1.Effect of refractive status before small incision lenticule extraction surgery on postoperative accommodative function

AIM: To investigate the abnormal conditions and change patterns of accommodative facility in patients with different refractive states before and after small incision lenticule extraction(SMILE)surgery.METHODS:A prospective clinical cohort study was conducted. A total of 59 patients(118 eyes)who underwent SMILE surgery and had visual function files established in our hospital from June to December 2023 were randomly selected, including 37 males and 22 females, aged 18-35 years(with an average age of 25.19±5.65 years). According to the preoperative spherical equivalent(SE), they were divided into two groups: the low-to-moderate myopia group(SE≥-6.00 DS)with 40 patients(80 eyes), and the high myopia group(SE<-6.00 DS)with 19 patients(38 eyes). The monocular and binocular accommodative facility before surgery and at 1 wk and 1 mo after surgery were compared, and the changes in accommodative facility before and after SMILE surgery in the two groups of patients were analyzed.RESULTS:All surgeries were completed successfully. In the low-to-moderate myopia group, 33 cases(66 eyes)completed the 1-month follow-up after surgery, with a loss to follow-up rate of 17.5%(7/40). In the high myopia group, 15 patients(30 eyes)completed the 1-month follow-up after surgery, with a loss to follow-up rate of 21.1%(4/19). After SMILE surgery, the uncorrected visual acuity and SE of both low-to-moderate myopia and high myopia were significantly improved(all P<0.05). The accommodative facility of the right eyes in all the patients at 1 mo after surgery was better than that before surgery and at 1 wk after surgery(P=0.002, 0.006), the accommodative facility of the left eyes was significantly increased at 1 mo after surgery than that at 1 wk after surgery(P=0.005), and the binocular accommodative facility at 1 mo after surgery was significantly increased compared with that before surgery(P<0.017). Furthermore, there were statistical significance in accommodative facility of the right eyes in the low-to-moderate group at 1 mo compared with that before surgery and at 1 wk after surgery(P=0.011, 0.004); it was significantly increased in the left eyes at 1 mo after surgery compared with that at 1 wk after surgery(P=0.001), and binocular accommodative facility at 1 mo after surgery was significantly better than that before surgery(P<0.001). Furthermore, there was no statistical significance in the right, left and binocular accommodative facility of patients in the high myopia group(all P>0.017).CONCLUSION: After SMILE surgery, the monocular accommodative facility shows a transient decrease and then exceeds the preoperative level at 1 mo after surgery, and the binocular accommodative facility gradually improves after surgery. SMILE surgery has a positive impact on the monocular and binocular accommodative facility in patients with low-to-moderate myopia, but has no significant impact on the accommodative facility in patients with high myopia. It is of clinical significance to strengthen the detection of monocular and binocular accommodative facility before and after SMILE surgery.

To investigate the association between neutrophil to lymphocyte ratio (NLR) andestimated glomerular filtration rate (eGFR) in patients with diabetes using large-scale data. Across-sectional analysis was conducted using data from diabetic patients in the National Health and Nutrition Examination Survey database from 2009 to 2014. Differences in NLR between patients with and without chronickidney disease (CKD) were compared. Pearson correlation analysis and multiple linear regression models wereapplied to assess the relationship between NLR and eGFR. A total of 857 diabetic patients were included, with 190 (22.2%) having CKD and 667 (77.8%) without CKD. NLR was significantly higher in patients with CKD compared to those without CKD (2.94±1.69 vs.2.36±1.98, NLR is independently negatively associatedwith eGFR in diabetic patients, demonstrating potential clinical value as an indicator of kidney function declineand CKD risk in this population.

Following hemorrhagic shock,locally released chemotactic molecules attract neutrophils to rapidly migrate to the damaged tissues,while effectively bind,engulf and kill microorganisms,along with chromatin releasing and forming a substance deco-rated with a meshwork of microbicidal proteins and enzymes,a DNA-protein structure named neutrophil extracellular traps(NETs).The formation and degradation of NETs are regulated by multiple factors,and an imbalance in the formation and degradation of NETs plays a role in cellular damage and the aggregation of inflammatory factors,which is one of the main mechanisms of uncontrolled in-flammatory response and further aggravates organ damage.Studies have shown that NETs are involved in the process of hemorrhagic shock-induced organ damage,but the relevant mechanisms have not been fully elucidated.In this paper,we review the role of NETs in organ damage after hemorrhagic shock and the related mechanisms,and in order to understand the role of NETs and provide a refer-ence for new targets for the prevention and treatment of hemorrhagic shock.

