This article focuses on the deepening and innovation of grassroots party building work in public hospitals,tak-ing Sijing Hospital in Songjiang District,Shanghai as a case study.It explores how public hospitals can improve service quality and promote high-quality development through party building work in the new era.By analyzing the main problems faced by party building work,this article proposes practical paths for innovative party building work,including establishing a party building grid management organizational framework,formulating and improving party building management standards and systems,standardi-zing party building project processes,and strengthening party building assessment and supervision.Through the deepening and innovation of party building work,the integration of party building work and hospital business is achieved,thereby enhancing the overall service capacity and management level of the hospital,cultivating high-quality medical and health personnel,and provi-ding solid political guarantees and organizational support for the sustainable development of public hospitals.

OBJECTIVE To investigate the risk factors for hypokalemia caused by amphotericin B liposome， and to provide reference for clinical use of drugs. METHODS A retrospective analysis was used to collect the information of patients who used amphotericin B liposome during the hospitalization in First Affiliated Hospital of Hainan Medical College from January 2012 to December 2021. The details of use information about amphotericin B liposome and the potassium supplementation were collected. The patients were divided into hypokalemia group and normal group according to the occurrence of hypokalemia. Univariate and multi-variate Logistic regression analyses were used to analyze the risk factors for hypokalemia induced by amphotericin B liposome. RESULTS Of the 121 patients included in this analysis， 60 patients were in hypokalemia group， 61 patients were in normal group. The following parameters of the hypokalemic group were significantly higher or longer than those of the normal group， such as the maintenance dose， cumulative dose and maximum daily dose （in patients with severe hypokalemia） of amphotericin B liposome， treatment days， the maintained days of hypokalemia， daily dose of potassium supplement （in patients with moderate or severe hypokalemia）， the duration of potassium supplement （in patients with moderate hypokalemia）. Results of single factor analysis showed that the cumulative dose of amphotericin B liposome ≥200 mg and the duration of treatment ≥5 days were independent risk factors of hypokalemia caused by this drug （P＜0.05）. Multi-variate analysis results showed that the presence of basic hypokalemia， body weight ＜50 kg， cumulative dose of amphotericin B liposome ≥200 mg and the duration of treatment ≥5 days were the independent risk factors for hypokalemia caused by amphotericin B liposome （P＜0.05）. CONCLUSIONS The incidence of hypokalemia caused by amphotericin B liposome is high， the independent risk factors for hypokalemia include cumulative dose ≥200 mg， treatment days ≥5 days， the presence of basic hypokalemia and body weight ＜ 50 kg. It is suggested that serum potassium should be elevated to normal level before amphotericin B liposome treatment， and the level of serum potassium should be monitored during medication to reduce the occurrence of hypokalemia.

A 70-year-old male patient with type 2 diabetes mellitus was treated with sitagliptin phosphate (sitagliptin) on the basis of previous oral metformin hydrochloride due to poor glycemic control. After 16 days of medication, the patient developed erythema and blisters with itching on the skin of the limbs. Laboratory tests showed that the white blood cell count was 12.2×10 9/L, neutrophil count was 8.5×10 9/L, and eosinophil count was 4.56×10 9/L. Histopathological examination of the skin lesion showed blisters under the epidermis, intact epidermis at the top of the blisters, dermal papillae at the bottom of the blisters, serous fluid and eosinophils in the blisters, and slight infiltration of eosinophils in the superficial dermis under the blisters. Bullous pemphigoid was diagnosed, which was considered to be related to sitagliptin. Therefore, the drug was discontinued. One week later, erythema and blisters gradually disappeared and no erythema or blisters recured.

Objective ToinvestigatethevalueofMSCTandpost-processingtechniquesforthepresentationoffishboneinesophagus, andtoexploretherelatedfactorsofcomplicationsafterfishboneincarceration.Methods TheclinicaldataandMSCTdataof81cases ofesophagealfishforeignbodiestreatedbyendoscopyorsurgerywereanalyzed,including46 malesand35females;moreoverttest andχ2testwereusedforintergrouptesting.Results (1)Therewerenosignificantdifferencesinthemaleandfemalepatients’age, distributionoffishboneincarceration,theperiodofbetweenmedicaltreatmentandfishbonepuncturingintotheesophagus,thelength ofthepuncturedfishbone,theincarcerationmethod,andtheperforationcomplications(P＞0.05).Thedistancebetweenthefishbone andtheincisorinthemalepatientswasfarlongerthanthatinthefemalepatients,andthedifferencewasstatisticallysignificant(P＝0.011).Therewere54cases(66.7%)inthehorizontalincarcerationand27cases(33.3%)inthelongitudinalincarceration.(2)9patients (11.1%)hadesophagealperforation.Theoccurrenceofesophagealperforationcomplicationwasnotrelatedtotheageofthepatient, thedistancefromthetopofthefishbonetotheincisor,andthelengthofthefishbone(P＞0.05);theincarcerationtimewaspositively correlatedtotheperforationcomplications(r＝0.258,P＝0.02).Horizontalincarcerationoffishbonewasmoreeasilyperforatedthan longitudinalincarceration (P＝0.000).(3)Alltheforeignbodiesoffishbonesshowedbone-likehighdensityonCT.Theforeign bodiesoffishbonesweremostlyembeddedintheupperesophagus.Thedistancebetweenthetopofthefishboneandtheincisorwas about(19.38±4.08)cm.Allcaseshadswellingandthickeningoftheesophagealwall.Conclusion Thefishbonehorizontalincarceration, andlongertimestayintheesophagusaremorelikelyhappentoperforationcomplications.MSCTandpost-processingtechniquesprovidea basisforthequalitativediagnosisoffishbones,displayingthelocation,incarceration,andperforationcomplicationsofthefishbones intheesophagus.

