1.Theta Oscillations Support Prefrontal-hippocampal Interactions in Sequential Working Memory.

The prefrontal cortex and hippocampus may support sequential working memory beyond episodic memory and spatial navigation. This stereoelectroencephalography (SEEG) study investigated how the dorsolateral prefrontal cortex (DLPFC) interacts with the hippocampus in the online processing of sequential information. Twenty patients with epilepsy (eight women, age 27.6 ± 8.2 years) completed a line ordering task with SEEG recordings over the DLPFC and the hippocampus. Participants showed longer thinking times and more recall errors when asked to arrange random lines clockwise (random trials) than to maintain ordered lines (ordered trials) before recalling the orientation of a particular line. First, the ordering-related increase in thinking time and recall error was associated with a transient theta power increase in the hippocampus and a sustained theta power increase in the DLPFC (3-10 Hz). In particular, the hippocampal theta power increase correlated with the memory precision of line orientation. Second, theta phase coherences between the DLPFC and hippocampus were enhanced for ordering, especially for more precisely memorized lines. Third, the theta band DLPFC → hippocampus influence was selectively enhanced for ordering, especially for more precisely memorized lines. This study suggests that theta oscillations may support DLPFC-hippocampal interactions in the online processing of sequential information.

2.Reference values of carotid intima-media thickness and arterial stiffness in Chinese adults based on ultrasound radio frequency signal: A nationwide, multicenter study

Background::Carotid intima-media thickness (IMT) and diameter, stiffness, and wave reflections, are independent and important clinical biomarkers and risk predictors for cardiovascular diseases. The purpose of the present study was to establish nationwide reference values of carotid properties for healthy Chinese adults and to explore potential clinical determinants.Methods::A total of 3053 healthy Han Chinese adults (1922 women) aged 18-79 years were enrolled at 28 collaborating tertiary centers throughout China between April 2021 and July 2022. The real-time tracking of common carotid artery walls was achieved by the radio frequency (RF) ultrasound system. The IMT, diameter, compliance coefficient, β stiffness, local pulse wave velocity (PWV), local systolic blood pressure, augmented pressure (AP), and augmentation index (AIx) were then automatically measured and reported. Data were stratified by age groups and sex. The relationships between age and carotid property parameters were analyzed by Jonckheere-Terpstra test and simple linear regressions. The major clinical determinants of carotid properties were identified by Pearson’s correlation, multiple linear regression, and analyses of covariance.Results::All the parameters of carotid properties demonstrated significantly age-related trajectories. Women showed thinner IMT, smaller carotid diameter, larger AP, and AIx than men. The β stiffness and PWV were significantly higher in men than women before forties, but the differences reversed after that. The increase rate of carotid IMT (5.5 μm/year in women and 5.8 μm/year in men) and diameter (0.03 mm/year in both men and women) were similar between men and women. For the stiffness and wave reflections, women showed significantly larger age-related variations than men as demonstrated by steeper regression slopes (all P for age by sex interaction <0.05). The blood pressures, body mass index (BMI), and triglyceride levels were identified as major clinical determinants of carotid properties with adjustment of age and sex. Conclusions::The age- and sex-specific reference values of carotid properties measured by RF ultrasound for healthy Chinese adults were established. The blood pressures, BMI, and triglyceride levels should be considered for clinical application of corresponding reference values.
3.Analysis of Whole Exome Sequencing Result in Adult Genetic Diseases