Chronic obstructive pulmonary disease (COPD) poses a significant public health challenge in China due to its high prevalence and substantial disease burden. Primary healthcare institutions (PHIs) serve as the cornerstone for COPD prevention and management. However, they currently face critical challenges, including inadequate diagnostic capacity, suboptimal treatment standardization, and limited patient awareness. In recent years, national efforts have initiated the development of a primary-level COPD prevention and management network. Key strategies include incorporating COPD into the Fundamental Public Health Service Program (FPHSP), strengthening lung function screening, promoting tiered diagnosis and treatment systems, establishing central-local coordination mechanisms, and implementing National Major Public Health Service Programs. Future directions necessitate focused efforts on: constructing integrated prevention and treatment models; optimizing the two-way referral system; enhancing screening for high-risk populations; leveraging digital health empowerment; and establishing a multi-dimensional quality evaluation system. These concerted actions are essential to build a comprehensive, high-quality COPD healthcare management system within primary care settings, ultimately contributing to the goals of the Healthy China Initiative (2019-2030).
6.Study on the AI-CDSS Using Behavior and Influencing Factors of Doctors:A Survey Study from Primary and Secondary Hospitals

Objective To study physicians'use of Artificial Intelligence-based Clinical Decision Support System(AI-CDSS)and its influencing factors in primary and secondary hospitals in China.Methods 443 physicians in prima-ry and secondary hospitals were surveyed by questionnaire.Univariate Chi-square test and logistic regression analy-sis were used to explore the influencing factors of doctors'use of Al-CDSS.Results Through univariate analysis,the effects of job title,working years,major,performance expectation,social influence,technical anxiety and indi-vidual innovation on use behavior were statistically significant(P<0.05).logistic regression analysis showed that in terms of working years,1 year or less,2 to 5 years,6 to 10 years,11 to 15 years,16 to 20 years and conve-nience were generally protective factors for physicians to use AI-CDSS,while attending physician,surgical direc-tion,non-existence of social influence,average social influence and personal innovation were generally risk factors.The differences were statistically significant(P<0.05).Conclusion Physicians'use of AI-CDSS is influenced by their working years and social influences.It is necessary to improve the cognition of physicians with long working years on AI-CDSS,give play to the radiation driving role of the surrounding environment,and pay attention to the cultivation of personal innovation.

9.Establishment of a zebrafish map3k15 knockout line using CRISPR/Cas9 gene editing technology

Objective To establish a homozygous zebrafish map3k15 knockout line using CRISPR/Cas9 gene editing technology so to provide an animal model for investigation of potential role of map3k15 in renal diseases.Methods 1)Analyzing the expression pattern of map3k15 in zebrafish;2)Establishing a homozygous map3k15 knockout zebrafish line using CRISPR/Cas9;3)Observing the phenotypic effects of map3k15 deficiency in ze-brafish.Results 1)map3k15 was expressed in the zebrafish pronephros,and map3k15/map3k15 protein was found highly conserved across species;2)map3k15-/-mutant zebrafish model was constructed with+2 bp mutation and+1 bp mutation;3)map3k15 mutant zebrafish exhibited edema in yolk sac,pericardium and head during embryonic development.Conclusions A homozygous zebrafish map3k15 knockout line is successfully developed which may provide an important model for future research on the role of map3k15 in renal development and disease.
10.Finite element modeling of knee joint based on semi-automatic segmentation technology

BACKGROUND:Knee finite element modelling can provide insight into knee mechanics,but its complex image segmentation is more difficult for researchers.With the development of deep learning techniques,deep learning techniques have been widely used in knee joint finite element modelling.OBJECTIVE:To replace the manual segmentation step in finite element modelling of the knee joint by using 3D Swin UNETR in combination with a semi-automatic segmentation technique for statistical shape models.METHODS:Manual(artificial)knee joint finite element model was developed based on MR and semi-automatic knee joint finite element model was developed based on 3D Swin UNETR+statistical shape model segmentation.The same loads and boundary conditions were applied to both models.Validation was performed by calculating the Dice similarity coefficient,mean distance,and comparing the peak equivalent stresses,maximum principal stresses,and maximum shear stresses of the two models.RESULTS AND CONCLUSION:(1)The Dice similarity coefficients of the manual and semi-automatic segmented femur and tibia were more than 98%,and the average distances were less than or equal to(0.35±0.08)mm.(2)With the longitudinal load of 750 N and 10 Nm internal overturning moment applied to the femur tip of both manual and semi-automatic finite element models,the peak equivalent stress,maximum principal stress,and maximum shear stresses of meniscus in manual finite element model were 14.12,18.54,and 7.35 MPa;peak equivalent force,maximum principal stress,and maximum shear stress of femoral cartilage were 2.22,2.15,and 1.18 MPa;peak equivalent force,maximum principal stress,and maximum shear stress of tibial cartilage were 2.50,1.91,and 1.41 MPa;semi-automatic finite element model of meniscus:peak equivalent force,maximum principal stress,and maximum shear stress were 14.93,18.53,and 7.75 MPa.The peak equivalent force,maximum principal stress,and maximum shear stress of femoral cartilage were 2.26,2.18,and 1.20 MPa;the peak equivalent stress,maximum principal stress,and maximum shear stress of tibial cartilage were 2.60,1.91,and 1.46 MPa.The peak equivalent stress,maximum principal stress,and maximum shear stress of manual and semi-automatic finite element models were basically consistent,with no significant difference(P>0.05).(3)The semi-automatic segmentation technique proposed in this study can replace manual segmentation in creating accurate finite element models of the knee joint.
