1.Study on mechanism of Chanbao zhichuang suppository in treating hemorrhoids based on network pharmacology and metabolomics

OBJECTIVE To explore the mechanism of improvement effect of Chanbao zhichuang suppository (CBZCS) on hemorrhoids in rats through network pharmacology and metabolomics. METHODS A hemorrhoid model was established by subcutaneous injection of rhododendron oil to induce anal swelling. SD rats were divided into blank group (NC group, 0.32 g/kg vaseline), model group (Model group, 0.32 g/kg vaseline), CBZCS low-, medium-, and high-dose groups (CBZCS-L, CBZCS- M, CBZCS-H groups, with dosages of 0.16, 0.32, and 0.64 g/kg respectively), and Mayinglong musk hemorrhoids suppository group (Positive group, 0.32 g/kg), with 9 rats in each group. Anal administration was performed at 6, 12, 24, 48, and 72 hours after modeling. After the last administration, the pathological changes of the anal tissues in rats were observed, and the serum levels of interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) in rats were detected. Differential metabolite analysis and enrichment analysis were conducted by metabolomics methods, and the target proteins of CBZCS in treating hemorrhoids were obtained by network pharmacology. The core metabolic pathways were screened by interaction and enrichment analysis of differential metabolites and proteins, and the core proteins were experimentally verified. RESULTS Compared with the NC group, the anal tissues of the Model group showed obvious lesions, and the levels of IL-6 and TNF- α in the serum were significantly increased (P<0.05); compared with the Model group, the pathological damage of the anal tissues in the treatment groups was alleviated to varying degrees, and serum levels of IL-6 in CBZCS-H group, CBZCS-M group, and Positive group as well as serum levels of TNF-α in CBZCS-H group were significantly reduced (P<0.05). The metabolomics results showed that 34 differential metabolites were screened from the anal tissues of rats, and 22 of them showed a return after CBZCS administration. The differential metabolites mainly enriched in arachidonic acid metabolism, histidine metabolism, and glycerophospholipid metabolism. Through the network pharmacology, 138 intersection genes of CBZCS against hemorrhoids were determined. The analysis results showed that differential metabolites and target proteins were mainly enriched in the arachidonic acid metabolism pathway, and the regulation of this pathway might be related to cyclooxygenase-2 (COX-2), Myc proto-oncogene protein (c-MYC), cytochrome P450 1B1 (CYP1B1), interleukin-1β (IL-1β), and IL-6 protein expression. The experimental verification results showed that the expression levels of key proteins (COX-2, c-MYC, CYP1B1, IL-6, IL-1β) in the anal tissues of the Model group were significantly higher than those in the NC group (P<0.05), and the levels of the above proteins in the anal tissues of CBZCS-H group and Positive group were significantly lower than those in the Model group (P<0.05). CONCLUSIONS The mechanism of CBZCS in treating hemorrhoids may be to inhibit the expression of COX-2, c-MYC and CYP1B1 proteins, thereby inhibiting arachidonic acid metabolism and reducing the release of inflammatory factors IL-6 and IL-1β.
2.Current status of perceived health competence and related influencing factors in patients with hepatolenticular degeneration

Objective To investigate the level of perceived health competence in patients with hepatolenticular degeneration, also known as Wilson disease (WD), as well as related influencing factors. Methods From January to December 2023, the convenience sampling method was used to select 60 patients with WD as the subjects for this study among the patients who were hospitalized in The Affiliated Hospital of the Neurology Institute of Anhui University of Chinese Medicine, and General Information Questionnaire, Perceived Health Competence Scale, Health Literacy Scale, Perceived Social Support Scale, and Perceived Control Scale were used to investigate the influencing factors for perceived health competence in patients with WD. Results The total score of perceived health competence was (25.85±5.03) in the patients with WD, with an outcome expectancy score of (12.12±3.55) and a behavioral expectancy score of (13.73±3.33), and the multivariate analysis showed that degree of education and health literacy assessment were influencing factors for perceived health competence in the patients with WD. Conclusions There is a moderate level of perceived health competence in patients with WD, which is influenced by the factors such as degree of education and health literacy assessment.
3.Mechanism of Linggui Zhugantang in Promoting Astrocyte Endocytosis and Degradation of Amyloid β

