Advanced hip arthrosis in young patients presents a unique problem. The goal is to provide a stable and pain-free hip joint for early return to function. Several surgical options are available, most of which however present with unpredictable results. Arthrodesis as a bone conserving procedure addresses pain relief and offers the pursuit of an active lifestyle, making it a suitable option for young, active individuals with high physical demands. We reviewed the cases of three pediatric patients with end stage hip arthrosis secondary to non-traumatic and non- septic etiologies managed by hip arthrodesis. After a mean follow up of 17 months, fusion was noted in all subjects at an average of 4 months. All subjects have become pain free as of last follow up although there was expectedly some degree of functional limitation, and an asymmetric and slightly slower gait compared to normal. Most importantly, all patients have returned to an active lifestyle both at school and at play.
