1.Application of progressive muscle relaxation training in relieving fatigue of elderly patients with primary hepatocellular carcinoma after receiving transcatheter arterial chemoembolization

Objective To investigate the effect of progressive muscle relaxation training intervention strategy in relieving fatigue of elderly patients with primary hepatocellular carcinoma(HCC)after receiving transcatheter arterial chemoembolization(T ACE),and to analyze its influencing factors.Methods Using convenience sampling method,a total of 150 elderly patients with HCC,who received TACE at a certain grade Ⅲ-A hospital at Peking of China from May 2021 to March 2023,were selected as the subjects of research.The patients were randomly divided into the study group and the control group,and progressive muscle relaxation training intervention strategy and conventional postoperative fatigue care method were employed respectively.The preoperative fatigue status and the postoperative fatigue recovery status were compared between the two groups,and the influencing factors were analyzed.Results In both groups,the postoperative one-day fatigue score was the highest,which was gradually decreased thereafter.The average recovery time of fatigue in the control group was 9.84 days,which in the study group was 6.16 days,the difference between the two groups was statistically significant(P=0.013).The body mass index(BMI),Child-Pugh classification,and preoperative grip strength index had an effect on the postoperative fatigue recovery time after intervention.A BMI of β=-0.953 and a preoperative grip strength index of β=-0.185 were negatively correlated with the postoperative fatigue recovery time after intervention,while a Child-Pugh classification of β=2.177 was positively correlated with the postoperative fatigue recovery time after intervention.Conclusion Progressive muscle relaxation training intervention strategy is helpful for shortening the postoperative fatigue recovery time in elderly patients with HCC after receiving TACE,and it is worth of promotion in clinical practice.The patient's nutrition and physical status such as BMI,hepatic reserve function and grip strength index,are the factors influencing the effectiveness of progressive muscle relaxation training intervention strategy.
2.Study on the association between systemic immune-inflammation index and metabolic types and characteristics of obesity in children and adolescents

Objective:To explore the association between the systemic immune-inflammation index (SII) and obesity metabolic phenotypes, as well as metabolic features in children and adolescents.Methods:A cross-sectional study was conducted using the random cluster sampling method from March 2023 to May 2024. Children and adolescents aged 9-17 years in Guangzhou were surveyed through questionnaires, physical measurements, and blood tests. According to BMI and metabolic status, participants were classified into normal-weight groups [metabolically healthy normal weight (MHNW) and metabolically unhealthy normal weight (MUNW)] and overweight/obese groups [metabolically healthy overweight/obese (MHO/O) and metabolically unhealthy overweight/obese (MUO/O)]. After natural log-transformation of SII values (lnSII), multinomial logistic regression was used to assess the association between SII and obesity metabolic phenotypes, while binary logistic regression was applied to assess the relationship between SII and metabolic phenotypes in the overweight/obese subgroup. Linear regression model and restricted cubic spline (RCS) were employed to examine the relationship between SII and metabolic features among the entire population.Results:A total of 3 749 participants were included. After adjusting for covariates, for every unit increase in lnSII, the risk of MHO/O and MUO/O increased by 93% ( OR=1.93, 95% CI: 1.56-2.40, P<0.001) and 156% ( OR=2.56, 95% CI: 2.02-3.25, P<0.001), respectively. In the overweight/obesity subgroup, for every unit increase in lnSII, the risk of MUO/O increased by 37% ( OR=1.37, 95% CI: 1.01-1.87, P=0.045). Linear regression model and RCS showed that lnSII was positively correlated with systolic blood pressure (SBP) and diastolic blood pressure (DBP) (SBP: β=1.39, 95% CI: 0.67-2.11, P<0.001; DBP: β=1.27, 95% CI: 0.79-1.75, P<0.001). lnSII also had a non-linear relationship with triglyceride ( Pnonlinear=0.032) and high-density lipoprotein cholesterol ( Pnonlinear=0.002). Conclusion:Elevated SII levels are associated with unfavorable obesity metabolic phenotypes, higher blood pressure, and altered lipid profiles in children and adolescents. SII may be a potential driving factor for metabolic heterogeneity in children and adolescents.