ObjectiveTo investigate the effects of Linggui Zhugantang (LGZGT)-containing serum on primary astrocytes (AS) induced by β amyloid 1-42 (Aβ1-42) in a rat model of Alzheimer's disease (AD) and explore the phagocytic and degradative effects of LGZGT on Aβ. MethodAn AD model was established by inducing AS with Aβ1-42. The cells were divided into normal group, model group, LGZGT low-, medium-, and high-dose (LGZGT-L, LGZGT-M, and LGZGT-H) groups, and donepezil hydrochloride group. The model group was treated with Aβ1-42 at a final concentration of 10 μmol∙L-1. The LGZGT-L, LGZGT-M, and LGZGT-H groups were treated with 10% serum containing LGZGT on the basis of the model group. Cell viability was assessed using a cell counting kit-8 (CCK-8), lactate dehydrogenase (LDH) activity was measured using an LDH assay kit, and cell morphology was observed using an inverted microscope. The expression of Aβ-related degradation enzymes insulin-degrading enzyme (IDE) and cathepsin D (CTSD) was detected using Western blot, and the fluorescence intensity of cathepsin B (CTSB) was measured using immunofluorescence. The content of Aβ1-42 in cells was determined using an enzyme-linked immunosorbent assay (ELISA). ResultCompared with the normal group, the viability of AS in all groups decreased, and Aβ1-42 at different concentrations had inhibitory effects on AS proliferation. After administration, compared with the normal group, the cell survival rate of the model group decreased significantly (P<0.05). Compared with the model group, the cell survival rates of the LGZGT-H group and donepezil hydrochloride group increased significantly (P<0.05). The LDH activity of cells in the model group was significantly increased compared with that in the normal group (P<0.05), and cell bodies were swollen and enlarged with increased protrusions and elongation, suggesting more obvious cell damage. Compared with the model group, the LDH activity of cells in the donepezil hydrochloride, LGZGT-L, LGZGT-M, and LGZGT-H groups decreased significantly (P<0.05). After administration, the cell swelling in the LGZGT-M, LGZGT-H, and donepezil hydrochloride groups improved, cell protrusions shortened, and cell clustering decreased. Compared with the normal group, the expression of IDE and CTSD in the model group decreased significantly (P<0.05). Compared with the model group, the expression of IDE increased significantly in the LGZGT-M and LGZGT-H groups (P<0.05). Compared with the model group, the expression of CTSD increased significantly in the LGZGT-L, LGZGT-M, LGZGT-H, and donepezil hydrochloride groups (P<0.05). The average fluorescence intensity of CTSB in the model group was significantly lower than that in the normal group (P<0.05). Compared with the model group, the average fluorescence intensity of CTSD in the LGZGT-L, LGZGT-M, LGZGT-H, and donepezil hydrochloride groups increased significantly (P<0.05). The intracellular content of Aβ1-42 in cells in the model group was significantly higher than that in the normal group (P<0.05). After administration, compared with the model group, the intracellular content of Aβ1-42 in cells in the LGZGT-L, LGZGT-M, LGZGT-H, and donepezil hydrochloride groups decreased significantly (P<0.05), and LGZGT-containing serum reduced Aβ1-42 in a dose-dependent manner (P<0.05). ConclusionLGZGT has a protective effect on Aβ1-42-induced AS and can promote the degradation of Aβ. Its mechanism may be related to reducing Aβ toxicity, enhancing cell viability, promoting the expression of IDE, CTSD, and CTSB, and restoring lysosomal function.
4.A qualitative study on the participation of patients with rheumatoid arthritis in shared medical appointments experience

Objective To explore the experience of patients with rheumatoid arthritis participating in the shared medical appointments(SMA),so as to provide references for medical staff to formulate targeted intervention strategies.Methods By the descriptive phenomenology research method in qualitative research,14 patients with rheumatoid arthritis who participated in the SMA in a tertiary A integrated traditional Chinese and western medicine hospital in Shanghai from August to October 2023 were selected for the in-depth interviews,and the data were analyzed and themes were extracted by Colaizzi 7-step analysis method.Results The experience of patients with rheumatoid arthritis participating in SMA can be summarized into 3 themes:①Driving factors of participation in SMA(seeking clarity and resolution,seeking support and hope);②Perceived benefits of participation in SMA(feelings of efficiency and convenience,reduced isolation,increased security,reduced stigma,increased self-efficacy,insights and experiences,lifestyle changes);(3)Barriers to participation in shared outpatient procedures(obscure medical terminology,inapplicability to some special groups,single content and form of sharing,worries and concerns about personal privacy disclosure,transportation inconvenience and time conflicts).Conclusion Medical staff should pay attention to the experience of patients with rheumatoid arthritis participating in the SMA,improve the training skills of medical staff,solve the obstacles in the process of participation in the SMA,and promote the continuous improvement and development of the SMA.
5.Effect of Danggui Shaoyaosan on Mitochondrial Homeostasis and AMPK/SIRT1/PGC-1α Signaling Pathway in Rats Model of Alzheimer's Disease

