1.Changing Pattern of Fungal Communities and Isoflavone Content during Fermentation of Sojae Semen Praeparatum

ObjectiveTo reveal the changing pattern of fungal communities and isoflavone content during the fermentation of Sojae Semen Praeparatum. MethodsInternal transcribed spacer (ITS) amplicon sequencing was used to analyze the changes in fungal communities during the fermentation. High-performance liquid chromatography (HPLC) was employed to determine the composition and content of isoflavones. Spearman correlation analysis was conducted to identify fungal communities positively correlated with isoflavone content. ResultsDuring the fermentation of Sojae Semen Praeparatum, the abundance of Penicillium continued to increase and became the dominant genus from the "yellow cladding" to the early "secondary fermentation" stage (9 days). However, in the mid-to-late "secondary fermentation" stage (12-21 days), the abundance of Penicillium decreased, while the abundance of Barnettozyma increased rapidly, becoming the dominant genus during the later stages (18-21 days). Alpha diversity analyses showed that fungal diversity was higher in the "secondary fermentation" stage than in the "yellow cladding" stage. Beta diversity analyses showed greater variability in fungal communities during the "yellow cladding" stage, with relatively stable fungal communities during the "secondary fermentation" stage. The aglycone isoflavone content exhibited a gradual increase, rising from 5.56% at the start of fermentation to 96.84% by the end. In contrast, the glucoside isoflavone content experienced a corresponding decline, decreasing from 94.44% to 3.16%. Spearman analysis showed that Penicillium, Barnettozyma, and Ilyonectria were positively correlated with the contents of daidzein, glycitein, and genistein, and promoted the conversion of isoflavones from glucoside to aglycone forms. ConclusionThe fungal communities of Penicillium, Barnettozyma, and Ilyonectria promote the conversion of isoflavones from glucoside to aglycone forms during the fermentation of Sojae Semen Praeparatum. The findings provide an important reference for improving the scalability and quality of Sojae Semen Praeparatu fermentation.
2.Coronary artery stenosis associated with right ventricular dysfunction in acute pulmonary embolism: A case-control study.

BACKGROUND:
The potential impact of pre-existing coronary artery stenosis (CAS) on right ventricular (RV) function during acute pulmonary embolism (PE) episodes remains underexplored. This study aimed to investigate the association between pre-existing CAS and RV dysfunction in patients with acute PE.
METHODS:
In this multicenter, case-control study, 89 cases and 176 controls matched for age were enrolled at three study centers (Peking Union Medical College Hospital, Fuwai Hospital, and the Second Affiliated Hospital of Harbin Medical University) from January 2016 to December 2020. The cases were patients with acute PE with CAS, and the controls were patients with acute PE without CAS. Coronary artery assessment was performed using coronary computed tomographic angiography. CAS was defined as ≥50% stenosis of the lumen diameter in any coronary vessel >2.0 mm in diameter. Conditional logistic regression analysis was used to evaluate the association between CAS and RV dysfunction.
RESULTS:
The percentages of RV dysfunction (19.1% [17/89] vs. 44.6% [78/176], P <0.001) and elevated systolic pulmonary artery pressure (sPAP) (19.3% [17/89] vs. 39.5% [68/176], P = 0.001) were significantly lower in the case group than those in the control group. In the multivariable logistic regression model, CAS was independently and negatively associated with RV dysfunction (adjusted odds ratio [OR]: 0.367; 95% confidence interval [CI]: 0.185-0.728; P = 0.004), and elevated sPAP (OR: 0.490; 95% CI: 0.252-0.980; P = 0.035), respectively.
CONCLUSIONS
Pre-existing CAS was significantly and negatively associated with RV dysfunction and elevated sPAP in patients with acute PE. This finding provides new insights into RV dysfunction in patients with acute PE with pre-existing CAS.

3.Analysis on current situation of drug use in Chinese aeromedical rescue

OBJECTIVE To analyze the current situation of drug use in domestic aeromedical rescue, and provide references for the development of aeromedical rescue services and the rational use of drugs on board. METHODS All literature on aeromedical rescue in China were retrieved from the databases of SinoMed, CNKI, VIP, and Wanfang data up to September 1st, 2024. Extracting descriptive analysis were conducted on the literature screened by the inclusion and exclusion criteria. RESULTS A total of 36 literature were included. Aeromedical rescue cases had been reported in China since 1985, with a cumulative total of 5 370 cases reported. Prehospital rescue performed 861 cases, with 96.40% of them involving the use of at least 9 categories, totaling at least 10 different drugs, primarily emergency drugs. Interhospital rescue performed 4 509 cases, and 85.23% of them used over 48 kinds of drugs across 19 categories, mainly emergency drugs supplemented by specialty drugs. From the view of transportation, 5 166 air transfers were made by helicopters, of which 88.00% involved the use of drugs, and 204 cases by fixed-wing aircraft, of which 91.18% involved the use of drugs. CONCLUSIONS Drugs are frequently used in aeromedical rescue involving a wide variety of types in China. It is imperative to strengthen the focus on the equipment and rational use of drugs in aeromedical rescue, thereby facilitating the establishment of a standardized theoretical framework.
4.Impact of peer dating behavior and cohabitation with parents on sexual behaviors among secondary vocational school students