4.Effect of Cichorium glandulosum Boiss.et Huet.on fecal bile acid profile in obese mice based on targeted metabolomics technique

Objective To investigate the effect of ethanolic extract of Cichorium glandulosum Boiss.et Huet.on fecal bile acid profiles in high-fat-diet-induced obese mice.Methods Twenty-four 6-week-old C57 BL/6 male mice were divided randomly into normal,model,drug-administration,and metformin groups.Mice in the normal group were fed a regular diet and mice in the other three groups were given high-fat diets.The drug-administration group was gavaged with 10 mL/kg ethanol extract of Cichorium glandulosum Boiss.et Huet.daily,and the metformin group was gavaged with 10 mL/kg metformin daily.After 10 weeks,livers were collected to measure hepatic total triglycerides(TG),total cholesterol(TC),low-density lipoprotein-cholesterol(LDL-C),and high-density lipoprotein(HDL)-C.Feces were collected and analyzed.Results Body weight(P<0.0001),liver TG(P<0.05),and TC(P>0.05)were all significantly higher in model mice compared with normal mice,while LDL-C(P>0.05)and HDL-C(P<0.001)were significantly lower,indicating abnormal weight gain and lipid metabolism.Alcoholic extract of Cichorium glandulosum Boiss.et Huet.significantly reduced body weight(P<0.0001),liver TG(P<0.0001),serum TG(P<0.05),TC(P<0.01),and LDL-C(P<0.05)in mice.Methodsological validation showed that the current method could accurately quantify 52 bile acids in feces.Analysis of the concentration of each type of bile acid revealed that alcoholic extract of Cichorium glandulosum Boiss.et Huet.significantly increased the secondary/primary bile acid ratio(P<0.05).Multivariate analysis showed that the bile acid metabolic pattern was significantly altered in all groups.Eight differential bile acids were screened in the drug-administration group relative to the model group using variable importance of projection>1 and P<0.05.A search of the Kyoto Encyclopedia of Genes and Genomes database revealed that the differential bile acids were mainly involved in the secondary bile acid biosynthesis pathway.Correlation analysis showed that four differential bile acids,deoxycholic acid(r,=0.6445,P<0.001),isolithocholic acid(r,=0.5879,P<0.01),3β-deoxycholic acid(r,=0.6649,P<0.001),and ω-rhamnoglutaric acid(rs=0.5387,P<0.01),in feces were strongly positively correlated with body weight.Conclusions Cichorium glandulosum Boiss.et Huet.alcoholic extract may play a role in weight reduction and amelioration of dyslipidemia by modulating secondary bile acid biosynthesis and altering fecal bile acid metabolic profiles.
5.Analysis of SHI Weiqun's Experience in Prescription Compounding in Clinical Practice

[Objective]To summarize and study Professor SHI Weiqun's thoughts and experience in the use of Xuanfu Daizhe Decoction as the main decoction for dysphagia.[Methods]Through outpatient study,case collection and review of related literature,the composition,efficacy,component analysis and modern pharmacology of Xuanfu Daizhe Decoction were investigated to summarize Prof. SHI Weiqun's experience in the use of as well as in the formulation of the said decoction,and to verify the results by citing two cases of poststroke dysphagia and pancreatic ataxia that were diagnosed and treated by Prof. SHI Weiqun.[Results]Xuanfu Daizhe Decoction is a representative prescription for treating stomach vacuity and phlegm blocking Qi and reverse in Treatise on Febrile Diseases. It is frequently utilized for treating digestive disorders. Modern medical research has broadened the application scope of Xuanfu Daizhe Decoction and demonstrated that it can also achieve favorable therapeutic effects in the treatment of other systemic diseases. Prof. SHI Weiqun maintains that in the clinical application of Xuanfu Daizhe Decoction,the pathogenesis should be grasped,it can be employed for gastric Qi inversion caused by various causes. The combination of the formula should be integrated with the regulation of Qi,concurrently reconcile the spleen and stomach,and verify the skillful application of the prescription. Eventually,it is necessary to review the entire formula through the overall concept of balancing the cold and heat of the prescription. The attached medical records encompassed the above four points throughout and attained good therapeutic effects.[Conclusion]Prof. SHI Weiqun has a unique understanding of the application of Xuanfu Daizhe decoction,and closely follows the four key points of "don't forget to regulate Qi,regulate the spleen and stomach,verify the demand cause,and balance cold and heat". His experience is unique,the clinical effect is remarkable,worthy of reference.

