1.Status and influencing factors of the continuity of care needs in patients with ruptured intracranial aneurysm hemorrhage

ObjectiveTo investigate the status and influencing factors of the continuity of care needs in patients with ruptured intracranial aneurysm hemorrhage. MethodsFrom January, 2022 to April, 2025, 121 patients with ruptured intracranial aneurysm hemorrhage admitted to the Fourth Affiliated Hospital of Anhui Medical University were selected. A questionnaire survey was conducted using general information questionnaire and continuity of care needs questionnaire. Multiple linear regression analysis was used to analyze the influencing factors of continuity of care needs. ResultsA total of 121 questionnaires were distributed, with 115 returned (95.04%). The scores of continuity of care needs were with an average score of (100.54±6.97). Statistically significant differences were observed in scores of continuity of care needs among patients with different ages, education levels, primary family caregivers, preoperative Hunt-Hess grades, number of comorbid diseases, whether they received postoperative rehabilitation training and availability of nearby healthcare services (|F| > 8.104, |t| > 2.651, P < 0.05). Multiple linear regression analysis showed that education level, primary family caregiver, preoperative Hunt-Hess grade, number of comorbid diseases and availability of nearby healthcare services were all influencing factors for the continuity of care needs in patients with ruptured intracranial aneurysm hemorrhage (|t| > 1.703, P < 0.05). ConclusionThe need for continuous care in patients with ruptured intracranial aneurysms is influenced by factors such as educational level, primary family caregivers, preoperative Hunt-Hess grade, number of comorbid diseases and accessibility to nearby healthcare services. Medical staff should comprehensively consider patients' specific conditions and actual needs to provide targeted continuous care services.

4.Advance in application of artificial intelligence in hospital infection prevention and control

Infection Prevention and Control(IPC)programs are crucial for prevention of hospital-associated infec-tions(HAIs),health and safety of patients and health care workers.The major task confronting the health care is to effectively carry out the IPC programs so as to raise the efficiency and quality of management of HAIs.In recent years,the rapid development of artificial intelligence(AI)technology has provided new ideas for the management of HAIs.The application and progress of research on application of AI technologies such as machine learning,nat-ural language processing and computer vision in surveillance,early warning,diagnosis,prevention and control,smart medical equipment and management of antibiotics were reviewed in the article.Meanwhile,the problems and challenges confronting the AI application,such as technical breakthroughs,data privacy and health care work-ers' acceptance,were also expounded.In the future,it is necessary to push forward the technology research and development,establish the safe and reliable data management system,increase the experience of health care work-ers and energize the interdisciplinary cooperation on IPC.
5.Study on the Improvement of Glucose-Lipid Metabolism in Type 2 Diabetes Mellitus by Compounding Seven-Flower Granules

Objective To investigate the ameliorative effect of Qihuakeli,a Hani formula,on glycolipid metabolism in type 2 diabetes mellitus by in vivo and in vitro experiments.Methods Rat liver mesenchymal stromal cells(BRL-3A)were inoculated in six-well plates and divided into blank,palmitic acid,fenofibrate,and Qihuakeli serum-containing 5.4,10.8,and 21.6 g/kg groups.Except for the blank group,the remaining groups were intervened with 0.2 mmol/L palmitic acid(PA)for 24 hour,and then added with drug-containing serum,and then continued to incubate for 24 hour.The proliferation rate of BRL-3A cells in each group was determined.Total cholesterol(T-CHO),low-density lipoprotein cholesterol(LDL-C),and high-density lipoprotein cholesterol(HDL-C)concentrations in the supernatant of each cell group were measured,cell culture medium was aspirated and discarded,triglyceride(TG)concentration in the cell lysate.The lipid content of the cells was determined by measuring and staining with red oil.Meanwhile,45 rats were taken and divided into blank group,model group,fenofibrate group(0.225 g/kg),Qihuakeli compound 5.4 g/kg group,and Qihuakeli compound 10.8 g/kg group,the blank group was given normal feed and the rest of the groups were given high-fat feed for 42 day.Beginning on the 43rd day,each group,except the blank group,was injected with a single intraperitoneal injection of Starting from the 43rd day,except the blank group,each group was given a one-time intraperitoneal injection of 0.25%streptozotocin(STZ)solution,and at the same time,the corresponding drugs were given by gavage for 14 day.The rats'weight gain and liver index were measured.Serum fasting blood glucose(FBG)and fasting insulin(FINS)were detected,and the insulin resistance index(ISI)was calculated.Serum free fatty acid(FFA)levels and tumor necrosis factor-α(TNF-α)in liver tissue were also detected.HE staining was used to detect pathological changes in the pancreas.Pathological changes were observed in the tissues,and islet α and β cell expression was detected by immunohistochemistry.Results Compared to the PA group,the accumulation rate of BRL-3A cells was significantly higher(P<0.01)in the 10.8 and 21.6 g/kg Qihuakeli-containing serum groups.The levels of T-CHO,LDL-C and TG in the 5.4 and 21.6 g/kg serum groups were significantly lower(P<0.05),and HDL-C levels significantly increased(P<0.05).Oil red staining results showed that lipids in the cytoplasm of the 5.4,10.8 and 21.6 g/kg.Qihuacel-containing groups significantly reduced.Compared to the model group,the body weight of the 10.8 g/kg group containing Qihuakeli granules increased significantly(P<0.05).The liver index of the 5.4 g/kg group containing Qihuakeli decreased significantly(P<0.05).The serum indices of FBG,FINS,FFA and insulin resistance of the 5 g/kg group containing Qihuakeli decreased significantly(P<0.05).In the 5.4,10.8 g/kg groups,all serum FBG,FINS,FFA and insulin resistance indices significantly reduced in the 5.4 and 10.8 g/kg Qihuakeli groups(P<0.05 or P<0.01).TNF-α levels were significantly reduced(P<0.01).HE staining showed that a small number of lymphocytes were scattered in the pancreatic ducts and perivascular area of the rats in the Qihuakeli 5.4 and 10.8 g/kg groups,the local vasodilatation was observed,the number of pancreatic islet cells and the area of islet cells significantly increased.Immunohistochemical study was further used.The results of immunohistochemistry showed that the area of pancreatic islet α-cells significantly reduced and the area of pancreatic islet β-cells significantly increased in Qihuakeli 5.4 and 10.8 g/kg groups.Conclusion Qihuakeli compound improved glucose-lipid metabolism in T2DM,probably by improving the function of pancreatic islet cells,increasing the sensitivity of insulin to blood glucose,improving insulin resistance,decreasing the secretion of insulin and glucagon,and thus lowering the level of fasting blood glucose.Meanwhile,by decreasing the content of TNF-α,inhibiting lipolysis in the body,and promoting the uptake of FFA by adipocytes,and further lowering the FFA.Thus,it regulates the levels of TG,T-CHO,HDL-C and LDL-C,improves the abnormalities of glucose and lipid metabolism,and alleviates T2DM.
6.Study on the processing technology and characteristic chromatogram of Epimedium koreanum roasted with suet oil