ObjectiveTo investigate the effect of Danggui Shaoyaosan (DSS) on the morphology and function of mitochondria in rats model of Alzheimer's disease (AD) and its possible mechanism. MethodRats model of AD was established by injection of streptozocin (STZ) into bilateral ventricles of SD rats. The 40 rats were randomly divided into sham group, model group, low, medium and high dosages of Danggui Shaoyaosan (12,24,36 g·kg-1·d-1) groups,observed the morphological changes of mitochondria in hippocampus of rats by electron microscopy after 14 days of continuous gavage. In situ end labeling(TUNEL) staining used to detect apoptosis and immunofluorescencereactive used to observe the expression of reactive oxygen species (ROS) and peroxisome proliferators-activated receptor γ coactivator lalpha (PGC-1α),quantitative Real-time polymerase chain reaction(Real-time PCR)detected the mRNA expression of dynamin-related protein 1 (Drp1),mitochondrial fusion protein 2 (MFN2) ,cytochrome C oxidase subunit Ⅳ (COX Ⅳ) and PGC-1α. Western blot detected the protein expression of adenosine 5'-monophosphate (AMP)-activated protein kinase (AMPK),phosphorylation(p)-AMPK,recombinant Sirtuin 1 (SIRT1) and PGC-1α. ResultCompared with the sham group,the results of model group showed that the damage of mitochondria in hippocampus was more obvious,accelerated the ROS production and apoptosis rate (P<0.01),decreased the mRNA level of MFN2,COX Ⅳ,PGC-1α,increased the mRNA level of Drp1,and descended the protein of p-AMPK/AMPK,SIRT1,PGC-1α (P<0.05,P<0.01).Compared with the model group,the medium and high dose of DSS group notably improved the damage of mitochondria,reduced the production of ROS and apoptosis rate (P<0.01),promoted the mRNA expression of MFN2,COX Ⅳ,PGC-1α,inhibited the mRNA expression of Drp1,and up-regulated the protein of p-AMPK/AMPK,SIRT1,PGC-1α (P<0.01). ResultDSS can significantly ameliorate the mitochondrial homeostasis imbalance in AD rats.
6.Health empowerment of elderly fragility fracture patients and its influencing factors

Objective? To explore the current situation of health empowerment of elderly fragility fracture patients and to analyze its influencing factors. Methods? We selected four Class Ⅲ Grade A hospitals randomly in Hengyang. From October 2017 to February 2018, a total of 176 elderly fragility fracture inpatients were investigated with the self-designed general information questionnaire, Elderly Frailty Assessment Scale, Osteoporosis Self-Efficacy Scale (OSES), Health Empowerment Scale for Elderly Patients with Chronic Disease. Multiple linear regression analysis was used to explore the influencing factors of health empowerment of patients. Results? The score of health empowerment of elderly fragility fracture patients was (89.00±14.31) and responsibility belief dimension was with the highest score (3.74±0.88). Patients' health empowerment had a negative correlation with the frailty (r=-0.576, P<0.01) and had a positive correlation with the self-efficacy (r=0.496, P< 0.01). Multiple linear regression analysis showed that the main influencing factors of health empowerment of patients included ages, education levels, medical payment methods, frailty and self-efficacy (P<0.05). Conclusions? Nurses should pay attention to patients' health empowerment ability and provide the individualized nursing to strengthen the health empowerment ability of elderly fragility fracture patients.
7.Effect of postoperative analgesia nursing on the comfort degree of laparotomy patients in hepatobiliary surgery department