7.Effect of rhythmic physical activity on executive function in children aged three to six:a systematic review

Objective To systematically analyze the effect of rhythmic physical activity on executive function and sub-components in healthy preschool children. Methods Literature in Chinese and English was retrieved from databases such as CNKI,Wanfang Data,EBSCO,PubMed,Google Scholar and Web of Science for researches about the intervention of rhythmic physical activity for executive function of healthy children aged three to six years,up to October,2024.The methodological quali-ty of the researches was evaluated with PEDro Scale,and data were extracted for a systematic review. Results Ten researches,published between 2019 and 2024,from seven countries,were included;and seven were random-ized controlled trials and three were quasi experiment,involving a total of 1 047 children.The scores of the PE-Dro scale ranged five to eight.The rhythmic physical activity intervention was 20 to 50 minutes a time,one to five times a week,for six to 20 weeks.The forms of rhythmic physical activity intervention included sports activi-ties(street dance,flower ball cheerleading,walking,running,jumping and other motor skill exercises),music teaching activities(music perception,rhythm synchronization and music games),dance activities(creative dance,action imitation and role playing)and music performance activities(body percussion,rhythm sticks,sand balls and drums).Rhythmic physical activity was effective on at least one sub-component of executive function.How-ever,five of the seven researches involving cognitive flexibility failed to demonstrate a positive effect.Six re-searches compared the effects of rhythmic physical activity versus other physical activities,and five found that rhythmic physical activity was more effective on executive function. Conclusion Rhythmic physical activity can improve inhibitory control and working memory in preschool children;but the effect on cognitive flexibility remain controversial.
8.Urodynamic study of bladder dysfunction after radical hysterectomy of cervical cancer

Objective:To explore the urodynamic characteristics of bladder function in patients with abnormal urination after radical hysterectomy of cervical cancer.Methods:In a prospective self-controlled study, a total of 84 patients with cervical cancer and clinical stage of ⅠB to ⅡA, meeting the preoperative inclusion criteria in our hospital from January 2016 to June 2018 were enrolled.All patients were tested for urodynamic testing 1 week before and 6 months after surgery.Patient bladder function status was observed and evaluated before and after surgery and urodynamic examination results were analyzed.Results:Of 84 study cases, the 58 patients developed abnormal urination after radical surgery, there were no urination abnormalities in 26 cases.There was no significant difference in age, clinical stage and pathological diagnosis between patients with and without urination abnormalities.Abnormal urination after radical surgery included difficulty in urinating(55%), frequent urination with a feeling of urination not complete(34%), stress urinary incontinence(7%), and urinary incontinence(4%). Among the 26 patients without urination abnormalities after radical surgery, only one case showed an abnormal urodynamic examination(abnormal bladder sensation). In patients without abnormal urination after surgery, differences in the urodynamic examination findings between pre-and post-surgery were not statistically significant(all P> 0.05). At the same time, in all cases of abnormal urination after radical hysterectomy of cervical cancer, 43 patients(74%)with bladder dysfunction had normal urination pattern before operation, but after operation, the urination abnormality required abdominal pressure.Of the patients with bladder dysfunction after surgery, the maximum flow rate(Qmax)was(12.9±10.3)ml/s, the average flow rate(Qave)was(6.0±4.2)ml/s, the voided volume was(148.0±36.8)ml, voiding time was(32.9±22.1)s, maximum flow time was(11.4±5.0)s, postvoid residual urine was(260.2±219.2)ml, maximal detrusor pressure was(12.1±8.9)cmH 2O, bladder compliance was(16.1±4.3)ml/cmH 2O, normal desire to void was(354.5±204.3)ml, maximal capacity was(587.4±152.5)ml, maximum urethral pressure was(97.6±33.1)cmH 2O, maximum urethral closure pressure was(89.9±36.4)cmH 2O, and function urethral length was(29.6±6.5)mm; In comparison, the above indexes-corresponding values at 1 week before surgery were respectively as follows: the Qmax was(25.1±11.4)ml/s, the Qave was(11.4±6.6)ml/s, the voided volume was(318.6±96.4)ml, voiding time was(29.2±18.5)s, maximum flow time was(6.7±3.9)s, postvoid residual urine was(29.9±21.5)ml, maximal detrusor pressure was(31.9±21.4)cmH 2O, bladder compliance was(78.1±33.9)ml/cmH 2O, normal desire to void was(258.2±185.5)ml, maximal capacity was(335.1±124.9)ml, maximum urethral pressure was(96.4±33.9)cm H 2O, maximum urethral closure pressure was(88.5±35.2)cmH 2O, and function urethral length was(37.2±7.2)mm.It can be seen that Qmax, Qave, voided volume, maximum detrusor pressure, bladder compliance, and functional urethral length in patients with abnormal urination are significantly lower after radical hysterectomy of cervical cancer than before the surgery.While, normal desire to void, maximum capacity, maximum flow time, and postvoid residual urine volume were higher after radical hysterectomy than before surgery( P<0.05). In addition, there was no significant difference in voiding time, maximum urethral pressure and maximum urethral closure pressure between pre-and post-operation.Besides, it is worth noting that there was no significant difference in preoperative urodynamic test results between patients without abnormal urination versus patients with abnormal urination( P>0.05), and the difference in urodynamic test results between the two groups is statistically significant( P<0.05). There was no statistically significant difference in abnormal urination and in urodynamic test results between different clinical stages and between different pathological types( P>0.05). Conclusions:The characteristics of urinary dynamics in patients with abnormal urination after radical hysterectomy of cervical cancer are mainly manifested as decreased bladder sensory function and abnormal detrusor function.And the urodynamic test can provide objective clinical indicators for early diagnosis.
9.Comparison of EB virus infection between short term and long term use of mycophenolate mofetil for prophylaxis of graft versus host disease after haploidentical hematopoietic stem cell transplantation

