1.Predictive value of cardiac MR feature tracking for adverse left ventricular remodeling in patients with acute anterior wall ST-segment elevation myocardial infarction

Objective To investigate the predicting value of cardiac magnetic resonance feature tracking(CMR-FT)for adverse left ventricular remodeling(ALVR)in patients with acute anterior wall ST-segment elevation myocardial infarction(STEMI).Methods The clinical data and cardiac magnetic resonance(CMR)images of 161 acute anterior wall STEMI patients within 1 week and 6 months after emergency percutaneous coronary intervention(PCI)were retrospectively analyzed.ALVR was defined as an increase of left ventricular end-diastolic volume(LVEDV)over 20%at the second CMR examination compared to the baseline.The CMR parame-ters were analyzed by CVI42 post-processing software.The logistic regression analysis was used to screen the independent predictors of ALVR,and the receiver operating characteristic(ROC)curve was used to evaluate the predictive efficiency of ALVR.Results The incidence of ALVR at 6 months was 21.7%(35/161).The logistic regression analysis showed that the left ventricular global circumferential strain(LVGCS)and right ventricular global longitudinal strain(RVGLS)at baseline were independent predictors for ALVR(P<0.001).When LVGCS was-13.89%and RVGLS was-15.07%at baseline,the sensitivity of predicting ALVR was 0.714 and 0.743,the specificity was 0.833 and 0.810,and the area under the curve(AUC)was 0.806 and 0.835,respectively.The sensitivity of LVGCS combined with RVGLS in predicting ALVR was 0.802,the specificity was 0.952,and the AUC was 0.888.The DeLong test showed that the AUC of LVGCS com-bined with RVGLS in predicting ALVR was significantly higher than that of individuals,and the difference was statistically significant(P<0.05).Conclusion The LVGCS and RVGLS at baseline are independent predictors for ALVR in patients with acute anterior wall STEMI,their combination can significantly improve the pre-dictive efficiency of ALVR in these patients.
2.Montelukast sodium inhibits airway inflammation through PHD2/HIF-1α pathway in asthmatic mice

Objective The study aimed to investigate whether montelukast sodium could alleviate airway inflammatory responses in asthmatic mice by affecting the PHD2/HIF-1α pathway.Methods An allergic asthma model was established by ovalbumin(OVA)induction,and 18 female BALB/c mice were randomly divided into a control group(Con group),an asthma group(OVA group),and an asthma group with montelukast sodium intervention(30 mg/kg montelukast sodium by oral administration 1 h before OVA challenge,Mon group).HE staining was used to analyze the pathological changes in the lungs of mice.Blood cell analyzer and kits were used to determine the number of inflammatory cells and the levels of cytokines,the content of lactic acid and pyruvic acid in the lungs,respectively.RT-PCR and Western blot were used to detect the mRNA and protein expression of HIF-1α,PHD2,E-cad and p120 in the lungs of mice.Results Compared with the Con group,there was a significant increase in the number of eosinophils,lymphocytes,neutrophils and monocytes,the levels of IL-5,IL-13,complement factor D(CFD)and contents of lactate and pyruvate in the lungs of mice in the OVA group.Lung HIF-1α,PHD2,p120 and E-cad mRNA levels were reduced,meanwhile HIF-1α and PHD2 protein expression were upregulated but E-cad and p120 protein expression were downregulated(all with P<0.05).After montelukast sodium intervention,the number of eosinophils and monocytes and CFD expression were significantly decreased in the lungs of Mon group,the contents of lactate and pyruvate were basically restored to normal,and the mRNA and protein expression of HIF-1α,PHD2,p120 and E-cad were effectively improved.Conclusion Montelukast sodium could alleviate the airway inflammatory responses in the lungs of asthmatic mice by regulating the PHD2/HIF-1α signaling pathway.