Objective:To investigate the effectiveness and feasibility of whole exome sequencing (WES), as a molecular diagnosis technique, for adult patients with genetic diseases.Methods:The present retrospective analysis included 445 adult patients (ages 18-80 years) with suspected genetic diseases who underwent whole exome sequencing (WES) from August 2021 to December 2022. The pathogenicity classification of each variant was assessed in accordance with the recommendations developed by the American Society of Medical Genetics and Genomics.Results:The overall positive rate of WES among adult patients with suspected genetic diseases was 28.08% (125/445). The highest positive rate was observed in the age group of 41-50 years (34.33%, 23/67). Among the diagnosed genetic diseases, those affecting the cardiovascular system (63.16%, 84/133), nervous system (18.05%, 24/133), and endocrine system (13.53%, 18/133) ranked as the top three. The most common genetic diseases identified through WES in adult patients were hypertrophic cardiomyopathy (18.80%, 25/133), dilated cardiomyopathy (16.54%, 22/133), Marfan syndrome (15.04%, 20/133), epilepsy (9.02%, 12/133), and familial hypercholesterolemia (4.51%, 6/133). The main causative genes identified included FBN1 (14.29%, 19/133), MYBPC3 (9.02%, 12/133), MYH7 (9.02%, 12/133), LDLR (3.76%, 5/133), TTN (3.76%, 5/133), and TNNI3 (3.01%, 4/133).Conclusion:Applying the WES technique in clinical practice can improve the diagnostic rate of adult genetic diseases, especially in adult patients with suspected genetic conditions involving the cardiovascular system, nervous system, and endocrine system.
4.Implementation and application of telephone recruitment for apheresis platelet donors: based on the whole process information management of blood collection and supply

【Objective】 To develop and implement a precise and efficient telephone recruitment strategy for apheresis platelet donors based on the information management of blood collection and supply process. 【Methods】 A telephone recruitment strategy based on the whole process information management of apheresis platelet donors (hereinafter referred to as telephone recruitment) was formulated From January 1, 2019, apheresis platelet donors in Hangzhou, registered in the intelligent blood donor recruitment system(hereinafter referred to as the recruitment system), were recruited by telephone calls or (and) SMS reminders to inform re-donation interval, key points of recruitment consultation, blood donation appointment time and other precautions. Response rate, pass rate and blood donation rate of donors before(in 2018) and after( in 2019) the implementation of telephone recruitment, as well as the above three rates during the COVID-19 epidemic in 2020 in Hangzhou were collected and analyzed by χ2 test. 【Results】 The response rate, pass rate, and blood donation rate of apheresis platelet donors in Hangzhou before /after the implementation of telephone recruitment and during the COVID-19 epidemic were 28.52%(3 901/13 678)vs 30.52%(4 429/14 511)vs 39.36%(4 064/10 326), 73.80%(2 879/3 901)vs 77.83%(3 447/4 429)vs 78.20%(3 178/4 064)and 6.77%(2 879/42 553)vs 7.77%(3 447/44 366)vs 8.13%(3 178/39 077, respectively(P<0.05). In 2020, 85.94%(3 178/3 698) of apheresis donors were recruited by telephone(P<0.05). The proportion of apheresis donations successfully recruited by telephone in 2018 and 2019 accounted for 11.74% (2 879/24 519) and 12.48% (3 447/27 628), respectively(P<0.05), contributing 10.59% (4 030 U/38 040 U) and 11.38% (4 875 U/42 853 U) of units(P<0.05) that issued to the clinical. Due to the COVID-19 epidemic, the number of blood donors recruited by telephone and its proportion in 2020 decreased significantly as compared with those before the implementation of telephone recruitment (2019). 【Conclusion】 The telephone recruitment strategy based on the whole-process information management (system) of blood collection and supply can effectively improve the targeted recruitment of apheresis platelet donors and enhance its proportion of apheresis collection that issued to the clinical.
5.Rapid Health Technology Assessment for Effectiveness ,Safety and Economics of Ginkgo biloba Extract in the Treatment of Alzheimer ’s Disease