Objective:To investigate the role of short-term use of mycophenolate mofetil (MMF) in EB viral infection and acute graft-versus host disease (GVHD) in patients receiving haploidentical hematopoietic stem cell transplantation (haplo-HSCT) .Method:Adult patients (≥14 years) who were diagnosed with hematological malignancies received haplo-HSCT in Peking University Institute of Hematology from May 2016 to December 2017 were retrospectively reviewed. The median age was 30 (14-60) years old. A total of 498 patients including 277 males and 221 females were enrolled. Donors' median age was 38 (8-66) years old. All patients were classified into long-term use of MMF ( n=199), which was defined as 500 mg every 12 hours from day 9 pre-transplant to 250 mg every 12 hours from day 30 after transplant then withdrawal on day 45 to 60 after transplant, and short-term use of MMF ( n=299), which was defined as 500 mg every 12 hour from day 9 pre-transplant then withdrawal till neutrophil engraftment. Kaplan-Meier model was used to analyze the cumulative incidence of EBV infection, and the Cox proportional regression model for multivariate analysis. Result:Characteristics including sex, age, disease types, mismatched HLA loci, donor-recipient relationship, donor-recipient blood type, donor age, and donor sex were comparable between two groups (all P>0.05). According to once, the incidence of EBV viremia, defined as EBV>10 3 copies/ml at least once, in short-term group and long-term group was 19.4% (58/299) and 27.6% (55/199) respectively ( P=0.046).Donor age and the duration of MMF prophylaxis (short-term group as reference) were associated with EBV viremia according to multivariate analysis [ HR=1.022(95% CI 1.006-1.038),1.600(95% CI 1.059-2.418); P=0.006 and 0.026, respectively]. The incidence of grade Ⅱ-Ⅳ and Ⅲ/Ⅳ acute GVHD in long-term and short-term group was 32.2% (64/199) versus 20.7% (62/299)( P=0.005) and 10.1% (20/199) versus 8.0% (24/299) ( P=0.427), respectively. Donor sex (female as reference) and duration of MMF prophylaxis (short-term group as reference) were associated with grade Ⅱ-Ⅳ acute GVHD [ HR=1.908(95% CI 1.079-3.373),1.752(95% CI 1.161-2.643); P=0.026 and 0.008, respectively].There were no statistical differences in the incidence of CMV viremia, refractory CMV viremia and hemorrhagic cystitis (all P>0.05) between the two groups. Conclusion:Short-term use of MMF can reduce EBV viremia without increasing the development of acute GVHD in haplo-HSCT patients.
10.Analysis of hot research areas for foreign nursing leadership based on bibliometric and biclustering

Objective To understand the status quo of foreign nursing leadership in recent 20 years,and compare and analyze the hotspots and trends of foreign nursing leadership.Methods The bibliometric analysis and cluster analysis were used for analyzing the literature of nursing leadership in PubMed from 1997 to 2006 and from 2007 to 2016,via Bicomb and gCLUTO softwares.Results A total of 482 articles were included.Bibliometric analysis showed that foreign nursing leadership literature number was increasing,which was mainly concentrated in the United States and England.The biclustering analysis showed that the research from 1997 to 2006 focused on patient safety and nursing leadership styles,education and training of nursing leadership,clinical nursing leadership models and nursing quality management;from 2007 to 2016,the research mainly focused on patient safety and nurses' psychological research,clinical nursing leadership theory,clinical nursing practice and competency training,and nnrses' role of leadership research.Conclusion The foreign nursing leadership theory has been developed well for its variety of topics and methods,and the object has been shifted from nursing administrators to clinical nurses.Future research should focus more on application of leadership in clinical nursing,and specialized leadership model and training system may be developed.