OBJECTIVE To optimize the processing technology of Epimedium koreanum roasted with suet oil and analyze its characteristic chromatogram before and after processing.METHODS The optimal processing technology was optimized by orthogonal experiments with frying power,frying time and medicinal temperature as factors using the comprehensive score of appearance traits(color+odor),alcohol-soluble extract,the contents of icariin and baohuoside Ⅰ as evaluation index,then proceed with verification.The E.koreanum roasted with suet oil was prepared with the optimal technology.The characteristic chromatograms of 15 batches of E.koreanum were established with the Similarity Evaluation System for Chromatographic Fingerprint of Traditional Chinese Medicine(2012 edition),and then similarity analysis was also conducted.Clustering analysis,principal component analysis,and orthogonal partial least squares discriminant analysis were used to evaluate the differences in E.koreanum before and after processing.RESULTS The optimal processing technique for E.koreanum roasted with suet oil was as follows:first,heat 4 g of suet oil at a power of 600 W until it melts;next,when the temperature at the bottom of the pan reaches 140 ℃,add 20 g of E.koreanum silk and stir-fry for 6 minutes;finally,remove it and let it cool.Comprehensive score of 3 validation tests was 98.94 points(RSD<3%).The established characteristic chromatogram of E.koreanum and E.koreanum roasted with suet oil were calibrated with 16 and 15 common peaks,respectively.Chromatographic peak 2 was determined to be chlorogenic acid,peak 5 to be chaohuoding A,peak 6 to be chaohuoding B,peak 7 to be chaohuoding C,peak 8 to be icariin,and peak 14 to be baohuoside Ⅰ.The similarities were all greater than 0.9.Results of cluster analysis and principal component analysis showed that E.koreanum and E.koreanum roasted with suet oil were clustered into distinct groups.The results of orthogonal partial least squares discriminant analysis showed that the variable importance projection values for peak 13,peak 15,peak 9,peak 1,peak 8,peak 12,peak 7,and peak 10 were all greater than 1.CONCLUSIONS The study successfully optimized the processing technology of suet oil-roasted E.koreanum.There are significant differences in the characteristic chromatograms of E.koreanum before and after processing.Among them,the chemical components corresponding to peak 13,peak 15,peak 9,peak 1,peak 8,peak 12,peak 7,and peak 10 may be the differential components of E.koreanum before and after processing.

9.Nodosin Induces Apoptosis of Colorectal Cancer Cells by Inhibiting mTOR Pathway and Activating Autophagy

Background:Colorectal cancer is a common malignant tumor in the digestive tract.The existing treatment methods and drugs have disadvantages such as significant toxicity and side effects,so there is an urgent need to develop new drugs with high efficiency and low toxicity.Aims:To investigate the potential mechanism of Nodosin in inducing apoptosis of colorectal cancer cells.Methods:Colorectal cancer cell lines SW480 and HCT116 were cultured with 0,5,10 and 20 μmol/L Nodosin for 24 hours,or cultured with 10 μmol/L Nodosin for 0,12,24 and 48 hours.The cell viability was measured by CCK-8 assay,and the protein expressions of PARP1,mTOR,p-mTOR and LC3BⅡ were detected by Western blotting.After pretreated with the autophagy inhibitor 3-methyladenine(3-MA)5 mmol/L for 4 hours,SW480 and HCT116 cells were cultured with 10 μmol/L Nodosin for 24 hours.The protein expressions of PARP1 and LC3BⅡ were detected by Western blotting.Results:Nodosin could decrease the cell viability of SW480 and HCT116 cells in a dose-and time-dependent manner,increase the expressions of apoptosis-related protein cleaved PARP1 and autophagy-related protein LC3BⅡ,and inhibit the protein expression of p-mTOR.Pretreatment of the autophagy inhibitor 3-MA could inhibit the expressions of apoptosis-related protein cleaved PARP1 and autophagy-related protein LC3BⅡ induced by Nodosin.Conclusions:Nodosin can induce apoptosis of colorectal cancer cells by inhibiting mTOR pathway and activating autophagy.