OBJECTIVE:To rapidly evaluate the effectiveness ,safety and economics of Ginkgo biloba extract(EGb)in the treatment of Alzheimer ’s disease (AD)patients,and to provide evidence-based reference for clinical drug selection and decision. METHODS:Retrieved from PubMed ,Embase,Cochrane Library ,Web of Science ,CNKI,CBM,Wanfang database ,health technology assessment (HTA)organization websites and database during the inception to Aug. 10,2020,HTA reports ,systematic reviews/Meta-analysis,and pharmacoeconomic studies of EGb versus placebo in the treatment of AD were collected. After literature screening and data extration ,HTA checklist ,AMSTAR-2 scale and CHEERS scale were used respectively to evaluate the literature quality of the included HTA report ,systematic review/Meta-analysis and pharmacoeconomics studies. The conclusion of the included studies were summarized by using qualitative description. RESULTS :A total of 9 literatures were included ,involving 8 systematic reviews and 1 economic studies. In terms of effectiveness ,there was no statistical significance in MMSE score of EGb group,ADAS-Cog score of 120 mg EGb group ,compared with placebo group (P>0.05). Dementia Quality of Life (DQoL)score of EGb group was significantly higher than that of placebo group. The scores of short cognitive aptitude tests ,neuropsychiatric inventory(NPI),NPI caregiver version score ,ADAS-Cog score of 160 mg EGb group and 240 mg EGb group were significantly lower than those of control group (P<0.05). ADL scores of patients were inconsistent ;ADL scores of EGb group were significantly lower than those o f placebo group (P<0.05),or there was no significant diff erence between 2 groups(P>0.05); . subgroup analysis by dose showed that there was no RDY2019-39) significant difference in ADL score between 120 mg EGb group and placebo group (P>0.05);ADL score of 240 mg E-mail:renxiaolei83@126.com EGb group were signicantly lower than that of placebo group (P<0.05). Subgroup analysis of clinical global impression 010-88325751。E-mail:lyi1267@126.com change (CGIC) score showed that there was no significant difference in CGIC score between EGb group and placebo group after receiving <200 mg EGb and 26 weeks of treatment (P> 0.05);CGIC score of EGb group was significantly higher than that of placebo group after receiving >200 mg EGb and 24 weeks of treatment (P<0.05). In terms of safety ,there was no statistical significance in the incidence of ADR or the incidence of severe ADR between EGb group and placebo group (P>0.05). Subgroup analysis by dose showed that the incidence of ADR in 240 mg EGb group was significantly higher than placebo group (P<0.05). Economically ,EGb treatment for AD is cost-effective ,which could indirectly save the nursing costs of AD patients. CONCLUSIONS :The efficacy of EGb in the treatment of AD is uncertain , and the safety and economy are good.
6.Efficacy and Safety of Alprostadil Injection in Treatment of Viral Hepatitis:A Meta-analysis

Objective To systematically evaluate the efficacy and safety of alprostadil injection in patients with viral hepatitis.Methods Medline,Embase,The Cochrane Library,CBMdisc,CNKI,Wanfang Database and VIP were searched.The quality of included studies such as randomization,blinding,allocation concealment and loss of follow-up were evaluated and meta-analysis was performed by RevMan5.1 software.Results In total,14 RCTs and 1 232 patients were included.Meta-analysis showed that in patients with viral hepatitis,the total effective rate of alprostadil injection treatment was significantly superior to that of conventional therapy (P<0.000 01).Serious adverse drug reactions (ADRs) induced by alprostadil injection were not reported.Conclusion Alprostadil injection is effective and safe for treating viral hepatitis.However,the evidence is not strong due to the generally low methodological quality of RCTs.Further high quality and large sample-sized randomized controlled trials and more pharmacoeconomics studies should be carried out.
7.Literature Analysis of 66 Cases of Drug-induced Hyperkalemia

OBJECTIVE:To investigate the characteristics and related factors of drug-induced hyperkalemia,present preven-tive measures and provide reference for the rational drug use. METHODS:46 literatures of individual cases and group cases of drug-induced hyperkalemia publicly reported in domestic medical journals from 1979 to 2014 were collected and analyzed statistical-ly,including 66 cases. RESULTS:66 cases with drug-induced hyperkalemia were mainly involved in cardiovascular drugs and im-munosuppressants. Most of them were improved after drug withdrawal and symptomatic treatment,and 2 cases died. CONCLU-SIONS:Drug-induced hyperkalemia are potentially dangerous. It is necessary to be vigilant,prevent and control timely and reduce the incidence of severe adverse reactions.
8.Crystal structure of SMU.2055 protein from Streptococcus mutans and its small molecule inhibitors design and selection.

