) for dichotomous and continuous outcomes, respectively. I2 statistics measured heterogeneity. GLP-1RAs were administered to 806 subjects (99.8%) in 16 trials. Median follow-up was 12 months (interquartile range, 1–49 months). The mean age was 57.05 ± 10.24 years, with 64.6% male patients. HbA1c levels (MD, –0.61% [95% CI, –0.82 to –0.40]; p < 0.01, I2 = 72%), weight, and BMI were statistically significantly reduced. Estimated glomerular filtration rate (eGFR; SMD, –0.38 mL/min/1.73 m2 [95% CI, –1.01 to 0.25]; p = 0.24, I2 = 0%), creatinine, and blood pressure did not change significantly. Additionally, total daily insulin dosage, lipid profile, fasting plasma glucose, and urine albumin-to-creatinine ratio and tacrolimus levels (MD, –0.40 ng/mL [95% CI, –0.85 to 0.05]; p = 0.08, I2 = 42%) did not yield statistically significant. GLP-1RAs caused increased nausea and vomiting (13.9%), urinary tract infections (21.1%), and drug discontinuation (13.4%). In SOT recipients, GLP-1RAs significantly reduced HbA1c, weight, and BMI without significantly altering tacrolimus trough levels, eGFR, creatinine, or cardiovascular outcomes. Gastrointestinal side effects were the most common adverse events.

Objective:To explore the effects of participatory interactive teaching in the clinical internship course of pediatric infectious diseases.Methods:217 undergraduate students of grade 2018 majoring in pediatrics were selected as the experimental group, and 208 undergraduate students of grade 2017 majoring in pediatrics were selected as the control group. The experimental group used the teaching method of BOPPPS combined with case-based learning and team-based learning, while the control group adopted the traditional teaching method. Evaluate the learning effectiveness by comparing the formative evaluation and theoretical exam scores of the two groups , and a questionnaire survey was conducted for the students in the experimental group to investigate teaching feedback. SPSS 19.0 was used to perform the t test for data analysis. Results:The experimental group had significantly better scores than the control group in final theoretical examination [(75.04±9.12) vs. (71.03±9.51), P<0.05] and formative evaluation [(81.03±6.13) vs. (70.02±10.32), P<0.05]. According to the results of the questionnaire survey on teaching satisfaction, the students were satisfied with the interactive participatory teaching method in the four aspects of improving learning interest, improving learning ability, classroom satisfaction level, and course acceptance level. Conclusions:The pragmatic teaching reform in the internship of pediatric infectious diseases is highly accepted by students and beneficial to improving students' clinical thinking ability and comprehensive quality, which is worth further application and promotion.

The full implementation of the dean's responsibility system under the leadership of the Party Committee trans-forms the hospital's Party Committee from its past role as the"political core"to the"leadership core".In the process of hospital management transformation,establishing a collaborative governance system between the Party and the government is key to imple-menting the dean's responsibility system under the leadership of the Party Committee.Taking the exploratory practices of Shenz-hen Hospital,Beijing University of Chinese Medicine(Longgang)as an example,this involves a comprehensive summary from improving decision-making mechanisms,leveraging the combat fortification role of Party branches and the exemplary role of Party members,and continuously expanding the service coverage of Party-building efforts.This discussion aims to explore effective im-plementation strategies for the dean's responsibility system under Party Committee leadership,to promote high-quality develop-ment of hospitals,and to provide meaningful references for the implementation of this system in public hospitals.

Objective To analyze the application effect of active intervention mode in improving the quality of de-tection of pathogens from department of obstetrics.Methods Inpatients receiving therapeutic antimicrobial therapy in the department of obstetrics in a hospital from January 2022 to December 2023 were selected as the research sub-jects,active intervention measures were implemented based on the pathogen detection rate and specimen qualified rate.January-December 2022 was pre-intervention stage and January-December 2023 was post-intervention stage.Relevant indicators before and after the active intervention were compared and analyzed to evaluate the intervention efficacy.Results Therapeutic antimicrobial use rate in obstetric inpatients after active intervention was lower than before intervention(5.08％vs 11.87％).The proportion of targeted pathogen specimen detection before antimicro-bial therapy after active intervention was higher than before active intervention(54.63％vs 8.04％),among which the proportions of blood culture(24.67％vs 5.15％),urine culture(4.41％vs 1.65％),fetal membrane culture(11.01％vs 0.41％),cervical secretion culture(5.29％vs 0.41％),and amniotic fluid culture(9.25％vs 0.41％)after intervention all increased compared with before intervention,and the differences were all statistically signifi-cant(all P＜0.05).The qualified rate of microbial culture specimens from department of obstetrics after active in-tervention was higher than before active intervention(91.13％vs 66.67％),with statistically significant difference(P＜0.001).Conclusion Active intervention mode not only improve the health care workers'awareness on targe-ted pathogen detection,but also increase the qualified rate of microbial culture detection,effectively improve the quality of detection of pathogens from department of obstetrics.