Objective To investigate the effect of postoperative analgesia nursing on the comfort degree of laparotomy patients in hepatobiliary surgery department.Methods A total of 86 laparotomy patients in our department were randomly divided into observation group and control group,receiving postoperative analgesia nursing and routine nursing,respectively.And postoperative analgesia and postoperative comfort were compared.Results The postoperative pain degree in the observation group was less than that in the control group,the incidence of incomplete analgesia was lower than that in the control group,pain call frequency in the operation day,1 day after operation,and 2 day after operation was lower than that in the control group,the difference was statistically significant (P <0.05).Comfort of physiological,psychological,social and cultural aspects in the observation group were higher than that in the control group,nursing satisfaction was higher than the control group,the difference was statistically significant (P < 0.05).Conclusion Postoperative analgesia nursing can significantly reduce degree of pain of laparotomy patients in hepatobiliary surgery department,and improve the postoperative comfort and nursing satisfaction.

9.Clinical observation of recurrent oral ulcer of heat accumulation in the heart and spleen treated with acupoint injection and fire needling therapy.

OBJECTIVETo compare the clinical efficacy between the combined acupuncture therapy of acupoint injection and fire needling and the oral administration of vitamin C and B2 as well as subcutaneous injection of transfer factor.
METHODSOne hundred and fifty-eight patients of recurrent oral ulcer (ROU) differentiated as heat accumulation in the heart and spleen were randomized into an observation group and a control group, 79 cases in each, one. In the observation group, Xinshu (BL 15), Pishu (BL 20) and Zusanli (ST 36) were selected for acupoint injection of shuang huanglian injection, 1 mL at each point, three times a week, for 2 weeks totally. Additionally, fire needling was applied to the ulcer lesion and peripheral mucosa, once a day, totally for 3 times. In the control group, vitamin C 100 mg and vitamin B2 20 mg were prescribed for oral administration, three times a day; and subcutaneous injection of transfer factor was applied, 2 mL (3 mg), twice a week, for 2 weeks totally. The score of symptoms and physical signs and the visual analogue scale (VAS) were observed before and after treatment, as well as the time of ulcer healing and ulcer recurrence in the patients of the two groups. The clinical efficacy was compared between the two groups.
RESULTSThe total effective rate was 92.4% (73/79) in the observation group, better than 77.2% (61/79) in the control group (P < 0.01). The differences in the score of symptoms and physical signs and VAS score before and after treatment were significant statistically in the observation group and the control group (all P < 0.01). The score of symptoms and physical signs and VAS score after treatment in the observation group were reduced significantly as compared with those in the control group (both P < 0.01). The time of ulcer healing in the observation group was earlier than that in the control group [(3.87 +/- 1.01) d vs. (6.22 +/- 0.53) d, P < 0.01]. The recurrence rate in the observation was lower than that in the control group [22.7% (17/75) vs. 50.0% (36/72), P < 0.01].
CONCLUSIONThe combined therapy of acupoint injection and fire needling achieves the better efficacy on ROU compared with oral administration of vitamin and subcutaneous injection of transfer factor. This therapy has the good and quick effect of analgesia, relieves the relevant symptoms, shortens the time of ulcer healing and reduces the recurrence rate.

10.Epidemiological Investigation and Genome Analysis of Duck Circovirus in Southern China

Duck circovirus(DuCV),a potential immunosuppressive virus,was investigated in Southern China from March 2006 to December 2009 by using a polymerase chain reaction(PCR)based method. In this study,a total of 138 sick or dead duck samples from 18 different farms were examined with an average DuCV infection rate of~35%. It was found that ducks between the ages of 40~60 days were more susceptible to DuCV. There was no evidence showing that the DuCV virus was capable of vertical transmission. Farms with positive PCR results exhibited no regularly apparent clinical abnormalities such as feathering disorders,growth retardation or lower-than-average weight. The complete genomes of 9. strains from Fujian Province and 1 from Zhejiang Province were sequenced and analyzed. The 10 DuCV genomes,compared with others genomes downloaded from GenBank,ranged in size from 1988 to 1996 base pairs,with sequence identities ranging from 83.2% to 99.8%. Phylogenetic analysis based on genome sequences demonstrated that DuCVs can be divided into two distinct genetic genotypes,Group I(the Euro-USA lineage)and Group II(the Taiwan lineage),with approximately 10.0% genetic difference between the two types. Molecular epidemiological data suggest there is no obvious difference among DuCV strains isolated from different geographic locations or different species,including Duck,Muscovy duck,Mule duck,Cheery duck,Mulard duck and Pekin duck.