OBJECTIVEThe aim of this study is to analyze the three-dimensional crystal structure of SMU.2055 protein, a putative acetyltransferase from the major caries pathogen Streptococcus mutans (S. mutans). The design and selection of the structure-based small molecule inhibitors are also studied.
METHODSThe three-dimensional crystal structure of SMU.2055 protein was obtained by structural genomics research methods of gene cloning and expression, protein purification with Ni²⁺-chelating affinity chromatography, crystal screening, and X-ray diffraction data collection. An inhibitor virtual model matching with its target protein structure was set up using computer-aided drug design methods, virtual screening and fine docking, and Libdock and Autodock procedures.
RESULTSThe crystal of SMU.2055 protein was obtained, and its three-dimensional crystal structure was analyzed. This crystal was diffracted to a resolution of 0.23 nm. It belongs to orthorhombic space group C222(1), with unit cell parameters of a = 9.20 nm, b = 9.46 nm, and c = 19.39 nm. The asymmetric unit contained four molecules, with a solvent content of 56.7%. Moreover, five small molecule compounds, whose structure matched with that of the target protein in high degree, were designed and selected.
CONCLUSIONProtein crystallography research of S. mutans SMU.2055 helps to understand the structures and functions of proteins from S. mutans at the atomic level. These five compounds may be considered as effective inhibitors to SMU.2055. The virtual model of small molecule inhibitors we built will lay a foundation to the anticaries research based on the crystal structure of proteins.

9.Effect of berberine on D-dimer, fibrinogen and interleukin-6 in peripheral blood of ovarian cancer patients

Objective To analyse the effect of berberine on D-dimer, fibrinogen (FIB) and interleukin-6(IL-6) in peripheral blood of ovarian cancer patients.Methods 130 cases selected from the hospital were diagnosed as unilateral ovarian cancer, treated by surgery and chemotherapy.The patients were divided into control group (25 cases) and experimental group (105 cases), and the experimental group were randomly divided into A, B, C three groups, 35 cases in each group.The experimental group received berberine hydrochloride tablets, orally, three times a day, on the basis of conventional chemotherapy.The experimental groups were divided into three subgroups according to the dosage: A group: 0.3 g, B group: 0.2 g, C group:0.1 g, 35 cases in each group, continuously treated for 2 weeks.The levels of D-dimer, FIB and IL-6 and quality of life score were detected before and after treatment.Results After treatment, D-dimer level in each subgroup of experimental group was lower than that in control group, respectively, and A group
10.Bilateral anterior internal capsulotomy for refractory anxiety disorders

Objective To study the clinical effect of bilateral anterior internal capsulotomy on patients with refractory anxiety disorders.Methods Twenty-one patients with anxiety disorders,failed to pharmacological and cognitive behavior therapies for 5 years and admitted to our hospital from November 2009 to January 2012,were chosen; they received MRI-guided stereotactic bilateral anterior capsulotomy.The coordinates of the lesion target were determined under high resolution MRI directly and confirmed by intra-operative impedance test and high frequency stimulation.The lesions were received radiofrequency at 80 ℃ for 60 seconds.Pre-and post-operative mini-mental state examination (MMSE),self-rating anxiety scale (SAS),Hamilton depression rating scale (HAMD) and Hamilton anxiety rating scale (HAMA) were performed by the same psychiatrists.Results In the 21 patients,8 had anxiety-free,10 had significant improvement,and 3 had no significant improvement.No serious surgery-related complications were noted.Nine patients had mild cognitive deficit and transient dysmnesia only at 1-2 weeks after operation.Personality changes,such as lack of motivation,apathy and indolence,appeared in 4 patients.Psychiatric evaluations demonstrated significant decrease of SAS,HAMD and HAMA scores after operation as compared with those before operation (P<0.05).However,the difference of MMSE scores between pre-and post-operation was not statistically significant (P>0.05).Conclusion MRI guided stereotactic bilateral capsulotomy is precise,safe and much an effective treatment for refractory anxiety disorders,whcih is promising to alleviate the symptoms of anxiety and depression,as well as improving the quality of life.