Objective: To analyze characteristics of virus infections in patients with acute exacerbations of chronic obstructive pulmonary disease (COPD), and to analyze the role of virus infection in acute exacerbations of COPD. Methods: Totally 193 patients of COPD with acute exacerbations admitted to the Hospital of Sichuan Provincial Armed Police Force from April 2015 to March 2016 were enrolled as group A, and 50 patients of COPD in stable stage in the same period were enrolled as group B. IgM antibodies to respiratory syncytial virus, adenovirus, parainfluenza virus, influenza A virus, and influenza B virus in two groups were detected. General conditions and total virus infection rate were analyzed. Infection rates of each virus in group A were calculated. Proportion of single virus infection, two virus infections and several virus infections in group A with virus infection were calculated. Virus infection rates in different seasons in group A were also analyzed. Results: There was no significant difference in age, sex, and course of disease between the two groups(P>0.05). The virus infection rate of group A was 13.47%, which was higher than that of group B(χ²=5.29, P<0.05). The most common virus found in group A was influenza B virus(infection rate 12.44%), followed by influenza A virus(infection rate 7.78%), parainfluenza virus(infection rate 7.25%), adenovirus(infection rate 2.63%), respiratory syncytial virus(infection rate 1.32%). Single virus infection accounted for 34.6% of acute exacerbation COPD patients with virus infection, and two virus infections accounted for 15.4%, and three or more virus infections accounted for 50%. Virus infection rate in summer and autumn were higher than that in winter and spring, but the differences were not statistically significant(P>0.05). Conclusions This study showed that 14.37% of patients with acute exacerbations of COPD had viral infections, and most of them were infected with two or more viruses. Virus infection may play an important role in the acute exacerbation of COPD, and we need to pay attention to the prevention and treatment of virus infection in patients with COPD.

ABSTRACT:Objective To explore the expression of chemokine receptor CX3CR1 in severe acute pancreatitis (SAP)rats model and to estimate whether it can be a potential predictor of SAP.Methods Forty Sprague-Dawley rats were randomly divided into sham-operation (SO)group (n =20)and SAP group (n =20).Rats were killed at 6 h,12 h,24 h and 48 h after model induction.The serum level of CX3CR1 was measured by enzyme-linked immunosorbent assay (ELISA).The expression of CX3CR1 protein in the pancreas,lung and kidney were detected by immunohistochemistry.Results The serum level of CX3CR1 in SAP rats increased gradually after model induction and reached the peak at 24 h (542.4 pg/mL),which were significantly higher than those in SO group (P <0.05).The expression of CX3CR1 was found in the pancreas,lung and kidney tissues of SAP rats and higher than that in SO group.Meanwhile,the expression of CX3CR1 reached the peak at 24 h in the pancreas and lung and at 48 h in the kidney.Conclusion This study suggests that the chemokine receptor CX3CR1 may be one effective index for predicting the severity of acute pancreatitis and deserves further research.

Objective To assess left ventricular systolic strain in patients with mitral valve replacement (MVR) by speckle tracking echocardiography. Methods Two-dimensional images were acquired from the apical four-chamber view, two-chamber view and long-axis of the left ventricular view in 30 MVR patients (MVR group) and 30 healthy subjects matched by age and gender(control group), and then were analysed off line to evaluate left ventricular systolic strain. The maximum systolic longitudinal strain(LS) of myocardial segments and global longitudinal strain(GLS) were measured in apical views. Left ventricular ejection fraction(LVEF) was calculated using Simpson's method. The correlation between GLS and LVEF was analyzed using Pearson's method. Results Compared with the control group, LS of corresponding segment and GLS in MVR group decreased significantly (P < 0.01), but there were no statistical differences within MVR group(P>0.05). There was a good linear positive correlation between GLS and LVEF in patients (r = 0.710, P <0.01). Conclusions Speckle tracking eehoeardiography is available for measuring left ventricular systolic strain and estimating global systolic function in patients with MVR. Bull's-eye strain map, created by speckle tracking imaging, can achieve an accurate real-time segmental wall motion analysis.

Objective To evaluate the clinical value of color M-mode Doppler echocardiography (CMM) in assessing left ventricular(LV) diastolic function after mitral valve replacement(MVR). Methods Fifty-two patients who had received the implantation for more than three months were examined by echoeardiography (MVR group). Thirty age and sex-adjusted normal volunteers served as control group. Dimensions of left atrium and ventricle,ejection fraction(EF) and mitral inflow velocity(E) were measured by two-dimensional and Doppler eehocardiography. Color M-mode Doppler flow propagation velocity(Vp) and TDl-derived diastolic mitral annular velocity(Em) were measured from the apical four-chamber and two-chamber views. Then two new indexes based on the ratio of E to Vp(E/Vp) and E to Era' (E/Em') were while E/Vp and E/Era' increased, there were significant differences(P <0.01). The correlation coefficient and atrial fibrillation(P <20.01) ,however E/Vp was lower affected hy EF or atrial fibrillation(P >0.05). dysfunction were 2.00. Conclusions CMM is an effective technique to assess LV diastolic function after MVR. E/Vp has good consistency with E/Era and may be a potential useful parameter for assessing LV diastolic function.