Objective:
To understand the moderating effect of cohabitation with parents on the association between peer dating behavior and sexual behaviors among secondary vocational school students, so as to provide a scientific basis for preventing sexual behaviors among secondary vocational school students.
Methods:
From March to April 2021, an electronic questionnaire survey was conducted among 3 180 students from 6 vocational schools in Shanghai (urban, suburban, exurban) and Shaanxi (Shangluo, Ankang, Baoji) using cluster sampling. Spearman correlation analysis was used to investigate the relationship of cohabitation with parents, peer dating behavior and sexual behaviors among secondary vocational school students. Binary Logistic regression analysis was performed to investigate the role of cohabitation with parents on peer dating behavior and sexual behaviors among secondary vocational students.
Results:
There was a significant negative between cohabitation with parents and sexual ( r =-0.04); and there was a positive correlation between peer dating behavior and sexual behaviors ( r =0.24), as well as cohabitation with parents and peer dating behavior ( r =0.04)( P <0.05). Multivariable Logistic regression analysis showed an association between peer dating behavior and the occurrence of sexual behaviors ( OR=2.79-12.95, P <0.05). Cohabitation with parents played a moderating role in the association between peer dating behavior and sexual behaviors, and a signification interaction was found between cohabitation with parents and reporting that a small part or about half of their peers had dating behavior ( OR =0.48, P <0.05).
Conclusions
The more peers dating behavior are associated with a higher risk of sexual behaviors among secondary vocational school students, and cohabitation with parents can partly reduce this risk. School and family sexuality education for secondary vocational students should be strengthened to improve their interpersonal skills and decision-making, and ability to resist peer pressure, so as to reduce their risk of sexual behaviors.
5.Nursing care for a patient with drug-eluting stent puncture in arteriovenous fistula

The nursing experience of buttonhole cannulation for an autogenous arteriovenous fistula with an Eluvia drug-eluting stent in a patient undergoing maintenance hemodialysis was summarized.Key nursing points included:developing a scientific and rational buttonhole cannulation plan for the autogenous arteriovenous fistula stent based on its characteristics post-stent implantation;conducting preoperative ultrasonic evaluations of the arteriovenous fistula and stent;establishing and maintaining a buttonhole tunnel at the stent site;regularly monitoring and following up on the cannulation site and the condition of the autogenous arteriovenous fistula;strengthening the observation and prevention of potential complications.Through rigorous and standardized assessments,procedures,and nursing care,the patient maintained good autogenous arteriovenous fistula function without stent-related complications such as fracture,stenosis,or infection over a 14-month follow-up period.
6.A retrospective study of BRAF inhibitors and EGFR inhibitors combined with immune checkpoint inhibitors in patients with microsatellite stable, BRAF V600E mutated metastatic colorectal cancer

Objective:To explore the efficacy and safety of B-Raf proto-oncogene, serine/threonine kinase (BRAF) inhibitor and epidermal growth factor receptor (EGFR) inhibitor combined with immune checkpoint inhibitor in microsatellite stable (MSS) BRAF V600E metastatic colorectal cancer (mCRC) patients.Methods:The data and outcomes of mCRC patients with MSS BRAF V600E who received BRAF inhibitor, EGFR inhibitor combined with immune checkpoint inhibitor in Tianjin Medical University Cancer Hospital from May 2022 to April 2024 were retrospectively collected.Results:A total of 12 mCRC patients were included in this study, the objective response rate was 50.0%, the disease control rate was 66.7%, and the median disease control time of patients who achieved objective response was 8.0 months. The median progression-free survival was 6.8 months and the median overall survival was 8.4 months. Overall adverse reactions were controllable, the most common treatment-related adverse events were fatigue (8 cases), fever (5 cases), and rash (4 cases). There were no grade 4 adverse event, serious adverse event, and treatment-related death.Conclusion:BRAF inhibitor and EGFR inhibitor combined with immune checkpoint inhibitor show good efficacy and controllable safety in BRAF V600E mCRC patients.
7.Cross-sectional study of drug resistance in newly diagnosed HIV-1 infected patients in Shanghai