4.Survey on self-compassion levels in hematologic malignant tumor patients and analysis of its influencing factors

Objective:To investigate the current self-compassion levels in hematologic malignant tumor patients and analyze the influencing factors, providing a basis for clinical interventions.Methods:A convenience sampling method was used to select 120 patients with hematologic malignant tumors treated at the First Affiliated Hospital of Wenzhou Medical University from January to December 2022. Clinical data, self-compassion levels, social support, and adult attachment status were assessed using general data questionnaires, the Self-Compassion Scale (SCS) , the Perceived Social Support Scale (PSSS) , and the Experiences in Close Relationship Scale-Short Form (ECR-S) . Pearson correlation analysis was performed to analyze the correlation between SCS scores and PSSS, ECR-S scores. Multiple linear regression analysis was conducted to explore the influencing factors of self-compassion levels.Results:A total of 120 questionnaires were distributed, and 114 valid questionnaires were returned, resulting in an effective response rate of 95.00%. The average SCS score of hematologic malignant tumor patients was (80.54±5.80) . Univariate analysis showed that self-compassion levels differed significantly among patients of different ages, education levels, per capita family monthly income, disease duration, and the first diagnosis or not of hematologic malignant tumors ( P<0.01) . Pearson correlation analysis revealed a positive correlation between SCS scores and PSSS scores ( r=0.659, P<0.001) , and a negative correlation with the attachment anxiety and avoidance dimensions of the ECR-S ( r=-0.152, -0.588, P<0.001) . Multiple linear regression analysis showed that education level, the first diagnosis or not, PSSS score, ECR-S attachment anxiety dimension, and ECR-S attachment avoidance dimension scores entered the regression equation. Conclusions:The influencing factors of self-compassion levels in hematologic malignant tumor patients include education level, the first diagnosis or not, social support level, and adult attachment status. Healthcare providers should strengthen psychological support for patients with low education, disease recurrence/treatment failure, and insecure attachment types, while also improving patients' social support levels to enhance their self-compassion status.

For the detection and identification of abnormal nodular tissues on the body surface, a microwave sensor structure loaded with a spiral resonator is proposed in this paper, a sensor simulation model is established using HFSS software, the structural parameters are optimized, and the actual sensor is fabricated. The S21 parameters of the tissue were obtained when nodules appeared by simulation, and the characteristic relationship between the difference of S21 parameters with position was analyzed and tested experimentally. The results showed that when nodules were present in normal tissues, the curve of S21 parameter difference with position change had obvious inverted bimodal characteristics, and the extreme value of S21 parameter difference appeared when the sensor was directly above the nodules, which was easy to identify the position of nodules. It provides an objective detection tool for the identification of abnormal nodular tissues on the body surface.

Objective To evaluate the correlation between cerebral blood flow perfusion and memory impairment in patients with severe stenosis of vertebral basilar artery (VBA).Methods 62 cases of patients with VBA stenosis diagnosed by digital subtraction angiography(DSA) in Beijing Tiantan Hospital from September 2016 to March 2017 were enrolled.Mental State Examination (MMSE),Clinical Memory Scale (CMS) test and CT perfusion(CTP) was performed.All patients were divided into memory normal group(n=24,including 1 excellent case,6 above normal cases,and 14 normal cases) and memory impairment group(n =38,including 18 below normal cases,12 periphery cases,8 impaired cases) according to CMS.The ratios of side-to-side period were compared between bilateral mesial temporal lobe and anterior circulation area.The relative time to peak (rTTP),relative mean transit time(rMTY),relative cerebral blood flow(rCBF) and relative cerebral blood volume (rCBV) were calculate.Results The incidence of CTP decompensation in the medial temporal lobe was higher than that in the patients with memory impairment(P<0.05).The difference of rTTP and rMTT value between the two groups in the bilateral medial temporal lobes was statistically significant (rTFP:(1.131 ±0.037),(1.437±0.139),t=10.520,P< 0.05);rMTT:(1.081 ±0.059),(1.281 ±0.174),t=5.423,P<0.05).Conclusion The patients with VBA severe stenosis are more likely to get memory impairment due to cerebral hypoperfusion.