Objective:To investigate the drug resistance of newly diagnosed HIV-1 infected patients in Shanghai and to provide reference value for clinical antiretroviral therapy (ART).Methods:The peripheral venous blood plasma of 196 newly diagnosed HIV-1 infected patients screened according to the inclusion and exclusion criteria at the Shanghai Public Health Clinical Center from April to June 2023 was collected, HIV-1 RNA was extracted, the pol region was amplified by reverse transcription-polymerase chain reaction (RT-PCR) for sequencing, the mutation sites and ART drug resistance were analyzed.Results:The plasma of 196 newly diagnosed HIV-1 infected patients was amplified successfully in 162 cases (amplification success rate was 82.65%). The subtypes consisted of CRF07_BC(51.23%), CRF01_AE (27.78%), and others (6.79%), CRF55_01B (5.56%), B (3.70%), CRF01_AE/B (3.70%) and CRF08_BC (1.23%). The overall transmitted drug resistance rate was 7.41%, the protease inhibitors (PIs), non-nucleoside/nucleotide reverse transcriptase inhibitors (NNRTIs), nucleoside/nucleotide reverse transcriptase inhibitors (NRTIs), integrase inhibitors (INSTIs) resistance rates were 3.09%, 3.70%, 0.00% and 0.62%, respectively. The proportion of NNRTIs-related mutation sites in B (66.67%) and CRF55_01B (88.89%) was higher than that in CRF07_BC (13.25%); the proportion of NNRTIs-related mutation sites in CRF55_01B (88.89%) was higher than that in CRF01_AE (22.22%) and other subtypes (18.18%), the difference was statistically significant (all P<0.05). Multivariate logistic regression analysis showed that the probability of PIs-related mutation sites in CRF01_AE/B was 21.71 times that of CRF07_BC[odds ratio ( OR)=21.71, 95% confidence interval ( CI): 3.36-140.27, P=0.001]. Conclusions:The transmitted drug resistance among newly diagnosed HIV-1 infected patients in Shanghai is at the moderate epidemic level, mainly NNRTIs and PIs-related drug resistance, and the INSTIs resistance rate is low, the use of INSTIs in ART regimens should be considered.
8.Association between beverage and snack intake and vitamin D insufficiency among primary and secondary school students in Yunnan Province

Objective:
To understand the association between beverage and snack intake and insufficient serum 25 hydroxyvitamin D [25(OH)D] among primary and secondary school students, so as to provide a scientific basis for targeted intervention measures.
Methods:
From October to December 2021, a stratified random sampling method was used to select 2 477 primary and secondary school students aged 8 to 15 years old from 9 counties in Yunnan Province implemented the Nutrition Improvement Plan for Rural Compulsory Education Students. The intake of beverages and snacks was investigated using the Rural Student Nutrition Monitoring Questionnaire from Chinese Center for Disease Control and Prevention. The snack intake intensity was calculated and classified into no intake, extremely low, low, medium, and high intensity. Serum 25(OH)D levels were measured in the laboratory, and levels <20 ng/mL were defined as insufficient. Chi square tests, LASSO regression, random forest and binary Logistic regression were used to analyze the association between 20 types of beverages and snacks and serum 25(OH)D insufficiency.
Results:
Insufficient serum 25(OH)D was detected in 564 boys (45.9%) and 855 girls (68.5%), with a total of 1 419 cases (57.3%). Binary Logistic regression results showed that extremely low intake intensity of carbonated beverages ( OR =1.51), plant protein beverages ( OR =1.61), and milk tea beverages ( OR =1.39) increased the risk of insufficient serum 25(OH)D, while protective factors were fruits and vegetables ( OR =0.77) and pure milk and yogurt ( OR =0.74) (all P <0.05). Subgroup analysis showed that extremely low intake intensity of carbonated beverages, milk containing beverages, tea beverages, fruit and vegetable juices, and plant protein beverages increased the risk of insufficient serum 25(OH)D in girls ( OR =2.22, 1.72, 1.67, 1.74, 1.92), and high intake intensity increased the risk of insufficient serum 25(OH)D in boys ( OR =1.73, 1.48, 1.52, 1.49, 1.97) (all P <0.05). Extremely low intake intensity of carbonated beverages, plant protein beverages, and milk tea beverages in junior high school students ( OR =1.92, 2.54, 1.68) and low intake intensity in primary school students ( OR =1.40, 1.33, 1.45) increased the risk of insufficient serum 25(OH)D (all P <0.05).
Conclusions
Frequent intake of beverages and highly processed snacks increases the risk of insufficient serum 25(OH)D in primary and secondary school students, while natural foods such as fruits, vegetables, pure milk and yogurt can reduce the risk. Girls and junior high school students are more susceptible to these effects.
9.Impacts of precision health management model on liver fibrosis,adipocytokines and metabolic indicators in patients with non-alcoholic fatty liver disease

Objective To investigate the impact of the precision health management model on liver fibrosis,adipocytokines,and metabolic indicators in patients with non-alcoholic fatty liver dis-ease(NAFLD).Methods A total of 600 NAFLD patients were selected as study subjects and divid-ed into conventional group and precision group according to different intervention methods,with 300 patients in each group.The conventional group received routine health management,while the preci-sion group received intervention through the precision health management model.Comparisons were made between the two groups in terms of obesity indicators[waist-to-hip ratio(WHR),body mass in-dex(BMI),body fat rate(BFR),degree of obesity],fatty liver grading,liver stiffness measure-ment(LSM),and liver fibrosis indicators[laminin(LN),type Ⅳ collagen(C Ⅳ),hyaluronic acid(HA),type Ⅲ procollagen(PC Ⅲ)],hepatic adipocytokines[nuclear factor-κB(NF-κB),adi-ponectin(APN),irisin,tumor necrosis factor-α(TNF-α)],and lipid metabolism indicators[tri-glycerides(TG),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),total cholesterol(TC)]levels.Results After the intervention,the levels of WHR,BFR,BMI,degree of obesity,LSM,as well as LN,C Ⅳ,HA,PC Ⅲ,NF-κB,TNF-α,TG,LDL-C,and TC in both groups were lower than those before the intervention,and the precision group showed lower levels than the conventional group,with statistically significant differences(P<0.05).After the intervention,the fatty liver grading in both groups was better than before intervention,and the precision group showed better fatty liver grading than the conventional group,with statistically signif-icant differences(P<0.05).After the intervention,the levels of APN,irisin,and HDL-C in both groups were higher than those before the intervention,and the precision group showed higher levels than the conventional group,with statistically significant differences(P<0.05).Conclusion The precision health management model can effectively improve the degree of obesity,fatty liver grading,and liver fibrosis status in NAFLD patients,reduce LSM,and regulate the levels of adipocytokines and lipid metabolism indicators,yielding significant effects.
10.Correlations between CD4+CD25+Treg,sCD30 and Clinical Characteristics of Lymphoma and Their Value in Predicting Infection after Chemotherapy

Objective To investigate the expression and significance of CD4+CD25+regulatory T cells(CD4+CD25+Treg)and soluble CD30(sCD30)in peripheral blood of lymphoma patients.Methods A total of 83 lymphoma patients admitted to Beijing Aerospace General Hospital from January 2018 to December 2022 were selected as the research subjects,and their peripheral blood CD4+CD25+Treg and sCD30 expressions were statistically analyzed.Spearman analysis was used to investigate the correlation between peripheral blood CD4+CD25+Treg and sCD30 expressions and bone marrow infiltration.At the same time,the research subjects were divided into an infection group(n=26)and a non-infection group(n=57)according to the presence or absence of infection after rituximab chemotherapy.The expressions and differences of peripheral blood CD4+CD25+Treg and sCD30 in the two groups were compared,and the receiver operating characteristic(ROC)curve and area under the curve(AUC)were used to analyze the predictive efficacy.Results The expression of CD4+CD25+Treg and sCD30 in peripheral blood of patients with bone marrow infiltration was higher than that of patients without bone marrow infiltration in the research group(P<0.05);the expression of CD4+CD25+Treg and sCD30 in peripheral blood of lymphoma patients was positively correlated with bone marrow infiltration(r=0.612,0.634,P<0.05);the expression of Tregs and sCD30 in peripheral blood of the infection group after chemotherapy was higher than that of the non-infection group,and the difference was higher than that of the non-infection group(P<0.05);the ROC curve showed that the combined prediction of the difference of CD4+CD25+Treg and sCD30 in peripheral blood for the infection after chemotherapy in lymphoma patients had an AUC of 0.916(0.834~0.965),which was superior to single prediction.Conclusion The high expression of CD4+CD25+Treg and sCD30 in peripheral blood of lymphoma patients is positively correlated with bone marrow infiltration.Combined testing can improve the predictive efficacy of infection after chemotherapy and guide clinical diagnosis and treatment.
